    Hicham Laraqui, Khalid Sair

Hicham Attifi, Mounir Hmidi, Ali Boukhari, Nabil Touihem, Mounir Kettani, Mohammed Zalagh, Abdelmjid Messary

Experiences in Tick Control by Acaricide in the Traditional Cattle Sector in Zambia and Burkina Faso: Possible Environmental and Public Health Implications

Daniele De Meneghi, Frédéric Stachurski, Hassane Adakal

Livestock, especially cattle, play a paramount role in agriculture production systems, particularly in poor countries throughout the world. Ticks and tick-borne diseases (TBDs) have an important impact on livestock and agriculture production in sub-Saharan Africa. The authors review the most common methods used for the control of ticks and TBDs. Special emphasis is given to the direct application of acaricides to the host animals. The possible environmental and public health adverse effects (i.e., risks for the workers, residues in the environment and in food products of animal origin) are mentioned. The authors present two case studies, describing different field experiences in controlling ticks in two African countries. In Zambia (Southern Africa), a strategic dipping regime was used to control Rhipicephalus appendiculatus ticks, vectors of theileriosis, a deadly disease affecting cattle in the traditional livestock sector in Southern Province. The dipping regime adopted allowed to reduce the tick challenge and cattle mortally rate and, at the same time, to employ less acaricide as compared to the intensive dipping used so far, without disrupting the building-up of enzootic stability. In Burkina Faso (West Africa), where dipping was never used for tick control, an acaricide footbath was employed as an alternative method to the traditional technique used locally (portable manual sprayers). This was developed from field observations on the invasion/attachment process of the Amblyomma variegatum ticks – vector of cowdriosis – on the animal hosts, leading to a control method aimed to kill ticks temporarily attached to the interdigital areas before their permanent attachment to the predilection sites. This innovative method has been overall accepted by the local farmers. It has the advantage of greatly reducing costs of treatments and has a minimal environmental impact, making footbath a sustainable and replicable method, adoptable also in other West African countries.

Experimental hut evaluation of DawaPlus 3.0 LN and DawaPlus 4.0 LN treated with deltamethrin and PBO against free-flying populations of Anopheles gambiae s.l. in Vallée du Kou, Burkina Faso

BackgroundIn view of widespread pyrethroid resistance in malaria vectors in Africa, two long-lasting insecticidal nets (LLINs) incorporated with a synergist, piperonyl butoxide (PBO), DawaPlus 3.0 (deltamethrin + PBO in the roof panel; deltamethrin alone in the side panels) and DawaPlus 4.0 (deltamethrin + PBO in all panels), were evaluated in an experimental hut trial in a rice growing irrigated area in Burkina Faso. Efficacy of nets was tested against free-flying malaria vector, Anopheles gambiae s.l., with high pyrethroid resistance involving L1014F kdr and CYP6P3P450 resistance mechanisms.MethodologyThe efficacy of unwashed and 20-times washed DawaPlus 3.0 (polyethylene roof panel with 120 mg/m2 deltamethrin and 440 mg/m2 PBO; polyester side panels with deltamethrin 100 mg/m2) and DawaPlus 4.0 (same composition as roof of DawaPlus 3.0) was evaluated against DawaPlus 2.0 (80 mg/m2 deltamethrin; positive control). Volunteer sleepers and treatments were rotated in huts using a Latin square design on 63 consecutive nights during August–October 2016. Mortality, human blood-feeding inhibition, deterrence and exit rates of An. gambiae s.l. were monitored.Principal findingsSignificantly higher rates of mortality and blood-feeding inhibition were observed with unwashed DawaPlus 4.0 (36%; 47.5%) than unwashed DawaPlus 3.0 (11.8%; 33.3%), DawaPlus 2.0 (4.3%; 6.4%) or untreated net (P < 0.05). Washing reduced personal protective efficacy yet PBO-LLINs were more protective and both met the WHO criteria.ConclusionsThe PBO-containing DawaPlus 4.0 significantly protected against An. gambiae s.l. in the study area. Unwashed DawaPlus 3.0 gave low to moderate protection against the positive control. PBO inhibits oxidase action; hence in areas with active malaria transmission having oxidase mechanisms, PBO nets could confer additional personal protection.

Exploiting Anopheles responses to thermal, odour and visual stimuli to improve surveillance and control of malaria

Frances M. Hawkes, Roch K. Dabiré, Simon P. Sawadogo, Stephen J. Torr, Gabriella Gibson

Mosquito surveillance and control are at the heart of efforts to eliminate malaria, however, there remain significant gaps in our understanding of mosquito behaviour that impede innovation. We hypothesised that a combination of human-associated stimuli could be used to attract and kill malaria vectors more successfully than individual stimuli, and at least as well as a real human. To test this in the field, we quantified Anopheles responses to olfactory, visual and thermal stimuli in Burkina Faso using a simple adhesive trap. Traps baited with human odour plus high contrast visual stimuli caught more Anopheles than traps with odour alone, showing that despite their nocturnal habit, malaria vectors make use of visual cues in host-seeking. The best performing traps, however, combined odour and visual stimuli with a thermal signature in the range equivalent to human body temperature. When tested against a human landing catch during peak mosquito abundance, this “host decoy” trap caught nearly ten times the number of Anopheles mosquitoes caught by a human collector. Exploiting the behavioural responses of mosquitoes to the entire suite of host stimuli promises to revolutionise vector surveillance and provide new paradigms in disease control.

Mainkouado Michael Samy, Rachid Laroussi, Tarik Adioui, Hassan Seddik

Exploration of the Antioxidant and Anti-inflammatory Potential of Cassia sieberiana DC and Piliostigma thonningii (Schumach.) Milne-Redh, Traditionally Used in the Treatment of Hepatitis in the Hauts-Bassins Region of Burkina Faso

Inflammation is the supreme biological response to illness. In the Hauts-Bassins region, in traditional medicine, all parts of Cassia sieberiana and Piliostigma thonningii are used to treat hepatitis and inflammation. The aim of this study was to evaluate the in vitro antioxidant and anti-inflammatory activities of their aqueous extracts. High performance liquid chromatography with photodiode array (HPLC-DAD) and ultra-high-performance liquid chromatography coupled with high-resolution mass spectrometry (UHPLC-MS/MS) analyses highlighted the presence of polyphenols and flavonoids. Antioxidant and anti-inflammatory activities were measured by various methods such as DPPH (2,2-diphenyl-1-picrylhydrazyl), ABTS 2,2’-azino-bis(3-ethylbenzothiazoline-6-sulfonic acid), TAC (total antioxidant capacity), anti-protease, anti-lipoxygenase, and membrane stabilization. The best antioxidant activity was observed in the bark (DPPH: IC50 = 13.45 ± 0.10 µg/mL) and roots (TAC = 29.68 ± 1.48 mg AAE/g DW) of Piliostigma thonningii and in the roots (ABTS: IC50 = 1.83 ± 0.34 µg/mL) of Cassia sieberiana. The best anti-inflammatory activity was observed in the bark (anti-lipoxygenase: IC50 = 13.04 ± 1.99 µg/mL) and leaves (anti-proteases: IC50 = 75.74 ± 1.07 µg/mL, membrane stabilization: IC50 = 48.32 ± 6.39 µg/mL) of Cassia sieberiana. Total polyphenols (ABTS: r = −0.679, TAC: r = 0.960) and condensed tannins (ABTS: r = −0.702, TAC: r = 0.701) were strongly correlated with antioxidant activity. Total flavonoids (anti-proteases: r = −0.729), condensed tannins (anti-proteases: r = 0.698), and vitamin C (anti-proteases: r = −0.953) were strongly correlated with anti-inflammatory activity. Total polyphenols, flavonoids, condensed tannins, and vitamin C could contribute to the antioxidant and anti-inflammatory activities of the two studied plants. These results could validate the traditional use of these plants to treat various inflammatory diseases.

    Hilami Khaldoune, Vincent Godard

Exploring Affordable Curative Therapy for Sickle Cell Disease in Africa: A Comprehensive Overview

Adetola A. Kassim, Alexis A. Thompson, Punam Malik

ABSTRACTThe practical aspects of developing curative treatments for sickle cell disease (SCD) in Africa, such as gene therapy and hematopoietic stem cell transplantation, involve strengthening healthcare infrastructure, training healthcare professionals, establishing regional treatment centers, and creating national SCD programs. The costs associated with gene therapy and stem cell transplants can be prohibitive, especially in low‐ and middle‐income countries. Strategies to address affordability, including local manufacturing, government funding, and partnerships with global health agencies, are essential to ensure equitable access. Establishing ethical and regulatory guidelines while raising awareness among patients and communities is critical. Early diagnosis through newborn screening and adequate clinical care programs are vital for identifying individuals with SCD who could benefit from curative therapies and other interventions. Addressing cultural beliefs and promoting positive attitudes toward SCD and its management is essential. While curative therapies offer hope, hydroxyurea and other disease‐modifying therapies with established clinical benefits are more accessible in many settings. Prioritizing these medications ensures that patients with SCD are medically prepared to receive curative therapies while simultaneously building capacity for advanced treatments, thus creating a pragmatic approach. Lastly, international collaboration and partnerships among researchers, global health agencies, and local organizations are vital for sharing knowledge, resources, and best practices in SCD management.

Exploring Antibiotic Use in the Community: A Household-Based Survey Using the Drug Bag Method in Rural Burkina Faso

In Burkina Faso, there is lack of awareness of antibiotic use at the community level. This study aims to generate information on the commonly used antibiotics along with the reasons for which they have been used in rural Burkina Faso. The drug bag method was employed to collect information from 423 households in the health district of Nanoro. Descriptive analyses were performed using R software version 4.2.1. Of the 33 antibiotics inventoried, amoxicillin tablets and oxytetracycline were the most recognized and used antibiotics. This study indicated that antibiotics were used for a range of health problems in the community, some of which were administered as painkillers. While primary healthcare facilities constituted the primary source of drugs for households (76.8%), informal drug sellers constituted an additional option (61.5%) for community members. This is a significant concern, given that some antibiotics classified as “Watch”—such as norfloxacin—were readily available in these outlets, despite not being included on the country’s Essential Medicines List. This study underscores the necessity of considering the role played by formal providers in the inappropriate use of antibiotics and the importance of understanding the circumstances and logical reasoning underlying communities’ access to and use of antibiotics.

IntroductionSub-Saharan Africa faces the highest maternal and newborn mortality and morbidity rates globally. The World Health Organization Safe Childbirth Checklist (WHO SCC) was developed to address this issue by promoting evidence-based practices during childbirth. This study explored the barriers and drivers to implementing a modified WHO SCC (mSCC) in Burkina Faso, Cote d'Ivoire, and Guinea.MethodsA qualitative multiple case study design was conducted from May to June 2023, involving individual interviews with diverse stakeholders (n = 110) across four regional hospitals in each country. The mSCC was implemented in these hospitals along with training and coaching. Data was analyzed using thematic analysis, guided by the updated CFIR framework. Nvivo 14 was used for coding.ResultsThe study identified 17 drivers and 7 barriers. Key drivers included the mSCC's clarity, simplicity and alignment with national guidelines, training, coaching, and stakeholders’ engagement. in these 3 countries. Barriers were mainly related to resource constraints (medicines, supplies, staffing, and space), increased workload, and lack of incentives. Specific barriers for Burkina Faso and Cote d’Ivoire were the lack of incentives and the positioning of the Kakemono in confined space only in Cote d'Ivoire. Despite these challenges, the mSCC was generally well-received, with strong support from leadership and implementation facilitators contributing to its integration into routine care.ConclusionThis study highlighted the importance of addressing resource limitations, optimizing workload, and providing incentives to ensure successful mSCC implementation. Findings underscored the need for context-specific strategies and strong leadership support when introducing similar interventions in resource-constrained settings.

Exploring differences in substance use behaviours among gender minority and non-gender minority youth: a cross-sectional analysis of the COMPASS study

Thepikaa Varatharajan, Karen A. Patte, Margaret de Groh, Ying Jiang, Scott T. Leatherdale

AbstractIntroduction:Research characterizing substance use disparities between gender minority youth (GMY) and non-GMY (i.e. girls and boys) is limited. The aim of this study was to examine the differences in substance use behaviours among gender identity (GI) groups and identify associated risk and protective factors.Methods:Cross-sectional data from Canadian secondary school students (n=42107) that participated in Year 8 (2019/20) or Year 9 (2020/21) of the COMPASS study were used. Hierarchal logistic regression models estimated current substance use (cigarettes, e-cigarettes, binge drinking, cannabis and nonmedical prescription opioids [NMPOs]). Predictor variables included sociodemographics, other substances, mental health outcomes, school connectedness, bullying and happy home life. Interaction terms were used to test mental health measures as moderators in the association between GI and substance use. Results:Compared to non-GMY, GMY reported a higher prevalence for all substance use outcomes. In the adjusted analyses, GMY had higher odds of cigarette, cannabis and NMPO use and lower odds for e-cigarette use relative to non-GMY. The likelihood of using any given substance was higher among individuals who were involved with other substances. School connectedness and happy home life had a protective effect for all substances except binge drinking. Bullying victimization was associated with greater odds of cigarette, e-cigarette use and NMPOs. Significant interactions between GI and all mental health measures were detected. Conclusion:Findings highlight the importance of collecting a GI measure in youth population surveys and prioritizing GMY in substance use–related prevention, treatment and harm reduction programs. Future studies should investigate the effects of GI status on substance use onset and progression among Canadian adolescents over time.

Exploring Factors That Affected Student Well-Being during the COVID-19 Pandemic: A Comparison of Data-Mining Approaches

COVID-19-related school closures caused unprecedented and prolonged disruption to daily life, education, and social and physical activities. This disruption in the life course affected the well-being of students from different age groups. This study proposed analyzing student well-being and determining the most influential factors that affected student well-being during the COVID-19 pandemic. With this aim, we adopted a cross-sectional study designed to analyze the student data from the Responses to Educational Disruption Survey (REDS) collected between December 2020 and July 2021 from a large sample of grade 8 or equivalent students from eight countries (n = 20,720), including Burkina Faso, Denmark, Ethiopia, Kenya, the Russian Federation, Slovenia, the United Arab Emirates, and Uzbekistan. We first estimated a well-being IRT score for each student in the REDS student database. Then, we used 10 data-mining approaches to determine the most influential factors that affected the well-being of students during the COVID-19 outbreak. Overall, 178 factors were analyzed. The results indicated that the most influential factors on student well-being were multifarious. The most influential variables on student well-being were students’ worries about contracting COVID-19 at school, their learning progress during the COVID-19 disruption, their motivation to learn when school reopened, and their excitement to reunite with friends after the COVID-19 disruption.

Exploring heterogeneity in mosquito exposure and attraction and its implications for malaria transmission

BackgroundMalaria transmission exhibits significant heterogeneity within communities, with small proportions of individuals experiencing disproportionate mosquito exposure.MethodsThis study addresses critical knowledge gaps in characterising and modelling this heterogeneity. Parameterising Bayesian hierarchical models to field data from Burkina Faso, we compared gamma and lognormal distributions for describing heterogeneity in mosquito biting rates. We then implemented this heterogeneity in an individual-based stochastic modelling platform, OpenMalaria, to assess its impact on transmission dynamics.FindingsThe gamma distribution better described the observed field data than the lognormal. This choice has a natural mathematical justification: when individual biting rates follow a gamma distribution and bites occur as a Poisson process, the resulting bite counts follow a negative binomial distribution, which is well-supported empirically for overdispersed count data of this kind. Furthermore, the gamma distribution’s lighter tail produces more moderate saturation and immunity effects compared to lognormal-based models, yielding more realistic transmission dynamics. When heterogeneity is introduced to malaria transmission simulations, both prevalence and incidence levels generally decrease across all age groups. Additionally, heterogeneity shifts disease burden towards younger age cohorts and alters the fundamental relationships between entomological inoculation rate and prevalence/incidence. The integration of appropriate heterogeneity distributions into transmission models substantially improved their ability to reproduce field-observed age-incidence curves.InterpretationOur findings highlight the importance of accounting for heterogeneous exposure when modelling malaria transmission, particularly in low-transmission and elimination settings where heterogeneity may be more pronounced. Exploring heterogeneity in treatment effects: The impact and interaction of asset-based wealth and mass azithromycin distribution on child mortality

ObjectiveTo examine how child mortality among children aged 1–59 months varies by asset-based wealth status in rural Burkina Faso, and to assess the interaction between mass azithromycin (AZ) distribution and wealth status on child mortality at both the household and community levels.MethodsWe used data from a cluster-randomized trial and population census data on household characteristics and assets. A wealth index score for each household, used to classify the population by wealth, was generated using principal component analysis. We used the Relative Index of Inequality (RII), the Slope Index of Inequality (SII), and the concentration index to assess wealth-related inequalities in mortality, and the Gini Index to assess variability in child mortality across households and communities. Poisson regression models were used, with person-time at risk included as an offset, and robust standard error to estimate changes in mortality rates by wealth and treatment arm. Interaction was assessed on both the multiplicative and additive scales.ResultsMortality declined with increasing wealth at both the household and community levels, with a significant gradient at the community level (RII = 1.17, 95% CI: 1.05–1.29; SII = 2.3 per 1,000 person-years, 95% CI: 0.2–4.4), reflecting higher mortality among the poorest. The effect of AZ did not vary significantly by wealth index, and changes in mortality rates across wealth levels were similar between the two treatment arms. There was no evidence of a statistically significant interaction between AZ and asset-based wealth on either the multiplicative or additive scale at the household or cluster level.ConclusionOur findings demonstrate a wealth gradient in child mortality, with the highest mortality rates observed among households and communities in the lowest wealth quintiles. These disparities were consistent across both AZ-treated and placebo groups, suggesting that the role of AZ in health disparities may be limited to addressin
