A Phase II, Randomized, Double-blind, Controlled Safety and Immunogenicity Trial of Typhoid Conjugate Vaccine in Children Under 2 Years of Age in Ouagadougou, Burkina Faso: A Methods Paper

Matthew B Laurens, Sodiomon B Sirima, Elizabeth T Rotrosen, Mohamadou Siribie, Alfred Tiono, Alphonse Ouedraogo, Yuanyuan Liang, Leslie P Jamka, Karen L Kotloff, Kathleen M Neuzil

AbstractThe recent Typhoid Fever Surveillance in Africa Program demonstrated an overall adjusted incidence of typhoid fever 2–3 times higher than previous estimates in Africa. Recently, a single-dose typhoid conjugate vaccine that allows infants as young as 6 months old to be vaccinated was prequalified by the World Health Organization (WHO). This Vi-based conjugate vaccine demonstrated robust immunogenicity after 1 dose in infants and children 6 through 23 months of age in India with no safety signal, and is currently being tested for the first time on the African continent in Malawi. The WHO Strategic Advisory Group of Experts recommends studies to evaluate co-administering Vi-typhoid conjugate vaccine (Vi-TCV) with routine childhood vaccines in typhoid-endemic countries. The Burkina Faso immunization schedule includes yellow fever vaccine (YFV) at 9 months and meningococcal A conjugate vaccine (MCV-A) at 15 months, in addition to measles-rubella vaccine at both 9 and 15 months. Co-administration testing of Vi-TCV with these routine vaccinations will provide the data needed to support large-scale uptake of Vi-TCV in sub-Saharan Africa. A randomized, controlled, Phase II trial of Vi-TCV co-administration with the vaccinations routinely given at 9 and 15 months of age is planned in Burkina Faso. The overall aim is to assess the safety and immunogenicity of Vi-TCV when co-administered with YFV at 9 months of age and with MCV-A at 15 months of age. A total of 250 participants (100 infants aged 9–11 months and 150 children aged 15–23 months) will be enrolled. Clinical Trials Registration. NCT03614533.

A Phylogeographic Analysis of Porcine Parvovirus 1 in Africa

Porcine parvovirus 1 (PPV1) is recognized as a major cause of reproductive failure in pigs, leading to several clinical outcomes globally known as SMEDI. Despite being known since the late 1960s its circulation is still of relevance to swine producers. Additionally, the emergence of variants such as the virulent 27a strain, for which lower protection induced by vaccines has been demonstrated, is of increasing concern. Even though constant monitoring of PPV1 using molecular epidemiological approaches is of pivotal importance, viral sequence data are scarce especially in low-income countries. To fill this gap, a collection of 71 partial VP2 sequences originating from eight African countries (Burkina Faso, Côte d’Ivoire, Kenya, Mozambique, Namibia, Nigeria, Senegal, and Tanzania) during the period 2011–2021 were analyzed within the context of global PPV1 variability. The observed pattern largely reflected what has been observed in high-income regions, i.e., 27a-like strains were more frequently detected than less virulent NADL-8-like strains. A phylogeographic analysis supported this observation, highlighting that the African scenario has been largely shaped by multiple PPV1 importation events from other continents, especially Europe and Asia. The existence of such an international movement coupled with the circulation of potential vaccine-escape variants requires the careful evaluation of the control strategies to prevent new strain introduction and persistence.

A Pill in the Lifeworld of Women in Burkina Faso: Can Misoprostol Reframe the Meaning of Abortion

    Seydou Drabo

In Burkina Faso, induced abortion is socially stigmatized, condemned, disapproved and legally restricted to cases of rape, incest, fetal malformation or endangerment to the life of the mother. Many women often resort to unsafe procedures to induce abortion, which puts their health at great risk. Misoprostol, which is officially restricted to the treatment of postpartum hemorrhage or post-abortion care, is also used illegally by women to terminate their pregnancies. Misoprostol represents an addition to the existing abortion methods, such as vacuum aspiration, which health workers have often used to induce abortion clandestinely. Many women also use misoprostol to self-induce abortions, replacing abortifacients such as herbal teas, potions, high doses of antimalarial drugs, or bleach. Despite the changes that occur in abortion access due to the use of misoprostol, little is known about what the drug means to its users and how this meaning can in turn influence the meaning of abortion. The aim of this paper is to describe how the use of misoprostol to terminate pregnancy contributes to changing women’s perception of the meaning of abortion. This paper is based on ethnographic fieldwork conducted between March 2016 and February 2017 in the city of Ouagadougou, Burkina Faso. By examining the relation between the use of misoprostol and the meaning that women give to abortion, this study found that women experience abortion either spontaneously or using emergency contraception with misoprostol. Through the experience of women, this paper claims that the meaning of abortion should be seen as a social construct and fundamentally rooted in individual practices and experiences rather than being subject to dichotomist global discourse.

A predictive algorithm for identifying children with sickle cell anemia among children admitted to hospital with severe anemia in Africa

Peter Olupot‐Olupot, Roisin Connon, Sarah Kiguli, Robert O. Opoka, Florence Alaroker, Sophie Uyoga, Margret Nakuya, William Okiror, Julius Nteziyaremye, Tonny Ssenyondo, Eva Nabawanuka, Juliana Kayaga, Cynthia Williams Mukisa, Denis Amorut, Rita Muhindo, Gary Frost, Kevin Walsh, Alexander W. Macharia, Diana M. Gibb, A. Sarah Walker, Elizabeth C. George, Kathryn Maitland, Thomas N. Williams

AbstractSickle cell anemia (SCA) is common in sub‐Saharan Africa where approximately 1% of births are affected. Severe anemia is a common cause for hospital admission within the region yet few studies have investigated the contribution made by SCA. The Transfusion and Treatment of severe anemia in African Children Trial (ISRCTN84086586) investigated various treatment strategies in 3983 children admitted with severe anemia (hemoglobin < 6.0 g/dl) based on two severity strata to four hospitals in Africa (three Uganda and one Malawi). Children with known‐SCA were excluded from the uncomplicated stratum and capped at 25% in the complicated stratum. All participants were genotyped for SCA at trial completion. SCA was rare in Malawi (six patients overall), so here we focus on the participants recruited in Uganda. We present baseline characteristics by SCA status and propose an algorithm for identifying children with unknown‐SCA. Overall, 430 (12%) and 608 (17%) of the 3483 Ugandan participants had known‐ or unknown‐SCA, respectively. Children with SCA were less likely to be malaria‐positive and more likely to have an affected sibling, have gross splenomegaly, or to have received a previous blood transfusion. Most outcomes, including mortality and readmission, were better in children with either known or unknown‐SCA than non‐SCA children. A simple algorithm based on seven admission criteria detected 73% of all children with unknown‐SCA with a number needed to test to identify one new SCA case of only two. Our proposed algorithm offers an efficient and cost‐effective approach to identifying children with unknown‐SCA among all children admitted with severe anemia to African hospitals where screening is not widely available.

A probabilistic method to estimate the burden of maternal morbidity in resource-poor settings: preliminary development and evaluation

Edward Fottrell, Ulf Högberg, Carine Ronsmans, David Osrin, Kishwar Azad, Nirmala Nair, Nicolas Meda, Rasmane Ganaba, Sourou Goufodji, Peter Byass, Veronique Filippi

BackgroundMaternal morbidity is more common than maternal death, and population-based estimates of the burden of maternal morbidity could provide important indicators for monitoring trends, priority setting and evaluating the health impact of interventions. Methods based on lay reporting of obstetric events have been shown to lack specificity and there is a need for new approaches to measure the population burden of maternal morbidity. A computer-based probabilistic tool was developed to estimate the likelihood of maternal morbidity and its causes based on self-reported symptoms and pregnancy/delivery experiences. Development involved the use of training datasets of signs, symptoms and causes of morbidity from 1734 facility-based deliveries in Benin and Burkina Faso, as well as expert review. Preliminary evaluation of the method compared the burden of maternal morbidity and specific causes from the probabilistic tool with clinical classifications of 489 recently-delivered women from Benin, Bangladesh and India.ResultsUsing training datasets, it was possible to create a probabilistic tool that handled uncertainty of women’s self reports of pregnancy and delivery experiences in a unique way to estimate population-level burdens of maternal morbidity and specific causes that compared well with clinical classifications of the same data. When applied to test datasets, the method overestimated the burden of morbidity compared with clinical review, although possible conceptual and methodological reasons for this were identified.ConclusionThe probabilistic method shows promise and may offer opportunities for standardised measurement of maternal morbidity that allows for the uncertainty of women’s self-reported symptoms in retrospective interviews. However, important discrepancies with clinical classifications were observed and the method requires further development, refinement and evaluation in a range of settings.

Hajar El Kouarty, Badr Sououd Benjelloun Dakhama

Cutaneous hemangiomas are benign vascular tumors, they can be complicated by very hemorrhagic ulcers and rarely by infections. Oral treatment with beta-blockers has revolutionized the management of complicated cutaneous hemangiomas. We report a case of a 4-month-old infant with no notable history, who consulted for purulent ulceration of a cutaneous hemangioma in the upper of thorax with fever from 3 days ago. The clinical examination finds a febrile infant at 39°C, the hemangioma of 10 cm large presents a central necrosis (A). The indication of hospitalization for management was indicated, but refused by the parents. The biological assessment revealed a CRP at 124mg/l with hyperleukocytosis at 15700/mm3 predominantly PNN. We started the outpatient treatment: daily local care of the infected ulceration, oral antibiotic therapy: Amoxicillin + clavulanic acid for 10 days and analgesics (Paracetamol). Faced with the hemorrhagic appearance of the ulceration measuring 3cm large diameter (B), we started the beta-blockers, outpatient, orally, after performing a normal ECG. (1mg/kg/day then 2mg/kg/day in two doses after the 1st week then 3mg/kg/day the third week). Twice-daily monitoring of capillary blood glucose, blood pressure and heart rate was performed at each dose increase. Clinical improvement was spectacular with significant involution of hemangioma (C) after 2 months of treatment with Propranolol. This observation recalls the importance of the introduction of beta-blockers in front of complicated hemangiomas as well as the possibility of their management on an outpatient basis.

Mohammed Afif, Jihane Khalil, Fadila Kouhen, Abdellah Aissa, Youssef Omour, Mustapha Elkabous, Hanan Elkacemi, Tayeb Kebdani, Noureddine Benjaafar

Hicham Sator, Rachida Dafiri, Latifa Chat

    Soumaya Nasri, Imane Kamaoui

We report a case of a 44-year old female patient with right back pain, apyretic, without associated signs. Clinical examination showed good overall physical condition without pain in the costovertebral angle region. Lymph nodes were free. The patient underwent abdominal ultrasound and abdominal computed tomography (CT) scan which showed a lesion process measuring 09 cm along its longer axis, in the right renal lodge, roughly defined encapsulated, heterogeneous, containing calcified bulkheads. This mass was in close contact with the lower edge of the liver which was the seat of three hypodense lesions. magnetic resonance imaging (MRI) showed: spontaneously T1-hyperintense lesion (a), heterogeneous in T2 (b,c), containing few diffusion hypersignals (d). After Gadolinium injection (e) and on subtracted sequences (f) there was no fleshy component while there was peripheral contrast enhancement. The three hepatic lesions were consistent with sclerosing angiomas. Histological examination of the specimen obtained from ultrasound-guided renal biopsy showed angiomyolipoma. Angiomyolipomas are benign slow growing tumors, characterized by a proliferation of adipose tissue, vascular elements, muscle fibers in variable proportions. Abdominal CT scan is the examination of choice to make a positive diagnosis of angiomyolipoma in the vast majority of cases. Diagnosis is difficult in patients with uncommon forms, including those with no greasy component (almost pathognomonic), and with intra-tumor bleeding masking the various contingents. Differential diagnosis includes renal cancers and other benign tumors of the kidney.

Salim Lachkar, Ahmed Ibrahimi, Imad Boualaoui, Hachem El Sayegh, Yassine Nouini

A psychosocial intervention to reduce gender-based violence and antepartum depressive symptoms in pregnant women in Kisumu County, Kenya: a quasi-experimental study

Redempta Kalekye Mutisya, Kenneth Ngure, Christina Mwachari

IntroductionGender-based violence during pregnancy and its associated adverse health effects are disproportionately higher in developing countries like Kenya where screening for and management of gender-based violence is currently not part of routine antenatal care. This study assessed the effect of a psychosocial intervention on gender-based violence and antepartum depressive symptoms in pregnant women.MethodsThis quasi-experimental study compared gender-based violence and antepartum depression scores of 288 pregnant women in the two arms; one exposed to a psychosocial intervention and another receiving usual antenatal care. We used analysis of covariance to estimate the intervention effect and Chi-square to test the equality of proportions.ResultsThe difference between the psychosocial intervention and the usual antenatal care group in the total intimate partner violence and physical violence scores was a significant, with small effect sizes of partial eta = 0.196 and 0.305 respectively. The two arms did not differ in terms of the proportion of women reporting other acts of gender-based violence by intimate and non-intimate partners post-intervention. The intervention group had significantly lower mean depression scores compared to the usual care group, post-intervention, with a medium effect size of 0.500.ConclusionThis intervention aimed at reduction of gender-based violence and improvement of mental health of pregnant women is promising. Primary health care facilities in resource-constrained settings can take advantage of local capacity to deliver focused non-specialized psychosocial support to pregnant women experiencing violence.

A qualitative case study of evaluation use in the context of a collaborative program evaluation strategy in Burkina Faso

Léna D’Ostie-Racine, Christian Dagenais, Valéry Ridde

BackgroundProgram evaluation is widely recognized in the international humanitarian sector as a means to make interventions and policies more evidence based, equitable, and accountable. Yet, little is known about the way humanitarian non-governmental organizations (NGOs) actually use evaluations.MethodsThe current qualitative evaluation employed an instrumental case study design to examine evaluation use (EU) by a humanitarian NGO based in Burkina Faso. This organization developed an evaluation strategy in 2008 to document the implementation and effects of its maternal and child healthcare user fee exemption program. Program evaluations have been undertaken ever since, and the present study examined the discourses of evaluation partners in 2009 (n = 15) and 2011 (n = 17). Semi-structured individual interviews and one group interview were conducted to identify instances of EU over time. Alkin and Taut’s (Stud Educ Eval 29:1–12, 2003) conceptualization of EU was used as the basis for thematic qualitative analyses of the different forms of EU identified by stakeholders of the exemption program in the two data collection periods.ResultsResults demonstrated that stakeholders began to understand and value the utility of program evaluations once they were exposed to evaluation findings and then progressively used evaluations over time. EU was manifested in a variety of ways, including instrumental and conceptual use of evaluation processes and findings, as well as the persuasive use of findings. Such EU supported planning, decision-making, program practices, evaluation capacity, and advocacy.ConclusionsThe study sheds light on the many ways evaluations can be used by different actors in the humanitarian sector. Conceptualizations of EU are also critically discussed.
