Correlates of HIV seropositivity in young West and Central African women: A pooled analysis of 17 Demographic and Health Surveys

Christian Bommer, Sebastian Vollmer, Noël Marie Zagre

BackgroundYoung women in West and Central Africa have been described by the United Nations as being especially vulnerable to HIV/AIDS. Despite a consensus that increased efforts are necessary to address the needs of this particular demographic, correlates of HIV seropositivity in young West and Central African women have not been systematically described. This study fills this gap using a rich set of publicly available survey data.MethodsFor this cross-sectional study, we combined HIV test results for young women (age 15-24 years) with information on demographic, cultural and socioeconomic correlates from 17 recent Demographic and Health Surveys (DHS) to estimate odds ratios (OR) from fixed effects logistic regression models accounting for potential individual, household-level and contextual risk factors of HIV seropositivity.ResultsThe prevalence of HIV seropositivity among young women is higher than for men of the same age in all included surveys, except for the Burkina Faso DHS. An important correlate of HIV seropositivity in young women is early sexual activity (OR = 1.510; 95% confidence interval (CI) = 1.100, 2.072), while higher education is associated with reduced odds of being HIV positive (OR = 0.215; 95% CI = 0.057, 0.820). No significant correlation has been found for individual HIV awareness, but HIV stigma is negatively associated with HIV seropositivity (OR = 0.495; 95% CI = 0.247, 0.990, in the fully adjusted model).ConclusionsThe results demonstrate the need to design effective policies addressing behavioral risks in young women. In particular, increasing HIV awareness alone is likely to be insufficient. Instead, information campaigns need to focus on transforming awareness into behavioral change. Moreover, fostering formal education may be an effective tool in the fight against HIV/AIDS.

Correlates of whole-blood polyunsaturated fatty acids among young children with moderate acute malnutrition

    C. W. Yaméogo, B. Cichon, C. Fabiansen, M. J. H. Rytter, D. Faurholt-Jepsen, K. D. Stark, A. Briend, S. Shepherd, A. S. Traoré, V. B. Christensen, K. F. Michaelsen, H. Friis, L. Lauritzen

BackgroundSevere acute malnutrition (SAM) has been associated with low polyunsaturated fatty acid (PUFA) status. However, investigations regarding PUFA status and correlates in children with moderate acute malnutrition (MAM) from low-income countries are scarce. The aim of this study was to describe whole-blood PUFA levels in children with moderate acute malnutrition (MAM) and to identify correlates of PUFAs.MethodsWe conducted a cross-sectional study using baseline data from a prospective nutritional intervention trial among 1609 children with MAM aged 6–23 months in Burkina Faso,West Africa. Whole-blood PUFAs were measured by gas chromatography and expressed as percent of total whole-blood fatty acids (FA%). Potential correlates of PUFAs including infection, inflammation, hemoglobin, anthropometry (difference between children diagnosed as having MAM based on low mid-upper-arm-circumference (MUAC) only, low MUAC and weight-for-height z-score (WHZ), or low WHZ only) and diet were assessed by linear regression adjusted for age and sex.ResultsChildren with MAM had low concentrations of whole-blood PUFAs, particularly n-3 PUFAs. Moreover, children diagnosed with MAM based only on low MUAC had 0.32 (95% confidence interval (CI), 0.14; 0.50) and 0.40 (95% CI, 0.16; 0.63) FA% lower arachidonic acid (AA) than those recruited based on both low WHZ as well as low MUAC and those recruited with low WHZ only, respectively. Infection and inflammation were associated with low levels of all long-chain (LC)-PUFAs, while hemoglobin was positively associated with whole-blood LC-PUFAs.ConclusionWhile PUFA deficiency was not a general problem, overall whole-blood PUFA concentrations, especially of n-3 PUFAs, were low. Infection, inflammation, hemoglobin, anthropometry and diet were correlates of PUFAs concentrations in children with MAM.Trial registrationThe trial is registered at http://www.isrctn.com (ISRCTN42569496).

Correlating the morphology of Anacardium occidentale L. fruits from 30 orchards with their physicochemical and nutritional properties

Roger Dakuyo, Kiessoun Konaté, David Bazié, Abdoudramane Sanou, Kabakdé Kaboré, Hemayoro Sama, Balmoussa Santara, Frédéric Anderson Konkobo, Mamoudou Hama Dicko

Cashew (Anacardium occidentale L.) is a cross-pollinating plant whose fruit consists of two parts, the nut, and the apple. This study aimed to carry out agro-morphological characteristics of cashew fruits to establish relationships with their physicochemical and nutritional properties. Thirty (30) cashew accessions fruits were sampled in different regions of Burkina Faso. The length, width, thickness, weight, yield, kernel output ratio (KOR), proximate composition, specific minerals, phenolic compounds, and anti-nutritional factors were assessed. Evaluations were made using standard methods. The results showed significant variations in agro-morphological, physicochemical and nutritional traits depending on the accessions and the organ. Also, the ratio of apple mass versus nut one was about 12.24 ± 1.24. Kernels are an important source of proteins, fat, total phenolic compounds, and flavonoids, with average contents of 22.84 ± 1.25 g/100 g, 51.65 ± 2.54 g/100 g, 9.78 ± 2.15 mg GAE/g, and 6.24 ± 12.15 mg QE/g, respectively. As for the apples, they contained substantial quantities of Potassium, Phosphorus, Chlorine, and Magnesium with contents of 611.24 ± 14.5 mg/100 g, 418.24 ± 16.47 mg/100 g, 332.24 ± 10.54 mg/100 g, and 224.95 ± 13.15 mg/100 g, respectively. Statistical analyses showed that mineral and phytate contents were positively correlated to cashew apples while phenolic compounds and tannins were strongly correlated with walnuts. Principal component analyses showed three groups of accessions based on apples and kernels characteristics. These data showed a direct relationship between cashew physicochemical, nutritional potentials, kernels, and apples’ agro-morphological characteristics. These data constitute an important basis for the identification of cashew accessions with high nutritional and economic potential.

Correlation Between Soluble Endothelial Adhesion Molecules and Nitric Oxide Metabolites in Sickle Cell Disease

Charles Antwi-Boasiako, John Ahenkorah, Eric S. Donkor, Bartholomew Dzudzor, Gifty B. Dankwah, Kate H. Otu, Robert Aryee, Charles Hayfron-Benjamin, Andrew D. Campbell

Nitric Oxide (NO) and soluble adhesion molecules are promising biomarkers, which predict endothelial dysfunction in sickle cell disease (SCD). Several studies have investigated the relationship between NO (as well as its metabolites) and endothelial adhesion molecules in SCD. However, these studies were done mainly in the developed world, and it is difficult to extrapolate the findings to SCD populations in other geographical regions such as Africa due to significant disparities in the results. The aim of the current study was to determine the correlation between levels of nitric oxide metabolites (NOx) and adhesion molecules in SCD patients in a tertiary hospital in Ghana. A case control cross-sectional study involving 100 SCD (made up of HbSS and HbSC patients) and 60 healthy controls was conducted. Concentrations of NOx and soluble endothelial adhesion molecules (ICAM-1, VCAM-1 and E-selectin) were measured in all the study participants (n = 160) by the Griess reagent system and enzyme-linked immunosorbent assay (ELISA). Correlation analysis was performed to determine a possible link between the variables. Levels of soluble adhesion molecules were higher in the HbSS patients. Correlation of NOx with ICAM-1 almost approached significance (r = 0.565, p = 0.058) in the HbSS patients. There were no correlations between NOx and E-selectin in both HbSS and HbSC patients. There were no significant correlations between NOx and VCAM-1 in all the study participants (p > 0.05). Of the soluble adhesion molecules, ICAM-1 showed a significant positive correlation with VCAM-1 in the HbSC patients. There were no significant differences between the adhesion molecules and the age of participants in the various study groups. Whether or not a significant correlation exists between NOx and soluble adhesion molecules may not depend on the sickle cell genotype. The expression of adhesion molecules may not depend on age.

Najib Zouhair, Sanaa Mallouk, Youssef Oukessou, Sami Rouadi, Redallah Larbi Abada, Mohamed Roubal, Mohamed Mahtar

Ahmed Guennoun, Yousra Krimou, Chahrazed Bouchikhi, Nisrine Mamouni, Sanaa Errarhay, Abdelaziz Banani

Mahmoud Amine Laffinti, Jalal El Ouadoudi, Hicham Guennouni Hassani, Rachid Najib, Abdeslam Benali

Cost analysis of a nationwide typhoid conjugate vaccine campaign in Burkina Faso

BackgroundIn Burkina Faso, typhoid fever remains a major public health concern, particularly among children under 15. In January 2025, a nationwide campaign introduced the typhoid conjugate vaccine targeting children aged 9 months to 14 years. This study aimed to estimate the cost of typhoid conjugate vaccine delivery during the national campaign and to identify the main cost drivers across different administrative levels.MethodsWe conducted a retrospective cross-sectional costing study using a microcosting approach from the perspective of the Ministry of Health. We collected data from fifty health facilities, eight health districts, five health regions, and the national level. Financial and economic costs were estimated for each level, excluding vaccine and syringe costs. All costs were converted to 2024 USD using the official exchange rate. Parameter uncertainty was explored through one-way sensitivity analysis on key assumptions.FindingsVaccinators administered a total of 10.5 million typhoid conjugate vaccine doses. The average financial cost per dose was $0.47 (95% CI: $0.39–$0.51), and the economic cost was $2.16 (95% CI: $1.71–$2.56). Human resources and per diem payments were the main contributors to costs. Costs varied by geography, delivery strategy, and security context, with higher costs observed in rural and conflict-affected areas. The mobile–temporary posts strategy had the highest economic cost per dose ($2.02; 95% CI: $1.64–$2.40), while the fixed strategy had the highest financial cost per dose ($0.41; 95% CI: ($0.32–$0.49). Sensitivity analyses indicated robust cost estimates, with volunteer time valuation having the greatest influence.ConclusionThe financial cost per dose remained within Gavi’s operational support range. The observed cost variations highlight the need for targeted funding and enhanced logistical support to ensure equitable access, particularly in rural and insecure areas. This study provides evidence to inform future vaccination

Cost analysis of an integrated disease surveillance and response system: case of Burkina Faso, Eritrea, and Mali

Zana C Somda, Martin I Meltzer, Helen N Perry, Nancy E Messonnier, Usman Abdulmumini, Goitom Mebrahtu, Massambou Sacko, Kandioura Touré, Salimata Ouédraogo Ki, Tuoyo Okorosobo, Wondimagegnehu Alemu, Idrissa Sow

BackgroundCommunicable diseases are the leading causes of illness, deaths, and disability in sub-Saharan Africa. To address these threats, countries within the World Health Organization (WHO) African region adopted a regional strategy called Integrated Disease Surveillance and Response (IDSR). This strategy calls for streamlining resources, tools, and approaches to better detect and respond to the region's priority communicable disease. The purpose of this study was to analyze the incremental costs of establishing and subsequently operating activities for detection and response to the priority diseases under the IDSR.MethodsWe collected cost data for IDSR activities at central, regional, district, and primary health care center levels from Burkina Faso, Eritrea, and Mali, countries where IDSR is being fully implemented. These cost data included personnel, transportation items, office consumable goods, media campaigns, laboratory and response materials and supplies, and annual depreciation of buildings, equipment, and vehicles.ResultsOver the period studied (2002–2005), the average cost to implement the IDSR program in Eritrea was $0.16 per capita, $0.04 in Burkina Faso and $0.02 in Mali. In each country, the mean annual cost of IDSR was dependent on the health structure level, ranging from $35,899 to $69,920 at the region level, $10,790 to $13,941 at the district level, and $1,181 to $1,240 at the primary health care center level. The proportions spent on each IDSR activity varied due to demand for special items (e.g., equipment, supplies, drugs and vaccines), service availability, distance, and the epidemiological profile of the country.ConclusionThis study demonstrates that the IDSR strategy can be considered a low cost public health system although the benefits have yet to be quantified. These data can also be used in future studies of the cost-effectiveness of IDSR.

Cost and impact of policies to remove and reduce fees for obstetric care in Benin, Burkina Faso, Mali and Morocco

    S. Witter, C. Boukhalfa, J. A. Cresswell, Z. Daou, V. Filippi, R. Ganaba, S. Goufodji, I. L. Lange, B. Marchal, F. Richard

BackgroundAcross the Africa region and beyond, the last decade has seen many countries introducing policies aimed at reducing financial barriers to obstetric care. This article provides evidence of the cost and effects of national policies focussed on improving financial access to caesarean and facility deliveries in Benin, Burkina Faso, Mali and Morocco.MethodsThe study uses a comparative case study design with mixed methods, including realist evaluation components. This article presents results across 14 different data collection tools, used in 4–6 research sites in each of the four study countries over 2011-13. The methods included: document review; interviews with key informants; analysis of secondary data; structured extraction from medical files; cross-sectional surveys of patients and staff; interviews with patients and observation of care processes.ResultsThe article finds that the policies have contributed to continued increases in skilled birth attendance and caesarean sections and a narrowing of inequalities in all four countries, but these trends were already occurring so a shift cannot be attributed solely to the policies. It finds a significant reduction in financial burdens on households after the policy, suggesting that the financial protection objectives may have been met, at least in the short term, although none achieved total exemption of targeted costs. Policies are domestically financed and are potentially sustainable and efficient, and were relatively thoroughly implemented. Further, we find no evidence of negative effects on technical quality of care, or of unintended negative effects on untargeted services.ConclusionsWe conclude that the policies were effective in meeting financial protection goals and probably health and equity goals, at sustainable cost, but that a range of measures could increase their effectiveness and equity. These include broadening the exempted package (especially for those countries which focused on caesarean section

Cost effectiveness of fortified bouillon in addressing Burkinabe children's vitamin A inadequacy: An economic optimization model

Armando R. Colina, Stephen A. Vosti, Michael Jarvis, Reina Engle‐Stone, Aleksandr Michuda, Karen Ortiz‐Becerra, Katherine P. Adams

AbstractVitamin A dietary inadequacy remains a serious public health problem among young children 6–59 months of age in Burkina Faso. Planners face several interrelated challenges: Selecting concrete policy objectives regarding vitamin A inadequacy reductions, identifying cost‐effective vitamin A intervention programs that can achieve those objectives, and being reasonably sure that proposed intervention programs are robust to uncertainty in program benefits and costs. A 10‐year, subnational economic optimization model making use of secondary dietary intake data and program cost data was developed and implemented to address these issues and included the following vitamin A program options: existing or improved edible oils fortification, a pair of hypothetical vitamin A‐fortified bouillon programs, and a set of subnational vitamin A supplementation (VAS) programs. The model consistently identified the improved edible oils and bouillon fortification programs as the core national programs upon which the more expensive subnational VAS programs could be layered, depending on policy objectives and available funding. These results were robust to uncertainty in program nutritional benefits and costs. However, even if the most impactful set of modeled programs was implemented, vitamin A inadequacy among children would remain a serious public health problem; hence, additional efforts to address it would be needed. Meeting young children's vitamin A needs will require multiple programs in Burkina Faso. Economic optimization modeling suggests that large‐scale food fortification programs, especially the improved edible oils program and a hypothetical fortified bouillon cube program delivering 30% of Codex Nutrient Reference Values for adults in 2.5 g/day, form the programmatic base upon which macro‐regional vitamin A supplementation programs can be cost‐effectively layered. However, even if the most impactful programs were implemented, ∼50% of young children would still suffer v

Cost of introducing and delivering malaria vaccine (RTS, S/AS01E) in areas of seasonal malaria transmission, Mali and Burkina Faso

Halimatou Diawara, Fadima Yaya Bocoum, Alassane Dicko, Ann Levin, Cynthia Lee, Fatoumata Koita, Jean Bosco Ouédraogo, Rosemonde Guissou, Seydou Yabré, Seydou Traoré, Winthrop Morgan, Clint Pecenka, Ranju Baral

BackgroundThe WHO recommends use of the RTS, S/AS01E (RTS, S) malaria vaccine for young children living in areas of moderate to high Plasmodium falciparum malaria transmission and suggests countries consider seasonal vaccination in areas with highly seasonal malaria. Seasonal vaccination is uncommon and may require adaptations with potential cost consequences. This study prospectively estimates cost of seasonal malaria vaccine delivery in Mali and Burkina Faso.MethodsThree scenarios for seasonal vaccine delivery are costed (1) mass campaign only, (2) routine Expanded Programme on Immunisation (EPI) and (3) mixed delivery (mass campaign and routine EPI)), from the government’s perspective. Resource use data are informed by previous new vaccine introductions, supplemented with primary data from a sample of health facilities and administrative units.FindingsAt an assumed vaccine price of US $5 per dose, the economic cost per dose administered ranges between $7.73 and $8.68 (mass campaign), $7.04 and $7.38 (routine EPI) and $7.26 and $7.93 (mixed delivery). Excluding commodities, the cost ranges between $1.17 and $2.12 (mass campaign), $0.48 and $0.82 (routine EPI) and $0.70 and $1.37 (mixed delivery). The financial non-commodity cost per dose administered ranges between $0.99 and $1.99 (mass campaign), $0.39 and $0.76 (routine EPI) and $0.58 and $1.28 (mixed delivery). Excluding commodity costs, service delivery is the main cost driver under the mass campaign scenario, accounting for 36% to 55% of the financial cost. Service delivery accounts for 2%–8% and 12%–23% of the total financial cost under routine EPI and mixed delivery scenarios, respectively.ConclusionVaccine delivery using the mass campaign approach is most costly followed by mixed delivery and routine EPI delivery approaches, in both countries. Our cost estimates provide useful insights for decisions regarding delivery approaches, as countries plan the malaria vaccine rollout.
