    Mohamed Amine Ennouhi, Abdenacer Moussaoui

Meriam Hajji, Amel Harzallah, Hayet Kaaroud, Mona Jerbi, Soumaya Chargui, Fethi El Younsi, Fethi Ben Hamida, Taieb Ben abdallah

Equipping Persons with Sickle Cell Disease, Transforming Care: A Protocol Paper on Feasibility and Acceptability of Self-Management Package in the Tribal Communities of Southern India

Manashri Bhuyar, Tanya Seshadri, Pooja Aggarwal, Anandhu KR, Deepa Bhat

IntroductionSickle cell disease affects tribal communities unequally. Self-management empowers the patient to take corrective actions, improve decision-making, and deepen understanding of the disease. Considering the unique challenges of the tribal community, implementing self-management complementary to the prescribed clinical management for sickle cell disease care can lead to greater autonomy, improved health outcomes, and, ultimately, quality of life. Hence, this study aims to develop a feasible and acceptable self-management package for tribal persons living with Sickle cell disease and caregivers residing in the Mysuru and Chamarajanagar districts of Karnataka.MethodThe study will be conducted in two phases. In the first phase, a self-management package will be developed with insights from health providers working with Persons living with sickle cell disease, non-government organisations, Subject experts and persons with sickle cell disease and caregiver. This package will be implemented in the second phase for persons living with sickle cell disease. Participants will undergo capacity-building training and receive regular follow-up for 12 months through monthly telephonic calls and a few home visits. Assessment will be done with Acceptability Intervention Measurement (AIM) and Feasibility Intervention Measurement (FIM) at regular intervals. Suggestions and feedback will be added to make the package more comprehensive. Thus, the expected outcome is an acceptable and feasible self-management package.ConclusionThis proposed study is an important step towards addressing a critical gap in sickle cell disease management in tribal populations in India. The anticipated outcome of this research is to enhance self-management practices in individuals with sickle cell disease. Plain Language SummaryA genetic condition known as Sickle Cell Disease (SCD) is a genetic condition that can lead to complications such as intense pain, fatigue, and sometimes death. In India, dise

Equity and health policy in Africa: Using concept mapping in Moore (Burkina Faso)

    Valéry Ridde

BackgroundThis methodological article is based on a health policy research project conducted in Burkina Faso (West Africa). Concept mapping (CM) was used as a research method to understand the local views of equity among stakeholders, who were concerned by the health policy under consideration. While this technique has been used in North America and elsewhere, to our knowledge it has not yet been applied in Africa in any vernacular language. Its application raises many issues and certain methodological limitations. Our objective in this article is to present its use in this particular context, and to share a number of methodological observations on the subject.MethodsTwo CMs were done among two different groups of local stakeholders following four steps: generating ideas, structuring the ideas, computing maps using multidimensional scaling and cluster analysis methods, and interpreting maps. Fifteen nurses were invited to take part in the study, all of whom had undergone training on health policies. Of these, nine nurses (60%) ultimately attended the two-day meeting, conducted in French. Of 45 members of village health committees who attended training on health policies, only eight were literate in the local language (Moore). Seven of these (88%) came to the meeting.ResultsThe local perception of equity seems close to the egalitarian model. The actors are not ready to compromise social stability and peace for the benefit of the worst-off. The discussion on the methodological limitations of CM raises the limitations of asking a single question in Moore and the challenge of translating a concept as complex as equity. While the translation of equity into Moore undoubtedly oriented the discussions toward social relations, we believe that, in the context of this study, the open-ended question concerning social justice has a threefold relevance. At the same time, those limitations were transformed into strengths. We understand that it was essential to resort to the focus

Equity monitoring for social marketing: use of wealth quintiles and the concentration index for decision making in HIV prevention, family planning, and malaria programs

Nirali M Chakraborty, Rebecca Firestone, Nicole Bellows

BackgroundThe majority of social marketing programs are intended to reach the poor. It is therefore essential that social marketing organizations monitor the health equity of their programs and improve targeting when the poor are not being reached. Current measurement approaches are often insufficient for decision making because they fail to show a program's ability to reach the poor and demonstrate progress over time. Further, effective program equity metrics should be benchmarked against a national reference population and consider exposure, not just health outcomes, to measure direct results of implementation. This study compares two measures of health equity, concentration indices and wealth quintiles, using a defined reference population, and considers benefits of both measures together to inform programmatic decision making.MethodsThree datasets from recent cross-sectional behavioral surveys on malaria, HIV, and family planning from Nepal and Burkina Faso were used to calculate concentration indices and wealth quintiles. Each sample was standardized to national wealth distributions based on recent Demographic and Health Surveys. Wealth quintiles were generated and concentration indices calculated for health outcomes and program exposure in each sample. Chi-square and t-tests were used to assess statistical significance of results.ResultsReporting wealth quintiles showed that recipients of Population Services International (PSI) interventions were wealthier than national populations. Both measures indicated that desirable health outcomes were usually concentrated among wealthier populations. Positive and significant concentration indices in all three surveys indicated that wealth and program exposure were correlated; however this relationship was not necessarily linear. In analyzing the equity of modern contraceptive use stratified by exposure to family planning messages in Nepal, the outcome was equitable (concentration index = 0.006, p = 0.68) among the expos

Equivalent susceptibility of Anopheles gambiae M and S molecular forms and Anopheles arabiensis to Plasmodium falciparum infection in Burkina Faso

Awa Gnémé, Wamdaogo M Guelbéogo, Michelle M Riehle, Antoine Sanou, Alphonse Traoré, Soumanaba Zongo, Karin Eiglmeier, Gustave B Kabré, N’Falé Sagnon, Kenneth D Vernick

BackgroundThe Anopheles gambiae sensu lato (s.l.) species complex in Burkina Faso consists of Anopheles arabiensis, and molecular forms M and S of Anopheles gambiae sensu stricto (s.s.). Previous studies comparing the M and S forms for level of infection with Plasmodium falciparum have yielded conflicting results.MethodsMosquito larvae were sampled from natural pools, reared to adulthood under controlled conditions, and challenged with natural P. falciparum by experimental feeding with blood from gametocyte carriers. Oocyst infection prevalence and intensity was determined one week after infection. DNA from carcasses was genotyped to identify species and molecular form.ResultsIn total, 7,400 adult mosquitoes grown from wild-caught larvae were challenged with gametocytes in 29 experimental infections spanning four transmission seasons. The overall infection prevalence averaged 40.7% for A. gambiae M form, 41.4% for A. gambiae S form, and 40.1% for A. arabiensis. There was no significant difference in infection prevalence or intensity between the three population groups. Notably, infection experiments in which the population groups were challenged in parallel on the same infective blood displayed less infection difference between population groups, while infections with less balanced composition of population groups had lower statistical power and displayed apparent differences that fluctuated more often from the null average.ConclusionThe study clearly establishes that, at the study site in Burkina Faso, there is no difference in genetic susceptibility to P. falciparum infection between three sympatric population groups of the A. gambiae s.l. complex. Feeding the mosquito groups on the same infective blood meal greatly increases statistical power. Conversely, comparison of the different mosquito groups between, rather than within, infections yields larger apparent difference between mosquito groups, resulting from lower statistical power and greater noise, and could

Asmae Lahlou, Salim Gallouj, Fatima-Zohra Mernissi

The bléomycine is an antitumor antibiotic. Erythrodermie desquamative

Majda Askour, Christelle Ebongo, Hafsae Bounniyt, Karima Senouci, Badredine Hassam

Aziza El Ouali, Yousra El Boussaadni, Fatima Ailal, Ahmed Aziz Bousfiha, Sihame Dikhaye, Noufissa Benajiba
