Saleh Muhemedi, Yakim Kabangu, Faustin Mpeli, Salomon Salumu, Philippe Kabeya, Emile Okitolonda

Hedfi Mohamed, Bouhafa Hala, Elcadhi Youssef, Cherif Abdelhedi, Sassi Karim, Sridi Azza, Chouchene Adnen

Lala Andriamasinavalona Rajaonarison, Nomena Finiavana Rasaholiarison, Naliniaina Robert Randrianantoandro, Manitra Niaina Rabeony, Julien Razafimahefa, Noël Zodaly, Alain Djacoba Tehindrazanarivelo

L´acce

Evolution of African cassava mosaic virus by recombination between bipartite and monopartite begomoviruses

Fidèle Tiendrébéogo, Pierre Lefeuvre, Murielle Hoareau, Mireille A Harimalala, Alexandre De Bruyn, Julie Villemot, Valentin SE Traoré, Gnissa Konaté, Alfred S Traoré, Nicolas Barro, Bernard Reynaud, Oumar Traoré, Jean-Michel Lett

BackgroundCassava mosaic disease (CMD) is a major constraint on cassava cultivation in Africa. The disease is endemic and is caused by seven distinct cassava mosaic geminiviruses (CMGs), some of them including several variants.FindingsFrom cassava leaf samples presenting CMD symptoms collected in Burkina Faso, four DNA-A begomovirus components were cloned and sequenced, showing 99.9% nucleotide identity among them. These isolates are most closely related to African cassava mosaic virus (ACMV) but share less than 89% nucleotide identity (taxonomic threshold) with any previously described begomovirus. A DNA-B genomic component, sharing 93% nucleotide identity with DNA-B of ACMV, was also characterized. Since all genomic components have a typical genome organization of Old World bipartite begomoviruses, this new species was provisionally named African cassava mosaic Burkina Faso virus (ACMBFV). Recombination analysis of the new virus demonstrated an interspecies recombinant origin, with major parents related to West African isolates of ACMV, and minor parents related to Tomato leaf curl Cameroon virus and Cotton leaf curl Gezira virus.ConclusionThis is the first report of an ACMV-like recombinant begomovirus arisen by interspecific recombination between bipartite and monopartite African begomoviruses.

Evolution of meningococcal carriage in serogroups X and Y before introduction of MenAfriVac in the health district of Kaya, Burkina Faso

Absatou Ky Ba, Idrissa Sanou, Paul A Kristiansen, Lassana Sangaré, Rasmata Ouédraogo, Kalifa Ouattara, Maxime Kienou, Simon Tiendrebeogo, Juliette Tranchot

BackgroundThe objective of this study was to evaluate the carriage of Neisseria meningitidis (Nm) serogroups X and Y in the health district of Kaya before the introduction of a serogroup A meningococcal conjugate vaccine in Burkina Faso.MethodsA repeated cross-sectional meningococcal carriage study was conducted in 2009 in eight randomly selected villages in the health district of Kaya, Burkina Faso. In each of 4 sampling rounds at least 1,500 people were enrolled within a 1-month period.ResultsFrom a total of 6,686 throat swabs we identified 419 Nm isolates (6.27%). The dominating serogroups were Y (3.19%) and X (1.05%). Overall carriage was higher in the dry season compared with the rainy season (OR, 1.51; 95% CI, 1.06–2.16). Carriage prevalence of serogroups Y and X varied by round and was highest at the end of the dry season (4.92% and 1.22%, respectively). The only risk factor associated with NmX carriage was vaccination status in contrast to serogroup Y, which was associated with age groups 5–9 years and 10–14 years.ConclusionThe presence of Nm serogroups X and Y, which could replace or be added to the serogroup A, is a warning sign. There is a need to strengthen surveillance and laboratory diagnosis of the various meningococcal serogroups circulating in Africa.

Evolution of Pfdhps and Pfdhfr mutations before and after adopting seasonal malaria chemoprevention in Nanoro, Burkina Faso

Francis Emmanuel Towanou Bohissou, Paul Sondo, Juliana Inoue, Toussaint Rouamba, Berenger Kaboré, Guétawendé Job Wilfried Nassa, A. Elisée Sié Kambou, Tiampan Edwig Traoré, Victor Asua, Steffen Borrmann, Halidou Tinto, Jana Held

Seasonal Malaria Chemoprevention consisting of monthly administration of amodiaquine/sulfadoxine-pyrimethamine to children aged 3–59 months during the transmission season could promote SP-resistance. Mutations in dihydrofolate reductase (Pfdhfr) and dihydropteroate synthase (Pfdhps) genes were assessed before and after SMC adoption in Burkina Faso. A total of 769 dried blood spots were selected from studies conducted in Nanoro, Burkina Faso, between 2010 and 2020. Of those, 299 were pre-SMC (2010–2012) and 470 were post-SMC-samples. Pfdhps and Pfdhfr genes were PCR-amplified and sequenced. A systematic review/meta-analysis of published studies conducted in Burkina Faso (2009–2023) was additionally performed. In Nanoro, the prevalence of Pfdhfr triple mutations (CIRNI) rose from 43.6% pre-SMC to 89.4% post-SMC (p < 0.0001). There was no mutation in Pfdhfr 164 and Pfdhps 540; Pfdhps A437G mutation increased from 63.9% (2010–2012) to 84.7% (2020) (p < 0.0001). The VAGKGS haplotype was 2.8% (2020). Pfdhfr/Pfdhps quintuple mutant IRN-436A437G rose from 18.6% (2010–2012) to 58.3% (2020) (p < 0.0001). Meta-analysis results from Burkina Faso showed an increase in mutations at Pfdhfr N51I, C59R, S108N, and Pfdhps A437G after SMC adoption. Post-SMC, the pyrimethamine-resistance marker prevalence increased, while the sulfadoxine-resistance marker prevalence remained stable. Detection of emerging PfdhpsVAGKGS haplotypes in 2020 underscores the importance of continuous SP-resistance monitoring.Supplementary InformationThe online version contains supplementary material available at 10.1038/s41598-024-75369-2.

Evolution of the nutritional status and factors associated with undernutrition in children under five of age between 2014 and 2017 in 24 health districts of Burkina Faso

The prevalences of undernutrition in children under five years of age appeared to decrease over the last decade in Burkina Faso. However, the country is now facing new health and security challenges that could threaten this progress. Therefore, it is essential to monitor the evolution of the situation within this specific context. We aimed to assess trends in undernutrition among children under 5 years of age, as well as the factors associated with it, between 2014 and 2017 in Burkina Faso. A study based on secondary analysis of the data from 2013 and 2017 surveys, conducted in 24 districts across six health regions, was carried out. We worked with the household databases to select two sub-samples: 9,259 children from 2014 and 12,119 children from 2017. We assessed anthropometric indicators using World Health Organization standards, analyzed their proportions between 2017 and 2014, and according to the health regions and children’s age groups. We examined the association of stunting and underweight with children, mothers, and households’ characteristics in 2017 and for two selected health regions, using logistic regression. From 2014 to 2017, the proportion of severe stunting and severe underweight decreased respectively from 24.8% to 7.9% (p < 0.001) and from 19.6% to 8.8% (p < 0.001) certainly due to nutrition and health initiatives. As in 2014, stunting was associated with sex, living in poorest household with AdjOR = 1.31 (95%CI: 1.14 -1.50), or in household having three and more children under five years with AdjOR = 1.28 (95%CI: 1.12 -1.47). This study demonstrated a period of significant progress in undernutrition in children. But it identified the persistence of associated factors contributing to the high prevalence of undernutrition in Burkina Faso, where insecurity has become a prevailing issue. Further studies are needed to assess the sustainability of encouraging progress in children’s nutritional status.

Younes Barbach, Hanane Baybay, Samia Mrabat, Mohammed Chaouche, Sara Elloudi, Fatima Zahra Mernissi

Evolutionary history and transmission dynamics of dengue virus type 2 in Africa

Muhammad Bashir Bello, Safia S. Aljedani, Nafi’u Lawal, Abdulrahman Alasiri, Mustapha Umar Imam

IntroductionDengue virus serotype 2 (DENV-2) is a major driver of severe dengue outbreaks globally, yet its evolutionary history and transmission dynamics across Africa remain incompletely understood.MethodsTo clarify its origins, introduction pathways, and patterns of regional spread, we compiled all available full-length DENV-2 genomes up to 16 July 2025, prioritizing African sequences while retaining representative global references. After quality filtering and down-sampling, a final dataset of 414 genomes was analyzed. Sequences were screened for recombination using RDP4 and analyzed with maximum-likelihood phylogenetics in IQ-TREE with 1,000 ultrafast bootstrap replicates. Temporal dynamics were inferred using time-scaled phylogenies reconstructed in BEAST under a strict molecular clock and exponential-growth coalescent prior, while viral dispersal was examined through discrete-trait phylogeographic analyses with Bayesian stochastic search variable selection. In parallel, single-nucleotide polymorphisms in the prM, E, and NS1 genes were characterized, and significant sites were mapped onto corresponding protein structures. Results and DiscussionBetween 1964 and 2024, African DENV-2 diversity was shaped primarily by repeated introductions of the Cosmopolitan genotype, giving rise to geographically structured lineages. Lineage IIB, likely originating from La Réunion in the late 1970s, subsequently disseminated across West, Central, and East Africa, with Burkina Faso emerging as a key regional hub. Lineage IIA, also linked to La Réunion, diverged into two sub-lineages that expanded through East and Central Africa and extended into South Asia. In contrast, lineages IID and IIF were detected only sporadically, with limited geographic representation and no evidence of sustained regional expansion. Despite substantial genetic variation across prM, E, and NS1, relatively few significant non-synonymous mutations were identified, and critical N-glycosylation motifs remain

Evolutionary history of sickle-cell mutation: implications for global genetic medicine

    Kevin Esoh, Ambroise Wonkam

AbstractResistance afforded by the sickle-cell trait against severe malaria has led to high frequencies of the sickle-cell mutation [HBB; c.20T>A, p.Glu6Val; OMIM: 141900 (HBB-βS)] in most parts of Africa. High-coverage sequencing and genotype data have now confirmed the single African origin of the sickle-cell gene variant [HBB; c.20T>A, p.Glu6Val; OMIM: 141900 (HBB-βS)]. Nevertheless, the classical HBB-like genes cluster haplotypes remain a rich source of HBB-βS evolutionary information. The overlapping distribution of HBB-βS and other disease-associated variants means that their evolutionary genetics must be investigated concurrently. In this review: (1) we explore the evolutionary history of HBB-βS and its implications in understanding human migration within and out of Africa: e.g. HBB haplotypes and recent migration paths of the Bantu expansion, occurrence of ~7% of the Senegal haplotype in Angola reflecting changes in population/SCD dynamics, and existence of all five classical HBB haplotype in Cameroon and Egypt suggesting a much longer presence of HBB-βS in these regions; (2) we discuss the time estimates of the emergence of HBB-βS in Africa and finally, (3) we discuss implications for genetic medicine in understanding complex epistatic interactions between HBB-βS and other gene variants selected under environmental pressure in Africa e.g. variants in HBB, HBA, G6PD, APOL1, APOE, OSBPL10 and RXRA.

Evolving influence of mental health stigma in Ontario public safety organizations: a qualitative study

Leslie Vesely, Cameron A. Mustard, Basak Yanar

AbstractIntroduction:Mental health stigma has been a long-standing issue in public safety professions and can deter public safety personnel (PSP) from accessing mental health support. This is concerning as PSP experience higher rates of post-traumatic stress injuries (PTSI) than the general population. Public safety employers play an important role in stigma reduction. However, there is little research on Ontario public safety employers’ perspectives on mental health stigma in their organization and the accompanying organizational challenges they face in addressing stigma and supporting PSPs’ mental health. Methods:A thematic analysis of 28 semi-structured interviews with 33 public safety employer representatives from fire services, paramedics, police, and provincial corrections within Ontario was conducted. Results:Employer representatives recognized mental health stigma existed historically. They described that stigma is reducing due to sociopolitical changes that restructured PTSI as a common occupational injury. Participants shared ways they are adjusting organizational practices and policies to further reduce stigma and support PSP. They also highlighted accompanying organizational challenges, including ongoing historic stigma, uncertainty in how to support PSP with PTSI, and difficulty finding meaningful accommodations. Conclusion:Participants perceived mental health stigma to be decreasing in their public safety organizations. However, ongoing stigma, organizational factors and uncertainty around how to support those experiencing PTSI can pose challenges to return-to-work and accommodation. While in various stages of implementing initiatives to support mental health, organizations need to continue to build PTSI awareness, take accountability for their role in reducing mental health stigma, and build trauma-informed practices and policies.

Evolving Landscape of Sickle Cell Anemia Management in Africa: A Critical Review

Sickle cell disease (SCD) is a prevalent inherited blood disorder, particularly affecting populations in Africa. This review examined the disease’s burden, its diverse clinical presentations, and the challenges associated with its management in African settings. Africa bears a significant burden of SCD, with prevalence varying across countries and age groups. Newborn screening programs have highlighted the high prevalence of SCD at birth, emphasizing the need for early diagnosis and intervention. The clinical manifestations of SCD in Africa are multifaceted, encompassing acute complications like vaso-occlusive crises, acute chest syndrome, and stroke, as well as chronic complications such as organ damage and leg ulcers. Biological factors, including fetal hemoglobin levels, and demographic factors, like age and sex, influence disease severity and outcomes. The management of SCD in Africa faces numerous challenges. Limited access to resources, including diagnostic tools, medications, and trained healthcare professionals, hinders optimal care. The high cost of advanced therapies further restricts patient access. Cultural stigma and a lack of awareness create additional barriers to effective management. To address these challenges, early diagnosis through newborn screening programs and point-of-care testing is crucial. Comprehensive care models, including hydroxyurea therapy, pain management, and patient education, are essential for improving outcomes. Collaboration with international networks and leveraging local resources can enhance the sustainability of SCD programs. In conclusion, SCD significantly impacts African populations. Overcoming the challenges associated with its management requires addressing resource limitations, affordability issues, and cultural barriers. Early diagnosis, comprehensive care models, and ongoing research focused on affordability and accessibility are crucial for improving the lives of individuals living with SCD in Africa.

Evolving trends and burden of iron deficiency among children, 1990–2019: a systematic analysis for the global burden of disease study 2019

Dan Long, Chenhan Mao, Yaxuan Liu, Tao Zhou, Yin Xu, Ying Zhu

ObjectivesWe aimed to provide a timely, comprehensive, and reliable assessment of the burden of iron deficiency (ID) in children between 1990 and 2019 at the global, regional, and national levels to inform policymakers in developing locally appropriate health policies.MethodsData related to ID among children younger than 15 years old were analyzed by sex, age, year, socio-demographic index (SDI), and location according to the Global Burden of Disease Study 2019 (GBD 2019). Age-standardized rates were used to compare the burden between different regions and countries. Furthermore, the Joinpoint regression model was used to assess temporal trends from 1990 to 2019.ResultsIn 2019, the number of prevalent cases and disability-adjusted life years (DALYs) for ID in children were 391,491,699 and 13,620,231, respectively. The global age-standardized prevalence and DALY rates for childhood ID in 2019 were 20,146.35 (95% confidence interval: 19,407.85 to 20,888.54) and 698.90 (466.54 to 1015.31) per 100,000, respectively. Over the past 30 years, the global prevalence of ID among children has been highest in low-SDI regions, particularly in Western Sub-Saharan Africa, South Asia, and Eastern Sub-Saharan Africa. Since 1990, the prevalence and DALY of ID in children have been declining in most geographic regions. Nationally, Ecuador, China, and Chile have shown the most significant decreases in prevalence. The greatest decline in age-standardized DALY rate was observed in Ecuador, while Burkina Faso experienced the highest increase. Bhutan had the highest prevalence and DALY rates in 2019. On the age level, the prevalence was relatively higher among the <5 years age group. At the gender dimension, the prevalence of ID in children overall was more pronounced in girls than in boys, as was the case for DALY.ConclusionAlthough the burden of ID in children has been declining, this disease remains a major public health problem, especially in countries with low SDI. Children younger th

Ex vivo anti-malarial drug susceptibility of Plasmodium falciparum isolates from pregnant women in an area of highly seasonal transmission in Burkina Faso

Marc C Tahita, Halidou Tinto, Sibiri Yarga, Adama Kazienga, Maminata Traore/Coulibaly, Innocent Valea, Chantal Van Overmeir, Anna Rosanas-Urgell, Jean-Bosco Ouedraogo, Robert T Guiguemde, Jean-Pierre van Geertruyden, Annette Erhart, Umberto D’Alessandro

BackgroundEx vivo assays are usually carried out on parasite isolates collected from patients with uncomplicated Plasmodium falciparum malaria, from which pregnant women are usually excluded as they are often asymptomatic and with relatively low parasite densities. Nevertheless, P. falciparum parasites infecting pregnant women selectively sequester in the placenta and may have a different drug sensitivity profile compared to those infecting other patients. The drug sensitivity profile of P. falciparum isolates from infected pregnant women recruited in a treatment efficacy trial conducted in Burkina Faso was determined in an ex vivo study.MethodsThe study was conducted between October 2010 and December 2012. Plasmodium falciparum isolates were collected before treatment and at the time of any recurrent infection whose parasite density was at least 100/µl. A histidine-rich protein-2 assay was used to assess their susceptibility to a panel of seven anti-malarial drugs. The concentration of anti-malarial drug inhibiting 50% of the parasite maturation to schizonts (IC50) for each drug was determined with the IC Estimator version 1.2.ResultsThe prevalence of resistant isolates was 23.5% for chloroquine, 9.2% for mefloquine, 8.0% for monodesethylamodiaquine, and 4.4% for quinine. Dihydroartemisinin, mefloquine, lumefantrine, and monodesethylamodiaquine had the lowest mean IC50 ranging between 1.1 and 1.5 nM respectively. The geometric mean IC50 of the tested drugs did not differ between chloroquine-sensitive and resistant parasites, with the exception of quinine, for which the IC50 was higher for chloroquine-resistant isolates. The pairwise comparison between the IC50 of the tested drugs showed a positive and significant correlation between dihydroartemisinin and both mefloquine and chloroquine, between chloroquine and lumefantrine and between monodesethylamodiaquine and mefloquine.ConclusionThese ex vivo results suggest that treatment with the currently available artemisi

Ex vivo anti-malarial drugs sensitivity profile of Plasmodium falciparum field isolates from Burkina Faso five years after the national policy change

Halidou Tinto, Léa N Bonkian, Louis A Nana, Isidore Yerbanga, Moussa Lingani, Adama Kazienga, Innocent Valéa, Hermann Sorgho, Hervé Kpoda, Tinga Robert Guiguemdé, Jean Bosco Ouédraogo, Petronella F Mens, Henk Schallig, Umberto D’Alessandro

BackgroundThe recent reports on the decreasing susceptibility of Plasmodium falciparum to artemisinin derivatives along the Thailand and Myanmar border are worrying. Indeed it may spread to India and then Africa, repeating the same pattern observed for chloroquine resistance. Therefore, it is essential to start monitoring P. falciparum sensitivity to artemisinin derivatives and its partner drugs in Africa. Efficacy of AL and ASAQ were tested by carrying out an in vivo drug efficacy test, with an ex vivo study against six anti-malarial drugs nested into it. Results of the latter are reported here.MethodsPlasmodium falciparum ex-vivo susceptibility to chloroquine (CQ), quinine (Q), lumefantrine (Lum), monodesethylamodiaquine (MDA), piperaquine (PPQ) and dihydroartemisinin (DHA) was investigated in children (6 months – 15 years) with a parasitaemia of at least ≥4,000/μl. The modified isotopic microtest technique was used. The results of cellular proliferation were analysed using ICEstimator software to determine the 50% inhibitory concentration (IC50) values.ResultsDHA was the most potent among the 6 drugs tested, with IC50 values ranging from 0.8 nM to 0.9 nM (Geometric mean IC50 = 0.8 nM; 95% CI [0.8 - 0.9]). High IC50 values ranged between 0.8 nM to 166.1 nM were reported for lumefantrine (Geometric mean IC50 = 25.1 nM; 95% CI [22.4 - 28.2]). MDA and Q IC50s were significantly higher in CQ-resistant than in CQ-sensitive isolates (P = 0.0001). However, the opposite occurred for Lum and DHA (P < 0.001). No difference was observed for PPQ.ConclusionArtemisinin derivatives are still very efficacious in Burkina Faso and DHA-PPQ seems a valuable alternative ACT. The high lumefantrine IC50 found in this study is worrying as it may indicate a decreasing efficacy of one of the first-line treatments. This should be further investigated and monitored over time with large in vivo and ex vivo studies that will include also plasma drug measurements.
