Background: The clinical research laboratory plays a pivotal role in the execution of clinical studies. The accurate and consistent registration of patients is dependent on the competent use of laboratory equipment and manual techniques by technicians, ensuring the reliability of the data collected. To support these activities, the Groupe de Recherche Action en Santé (GRAS) has been registered with the College of American Pathologists (CAP) and the Clinical Laboratories Services (CLS) in Johannesburg, South Africa, for external proficiency testing (EPT) of its laboratory, as part of our commitment to quality assurance. The following report details the performance achievements over the past five years. Methods: Proficiency testing (PT) samples are dispatched to GRAS Lab three times a year (quarterly) and the results are generally returned within two to three weeks. In the field of parasitology, challenge specimens were prepared as follows: thick and thin blood films were stained with Giemsa and mounted with strips to protect them for multiple uses. Photographs, also known as whole slide images (WSIs), were also taken. For the biochemistry and haematology tests, a set of five samples were received for processing. All evaluations were carried out in accordance with the GRAS laboratory’s internal procedures. Results: The CAP laboratory’s performance in terms of the diagnosis of malaria and other blood parasites from 2020 to 2024 was 97.3% accurate (ranging from 93.33% to 100%), with 93.33%, 100%, 100%, 93.33% and 100% achieved in 2020, 2021, 2022, 2023 and 2024, respectively. The number of microscopists evaluated annually has been subject to variation according to operational staff at the time of evaluation. A total of 31 microscopists were enrolled in the CLS PT scheme, of which 73.9% were classified as ‘experts’ and 19.2% as ‘reference’ microscopists. In the field of haematology, the PT demonstrated 100% accuracy over the four-year study period. This outcome is indica

BackgroundThe foundation of recommended diarrhea management in young children is increased fluids and continued feeding. This increase in fluids is necessary to replace those lost during diarrhea and ultimately prevent dehydration. There may be an opportunity to prevent deaths in children under five by discouraging the practice of reducing or curtailing fluids during diarrhea episodes across different settings worldwide.MethodsWe quantify and describe the extent of fluid curtailment in children with diarrhea in a selection of countries (Burkina Faso, Democratic Republic of Congo, Ethiopia, Nigeria, Tanzania, and Uganda) with high burden of diarrhea-related mortality with national cross sectional survey data. We examine the practice of fluid curtailment in these countries and its relationship to child and household traits and to characteristics of diarrhea management.ResultsThe prevalence of fluid curtailment among children under five with diarrhea is strikingly high in these countries: 55 % in Nigeria, 49 % in Ethiopia, 44 % in Uganda, 37 % in Tanzania, 36 % in DR Congo and 32 % in Burkina Faso. Fluid curtailment is associated with giving less food, potentially worsening the impact of this harmful practice. Children who were reported to have had fluids curtailed during diarrhea episodes were also 3.51 (95 % confidence, 2.66 - 4.64) times more likely to be reported to have food withheld (α = 0.05; p < 0.001). Children who received care from non-governmental providers, and those who were breastfed were more likely to have their fluids curtailed, as were children with an unimproved water source. Children of poorer or less educated mothers and those living in rural areas are more likely to have curtailed fluids, compared to children of less poor or more educated mothers, or those living in urban areas.ConclusionsThe harmful practice of curtailing fluids for a child with diarrhea is highly prevalent, representing an increased risk of dehydration and complications due to

IntroductionThe food environment is an important factor in the efforts of countries worldwide to achieve a transition to sustainable food systems. The objective of this study is to formulate and prioritize actions to be addressed to the government of Burkina Faso for the creation of a healthy food environment, which will contribute to reducing malnutrition in all its forms and non-communicable diseases.MethodsNational experts were brought together to identify and prioritize actions to fill the gaps identified through a multi-step assessment process following the methodology of the Healthy Food and Environment Policy Index (Food-EPI).ResultsUp to 20 priority policy actions were recommended to the Burkina Faso government. Actions in the policy component focused mainly on regulation of food promotion and marketing, particularly to children, and others in the infrastructure support component focused largely on political leadership, i.e., strong and visible political support from the government to improve the food environment, population nutrition, diet-related non-communicable diseases and their inequalities.ConclusionThe priority actions to be recommended to the government will strengthen advocacy for government decisions to create a healthier food environment in the country.

The outbreak of COVID-19 led to the implementation of lockdowns and social distancing regulations to curb the spread of infections. Consequently, the lockdowns impeded the movement of smallholder farmers, agricultural inputs, and food products thereby disrupting the food supply chains in SSA. Therefore, this paper examines the relationship between food security indicators (accessibility, availability, utilization, stability) and COVID-19. This study uses ordinary least square regression (OLS) models to study the relationship between the food security indicators and COVID-19. The study considers 9 out of 48 sub-Saharan African countries (Benin, Burkina Faso, Cameroon, Chad, Madagascar, Mali, Mauritania, Nigeria, Senegal) due to data availability restrictions. The result of the analysis indicated that a rise in COVID-19 levels negatively impacts all the 4 indicators of food security without exception. This paper underscores the need to consider the disruptions of food security indicators such as diet, nutritional content, access and availability, affordability, and food supply chains. Moreover, the paper discusses mitigating strategies that may alleviate SSA’s food security amidst the COVID-19 pandemic. We recommend that SSA countries invest in quality agricultural and food production infrastructure and supporting industries that contribute directly to the food supply chain, such as agro-processing, fertilizer production and transport. Another important dimension of the COVID-19 and food insecurity syndemic is the income shocks that occurred as a consequence of the COVID-19 outbreak. Like many factories, companies, and service providers closed shop (especially the informal sector), people lost their incomes as a result of loss of employment and, in many instances, no social protection. Therefore, we recommend that SSA governments develop affordable, sustainable, and targeted social protection/insurance systems that extend to the informal sector of the economy.

AbstractIntroduction:There is growing interest in food prescriptions, which leverage health care settings to provide patients access to healthy foods through vouchers or food boxes. In this commentary, we draw on our experiences and interest in food prescribing to provide a summary of the current evidence on this intervention model and critically assess its limitations and opportunities.Rationale:Food insecurity is an important determinant of health and is associated with compromised dietary adequacy, higher rates of chronic diseases, and higher health service utilization and costs. Aligning with recent discourse on social prescribing and “food is medicine” approaches, food prescribing can empower health care providers to link patients with supports to improve food access and limit barriers to healthy diets. Food prescribing has been shown to improve fruit and vegetable intake and household food insecurity, although impacts on health outcomes are inconclusive. Research on food prescribing in the Canadian context is limited and there is a need to establish evidence of effectiveness and best practices. Conclusion:As food prescribing continues to gain traction in Canada, there is a need to assess the effectiveness, cost-efficiency, limitations and potential paternalism of this intervention model. Further, it is necessary to assess how food prescribing fits into broader social welfare systems that aim to address the underlying determinants of food insecurity.

Food security, defined as the capacity to acquire preferred food at all times, can manifest in many dimensions. Following a mixed methods approach used in India and Burkina Faso, we developed a 58-item experience-based measure in the Peruvian Amazon, based on investigator observations, relevant literature, and pre-testing with community field workers. The tool encompasses seven dimensions of food security and included measures of (1) food purchases, frequency of purchase, and location of acquisition, (2) food expenses, (3) coping mechanisms, (4) preparation of leftover food, (5) food safety (refrigerator access), (6) fishing intensity and (7) selling food. The survey was piloted among 35 randomly selected families from the Malnutrition Enteric Disease (MAL-ED) birth cohort in Santa Clara, Peru and the surrounding communities. Subsequently, based on a focus group discussion, a pile-sorting exercise, and pilot results, we reduced the survey to 36 items to be collected monthly among 203 MAL-ED households from November 2013 to January 2015. Validity and reliability were then assessed using principal component analysis and exploratory factor analysis, revealing four groups of purchase and coping strategy behaviors: (1) Sweets and sugary items, (2) Less preferred, (3) More preferred, and (4) Minimum meal. Internal consistency of the final 22-item scale had an acceptable cutoff of Cronbach’s α of 0.73. Criterion and construct validity of the factor groups revealed there were: (1) food purchase patterns that were distinctive to quality and quantity aspects of the Household Food Insecurity Access scale, (2) unique correlations of child’s intake of fats, animal source protein, fiber and other micronutrients, (3) household purchase patterns from the “more preferred” group (fish, red meat) associated with child’s weight-for-age. Food purchase and frequency, and context-specific behaviors at the household level can be used as surrogates for dietary intake patterns and nutritiona

Rural communities are more likely to encounter constraints in achieving food security and adequate nutritional supply. This study explores food security, nutritional supply, nutrient adequacy, macronutrient balance, recipes, and nutrient sources based on bi-monthly household surveys in rural villages in Northern and Southern Burkina Faso from 2019 to 2020. Food security across time and its quantity dimensions were measured using the food consumption score (FCS). Ordered logit regression showed that FCS was significantly influenced by season, region, and household characteristics such as the head’s education and women’s possession of personal plots. The regional differences were large: Households categorized as having “poor” diets were approximately 1% and 38% in the south and north, respectively. Nutrient adequacy was assessed by converting 24 h dietary recall into nutrient supply and comparing the results to the requirement. While macronutrient balance appeared adequate in the pooled sample, it became unacceptable when the two regions were considered separately. Most micronutrient supplies were insufficient. Cereals were the main nutrient sources, and leaves of crops and potash (additive containing potassium) were also non-negligible for micronutrient supplies. Overall, we found large regional differences in nutrition and food security, indicating that various local contexts must be considered for effective nutritional improvement.

AbstractIntroduction:Household food insecurity (HFI) is a persistent and important public health and policy concern within Canada that continues to be widespread in the face of economic uncertainties and inflation. The objective of this systematic review was to synthesize the evidence on food-based interventions that could reduce HFI in Canada. Methods:Studies that assessed a food-based intervention that might reduce food insecurity and measured HFI were included, regardless of whether that was the primary purpose of the study. Four databases were searched up to 19 February 2025. Screening of abstracts and full texts, data extraction, assessments of risks of bias and certainty of the evidence were conducted independently by two reviewers. PROSPERO CRD42021254450.Results:Exposure to food voucher programs may reduce HFI, but exposure to food box, community gardening, school food, hunting and fishing, and food charity programs may have little to no effect on HFI. The rate of utilization of food banks by food-insecure households may be low and depends upon food insecurity level and population group. Conclusion:Food charities may be a last resort for those in need of short-term access to emergency food (i.e. populations experiencing homelessness). However, given the pervasive nature of HFI as a marker of deprivation, it is unlikely that food-based responses will have a major impact on overall HFI, which is primarily an economic problem. A more comprehensive public policy approach to mitigate HFI is likely required.

The aim of this study was to explore molecular measures of P. falciparum malaria burden (FOI and MOI) in the context of seasonal malaria chemoprevention. We analyzed malaria cases collected as part of a longitudinal cohort study. The cohort included P. falciparum-negative children aged 1.5 to 12, as confirmed by PCR 21 days after a radical cure using DHA-PQ or AS. Children were followed up for six months using active and passive case detection methods. At each visit, dried blood spots and blood smears were collected by finger prick, along with clinical data. Parasite DNA was extracted and analyzed by nested PCR for detection and genotyping of P. falciparum parasites. A total of 458 P. falciparum isolates collected during follow-up from October 2020 to March 2021 were genotyped. During the follow-up, children contracted 1.05 (95% IC [0.81–1.30]) new P. falciparum infections/child/time of exposure, and the MOI value was 3.00 (SD 1.60). Age is a protective factor (IRR: 0.74; 95% CI: 0.61, 0.90) against the occurrence of an episode of malaria, unlike an increase in MOI (IRR: 1.63; 95% CI: 1.04, 1.99), which is a favorable factor (p < 0.05). This study confirms the reduction in malaria transmission in our study area, probably due to the massive deployment of control tools.

Tandem repeat genetic profiles used in forensic applications varies between populations. Despite the diversity and security issues in the Sahel that require the identification of victims (soldiers and civilians), Burkina Faso (BF) remains understudied. To fill this information gap, 396 unrelated individuals from BF were genotyped using a MICROREADER 21 ID System kit. All 20 short tandem repeat (STR) loci tested passed the Hardy–Weinberg equilibrium (HWE) test. The combined powers of exclusion for duos (CPE duos) and trios (CPE trios) for the 20 tested loci were 0.9999998 and 0.9999307, respectively. The probability that two individuals would share the same DNA profiles among the BF population was 9.80898 × 10–26. For the X-chromosome STR analysis, 292 individuals were included in this study using a MICROREADER 19X Direct ID System kit. Among the 19 loci, no significant deviations from HWE test were observed in female samples after Bonferroni correction (p < 0.05/19 = 0.0026), except for loci GATA165B12 and DXS7423. The results showed that the combined power of exclusion (CPE) and the combined power of discrimination in females (CPDF) and males (CPDM) were 0.999999760893, 0.999999999992, and 1, respectively. Comparison with other African sub-populations showed that geographical proximity is a reliable indicator of genetic relatedness.

ABSTRACTBackgroundBurkina Faso joined the Global Financing Facility for Women, Children and Adolescents (GFF) in 2017 to address persistent gaps in funding for reproductive, maternal, newborn, child, and adolescent health and nutrition (RMNCAH-N). Few empirical papers deal with how global funding mechanisms, and specifically GFF, support resource mobilisation for health nationally.ObjectiveThis study describes the policy processes of developing the GFF planning documents (the Investment Case and Project Appraisal Document) in Burkina Faso.MethodsWe conducted an exploratory qualitative policy analysis. Data collection included document review (N = 74) and in-depth semi-structured interviews (N = 23). Data were analysed based on the components of the health policy triangle.ResultsThere was strong national political support to RMNCAH-N interventions, and the process of drawing up the investment case (IC) and the project appraisal document was inclusive and multi-sectoral. Despite high-level policy commitments, subsequent implementation of the World Bank project, including the GFF contribution, was perceived by respondents as challenging, even after the project restructuring process occurred. These challenges were due to ongoing policy fragmentation for RMNCAH-N, navigation of differing procedures and perspectives between stakeholders in the setting up of the work, overcoming misunderstandings about the nature of the GFF, and weak institutional anchoring of the IC. Insecurity and political instability also contributed to observed delays and difficulties in implementing the commitments agreed upon. To tackle these issues, transformational and distributive leaderships should be promoted and made effective.ConclusionsFew studies have examined national policy processes linked to the GFF or other global health initiatives. This kind of research is needed to better understand the range of challenges in aligning donor and national priorities encountered across diverse health s
