From farmers to livestock keepers: a typology of cattle production systems in south-western Burkina Faso

    B. Zoma-Traoré, A. Soudré, S. Ouédraogo-Koné, N. Khayatzadeh, L. Probst, J. Sölkner, G. Mészáros, P. A. Burger, A. Traoré, M. Sanou, G. M. S. Ouédraogo, L. Traoré, D. Ouédraogo, B. Yougbaré, M. Wurzinger

Cattle production is an essential livelihood strategy in south-western Burkina Faso. Although having a distinct cultural role and known to be resistant against African animal trypanosomosis, the Lobi taurine cattle breed is endangered due to its low market value. As the first step in preservation efforts, our study aimed to develop a typology of production systems at the farm level. We used a structured questionnaire and focus group discussions for collecting data on household characteristics, socioeconomic activities, livestock, and access to services. The sample comprised 169 households in three communities. The analytical strategy included factor analysis of mixed data and hierarchical clustering. We identified four distinct types of cattle production systems: (1) sedentary Lobi farms, (2) sedentary crossbreed farms, (3) semi-transhumant Fulani zebu farms, and (4) transhumant Fulani zebu farms. Significant factors in developing this typology were the farmers’ ethnic group, crop diversity, cattle herd size, cattle herd composition, number of small ruminants, and livestock management strategies. Across all production systems, men were considered being primary decision-makers in cattle production, with women, herders, and children being responsible for specific tasks. All identified production systems are increasingly confronting disease pressure and scarcity of water and land. Future efforts in preservation and breeding will need to respond to these trends in the agroecosystem, integrate risk management measures, and resonate with the specific needs of the different household members involved in cattle rearing.

From Lineage Property to Individual Property: Dynamics of Land Management in the Villages of Nasso and Dindéresso, Hauts-Bassins, Burkina Faso

    Abdoulaye Sawadogo

BackgroundThis article analyses the dynamics of relations between communities and land resources in a context of scarcity resulting from land speculation. Faced with this reality, the changes brought about by social dynamics in the land sector have ultimately led to the introduction of new forms of land ownership in the villages of Nasso and Dindéresso in Burkina Faso. This issue is a corollary of spatial expansion following urban sprawl due to population growth and speculation around peri-urban land. With increased interest in peri-urban land, traditional management norms are eroding, giving way to more modern management norms that offer more power to individuals.MethodsThis research is based on a qualitative approach using data from documentary reviews, semi-structured interviews with community and administrative actors, and direct observation. This approach has made it possible to analyse land management through the prism of public policies and internal community dynamics. Property rights theory has been used to examine this issue. The combination of methodological and theoretical approaches has yielded illustrative results.ResultsThese results reveal the nature and challenges of the fragmentation of land ownership rights within communities. To this end, it appears that land dynamics have had a significant impact on forms of intra-community land ownership, leading to the individualisation of land rights following the advent of land commodification.ConclusionsIn conclusion, it appears that land is a cross-cutting issue that impacts all spheres of society. Hence the multiple and multifaceted challenges in terms of land governance and the consolidation of social cohesion.

From policy to practice: World Health Organization’s contribution to safer blood transfusions in Africa for sickle cell management – a review

    Emmanuel Ifeanyi Obeagu

Sickle cell disease (SCD) is a major public health challenge in Africa, where the prevalence of the condition is high, and it contributes to significant morbidity and mortality, especially among children. Blood transfusions are a crucial therapeutic intervention for managing complications of SCD, such as severe anemia and stroke, which are common among affected individuals. However, many African countries face significant barriers in providing safe, adequate, and timely blood transfusions due to underdeveloped healthcare infrastructure, limited voluntary blood donations, and a shortage of trained healthcare professionals. This review explores the World Health Organization’s (WHO) role in enhancing blood transfusion services for SCD patients in Africa, highlighting its advocacy efforts, policy development, infrastructure strengthening, and capacity-building initiatives. The WHO has been pivotal in advocating for the integration of blood transfusion services into national healthcare systems and developing guidelines for safe blood collection, storage, and distribution. Additionally, WHO has worked to promote voluntary blood donation campaigns, ensuring a steady and safe supply of blood products. WHO’s efforts also include providing technical support to healthcare systems in Africa to improve blood transfusion infrastructure, increase public awareness about the importance of regular blood donation, and ensure that healthcare professionals are properly trained in transfusion medicine. These initiatives aim to enhance the quality and accessibility of blood transfusion services, ultimately improving outcomes for individuals living with SCD.

Benoît van Gastel, Paulo César Peiter, Valérie Morel, Emmanuel Roux, Vivian da Cruz Franco, Anapaula Martins Mendes, Nadia Cristine Coelho Eugenio, Margarete Gomes

    Fumonisins in African Countries

    Tapani Yli-Mattila, Leif Sundheim

Maize and other cereals are the commodities most contaminated with fumonisins. The maize acreage is increasing in Africa, and the maize harvest provides important foods for humans and feeds for domestic animals throughout the continent. In North Africa, high levels of fumonisins have been reported from Algeria and Morocco, while low levels have been detected in the rather few fumonisin analyses reported from Tunisia and Egypt. The West African countries Burkina Faso, Cameroon, Ghana, and Nigeria all report high levels of fumonisin contamination of maize, while the few maize samples analysed in Togo contain low levels. In Eastern Africa, high levels of fumonisin contamination have been reported from the Democratic Republic of Congo, Ethiopia, Kenya, Tanzania, and Uganda. The samples analysed from Rwanda contained low levels of fumonisins. Analysis of maize from the Southern African countries Malawi, Namibia, South Africa, Zambia, and Zimbabwe revealed high fumonisin levels, while low levels of fumonisins were detected in the few analyses of maize from Botswana and Mozambique.

Functional antibodies against Plasmodium falciparum sporozoites are associated with a longer time to qPCR-detected infection among schoolchildren in Burkina Faso

Aissata Barry, Marije C. Behet, Issa Nébié, Kjerstin Lanke, Lynn Grignard, Alphonse Ouedraogo, Issiaka Soulama, Chris Drakeley, Robert Sauerwein, Judith M. Bolscher, Koen J. Dechering, Teun Bousema, Alfred B. Tiono, Bronner P. Gonçalves

Background: Individuals living in malaria-endemic regions develop immunity against severe malaria, but it is unclear whether immunity against pre-erythrocytic stages that blocks initiation of blood-stage infection after parasite inoculation develops following continuous natural exposure. Methods: We cleared schoolchildren living in an area (health district of Saponé, Burkina Faso) with highly endemic seasonal malaria of possible sub-patent infections and examined them weekly for incident infections by nested PCR. Plasma samples collected at enrolment were used to quantify antibodies to the pre-eryhrocytic-stage antigens circumsporozoite protein (CSP) and Liver stage antigen 1 (LSA-1). In vitro sporozoite gliding inhibition and hepatocyte invasion inhibition by naturally acquired antibodies were assessed using Plasmodium falciparum NF54 sporozoites. Associations between antibody responses, functional pre-erythrocytic immunity phenotypes and time to infection detected by 18S quantitative PCR were studied. Results: A total of 51 children were monitored. Anti-CSP antibody titres showed a positive association with sporozoite gliding motility inhibition (P<0.0001, Spearman’s ρ=0.76). In vitro hepatocyte invasion was inhibited by naturally acquired antibodies (median inhibition, 19.4% [IQR 15.2-40.9%]), and there were positive correlations between invasion inhibition and gliding inhibition (P=0.005, Spearman’s ρ=0.67) and between invasion inhibition and CSP-specific antibodies (P=0.002, Spearman’s ρ=0.76). Survival analysis indicated longer time to infection in individuals displaying higher-than-median sporozoite gliding inhibition activity (P=0.01), although this association became non-significant after adjustment for blood-stage immunity (P = 0.06). Conclusions: In summary, functional antibodies against the pre-erythrocytic stages of malaria infection are acquired in children who are repeatedly exposed to Plasmodium parasites. This immune response does not prevent them f

Functional Characterization and Comparison of Plasmodium falciparum Proteins as Targets of Transmission-blocking Antibodies*

Daria Nikolaeva, Joseph J. Illingworth, Kazutoyo Miura, Daniel G. W. Alanine, Iona J. Brian, Yuanyuan Li, Alex J. Fyfe, Dari F. Da, Anna Cohuet, Carole A. Long, Simon J. Draper, Sumi Biswas

The malaria agent Plasmodium falciparum continues to evade control efforts. Despite multiple datasets for the Plasmodium sexual-stage transcriptome and proteome, there have been no rational screens to identify candidate antigens for transmission-blocking vaccine (TBV) development. We demonstrate the use of rational screens and comparative functional assessments in identifying proteins of the P. falciparum transmission pathway and establishing a robust pre-clinical TBV pipeline. Graphical AbstractHighlightsCharacterization of 12 proteins from across the P. falciparum sexual-stages as possible TBV targets.Heterologously expressed recombinant proteins recapitulate native parasite epitopes. Some recombinant proteins exhibit immunoreactivity when tested against sera from individuals from malaria-endemic Burkina Faso and Mali.Purified IgG against the antigen enolase moderately inhibits parasite development in the mosquito midgut. Plasmodium falciparum malaria continues to evade control efforts, utilizing highly specialized sexual-stages to transmit infection between the human host and mosquito vector. In a vaccination model, antibodies directed to sexual-stage antigens, when ingested in the mosquito blood meal, can inhibit parasite growth in the midgut and consequently arrest transmission. Despite multiple datasets for the Plasmodium sexual-stage transcriptome and proteome, there have been no rational screens to identify candidate antigens for transmission-blocking vaccine (TBV) development. This study characterizes 12 proteins from across the P. falciparum sexual-stages as possible TBV targets. Recombinant proteins are heterologously expressed as full-length ectodomains in a mammalian HEK293 cell system. The proteins recapitulate native parasite epitopes as assessed by indirect fluorescence assay and a proportion exhibits immunoreactivity when tested against sera from individuals living in malaria-endemic Burkina Faso and Mali. Functional difficulties in children and youth with autism spectrum disorder: analysis of the 2019 Canadian Health Survey on Children and Youth

Amy Farrow, Ahmed A. Al-Jaishi, Siobhan O’Donnell, Sarah Palmeter, Stelios Georgiades, Yun-Ju Chen, Patrick G. McPhee, Rojiemiahd Edjoc

AbstractIntroduction:This study examined the prevalence of functional difficulties and associated factors in Canadian children/youth aged 5 to 17 years diagnosed with autism spectrum disorder (ASD).Methods:We analyzed data from the 2019 Canadian Health Survey on Children and Youth (CHSCY), a nationally representative survey of Canadian children/youth that used the Washington Group Short Set on Functioning (WG-SS) to evaluate functioning in six daily tasks. For each functional domain, binary outcomes were derived (no/some difficulty, a lot of difficulty/no ability). We used logistic regression to identify associations between demographic characteristics, educational experiences, and perceived mental and general health and the most common functional difficulties, namely those related to remembering/concentrating, communication and self-care. All estimates were weighted to be representative of the target population. The bootstrap method was used to calculate variance estimates. Results:Analysis of the records of 660 children/youth with ASD revealed that the most common functional difficulties were remembering/concentrating (22%; 95% CI: 18–27), communicating (19%; 95% CI: 15–23) and self-care (13%; 95% CI: 10–17). Lower perceived mental health was associated with increased functional difficulties with remembering/concentrating. ASD diagnosis at a lower age and lower perceived general health were associated with increased functional difficulty with communication. Parental expectations for postsecondary education were associated with decreased functional difficulty for self-care.Conclusion: One or more functional difficulties from the WG-SS was present in 39% of Canadian children/youth aged 5 to 17 years with ASD. Functional difficulties with remembering/concentrating, communication and self-care were most common.

Future Directions for Meningitis Surveillance and Vaccine Evaluation in the Meningitis Belt of Sub-Saharan Africa

Ryan T Novak, Olivier Ronveaux, André F Bita, Honoré Flavien Aké, Fernanda C Lessa, Xin Wang, Ado M Bwaka, LeAnne M Fox

AbstractIn sub-Saharan Africa, bacterial meningitis remains a significant public health problem, especially in the countries of the meningitis belt, where Neisseria meningitidis serogroup A historically caused large-scale epidemics. In 2014, MenAfriNet was established as a consortium of partners supporting strategic implementation of case-based meningitis surveillance to monitor meningitis epidemiology and impact of meningococcal serogroup A conjugate vaccine (MACV). MenAfriNet improved data quality through use of standardized tools, procedures, and laboratory diagnostics. MenAfriNet surveillance and study data provided evidence of ongoing MACV impact, characterized the burden of non-serogroup A meningococcal disease (including the emergence of a new epidemic clone of serogroup C), and documented the impact of pneumococcal conjugate vaccine. New vaccines and schedules have been proposed for future implementation to address the remaining burden of meningitis. To support the goals of “Defeating Meningitis by 2030,” MenAfriNet will continue to strengthen surveillance and support research and modeling to monitor the impact of these programs on meningitis burden in sub-Saharan Africa.

G6PD and HBB polymorphisms in the Senegalese population: prevalence, correlation with clinical malaria

BackgroundHost genetic factors contribute to the variability of malaria phenotypes and can allow a better understanding of mechanisms involved in susceptibility and/or resistance to Plasmodium falciparum infection outcomes. Several genetic polymorphisms were reported to be prevalent among populations living in tropical malaria-endemic regions and induce protection against malaria. The present study aims to investigate the prevalence of HBB (chr11) and G6PD (chrX) deficiencies polymorphisms among Senegalese populations and their associations with the risk for severe Plasmodium falciparum malaria occurrence.MethodsWe performed a retrospective study with 437 samples, 323 patients recruited in hospitals located in three different endemic areas where malaria episodes were confirmed and 114 free malaria controls. The patients enrolled were classified into two groups: severe malaria (SM) (153 patients) and uncomplicated malaria (UM) (170 patients). PCR and DNA sequencing assessed host genetic polymorphisms in HBB and G6PD. Using a multivariate regression and additive model, estimates of the impact of human HBB and G6PD polymorphisms on malaria incidence were performed.ResultsSix frequent SNPs with minor allele frequencies (MAF) > 3% were detected in the HBB gene (rs7946748, rs7480526, rs10768683, rs35209591, HbS (rs334) and rs713040) and two in the G6PD gene (rs762515 and rs1050828 (G6PD-202 G > A). Analysis of selected HbS polymorphism showed significant association with protective effect against severe malaria with a significant p-value = 0.033 (OR 0.38, 95% CI [0.16–0.91]) for SM vs. UM comparison. Surprisingly, our study did not identify the protective effect of variant HbC polymorphism against severe malaria. Finally, we found some of the polymorphisms, like HbS (rs334), are associated with age and biological parameters like eosinophils, basophils, lymphocytes etc.ConclusionOur data report HBB and G6PD polymorphisms in the Senegalese population and their correlation w

Anass Ayyad, Sahar Messaoudi, Rim Amrani

Tarik Salama, El Mohtadi Aghoutane, Rédouane El Fezzazi

Gaps in the implementation and uptake of maternal nutrition interventions in antenatal care services in Bangladesh, Burkina Faso, Ethiopia and India

Tina Sanghvi, Phuong H. Nguyen, Manisha Tharaney, Sebanti Ghosh, Jessica Escobar‐Alegria, Zeba Mahmud, Tamirrat Walissa, Maurice Zafimanjaka, Sunny Kim

AbstractAntenatal care (ANC) is the largest health platform globally for delivering maternal nutrition interventions (MNIs) to pregnant women. Yet, large missed opportunities remain in nutrition service delivery. This paper examines how well evidence‐based MNIs were incorporated in national policies and programs in Bangladesh, Burkina Faso, Ethiopia and India. We compared the nutrition content of ANC protocols against global recommendations. We used survey data to elucidate the coverage of micronutrient supplementation, weight gain monitoring, dietary and breastfeeding counselling. We reviewed literature, formative research and program assessments to identify barriers and enabling factors of service provision and maternal nutrition practices. Nutrition information in national policies and protocols was often fragmented, incomplete and did not consistently follow global recommendations. Nationally representative data on MNIs in ANC was inadequate, except for iron and folic acid supplementation. Coverage data from subnational surveys showed similar patterns of strengths and weaknesses. MNI coverage was consistently lower than ANC coverage with the lowest coverage of weight gain monitoring and variable coverage of dietary and breastfeeding counselling. Key common factors associated with coverage were micronutrient supply disruptions; suboptimal counselling on maternal diet, weight gain, and breastfeeding; and limited or no record keeping. Adherence of women to micronutrient supplementation and dietary recommendations was low and associated with late and too few ANC contacts, poor maternal knowledge and self‐efficacy, and insufficient family and community support. Models of comprehensive nutrition protocols and health systems that deliver maternal nutrition services in ANC are urgently needed along with national data systems to track progress. Key points Delivering nutrition services to all pregnant women is essential for maternal and child health outcomes but remains a

Sana Ben Slama, Dorra Ben Ghachem, Amen Dhaoui, Mohamed Taieb Jomni, Mohamed Hédi Dougui, Khadija Bellil

Sara Darhoua, Fatima-Zahra Belabbes, Hanane Delsa, Anass Nadi, Omar Bahlaoui, Wafaa Khannoussi, Imane Ben El Barhdadi

AbstractCircular replication-associated protein encoding single stranded (CRESS) deoxyribonucleic acid (DNA) viruses have emerged to become one of the largest and most devastating groups of crops-infecting viruses in tropical and subtropical regions of the world. In West Africa, renewed outbreaks of disease-like symptoms in the field have prompted surveys that revealed the circulation of multiple CRESS DNA virus species. Recognizing that plant viral diseases may be best understood at the ecosystem scale, we conducted a survey of plant-associated CRESS DNA virus communities in two localities of Burkina Faso (1068 plant samples). CRESS DNA viruses were detected in 41% and 13% of samples from cultivated and uncultivated plants, respectively. Virus community maps constructed from the 30 uncovered species of geminivirus and associated satellites revealed strong modularity. Whereas few co-infections involving viruses belonging to different host–virus modules were observed, evidence of recombination was nevertheless detected between viruses belonging to different modules. This suggests that co-infections between viruses in different modules does at least occasionally occur: possibly as a consequence of seasonal variations in virus–host community structures with the inter-cropping period in particular promoting such “inter-module” co-infections.

ABSTRACT Background: The global health transition is linked with an increased burden of non-communicable diseases with cardiovascular diseases leading the epidemic. In sub-Saharan Africa (SSA), the prevalence of obesity has increased during the past decades and there is a need to investigate the associated driving factors. In Burkina Faso obesity remains low, especially in rural areas. In this study we recruited middle-aged adults, as part of a larger study on genetic and environmental contributions to cardiometabolic disease among Africans. Objectives: To investigate the distribution of BMI and prevalence of obesity in a cross-sectional population-based study and to determine the sociodemographic and behavioural correlates with BMI. Methods: Participants (N = 2,076) were recruited from the Nanoro Health and Demographic Surveillance System area and were aged 40–60 years. We applied hierarchical modelling to identify factors associated with BMI and structural equation modelling to identify mediated effects of sociodemographic and behavioural variables on BMI. Results: Data are presented on 2,076 participants (49.9% female). Men had significantly higher BMI than women with medians of 21.1 (19.2 – 23.4) vs 19.8 (18.1 – 21.6) (p < 0.001), and there were significantly more underweight women compared to men (31.0% vs 17.4%) (p < 0.001). More men were overweight and obese than women (11.9% vs 5.2% and 2.2% vs 1.4%). Socioeconomic status was the major contributor to increased BMI for men, and education was the main contributor in women. Tobacco smoking and chewing, and problematic alcohol consumption were associated with a decrease in BMI in men and women. Conclusion: Overweight and obesity are relatively low among adults in rural Burkina Faso, and men had a higher median BMI than women. Behavioural factors, including tobacco use and alcohol consumption, contributed to a decrease in BMI, whereas socioeconomic status and education (which were both generally low in this commu
