Affangla Désiré Alain, Leye Mohamed, Dia Aliou Amadou, Ndiaye El Hadj Mohamed, Aw Fatou, Bazolo Georges Antoine, Kane Abdoul

Amine Chouchaine, Mahmoud Fodha, Mohamed Taha Abdelkefi, Kamel Helali, Mohamed Fodha

Aging partially restores the efficacy of malaria vector control in insecticide-resistant populations of Anopheles gambiae s.l. from Burkina Faso

Christopher M Jones, Antoine Sanou, Wamdaogo M Guelbeogo, N'Fale Sagnon, Paul CD Johnson, Hilary Ranson

BackgroundThe operational impact of insecticide resistance on the effectiveness of long-lasting insecticide nets (LLINs) and indoor residual spraying (IRS) is poorly understood. One factor which may prolong the effectiveness of these tools in the field is the increase in insecticide susceptibility with mosquito age. In this study, LLINs and IRS were tested against young (three to five days) and old (17-19 days) pyrethroid resistant Anopheles gambiae s.l. from Burkina Faso.MethodsBlood-fed adult Anopheles gambiae s.l. were collected from south-west Burkina Faso and identified to species/form level. Cohorts of the F1 progeny of An. gambiae s.s. S-forms were exposed to deltamethrin (0.05%) at three to five or 17-19 days post-emergence and tested for the frequency of the resistance allele 1014F. Isofemale lines of the M, S- form of An. gambiae s.s. and Anopheles arabiensis were exposed in WHO cone tests to either a) LLINs deployed in households for two years or (b) bendiocarb sprayed walls.ResultsMortality rates in response to deltamethrin (0.05%) increased from levels indicative of strong resistance in three to five day old F1 mosquitoes, to near full susceptibility in the 17-19 day old cohort. On exposure to LLINs sampled from the field, the mortality rate in isofemale lines was higher in older mosquitoes than young (OR = 5.28, CI 95% = 2.81-9.92), although the mortality estimates were affected by the LLIN tested. In general, the LLINs sampled from the field performed poorly in WHO cone bioassays using either laboratory susceptible or field caught mosquito populations. Finally, there was a clear relationship between mortality and age on exposure to bendiocarb-sprayed walls, with older mosquitoes again proving more susceptible (OR = 3.39, CI 95% = 2.35-4.90).ConclusionsAge is a key factor determining the susceptibility of mosquitoes to insecticides, not only in laboratory studies, but in response to field-based vector control interventions. This has important implicati

    Agressions sexuelles à Port-Gentil

Mohamed Maniboliot Soumah, Gladys Rita Olendo, Mor Ndiaye, Mamadou Lamine Sow

Agricultural productivity in relation to climate and cropland management in West Africa

    Altaaf Mechiche-Alami, Abdulhakim M. Abdi

The climate of West Africa is expected to become more arid due to increased temperature and uncertain rainfall regimes, while its population is expected to grow faster than the rest of the world. As such, increased demand for food will likely coincide with declines in agricultural production in a region where severe undernutrition already occurs. Here, we attempt to discriminate between the impacts of climate and other factors (e.g. land management/degradation) on crop production across West Africa using satellite remote sensing. We identify trends in the land surface phenology and climate of West African croplands between 2000 and 2018. Using the combination of a an attribution framework and residual trend anlaysis, we discriminate between climate and other impacts on crop productivity. The combined effect of rainfall, land surface temperature and solar radiation explains approximately 40% of the variation in cropland productivity over West Africa at the 95% significance level. The largest proportions of croplands with greening trends were observed in Mali, Niger and Burkina Faso, and the largest proportions with browning trends were in Nigeria, The Gambia and Benin. Climate was responsible for 52% of the greening trends and 25% of the browning trends. Within the other driving factors, changes in phenology explained 18% of the greening and 37% of the browning trends across the region, the use of inputs and irrigation explained 30% of the greening trends and land degradation 38% of the browning trends. These findings have implications for adaptation policies as we map out areas in need of improved land management practices and those where it has proven to be successful.

Aid cut, lives lost: estimating the impact of USAID’s withdrawal on maternal mortality in six African countries

    Matthew Cummins

AbstractIn January 2025, the US government suspended and subsequently terminated the majority of United States Agency for International Development (USAID) programs. This study estimates the impact of that decision on maternal mortality in six highly vulnerable countries in West and Central Africa: Burkina Faso, Central African Republic, Chad, Mali, Niger, and Nigeria. Using a deterministic model grounded in regional health expenditure elasticities, the analysis projects how the sudden withdrawal of foreign aid affects health spending among populations in humanitarian need, under the assumption that no immediate domestic or external financing substitutes for the lost resources, and the resulting changes in maternal mortality ratios (deaths per 100 000 live births). The results indicate that the funding cuts could cause maternal deaths to increase by 45%, on average, among populations in need. This increase is estimated relative to a baseline of approximately 2900 maternal deaths predicted in 2025, yielding approximately 1000 additional deaths across the countries within a single year. The magnitude of impact varies, with Niger experiencing the largest proportional increase (over 90%) and Nigeria the largest absolute increase (more than 300 additional deaths). Sensitivity analyses confirm that the results are robust to alternative elasticity scenarios. The findings illustrate the degree to which maternal health outcomes in fragile settings are sensitive to financing discontinuities. The results are presented as conditional estimates and are intended to inform ongoing discussions on health financing sustainability, transition planning, and risk mitigation.

Rajae Derrar, Nourredinne Boutimzine, Amina Laghmari, Amal Alouane, Rajae Daoudi

ALARUM: Active One Health surveillance in LMICs to monitor and predict Antimicrobial Resistance Using Metagenomics – a cross-sectional study protocol

Marianne A B van der Sande, Daniel Valia, Caroline Tigoi, Nicole Stoesser, Linda Stamm, Andrea Marten, Bram Riems, Robert Musyimi, Yougbare Sibidou, Anita C Schurch, Eric W Tiendrebeogo, Shalton Mwaringa, Malte Kohns Vasconcelos, Brecht Ingelbeen, Halidou Tinto, Julia A Bielicki, Ben S Cooper, James A Berkley, Esther van Kleef

AbstractBackgroundIn rural sub-Saharan Africa (sSA), the burden of antimicrobial resistance (AMR) remains high. As AMR continues to rise, there is a strong need for practical, implementable surveillance to monitor and mitigate risks, as well as inform timely, evidence-based clinical decision-making. Emerging evidence points to possible community-level drivers, such as transmission between human, animal and environmental reservoirs as contributing factors, yet microbiological surveillance or opportunities for wastewater-based surveillance are often limited and insufficient in these settings. Therefore, alternative sustainable and affordable approaches are needed. We intend to build on the demonstrated potential of metagenomic profiling of pooled faecal material, which accurately predicted population-level AMR prevalence in invasive Enterobacterales infections. Methods and analysisWe aim to validate this metagenomic pooled approach on additional populations, and to evaluate whether AMR patterns could be similarly predicted from surveillance of community One Health reservoirs. We will assemble existing data from hospital-based microbiology diagnostic laboratories in rural Burkina Faso and Kenya, and determine to what extent community-level metagenomic data, and/or faecal material of patients on hospital admission, can predict AMR in clinical isolates. We will perform community-level surveys in eight clusters per country, randomly selecting 15 households per cluster. We will systematically sample suspected environmental AMR exposure sites in and around households (soil, drinking water, latrines, chicken faeces) and collect data on community-level antibiotic use, hygiene practices, contact with domestic animals and sanitary facilities. Samples and data will be collected twice: during the dry and during the rainy season. In addition to evaluating the accuracy of predicting resistance in clinical isolates, we will quantify community-level exposure risks. We will conduct meta

Alcohol abstinence and drinking among African women: data from the World Health Surveys

Priscilla Martinez, Jo Røislien, Nirmala Naidoo, Thomas Clausen

BackgroundAlcohol use is increasing among women in Africa, and comparable information about women's current alcohol use is needed to inform national and international health policies relevant to the entire population. This study aimed to provide a comparative description of alcohol use among women across 20 African countries.MethodsData were collected as part of the WHO World Health Survey using standardized questionnaires. In total, 40,739 adult women were included in the present study. Alcohol measures included lifetime abstinence, current use (≥1 drink in previous week), heavy drinking (15+ drinks in the previous week) and risky single-occasion drinking (5+ drinks on at least one day in the previous week). Country-specific descriptives of alcohol use were calculated, and K-means clustering was performed to identify countries with similar characteristics. Multiple logistic regression models were fitted for each country to identify factors associated with drinking status.ResultsA total of 33,841 (81%) African women reported lifetime abstinence. Current use ranged from 1% in Malawi to 30% in Burkina Faso. Among current drinkers, heavy drinking varied between 4% in Ghana to 41% in Chad, and risky single-occasion drinking ranged from <1% in Mauritius to 58% in Chad. Increasing age was associated with increased odds of being a current drinker in about half of the countries.ConclusionsA variety of drinking patterns are present among African women with lifetime abstention the most common. Countries with hazardous consumption patterns require serious attention to mitigate alcohol-related harm. Some similarities in factors related to alcohol use can be identified between different African countries, although these are limited and highlight the contextual diversity of female drinking in Africa.

Alcohol consumption and associated risk factors in Burkina Faso: results of a population-based cross-sectional survey

Bruno Bonnechère, Sékou Samadoulougou, Kadari Cisse, Souleymane Tassembedo, Seni Kouanda, Fati Kirakoya-Samadoulougou

ObjectivesLifestyle modifiable risk factors are a leading preventable cause of non-communicable diseases, with alcohol consumption among the most important. Studies characterising the prevalence of alcohol consumption in low-income countries are lacking. This study describes the prevalence of different levels of alcohol consumption in Burkina Faso and its associated factors.DesignData from the 2013 Burkina Faso WHO STEPwise Approach to Surveillance survey were analysed. The prevalence of alcohol consumption over the last 30 days was recoded into categories according to WHO recommendations: low, mid or abusive alcohol consumption. Multinomial logistic regression analyses identified factors associated with the different levels of alcohol consumption.SettingPopulation-based cross-sectional survey in Burkina Faso.Participants4692 participants of both sexes aged 25–64 years were included in the study.ResultsIn the whole sample, 3559 participants (75.8% (72.5%–78.7%)) were not consuming any alcohol, 614 (12.9% (10.9%–15.3%)) had low alcohol consumption, 399 (8.5% (7.1%–10.1%)) had mid alcohol consumption and 120 (2.7% (2.0%–3.7%)) had abusive consumption. Age was associated with alcohol intake with a gradient effect and older people having a higher level of consumption (adjusted OR (AOR): 2.36, 95% CI (1.59 to 3.51) for low consumption, 2.50 (1.54 to 4.07) for mid consumption and 2.37 (1.01 to 5.92) for abusive consumption in comparison with no consumption). Tobacco consumption was also significantly associated with alcohol intake with a gradient effect, those with higher tobacco consumption being at higher risk of abusive alcohol intake (AOR: 6.08 (2.75 to 13.4) for moderate consumption and 6.58 (1.96 to 22.11) for abusive consumption).ConclusionOur data showed an important burden of alcohol consumption in Burkina Faso, which varied with age and tobacco use. To effectively reduce alcohol consumption in Burkina Faso, comprehensive control and prevention campaigns should c

Algorithms for sequential interpretation of a malaria rapid diagnostic test detecting two different targets of Plasmodium species to improve diagnostic accuracy in a rural setting (Nanoro, Burkina Faso)

BackgroundMalaria rapid diagnostic tests (RDT) have limitations due to the persistence of histidine-rich protein 2 (HRP2) antigen after treatment and low sensitivity of Plasmodium lactate dehydrogenase (pLDH) based RDTs. To improve the diagnosis of malaria in febrile children, two diagnostic algorithms, based on sequential interpretation of a malaria rapid diagnostic test detecting two different targets of Plasmodium species and followed by expert microscopy, were evaluated.MethodsTwo diagnostic algorithms were evaluated using 407 blood samples collected between April and October 2016 from febrile children and the diagnostic accuracy of both algorithms was determined. Algorithm 1: The result of line T1-HRP2 were read first; if negative, malaria infection was considered to be absent. If positive, confirmation was done with the line T2-pLDH. If T2-pLDH test was negative, the malaria diagnosis was considered as “inconclusive” and microscopy was performed; Algorithm 2: The result of line T2-pLDH were read first; if positive, malaria infection was considered to be present. If negative, confirmation was done with the line T1-HRP2. If T1-HRP2 was positive the malaria diagnosis was considered as “inconclusive” and microscopy was performed. In absence of malaria microscopy, a malaria infection was ruled out in children with an inconclusive diagnostic test result when previous antimalarial treatment was reported.ResultsFor single interpretation, the sensitivity of PfHRP2 was 98.4% and the specificity was 74.2%, and for the pLDH test the sensitivity was 89.3% and the specificity was 98.8%. Malaria was accurately diagnosed using both algorithms in 84.5% children. The algorithms with the two-line malaria RDT classified the test results into two groups: conclusive and inconclusive results. The diagnostic accuracy for conclusive results was 98.3% using diagnostic algorithm 1 and 98.6% using algorithm 2. The sensitivity and specificity for the conclusive results were 98.2% and 98.4

ALIMUS—We are feeding! Study protocol of a multi-center, cluster-randomized controlled trial on the effects of a home garden and nutrition counseling intervention to reduce child undernutrition in rural Burkina Faso and Kenya

Isabel Mank, Raissa Sorgho, Fanta Zerbo, Moubassira Kagoné, Boubacar Coulibaly, John Oguso, Michael Mbata, Sammy Khagayi, Erick M. O. Muok, Ali Sié, Ina Danquah

BackgroundClimate change heavily affects child nutritional status in sub-Saharan Africa. Agricultural and dietary diversification are promising tools to balance agricultural yield losses and nutrient deficits in crops. However, rigorous impact evaluation of such adaptation strategies is lacking. This project will determine the potential of an integrated home gardening and nutrition counseling program as one possible climate change adaptation strategy to improve child health in rural Burkina Faso and Kenya.MethodsBased on careful co-design with stakeholders and beneficiaries, we conduct a multi-center, cluster-randomized controlled trial with 2 × 600 households in North-Western Burkina Faso and in South-Eastern Kenya. We recruit households with children at the age of complementary feed introduction (6–24 months) and with access to water sources. The intervention comprises the bio-diversification of horticultural home gardens and nutritional health counseling, using the 7 Essential Nutrition Action messages by the World Health Organization. After 12-months of follow-up, we will determine the intervention effect on the primary health outcome height-for-age z-score, using multi-level mixed models in an intention-to-treat approach. Secondary outcomes comprise other anthropometric indices, iron and zinc status, dietary behavior, malaria indicators, and household socioeconomic status.DiscussionThis project will establish the potential of a home gardening and nutrition counseling program to counteract climate change-related quantitative and qualitative agricultural losses, thereby improving the nutritional status among young children in rural sub-Saharan Africa.Trial registrationGerman Clinical Trials Register (DRKS) DRKS00019076.

Léon Kabamba Ngombe, Ignace Bwana Kangulu, Michel Kabamba Nzaji
