Conclusion : Cette analyse sur donnée

Il faudrait i

Abstract.Seasonal malaria chemoprevention (SMC) for children < 5 is a strategy that is gaining popularity in West African countries. Although its efficacy to reduce malaria incidence has been demonstrated in trials, the effects of SMC implemented in routine program conditions, outside of experimental contexts, are unknown. In 2014 and 2015, a survey was conducted in 1,311 households located in Kaya District (Burkina Faso) where SMC had been recently introduced. All children < 72 months were tested for malaria and anemia. A pre–post study with control group was designed to measure SMC impact during high transmission season. A difference-in-differences approach was coupled in the analysis with propensity score weighting to control for observable and time-invariant nonobservable confounding factors. SMC reduced the parasitemia point and period prevalence by 3.3 and 24% points, respectively; this translated into protective effects of 51% and 62%. SMC also reduced the likelihood of having moderate to severe anemia by 32%, and history of recent fever by 46%. Self-reported coverage for children at the first cycle was 83%. The SMC program was successfully added to a package of interventions already in place. To our knowledge, with prevalence < 10% during the peak of the transmission season, this is the first time that malaria can be reported as hypo-endemic in a sub-Sahelian setting in Burkina Faso. SMC has great potential, and along with other interventions, it could contribute to approaching the threshold where elimination strategies will be envisioned in Burkina Faso.

BackgroundSeveral studies highlighted the impact of community-based interventions whose purpose was to reduce the vectors’ breeding sites. These strategies are particularly interesting in low-and-middle-income countries which may find it difficult to sustainably assume the cost of insecticide-based interventions. In this case study we determine the spatial distribution of a community-based intervention for dengue vector control using different entomological indices. The objective was to evaluate locally where the intervention was most effective, using spatial analysis methods that are too often neglected in impact assessments.MethodsTwo neighbourhoods, Tampouy and Juvenat in Ouagadougou, Burkina Faso, were chosen among five after a survey was conducted, as part of an assessment related to the burden of dengue. As part of the community-based intervention conducted in Tampouy between August and early October 2016, an entomological survey was implemented in two phases. The first phase consisted of a baseline entomological characterization of potential breeding sites in the neighbourhood of Tampouy as well as in Juvenat, the control area. This phase was conducted in October 2015 at the end of the rainy season. The mosquito breeding sites were screened in randomly selected houses: 206 in Tampouy and 203 in Juvenat. A second phase took place after the intervention, in October 2016. The mosquito breeding sites were investigated in the same yards as during the baseline phase. We performed several entomological analyses to measure site productivity as well as before and after analysis using multilevel linear regression. We used Local Indicators of Spatial Association (LISAs) to analyse spatial concentrations of larvae.ResultsAfter the intervention, it is noted that LISAs at Tampouy reveal few aggregates of all types and the suppression of those existing before the intervention. The analysis therefore reveals that the intervention made it possible to reduce the number of conc

BackgroundThe management of acute febrile illnesses places a heavy burden on clinical services in many low- and middle-income countries (LMICs). Bacterial and viral aetiologies of acute fevers are often clinically indistinguishable and, in the absence of diagnostic tests, the ‘just-in-case’ use of antibiotics by many health workers has become common practice, which has an impact on drug-resistant infections. Our study aims to answer the following question: in patients with undifferentiated febrile illness presenting to outpatient clinics/peripheral health centres in LMICs, can we demonstrate an improvement in clinical outcomes and reduce unnecessary antibiotic prescription over current practice by using a combination of simple, accurate diagnostic tests, clinical algorithms, and training and communication (intervention package)?MethodsWe designed a randomized, controlled clinical trial to evaluate the impact of our intervention package on clinical outcomes and antibiotic prescription rates in acute febrile illnesses. Available, point-of-care, pathogen-specific and non-pathogen specific (host markers), rapid diagnostic tests (RDTs) included in the intervention package were selected based on pre-defined criteria. Nine clinical study sites in six countries (Burkina Faso, Ghana, India, Myanmar, Nepal and Uganda), which represent heterogeneous outpatient care settings, were selected. We considered the expected seasonal variations in the incidence of acute febrile illnesses across all the sites by ensuring a recruitment period of 12 months. A master protocol was developed and adapted for country-specific ethical submissions. Diagnostic algorithms and choice of RDTs acknowledged current data on aetiologies of acute febrile illnesses in each country. We included a qualitative evaluation of drivers and/or deterrents of uptake of new diagnostics and antibiotic use for acute febrile illnesses. Sample size estimations were based on historical site data of antibiotic prescription

According to the World Health Organization, air pollution is responsible for 90 % of deaths in Africa. However, limited data on exposure to air pollution is available, and studies are rare, particularly in French-speaking Africa. This study aims to investigate the impact of air pollution on mortality in 12 French-speaking African countries (Algeria, Burkina Faso, Burundi, Cameroon, Guinea, Ivory Coast, Madagascar, Mali, Morocco, Democratic Republic of Congo, Senegal, Tunisia). Using data from the Institute for Health Metrics and Evaluation (IHME), annual concentrations of the PM2.5 pollutant from different cities were integrated into a spatial interpolation model (IDW) at the scale of each country. The interpolation was validated using cross-validation models. For each of the considered cardiorespiratory diseases (LRI, stroke, COPD, IHD), the attributable mortality fraction was estimated using literature data and population exposure calculated using demographic data from each country. Large variations in ambient PM2.5 concentration between the 12 countries are observed, with concentrations ranging from 1.76 μg/m3 in Morocco to 64.99 μg/m3 in Cameroon. Concentrations are higher in West Africa than in Central or North Africa. In 2019, 136 457 deaths attributable to air pollution were recorded in the 12 countries: 40.8 % from ischemic heart disease, 38.5 % from stroke, 13.2 % from lower respiratory infections and 7.5 % from chronic obstructive pulmonary disease. Our model allowed us to obtain a spatial distribution and the number of deaths related to air pollution. However, the estimation of the health impact from air pollution could be improved by more systematic and comprehensive local exposure assessments from a robust air quality monitoring system. Highlights•Study estimates over 136 000 deaths from air pollution in 2019 across 12 countries.•GIS methods reveal spatial PM2.5 distribution in 12 French-speaking African countries.•West Africa has higher PM2.5 annual co

Studies about the impact of future climate change on diseases have mostly focused on standard Representative Concentration Pathway climate change scenarios. These scenarios do not account for the non-linear dynamics of the climate system. A rapid ice-sheet melting could occur, impacting climate and consequently societies. Here, we investigate the additional impact of a rapid ice-sheet melting of Greenland on climate and malaria transmission in Africa using several malaria models driven by Institute Pierre Simon Laplace climate simulations. Results reveal that our melting scenario could moderate the simulated increase in malaria risk over East Africa, due to cooling and drying effects, cause a largest decrease in malaria transmission risk over West Africa and drive malaria emergence in southern Africa associated with a significant southward shift of the African rain-belt. We argue that the effect of such ice-sheet melting should be investigated further in future public health and agriculture climate change risk assessments. Release of freshwater into the oceans as a result of ice sheet melting could impact the distribution of climate-sensitive diseases. Here, the authors show that a rapid ice sheet melting in Greenland could cause an emergence of malaria in Southern Africa whilst transmission risks in West Africa may decline.

BackgroundTsetse flies are the sole vectors of human and animal trypanosomosis. In Burkina Faso, a project aiming to create zones free of tsetse flies and trypanosomosis was executed from June 2006 to December 2013. After the determination of tsetse distribution in the intervention area from December 2007 to November 2008, the control campaign was launched in November 2009 and ended in December 2013. The goal was to eliminate tsetse flies from 40,000 km2 of area, through an integrated control campaign including insecticide targets, traps and cattle, sequential aerial treatment (SAT) and the mass treatment of livestock using trypanocides. The campaign involved assistance of the beneficiary communities at all the steps of the control strategy with insecticide impregnated targets.MethodsThis study was carried out to assess the impact of the control project on tsetse apparent density per trap per day (ADT). To evaluate the effectiveness of tsetse control, 201 sites were selected based on the baseline survey results carried out from December 2007 to November 2008. These sites were monitored bi-monthly from January 2010 to November 2012. At the end-of-study in 2013 a generalized entomological survey was carried out in 401 infested sites found during the longitudinal survey done before the control. Barrier and tsetse persistence areas were treated by ground spraying and evaluated. Controls were also done before and after aerial spraying.ResultsIn the insecticide-impregnated target area, the control showed that ADT of tsetse flies declined from 10.73 (SD 13.27) to 0.43 (SD 2.51) fly/trap/day from the third month of campaign onwards (P < 0.0001) and remained low thereafter. At the end of the campaign in 2013, an 83% reduction of ADT was observed for Glossina palpalis gambiensis and a 92% reduction for G. tachinoides. Tsetse flies were captured only in 29% of the sites found infested in 2008.ConclusionsTsetse flies could be suppressed efficiently but their elimination from th

ObjectiveAnaemia is increasingly being described as a negative predictor of outcome after myocardial infarction. The objective of our study was to assess the prognosis post myocardial infarction in the short and medium term in black Africans with chronic anaemia.MethodsWe carried out a comparative case-control study on 272 patients (93 anaemic and 179 non-anaemic) hospitalised for myocardial infarction at the Cardiology Institute of Abidjan. One group included 93 patients who presented with anaemia concurrent with the myocardial infarction (haemoglobin level low: 13 g/dl for males and 12 g/dl for females, respectively). The other group comprised 172 patients who presented without anaemia during the acute phase of myocardial infarction. The haemoglobin rate was measured at admission, as were the biological markers of myocardial infarction.ResultsThe mean age was 53.5 years for the anaemic patients and 52.6 years for the non-anaemic patients. We noticed a clear male predominance in both populations (81.7 vs 78.8%; p = 0.56). The mean haemoglobin level was lower in the anaemic patients compared to that in the non-anaemic patients (10.2 vs 15 g/dl). The anaemic patients were eight times more at risk for an unfavourable outcome (complications or death) compared to the non-anaemic patients (91.4 vs 57%; OR = 8.02; 95% CI: 3.5–19.07; chi2 = 33.74; p < 0.0001). The anaemic patients were 3.7 times more at risk for right ventricular failure (NYHA class II and III) compared to the control population (69.9 vs 38.5%; OR = 3.7; 95% CI: 08–6.60; chi2 = 24.06; p < 0.0001) and six times more at risk for cardiogenic shock (24.7 vs 5.3%; OR = 6.21; 95% CI: 2.56–15.43; chi2 = 22.89; p < 0.0001). The mortality rate was significantly higher in the anaemic than the non-anaemic patients (35.5 vs 12.8%; OR = 3.73; 95% CI: 1.94–7.19; chi2 = 19.18; p < 0.0001).ConclusionAnaemia is an independent risk factor for a poor prognosis during the acute phase of myocardial infarction in black Africans

Background: Malaria remains a climate-driven public health issue in Burkina Faso, yet the interactions between climatic factors and malaria interventions across different zones are not well understood. This study estimates time delays in the effects of climatic factors on malaria incidence, develops forecasting models, and assesses their short-term forecasting performance across three distinct climatic zones: the Sahelian zone (hot/arid), the Sudano-Sahelian zone (moderate temperatures/rainfall); and the Sudanian zone (cooler/wet). Methods: Monthly confirmed malaria cases of children under five during the period 2015–2021 were analyzed using Bayesian generalized autoregressive moving average negative binomial models. The predictors included land surface temperature (LST), rainfall, the coverage of insecticide-treated net (ITN) use, and the coverage of artemisinin-based combination therapies (ACTs). Bayesian variable selection was used to identify the time delays between climatic suitability and malaria incidence. Wavelet analysis was conducted to understand better how fluctuations in climatic factors across different time scales and climatic zones affect malaria transmission dynamics. Results: Malaria incidence averaged 9.92 cases per 1000 persons per month from 2015 to 2021, with peak incidences in July and October in the cooler/wet zone and October in the other zones. Periodicities at 6-month and 12-month intervals were identified in malaria incidence and LST and at 12 months for rainfall from 2015 to 2021 in all climatic zones. Varying lag times in the effects of climatic factors were identified across the zones. The highest predictive power was observed at lead times of 3 months in the cooler/wet zone, followed by 2 months in the hot/arid and moderate zones. Forecasting accuracy, measured by the mean absolute percentage error (MAPE), varied across the zones: 28% in the cooler/wet zone, 53% in the moderate zone, and 45% in the hot/arid zone. ITNs were not statist

BackgroundThe widespread emergence of insecticide resistance in malaria vectors, particularly Anopheles gambiae, poses a significant challenge to malaria control in sub-Saharan Africa. In Burkina Faso, where pyrethroid resistance is driven by both target-site mutations and enhanced metabolic detoxification, the efficacy of key vector control tools such as insecticide-treated nets (ITNs) and indoor residual spraying (IRS) has declined. In response, alternative non-pyrethroid interventions are being implemented. This study investigated the impact of non-pyrethroid IRS and piperonyl butoxide (PBO)-synergist pyrethroid ITNs on insecticide resistance dynamics in An. gambiae populations in Kampti and Gaoua, southwestern Burkina Faso.MethodsAnopheles gambiae larvae were collected annually from 2021 to 2023, between the months of August and September. Adult susceptibility to pyrethroids insecticides was assessed using WHO tube assays. Molecular assays were used for species identification within the An. gambiae complex and detection of the kdr mutations. Expression of metabolic resistance genes was quantified using qPCR. KD60 and mortality rates were compared across intervention: during IRS use (2021), after its replacement with PBO-ITNs (2022), and after 2 years of PBO-ITN implementation (2023).ResultsThree Anopheles species were identified, 397 An. gambiae sensu stricto (66.17%; 95%CI 62.30–69.80) 149 Anopheles arabiensis (; 24.83%; 95%CI 21.50–28.40) and 54; Anopheles coluzzii (9%; 95CI 6.96–11.60). with An. gambiae being predominant. The L1014F mutation was highly prevalent, while L1014S was undetected. Following the withdrawal of IRS and the introduction of PBO-ITNs in 2022, pyrethroid resistance intensified in Kampti, with reduced mortality to permethrin and alphacypermethrin. PBO pre-exposure partially restored deltamethrin susceptibility in Kampti and fully restored it in Gaoua. By 2023, resistance remained high, particularly to alphacypermethrin, and PBO’s synergist

ABSTRACT.Coronavirus disease 2019 (COVID-19) can have far-reaching consequences for developing countries through the combined effects of infection and mortality, and the mitigation measures that can impact food systems and diets. Using a mobile platform, this cross-sectional study evaluated the effect of COVID-19 on food prices and dietary quality for 1797 households in Nouna and Ouagadougou in Burkina Faso, Addis Ababa and Kersa in Ethiopia, and Lagos and Ibadan in Nigeria. We assessed the consumption of 20 food groups during the previous 7 days. The dietary diversity scores (DDS) and Prime Diet Quality Scores (PDQS) were used to assess dietary diversity and quality. We used generalized estimating equation (GEE) linear models to evaluate associations between price changes for staples, pulses, vegetables, fruits, and animal source foods (ASFs) with the DDS and PDQS PDQS. Most participants reported increasing prices of staples, pulses, fruits, vegetables and ASF, and ≥ 40% reported the decreased consumption of staples, legumes, and other vegetables and fruits. The DDS (except in Kersa and Ouagadougou) and PDQS were lower during the COVID-19 pandemic. Higher pulse prices were associated with lower DDS (estimate, −0.35; 95% confidence interval [CI], −0.74 to 0.03; P = 0.07) in the combined analysis and in Burkina Faso (estimate, −0.47; 95% CI, −0.82 to −0.11). Higher vegetable prices were positively associated with the DDS (estimate, 0.22; 95% CI, 0.08 to 0.37). Lower crop production (estimate, −0.54; 95% CI, −0.80 to −0.27) was associated with lower DDS. The price increases and worsening dietary diversity and quality call for social protection and other strategies to increase the availability and affordability of nutrient-rich foods during the COVID-19 pandemic and public health emergencies.

ABSTRACT.Although the numbers of SARS–CoV-2 infections and related deaths are relatively low in sub-Saharan Africa, the pandemic might lead to a high indirect death toll there. We determined the impact of the COVID-19 pandemic on the management of malnourished children in urban and rural areas. We analyzed data from two Centers for Rehabilitation, Education & Nutrition (CRENs), one in the capital and one in a rural center, both run by the Camillian Fathers. We compared data from the year before the pandemic (2019) with the first 2 years during the pandemic (2020/2021). In the urban CREN, there was a sharp reduction in new patients enrolled, from 340 in the pre-pandemic year to 189 during the first pandemic year and 202 in the second year. The follow-up was significantly shorter during the first pandemic year, with a rebound in the second year (pre: 57 days versus 42 and 63 days for the first and second years, respectively). In the rural CREN, the situation was different: The numbers of patients did not show any significant variation between the pre-pandemic year (191) and the first and second pandemic years (223 and 179, respectively). Different perceptions of the pandemic in urban (high, more testing, more COVID) and rural (low, less information and testing) areas may partly explain this difference. The discrepancy between the decreasing numbers of malnourished children in specialized care during the pandemic—especially in the urban area—is contrary to the lockdown-induced increase in food insecurity and warrants attention to avoid an increase in the silent epidemic of malnourished children in Africa.

BackgroundCOVID-19 pandemic has put tremendous burden on health services. Only limited evidence, however, is available to identify the impact of COVID-19 on sexual and reproductive health (SRH). This world health organization (WHO)-led research sought to evaluate health systems focusing on SRH in Brazil, Burkina Faso, China, Ghana, Italy, Pakistan, Thailand, and the United Kingdom.MethodsThe study was conducted in Thailand on two levels using a mixed-methods design: 1) Individual level included in-depth interviews and focus group discussions with clients (and their partners, where applicable) and healthcare providers (HCPs) to investigate service perceptions and obstacles to SRH service utilization; 2) health facility level, a quantitative evaluation of health facility preparedness for SRH service provision was performed using an adapted version of the WHO Service Availability and Readiness Assessment (SARA) tool. The data was collected at two timepoints, baseline and endline, at time intervals of 9 months.ResultsAlmost all SRH services were maintained with some shortage of supply such as medications for safe abortion during the first few months. Both providers and clients perceived that all SRH services should be maintained. Some clients were concerned about fear of getting COVID-19 infection while visiting the facility. Some clients switched from short-acting to long-acting contraceptive methods. At the endline, this affect was less obvious since a large proportion of clients were familiar with the pandemic and already received vaccination. The Centre for COVID-19 Situation Administration (CCSA) was established to update COVID-19 pandemic situation, new government policy and intervention to reduce fake news. Telemedicine was used to reduce avoidable appointments. For postpartum women, appointment tended to be more individualized. For those who required pregnancy protection, contraceptive methods were offered to clients before discharge from the hospital. A follow-u

Background:Childhood immunisation services have been disrupted by the COVID-19 pandemic. WHO recommends considering outbreak risk using epidemiological criteria when deciding whether to conduct preventive vaccination campaigns during the pandemic. Methods:We used two to three models per infection to estimate the health impact of 50% reduced routine vaccination coverage in 2020 and delay of campaign vaccination from 2020 to 2021 for measles vaccination in Bangladesh, Chad, Ethiopia, Kenya, Nigeria, and South Sudan, for meningococcal A vaccination in Burkina Faso, Chad, Niger, and Nigeria, and for yellow fever vaccination in the Democratic Republic of Congo, Ghana, and Nigeria. Our counterfactual comparative scenario was sustaining immunisation services at coverage projections made prior to COVID-19 (i.e. without any disruption). Results:Reduced routine vaccination coverage in 2020 without catch-up vaccination may lead to an increase in measles and yellow fever disease burden in the modelled countries. Delaying planned campaigns in Ethiopia and Nigeria by a year may significantly increase the risk of measles outbreaks (both countries did complete their supplementary immunisation activities (SIAs) planned for 2020). For yellow fever vaccination, delay in campaigns leads to a potential disease burden rise of >1 death per 100,000 people per year until the campaigns are implemented. For meningococcal A vaccination, short-term disruptions in 2020 are unlikely to have a significant impact due to the persistence of direct and indirect benefits from past introductory campaigns of the 1- to 29-year-old population, bolstered by inclusion of the vaccine into the routine immunisation schedule accompanied by further catch-up campaigns. Conclusions:The impact of COVID-19-related disruption to vaccination programs varies between infections and countries. Planning and implementation of campaigns should consider country and infection-specific epidemiological factors and local immunity ga
