La tuberculose sternale: à propos de 2 cas

Aziz Ouarssani, Fouad Atoini, Fatima Ait Lhou, Mustapha Idrissi Rguibi

Ahlem Bchir, Ahlem Bdioui, Sarra Mestiri, Samia Ayachi, Hbib Khochtali, Sihem Hmissa, Moncef Mokni

    Youssef Kadouri, Yassine Nouini

Buschke-Lowenstein’ tumor (BLT) or giant condylomata acuminata (GCA) is a pseudo-epithelial proliferation belonging to the verrucous carcinomas. It was first described in 1896. In 1925 Buschke and Lowenstein showed that it was characterized by disease-specific features. It is caused by human papilloma virus (HPV) a sexually transmitted infection mainly affecting the anogenital region. It can result in progressive degeneration and recur after treatment. Buschke-Lowenstein’s tumor is relatively rare and it is always preceded by condylomata acuminata. Its annual incidence appears to be 0.1% among the sexually active adult population. It can occur at any age after puberty but mainly between the 4thand 6thdecades. The infection can affect both sexes but it most commonly occurs in males. Development, persistence, and recurrence rates of condylomas largely depend on the host’s immune status. Immunodepression, chronic inflammation, lack of hygiene and HIV infection appear to be risk factors for this disease. BLT is known to be caused by papillomavirus and, in particular, by HPV serotypes 6, 11. Prevention is essential and it should be based on the treatment of condylomata acuminata and the battle against sexually transmitted diseases. Early wide excision is mandatory. We here report the case of a 55-year old patient with a history of sexual vagrancy and recurrent urethritis, presenting with penile tumor progressing over the last 8 years. Clinical examination showed several fetid, painless, invasive, cauliflower papillomatous tumor lesions involving the root and ventral face of the penis. Lymph nodes were free. Serological tests for HIV, syphilis and hepatitis B and C were negative. Treatment was based on surgical excision with skin coverage using adjacent skin. Histological examination of the surgical specimen showed acute papillomatous hyperplasia of the epidermis and some koilocytes suggesting giant condyloma. At 2-year follow-up the patient didn’t show any recurrence.

Noureddine Njoumi, Mohamed Tarchouli, Moulay Brahim Ratbi, Mohamed Reda Elochi, Rajae Yamoul, Hafid Hachi, Abdesslam Bougtab

Majdouline Boujoual, Mariam Amrani, Abdesslam Bougtab

Mohamed Amine Mahraoui, Merouane Abouchane, Yassir El Andaloussi, Ahmed Reda Haddoun, Mohamed Nechad

Anass Ayyad, Maria Rkain, Abdeladim Babakhouya, Noufissa Benajiba

Jihène Methlouthi, Nabiha Mahdhaoui, Manel Bellalah, Hedia Ayache, Sonia Nouri, Hassen Seboui

Mohamed Rami, Achraf El Bakkaly, Jaouad Bouljrouf, Toualouth Lafia, Mohammed Amine Bouhafs, Rachid Belkacem

    Jean-Pierre Fina-Lubaki, Williams R. Phillips, Klaus B. von Pressentin

The French version of the CRISP checklist: A practical tool for publishing primary care research in French. This editorial introduces the French version of the Consensus Reporting Items for Studies in Primary Care (CRISP) checklist, a reporting tool developed to improve the quality, relevance, and transparency of primary care publications. It notes that primary care research has unique methods, populations, and health system contexts, and thus needs tailored reporting guidance. The article describes the checklist’s development via international consensus and its flexibility for quantitative, qualitative, and mixed-methods research. The checklist assists authors, reviewers, editors, teachers, and study planners. The editorial stresses the importance of a French version, given French’s global presence, especially in Africa, while English dominates scientific publishing. This language barrier hinders French-speaking researchers. Offering a French version improves access and supports clearer communication of research in Francophone and African settings. The editorial concludes that the French CRISP checklist strengthens reporting and helps to interpret and apply primary care research findings.

Sylvie Linsuke, Gisèle Nabazungu, Gillon Ilombe, Steve Ahuka, Jean-Jacques Muyembe, Pascal Lutumba

Laboratory An. gambiae s.l. mosquito colonies show sustained high transmission of Microsporidia sp. MB and a small decrease in egg viability

Beth Crawford Poulton, Deepak-Kumar Purusothaman, Abdelhakeem Ismail Adam, Issiaka Sare, Ewan R. S. Parry, Roland Pevsner, Thomas H. Ant, Etienne Bilgo, Abdoulaye Diabaté, Steven Paul Sinkins

IntroductionMicrosporidia sp. MB, a microsporidian symbiont found naturally in Anopheles mosquitoes, has potential as a novel malaria control tool since it can inhibit Plasmodium development and transmission. The most feasible MB-based Plasmodium control strategy would involve dissemination through live mosquito releases, or release of spores infective to mosquito larvae. To implement either strategy, establishment of stable mosquito colonies carrying MB at a high frequency is likely to be essential.MethodsThe progeny of field caught An. gambiae s.l from Burkina Faso were isolated for individual egg laying and tested for MB. The progeny of the MB positive females were pooled and this process was repeated for multiple generations. The relative density of MB in different life stages and tissues of the An. coluzzii host was examined using a novel duplex qPCR assay. We also examined the impact of MB on egg production through individualization for egg laying and counting of eggs. Finally, we examined laid eggs for presence of MB spores.ResultsThree An. coluzzii colonies and one An. gambiae s.l hybrid colony were established with high prevalence and density of MB and were maintained for more than two years with minimal intervention. MB prevalence and density was highest in eggs and adult females and lowest in L4 larvae; in adults density was highest in the gonads. Additionally, MB density increased in ovary samples following blood feeding which was likely due to the activation of sporogony.DiscussionThe production of spores is the reason why MB-carrying females lay more white non-hatching eggs and show a small reduction in the median number of eggs laid. Establishment of several stable MB carrying An. gambiae s.l colonies and understanding the impact of spores on egg production are significant steps forward in developing MB as a malaria control tool.

Laboratory and experimental hut trial evaluation of VECTRON ™ T500 for indoor residual spraying (IRS) against insecticide resistant malaria vectors in Burkina Faso

Koama Bayili, Hyacinthe D. Ki, Bazoma Bayili, Bazoumana Sow, Abdoulaye Ouattara, Graham Small, Aristide S. Hien, Roch K. Dabire, Abdoulaye Diabate

Background: Malaria cases in some areas could be attributed to vector resistant to the insecticide. World Health Organization recommended insecticides for vector control are limited in number. It is essential to find rotational partners for existing Indoor Residual Spraying (IRS) products. VECTRON ™ T500 is a novel insecticide with broflanilide as active ingredient. It has a mode of action on mosquitoes completely different to usually used. The aim of this study was to determine the optimum effective dose and efficacy of VECTRON TM T500 against susceptible and resistant strains of Anopheles in Burkina Faso. Methods: VECTRON™T500 was sprayed at 50, 100 and 200 mg/m² doses onto mud and concrete blocks using Potter Spray Tower. The residual activity of broflanilide was assessed through cone bioassays 1 week and then monthly up to 14 months post spraying. Its efficacy was evaluated at 100 and 150 mg/m² against wild free-flying mosquitoes in experimental huts on both substrates. Actellic 300CS was applied at 1000 mg/m² as reference product. Cone assays were conducted monthly, using susceptible and resistant mosquito strains. Results: In the laboratory, VECTRON ™ T500 showed residual efficacy (≥80% mortality) on An. gambiae Kisumu up to 12 and 14 months, respectively, on concrete and mud blocks. Similar results were found with 100 and 200 mg/m² using An. coluzzii pyrethroid resistant strain. In experimental huts, a total of 19,552 An. gambiae s.l. were collected. Deterrence, blood-feeding inhibition and exophily with VECTRON™ treated huts were very low. At 100 and 150 mg/m², mortality of wild An. gambiae s.l. ranged between 55% and 73%. Monthly cone bioassay mortality remained >80% up to 9 months. Conclusions: VECTRON™ T500 shows great potential as IRS formulation for malaria vector control. It can be added to the arsenal of IRS products for use in rotations to control malaria and manage mosquito insecticide resistance.

Laboratory and field evaluation of MAA, an ointment containing N,N-diethyl-3-methylbenzamide (DEET) against mosquitoes in Burkina Faso

Alphonse Traor, Grard Niyondiko, Antoine Sanou, Franck Langevin, NFal Sagnon, Adama Gansan, Moussa Wamdaogo Guelbeogo

BackgroundMalaria vector control relies upon the use of insecticide-treated nets and indoor residual spraying. However, as the emergency of insecticide resistance in malaria vectors grows, the effectiveness of these measures could be limited. Alternative tools are needed. In this context, repellents can play an important role against exophagic and exophilic mosquitoes. This study evaluated the efficacy of MAA, a novel repellent ointment, in laboratory and field conditions in Burkina Faso.MethodsFor laboratory and field assessment, 20 volunteers were enrolled and trained for nocturnal collection of mosquitoes using human landing catches (HLC). In the laboratory tests, 2mg/sq cm of treatment (either MAIA or 20% DEET) were used to assess median complete protection time (CPT) against two species: Anopheles gambiae and Aedes aegypti, following WHO guidelines. For both species, two strains consisting of susceptible and local strains were used. The susceptible strains were Kisumu and Bora Bora for An. gambiae and Ae. aegypti, respectively. For the field test, the median CPT of MAA was compared to that of a negative (70% ethanol) and positive (20% DEET) after carrying out HLCs in rural Burkina Faso in both indoor and outdoor settings.ResultsLaboratory tests showed median Kaplan-Meier CPT of 6h 30min for An. gambiae (Kisumu), 5h 30min for An. gambiae (Goden, local strain), and 4h for Ae. aegypti for both the local and sensitive strain. These laboratory results suggest that MAA is a good repellent against the three mosquito species. During these field tests, a total of 3979 mosquitoes were caught. In this population, anophelines represented 98.5%, with culicines (Aedes) making up the remaining 1.5%. Among anopheline mosquitoes, 95% belonged to the An. gambiae complex, followed by Anopheles funestus and Anopheles pharoensis. The median CPT of 20% DEET and MAA were similar (8h) and much longer than that of the negative control (2h).ConclusionsResults from the present studies sh

Lack of Association of CD55 Receptor Genetic Variants and Severe Malaria in Ghanaian Children

Kathrin Schuldt, Christa Ehmen, Juergen Sievertsen, Jennifer Evans, Juergen May, Daniel Ansong, Birgit Muntau, Gerd Ruge, Christian Timmann, Tsiri Agbenyega, Rolf D. Horstmann, Thorsten Thye

In a recent report, the cellular receptor CD55 was identified as a molecule essential for the invasion of human erythrocytes by Plasmodium falciparum, the causal agent of the most severe form of malaria. As this invasion process represents a critical step during infection with the parasite, it was hypothesized that genetic variants in the gene could affect severe malaria (SM) susceptibility. We performed high-resolution variant discovery of rare and common genetic variants in the human CD55 gene. Association testing of these variants in over 1700 SM cases and unaffected control individuals from the malaria-endemic Ashanti Region in Ghana, West Africa, were performed on the basis of single variants, combined rare variant analyses, and reconstructed haplotypes. A total of 26 genetic variants were detected in coding and regulatory regions of CD55. Five variants were previously unknown. None of the single variants, rare variants, or haplotypes showed evidence for association with SM or P. falciparum density. Here, we present the first comprehensive analysis of variation in the CD55 gene in the context of SM and show that genetic variants present in a Ghanaian study group appear not to influence susceptibility to the disease.

AbstractThe endosymbiont Wolbachia can have major effects on the reproductive fitness, and vectorial capacity of host insects and may provide new avenues to control mosquito‐borne pathogens. Anopheles gambiae s.l is the major vector of malaria in Africa but the use of Wolbachia in this species has been limited by challenges in establishing stable transinfected lines and uncertainty around native infections. High frequencies of infection of Wolbachia have been previously reported in An. gambiae collected from the Valle du Kou region of Burkina Faso in 2011 and 2014. Here, we re‐evaluated the occurrence of Wolbachia in natural samples, collected from Valle du Kou over a 12‐year time span, and in addition, expanded sampling to other sites in Burkina Faso. Our results showed that, in contrast to earlier reports, Wolbachia is present at an extremely low prevalence in natural population of An. gambiae. From 5341 samples analysed, only 29 were positive for Wolbachia by nested PCR representing 0.54% of prevalence. No positive samples were found with regular PCR. Phylogenetic analysis of 16S rRNA gene amplicons clustered across supergroup B, with some having similarity to sequences previously found in Anopheles from Burkina Faso. However, we cannot discount the possibility that the amplicon positive samples we detected were due to environmental contamination or were false positives. Regardless, the lack of a prominent native infection in An. gambiae s.l. is encouraging for applications utilizing Wolbachia transinfected mosquitoes for malaria control.

    Mohamed Amine Oukhouya, Hind Cherrabi

We report the case of an 11-year-old female child who injured her ring finger (ring on the 4th finger hung on a nail fixed to the wall during a jump). Clinical examination showed 4th finger degloving injury with exposure of bone and tendon system and 3rd phalanx amputation (A). Following consultation with the family, the finger was preserved and McGregor inguinal flap was performed for aesthetic and functional outcome. Emergency surgery was performed under general anesthesia, with the patient in a supine position and preparation of the right upper limb and right flank. McGregor inguinal flap was performed after anatomical identification of the flap to place on the pathway of the superficial iliac artery (B). After dissecting the flap, it was tubularized on the finger to cover it (C). The postoperative course was uneventful and after three weeks the flap was weaned under general anesthesia (D); the result after one month was very satisfactory (E).
