Liver Fibrosis: Mechanistic Concepts and Therapeutic Perspectives.

Natascha Roehlen, Emilie Crouchet, Thomas F. Baumert

Liver fibrosis due to viral or metabolic chronic liver diseases is a major challenge of global health. Correlating with liver disease progression, fibrosis is a key factor for liver disease outcome and risk of hepatocellular carcinoma (HCC). Despite different mechanism of primary liver injury and disease-specific cell responses, the progression of fibrotic liver disease follows shared patterns across the main liver disease etiologies. Scientific discoveries within the last decade have transformed the understanding of the mechanisms of liver fibrosis. Removal or elimination of the causative agent such as control or cure of viral infection has shown that liver fibrosis is reversible. However, reversal often occurs too slowly or too infrequent to avoid life-threatening complications particularly in advanced fibrosis. Thus, there is a huge unmet medical need for anti-fibrotic therapies to prevent liver disease progression and HCC development. However, while many anti-fibrotic candidate agents have shown robust effects in experimental animal models, their anti-fibrotic effects in clinical trials have been limited or absent. Thus, no approved therapy exists for liver fibrosis. In this review we summarize cellular drivers and molecular mechanisms of fibrogenesis in chronic liver diseases and discuss their impact for the development of urgently needed anti-fibrotic therapies.

Living at the edge: biogeographic patterns of habitat segregation conform to speciation by niche expansion in Anopheles gambiae

Carlo Costantini, Diego Ayala, Wamdaogo M Guelbeogo, Marco Pombi, Corentin Y Some, Imael HN Bassole, Kenji Ose, Jean-Marie Fotsing, N'Falé Sagnon, Didier Fontenille, Nora J Besansky, Frédéric Simard

BackgroundOngoing lineage splitting within the African malaria mosquito Anopheles gambiae is compatible with ecological speciation, the evolution of reproductive isolation by divergent natural selection acting on two populations exploiting alternative resources. Divergence between two molecular forms (M and S) identified by fixed differences in rDNA, and characterized by marked, although incomplete, reproductive isolation is occurring in West and Central Africa. To elucidate the role that ecology and geography play in speciation, we carried out a countrywide analysis of An. gambiae M and S habitat requirements, and that of their chromosomal variants, across Burkina Faso.ResultsMaps of relative abundance by geostatistical interpolators produced a distinct pattern of distribution: the M-form dominated in the northernmost arid zones, the S-form in the more humid southern regions. Maps of habitat suitability, quantified by Ecological Niche Factor Analysis based on 15 eco-geographical variables revealed less contrast among forms. M was peculiar as it occurred proportionally more in habitat of marginal quality. Measures of ecological niche breadth and overlap confirmed the mismatch between the fundamental and realized patterns of habitat occupation: forms segregated more than expected from the extent of divergence of their environmental envelope – a signature of niche expansion. Classification of chromosomal arm 2R karyotypes by multilocus genetic clustering identified two clusters loosely corresponding to molecular forms, with 'mismatches' representing admixed individuals due to shared ancestral polymorphism and/or residual hybridization. In multivariate ordination space, these karyotypes plotted in habitat of more marginal quality compared to non-admixed, 'typical', karyotypes. The distribution of 'typical' karyotypes along the main eco-climatic gradient followed a consistent pattern within and between forms, indicating an adaptive role of inversions at this geographica

Local adaptation and colonization are potential factors affecting sexual competitiveness and mating choice in Anopheles coluzzii populations

Charles Nignan, Bèwadéyir Serge Poda, Simon Péguédwindé Sawadogo, Hamidou Maïga, Kounbobr Roch Dabiré, Olivier Gnankine, Frédéric Tripet, Olivier Roux, Abdoulaye Diabaté

The mating behaviour of the malaria vector Anopheles gambiae complex is an important aspect of its reproduction biology. The success of mosquito release programmes based on genetic control of malaria crucially depends on competitive mating between both laboratory-reared and wild individuals, and populations from different localities. It is known that intrinsic and extrinsic factors can influence the mating success. This study addressed some of the knowledge gaps about factors influcencing mosquito mating success. In semi-field conditions, the study compared the mating success of three laboratory-reared and wild allopatric An. coluzzii populations originating from ecologically different locations in Burkina Faso. Overall, it was found that colonization reduced the mating competitiveness of both males and females compared to that of wild type individuals. More importly, females were more likely to mate with males of their own population of origin, be it wild or colonised, suggesting that local adaptation affected mate choice. The observations of mating behaviour of colonized and local wild populations revealed that subtle differences in behaviour lead to significant levels of population-specific mating. This is the first study to highlight the importance of local adaptation in the mating success, thereby highlighting the importance of using local strains for mass-rearing and release of An. coluzzii in vector control programmes.

Local agro-pastoralists’ perspectives on forage species diversity, habitat distributions, abundance trends and ecological drivers for sustainable livestock production in West Africa

    John-Baptist S. N. Naah, Boris Braun

Despite the importance of local ecological knowledge of forage plants, there has been little discussion on how local agro-pastoralists perceive forage species diversity, abundance trends, habitat distributions and ecological drivers influencing changing abundance trends over time in rural West Africa’s savannas. In estimating, assessing and investigating the ecological variables, we performed elaborate ethnobotanical surveys in seven villages in northern Ghana and nine villages in southern-central Burkina Faso. Data were analyzed using descriptive statistics, bivariate correlation analysis and cognitive salience index calculations to disentangle the dynamics of local responses to ecological variables considered in this study. Our results revealed that agro-pastoralists exhibited extensive knowledge on forage species diversity, habitat types, abundance trends and ecological drivers. According to agro-pastoralists interviewed, about 82 percent of all forage species known to them were commonly available in local landscapes, while a majority of our interviewees indicated that available forage resources have shown a gradually increasing trend over the past few years. Rainfall variability, tree cutting and drought were the topmost perceived threats causing changes in the trends of forage species abundance. Given our findings, local perceptions of agro-pastoralists could have substantial practical implications in favor of forage-related biodiversity conservation and sustainable livestock production.

Local Ancestry to Identify Selection in Response to Trypanosome Infection in Baoulé x Zebu Crossbred Cattle in Burkina Faso

Tamboura, Amadou Traoré, Okeyo Ally Mwai, Johann Sölkner, Negar Khayatzadeh, Gábor Mészáros, Pamela A. Burger

The genomes of crossbred (admixed) individuals are a mosaic of ancestral haplotypes formed by recombination in each generation. The proportion of these ancestral haplotypes in certain genomic regions can be responsible for either susceptibility or tolerance against pathogens, and for performances in production traits. Using a medium-density genomic marker panel from the Illumina Bovine SNP50 BeadChip, we estimated individual admixture proportions for Baoulé x Zebu crossbred cattle in Burkina Faso, which were tested for trypanosome infection by direct ELISA from blood samples. Furthermore, we calculated local ancestry deviation from average for each SNP across 29 autosomes to identify potential regions under selection in the trypanotolerant Baoulé cattle and their crossbreds. We identified significant deviation from the local average ancestry (above 5 and 10% genome-wide thresholds) on chromosomes 8 and 19 in the positive animals, while the negative ones showed higher deviation on chromosomes 6, 19, 21, and 22. Some candidate genes on chromosome 6 (PDGFRA) and chromosome 19 (CDC6) have been found associated to trypanotolerance in West African taurines. Screening for FST outliers in trypanosome positive/negative animals we detected seven variants putatively under selection. Finally, we identified a minimum set of highly ancestry informative markers for routine admixture testing. The results of this study contribute to a better understanding of the genetic basis of trypanotolerance in Baoulé cattle and their crossbreeds. Furthermore, we provide a small informative marker set to monitor admixture in this valuable indigenous breed. As such, our results are important for conserving the genetic uniqueness and trypanotolerance of Baoulé cattle, as well as for the improvement of Baoulé and Zebu crossbreds in specific community-based breeding programs.

Local knowledge, practices, challenges of ethnopharmacologically used medicinal plants in Benin and implications for brain illnesses

Godfried Dougnon, Victorien Tamègnon Dougnon, Jean Robert Klotoé, Eric Agbodjento, Dannialou Zoumarou, Boris Lègba, Hornel Koudokpon, Phénix Assogba, Leena Hanski, Eléonore Yayi Ladékan

Traditional medicine (TM) is a significant resource for primary healthcare management all over the world, and principally in Africa. Quality improvement activities that promote evidence-based practices and the integration of traditional medicine into primary healthcare systems can help improve the quality of patient care. In the Republic of Benin (West Africa), traditional medicine practitioners (TMPs) provide different treatments and ways of use, depending on the ailments and the medicinal plants used. The present study aimed at documenting the knowledge, attitudes and practices of Beninese TMPs regarding the use of medicinal plants and the challenges associated with their activities. A focus group survey was conducted using semi-structured interviews with a sample of 91 TMPs in 8 departments of the Republic of Benin. The respondents had an average age of 50 years old and belonged to various categories of TMPs. Medicinal plants are harvested depending on the season and time of the day, and are dried in the shade before being used as decoctions or infusions. Nevertheless, the majority of TMPs do not conduct the necessary tests for quality control, efficacy or toxicity of the proposed remedies, which raises several scientific interrogations, particularly for the treatment of mental and brain-related disorders. Among ~ 110 plants used in the treatment of several pathologies, 66 were revealed as threatened species. The challenges faced by TMPs are mainly material, financial and technical difficulties. The present study reports the importance of intervention to modernize TM practices in Benin. Quality improvement could enhance healthcare delivery and provide support for evidence-based interventions aimed at addressing behavioral, social, and environmental determinants of health.

Local perception of ecosystem services and their conservation in Sudanian savannas of Burkina Faso (West Africa)

Assétou Nabaloum, Dethardt Goetze, Amadé Ouédraogo, Stefan Porembski, Adjima Thiombiano

ContextIn Burkina Faso, Sudanian savannas are important ecosystems for conservation of plant diversity. Due to desertification and insecurity, population migration from the North has increased human density and anthropogenic pressure on southern savannas. This study aims to investigate knowledge of local populations on ecosystem services (ES) and perception of their conservation.MethodIndividual semi-structured interviews about knowledge on ES and ecosystem conservation issues were conducted. Informants were selected according to sociocultural groups and sex in three areas of different land use intensity: the communal area of Dano (CAD), the Total Wildlife Reserve of Bontioli (TWRB) and the Game Ranch of Nazinga (GRN). The use value and vulnerability index of each plant species were determined. A cluster analysis and a principal component analysis were carried out to identify the particular knowledge of different ethnic groups.ResultsOverall, 163 plant species were cited for fifteen ES. Provisioning services were most frequently cited (100%), regulating services second most frequently (92.47%). Entire plants were exclusively used for ES with non-material benefits (protection against wind, for shading, soil fertility, erosion prevention, tourism and religion). The ten species contributing most to ES provision were Vitellaria paradoxa, Parkia biglobosa, Diospyros mespiliformis, Adansonia digitata, Lannea microcarpa, Faidherbia albida, Khaya senegalensis, Afzelia africana, Ficus sycomorus, Pterocarpus erinaceus. Seven of them were identified as highly vulnerable. Around GRN, migrants and natives shared the same knowledge, while migrants in TWRB used the ES only to a small extent due to restricted contact with the native population. Migrants and natives of GRN had more knowledge on tourism and crafts services while the natives of CAD and TWRB made use of the services that sustain the quality of the agricultural land and meet their primary needs. To reduce further degrad

Local preferences for three indigenous oil-seed plants and attitudes towards their conservation in the Kénédougou province of Burkina Faso, West-Africa

Fanta Reine Sheirita Tiétiambou, Kolawolé Valère Salako, Jésukpégo Roméo Tohoun, Amadé Ouédraogo

BackgroundCarapa procera, Lophira lanceolata, and Pentadesma butyracea are three underutilized but increasingly threatened indigenous oil-seed tree species (IOS) in tropical Africa. Because local knowledge is vital for sustainable management, this study investigated the socio-economic factors that explain local people’s (i) preferences for these IOS, (ii) attitudes toward their conservation, and (iii) ability to identify “plus trees” based on seed traits. We predicted a positive relationship between response variables and informants’ age, residence status, gender (femaleness), and existence of market opportunities for each IOS. We also predicted that a higher preference for a given IOS has a positive effect on people’s attitudes for its conservation and the aptitude to identify its “plus trees.” We additionally expected significant differences among ethnic groups for each response variable.MethodsData were collected through individual semi-structured interviews with 336 informants from 14 randomly selected villages in the species distribution area of Kénédougou province. For each species, the collected data were the number of actual uses reported (converted to use value—UV, as a measure of the species preference), practiced conservation actions (converted to conservation attitude using a four-scale scoring method), and possible criteria for selecting preferred trees for seed oil extraction. Generalized linear mixed models were used to test for the fixed effects of socio-economic factors, and account for the random variation across villages.ResultsThe results showed species-specific patterns. Carapa procera had the highest UV and hence was the most preferred IOS, particularly by women. Informants from the Siamou ethnic group had the highest UV irrespective of IOS. The most cited conservation actions were assisted natural regeneration and banning of tree cutting, which were practiced for C. procera and L. lanceolata. No conservation measure was cited for P. butyracea.

Local sustainability and scaling up for user fee exemptions: medical NGOs vis-à-vis health systems

Jean-Pierre Olivier de Sardan, Aïssa Diarra, Félix Yaouaga Koné, Maurice Yaogo, Roger Zerbo

Free healthcare obviously works when a partner from abroad supplies a health centre or a health district with medicines and funding on a regular basis, provides medical, administrative and managerial training, and gives incentive bonuses and daily subsistence allowances to staff. The experiments by three international NGO in Burkina Faso, Mali and Niger have all been success stories. But withdrawing NGO support means that health centres that have enjoyed a time of plenty under NGO management will return to the fold of health centres run by the state in its present condition and the health system in its present condition, with the everyday consequences of late reimbursements and stock shortages. The local support given by international NGOs has more often than not an effect of triggering an addiction to aid instead of inducing local sustainability without infusion. In the same way, scaling up to the entire country a local pilot experiment conducted under an NGO involves its insertion into a national bureaucratic machine with its multiple levels, all of which are potential bottlenecks. Only experiments carried out under the "ordinary" management of the state are capable of laying bare the problems associated with this process. Without reformers 'on the inside' (within the health system itself and among health workers), no real reform of the health system induced by reformers 'from the outside' can succeed. The problems relating to the sustainability of public policies in Africa, especially when the policies benefit from development aid, in the area of health among others, are familiar to researchers and policy-makers. However, as far as user fee exemptions are concerned, debates about these problems have extended well beyond the narrow circle of experts and into the public domain in the countries concerned. Throughout our research, we have observed that the sustainability of free healthcare policies is a major concern of all the actors (health workers, users, managers

Localisation abdominale d’un dermatofibrosarcome de Darrier et Ferrand: à propos d’un cas

Issam Loukil, Amine Zouari, Bassem Abid

Lamia Kissi, Rajaa El Bouihi, Mouna Lamchahab, Ahmed Alaoui, Ihsane Benyahya
