Loss to Follow-Up from HIV Pre-Exposure Prophylaxis Care in Men Who Have Sex with Men in West Africa

Loss to follow-up (LTFU) from HIV pre-exposure prophylaxis (PrEP) care compromises the goal of HIV elimination. We investigated the proportion of LTFU and associated risk factors among men who have sex with men (MSM) enrolled in a PrEP demonstration project in Burkina Faso, Côte d’Ivoire, Mali, and Togo. CohMSM-PrEP, a prospective cohort study, was conducted between November 2017 and June 2021 in community-based clinics. MSM aged 18 years or older at substantial risk of HIV infection received a comprehensive prevention package, including PrEP and peer education. LTFU was defined as not returning to the clinic for six months. Associated risk factors were investigated using a time-varying Cox’s model. Of 647 participants followed up for a median time of 15 months, 372 were LTFU (57.5%). LTFU was associated with younger age (adjusted hazard ratio [95% Confidence Interval]; 1.50 [1.17–1.94]), unemployment (1.33 [1.03–1.71]), depression (1.63 [1.12–2.38]), and perceiving no HIV risk with stable male partners (1.61 [1.23–2.10]). Contacting peer educators outside of scheduled visits was protective (0.74 [0.56–0.97]). Our findings show that LTFU from PrEP care in West African MSM is a major challenge to achieving HIV elimination, but that the involvement of peer educators in PrEP delivery helps to limit LTFU by providing users with adequate support.

Low birth weight and its associated risk factors in a rural health district of Burkina Faso: a cross sectional study

Moussa Lingani, Serge Henri Zango, Innocent Valéa, Georges Somé, Maïmouna Sanou, Sékou O. Samadoulougou, Serge Ouoba, Eli Rouamba, Annie Robert, Michèle Dramaix, Philippe Donnen, Halidou Tinto

BackgroundLow birth weight (LBW) is a major factor of neonate mortality that particularly affects developing countries. However, the scarcity of data to support decision making to reduce LBW occurrence is a major obstacle in sub-Saharan Africa. The aim of this research was to determine the prevalence and associated factors of LBW at the Yako health district in a rural area of Burkina Faso.MethodsA cross sectional survey was conducted at four peripheral health centers among mothers and their newly delivered babies. The mothers’ socio-demographic and obstetrical characteristics were collected by face-to-face interview or by review of antenatal care books. Maternal malaria was tested by standard microscopy and neonates’ birth weights were documented. Multivariate logistic regression was used to determine factors associated with LBW. A p-value < 0.05 was considered statistically significant.ResultsOf 600 neonates examined, the prevalence of low birth weight was 11.0%. Adjustment for socio-demographic characteristic, medical conditions, obstetrical history, malaria prevention measures by multivariate logistic regression found that being a primigravid mother (aOR = 1.8, [95% CI: 1.1–3.0]), the presence of malaria infection (aOR = 1.9, [95% CI: 1.1–3.5]), the uptake of less than three doses of sulfadoxine-pyrimethamine for the intermittent preventive treatment of malaria in pregnancy (IPTp-SP) (aOR = 2.2, [95% CI: 1.3–3.9]), the presence of maternal fever at the time of delivery (aOR = 2.8, [95% CI: 1.5–5.3]) and being a female neonate (aOR = 1.9, [95% CI: 1.1–3.3]) were independently associated with an increased risk of LBW occurrence. The number of antenatal visits performed by the mother during her pregnancy did not provide any direct protection for low birth weight.ConclusionThe prevalence of LBW remained high in the study area. Maternal malaria, fever and low uptake of sulfadoxine-pyrimethamine doses were significantly associated with LBW and should be adequately addr

Low coverage but few inclusion errors in Burkina Faso: a community-based targeting approach to exempt the indigent from user fees

Valéry Ridde, Slim Haddad, Béatrice Nikiema, Moctar Ouedraogo, Yamba Kafando, Abel Bicaba

BackgroundUser fees were generalized in Burkina Faso in the 1990 s. At the time of their implementation, it was envisioned that measures would be instituted to exempt the poor from paying these fees. However, in practice, the identification of indigents is ineffective, and so they do not have access to care. Thus, a community-based process for selecting indigents for user fees exemption was tested in a district. In each of the 124 villages in the catchment areas of ten health centres, village committees proposed lists of indigents that were then validated by the health centres' management committees. The objective of this study is to evaluate the effectiveness of this community-based selection.MethodsAn indigent-selection process is judged effective if it minimizes inclusion biases and exclusion biases. The study compares the levels of poverty and of vulnerability of indigents selected by the management committees (n = 184) with: 1) indigents selected in the villages but not retained by these committees (n = 48); ii) indigents selected by the health centre nurses (n = 82); and iii) a sample of the rural population (n = 5,900).ResultsThe households in which the three groups of indigents lived appeared to be more vulnerable and poorer than the reference rural households. Indigents selected by the management committees and the nurses were very comparable in terms of levels of vulnerability, but the former were more vulnerable socially. The majority of indigents proposed by the village committees who lived in extremely poor households were retained by the management committees. Only 0.36% of the population living below the poverty threshold and less than 1% of the extremely poor population were selected.ConclusionsThe community-based process minimized inclusion biases, as the people selected were poorer and more vulnerable than the rest of the population. However, there were significant exclusion biases; the selection was very restrictive because the exemption had to be

Low occurrence of Pseudomonas aeruginosa in agricultural soils with and without organic amendment

Amélie Deredjian, Céline Colinon, Edmond Hien, Elisabeth Brothier, Benjamin Youenou, Benoit Cournoyer, Samuel Dequiedt, Alain Hartmann, Claudy Jolivet, Sabine Houot, Lionel Ranjard, Nicolas P. A. Saby, Sylvie Nazaret

The occurrence of Pseudomonas aeruginosa was monitored at a broad spatial scale in French agricultural soils, from various soil types and under various land uses to evaluate the ability of soil to be a natural habitat for that species. To appreciate the impact of agricultural practices on the potential dispersion of P. aeruginosa, we further investigated the impact of organic amendment at experimental sites in France and Burkina Faso. A real-time quantitative PCR (qPCR) approach was used to analyze a set of 380 samples selected within the French RMQS (“Réseau de Mesures de la Qualité des Sols”) soil library. In parallel, a culture-dependent approach was tested on a subset of samples. The results showed that P. aeruginosa was very rarely detected suggesting a sporadic presence of this bacterium in soils from France and Burkina Faso, whatever the structural and physico-chemical characteristics or climate. When we analyzed the impact of organic amendment on the prevalence of P. aeruginosa, we found that even if it was detectable in various manures (at levels from 103 to 105 CFU or DNA targets (g drywt)−1 of sample), it was hardly ever detected in the corresponding soils, which raises questions about its survival. The only case reports were from a vineyard soil amended with a compost of mushroom manure in Burgundy, and a few samples from two fields amended with raw urban wastes in the sub-urban area of Ouagadougou, Burkina Faso. In these soils the levels of culturable cells were below 10 CFU (g drywt)−1.

Low prevalence of high blood pressure in pregnant women in Burkina Faso: a cross-sectional study

Franck Garanet, Sekou Samadoulougou, Adama Baguiya, Bruno Bonnechère, Tieba Millogo, Jean-Marie Degryse, Fati Kirakoya-Samadoulougou, Seni Kouanda

BackgroundHigh blood pressure (HBP) during pregnancy causes maternal and fetal mortality. Studies regarding its prevalence and associated factors in frontline level health care settings are scarce. We thus aimed to evaluate the prevalence of HBP and its associated factors among pregnant women at the first level of the health care system in Burkina Faso.MethodsThis cross-sectional study was conducted in six health facilities between December 2018 and March 2019. HBP was defined as systolic blood pressure ≥ 140 mmHg and/or diastolic blood pressure ≥ 90 mmHg. Multivariable logistic regression analysis was performed to identify factors associated with HBP.ResultsA total of 1027 pregnant women were included. The overall prevalence of HBP was 1.4% (14/1027; 95% confidence interval [CI] 0.7–2.3), with 1.6% (7/590; 95% CI 0.8–3.3) in rural and 1.2% (7/437; 95% CI 0.6- 2.5) in semi-urban areas. The prevalence was 0.7% (3/440; 95% CI 0.2–2.1) among women in the first, 1.5% (7/452; 95% CI 0.7–3.2) in the second and 3% (4/135; 95% CI 1.1–7.7) in the third trimester. In the multivariable analysis, pregnancy trimester, maternal age, household income, occupation, parity, and residential area were not associated with HBP during pregnancy.ConclusionThe prevalence of HBP among pregnant women at the first level of health system care is significantly lower compared to prevalence’s from hospital studies. Public health surveillance, primary prevention activities, early screening, and treatment of HDP should be reinforced in all health facilities to reduce the burden of adverse pregnancy outcomes in Burkina Faso.

Low-dose hydroxycarbamide therapy may offer similar benefit as maximum tolerated dose for children and young adults with sickle cell disease in low-middle-income settings

Baba Psalm Duniya Inusa, Wale Atoyebi, Abdul Aziz Hassan, Tushar Idhate, Livingstone Dogara, Ifeoma Ijei, Yewen Qin, Kofi Anie, Juliana Olufunke Lawson, Lewis Hsu

The multiple clinical benefits of hydroxycarbamide in sickle cell disease are supported by a large body of evidence. The maximum tolerated dose (MTD) is the regimen recommended by guidelines from a panel of National Heart, Lung, and Blood Institute (NHLBI) experts, but other dosage regimens have been used in babies (BABY-HUG) 9 to 18 months old (20 mg/kg per day) and developing countries such as India (10 mg/kg per day); however, there has been no direct comparison of the efficacy, effectiveness, or cost-effectiveness of these different regimens. The purpose of this review was to investigate the current situation with various hydroxycarbamide regimens with particular relevance to low-middle-income countries. In regard to methodology, a literature review was undertaken by using multiple databases in PubMed and Google and the search terms included sickle cell disease, hydroxyurea, hydroxycarbamide, sickle cell anaemia, low-middle-income countries, Sub-Saharan Africa, and India. Although MTD regimens have been widely used in research, especially within North America, clinical trials elsewhere tend to use fixed-dose regimens. In a survey of haematologists across Europe and Africa, 60% (75% response rate) did not use the MTD regimen for hydroxycarbamide treatment of sickle cell disease. The recommendations are (1) for practical purposes to commence using fixed-dose hydroxycarbamide in line with BABY-HUG recommendations and then (2) to consider or propose a trial comparing MTD escalation with various fixed doses and to include as end points health-related quality of life, haemoglobin F levels, adherence, and cost-effectiveness.

Lung function variability and the burden of COPD and lung restriction in South Africa and Burkina Faso: results from the AWI-Gen study

Emmanuel Adonyo, Jing Chen, Catherine John, Richard Packer, Michele Ramsay, Chiara Batini, Richard van Zyl-Smit, Mervyn Mer, Palwende Romuald Boua, Godfred Agongo, F Xavier Gómez-Olivé, Reneilwe Given Mashaba, Martin D. Tobin, Anna L. Guyatt, Lisa K. Micklesfield

AbstractBackgroundPopulation-based spirometry data from sub-Saharan Africa remain limited. We characterised lung function, chronic obstructive pulmonary disease (COPD), lung restriction and associated factors from two Sub-Saharan African countries. Methods We performed spirometry using ndd Easy On-PC spirometers among adults aged ≥40 years from population-based cohorts in four communities in Burkina Faso and South Africa from January 2019–April 2022. We included 1,619 participants (936 women, 683 men) with repeatable forced expiratory volume in one second (FEV 1 ) and forced vital capacity (FVC). COPD was defined as FEV 1 /FVC<0.70 and lung restriction as FEV 1 /FVC≥0.70, with FVC<80% predicted. We used multivariable models to test factors associated with lung function measurements (FEV 1 , FVC and FEV 1 /FVC), COPD and lung restriction. Results Median FEV 1 and FVC ranged from 1.940–2.273L and 2.521–2.883L, respectively. COPD prevalence was 16.1% (95% CI 14.4–18.0; site range 10.8%–24.5%) and lung restriction prevalence was 21.3% (95% CI 19.4–23.4; site range 11.3%–36.2%). Prior tuberculosis was associated with higher odds of COPD (OR 2.86, 95% CI 1.75–4.59) and lower FEV 1 , FVC and FEV 1 /FVC. HIV infection was associated with higher COPD odds (OR 1.67, 95% CI 1.01–2.72) and lower FEV 1 /FVC. Hypertension was associated with higher lung restriction odds (OR 1.46, 95% CI 1.07–2.00) and lower FEV 1 and FVC. Diabetes was associated with lower COPD odds (OR 0.50, 95% CI 0.24–0.94), lower FVC and higher FEV 1 /FVC. Ever-smoking and older age were associated with higher COPD odds (OR 2.21, 95% CI 1.48–3.32; OR 1.05, 95% CI 1.03–1.07, respectively); older age was associated with higher lung restriction odds (OR 1.02, 95% CI 1.00–1.04). ConclusionThis multisite study highlights substantial heterogeneity in COPD and lung restriction prevalence across African populations and identifies respiratory multimorbidity with infectious and cardiometabolic conditions. These findings

Lupus érythémateux systémique à début pédiatrique: à propos d'un cas

Sana Khlif, Hend Hachicha, Faten Frikha, Sawsan Feki, Mourad Ben Ayed, Zouhir Bahloul, Hatem Masmoudi

Mahbouba Jguirim, Amna Jbeli, Hajer Ben Brahim, Amira Mhenni, Monia Youssef, Mongi Touzi, Sawssen Zrour, Ismail Bejia, Naceur Bergaoui

Kenza Baline, Karim Zaher, Hassan Fellah, Hakima Benchikhi

Fahd Idarrha, Yassine Fath El Khir, Ahmed idoukitar, Mehdi Maskouf, Abdesslam Achkoun, Mohammed Amine Benhima, Imad Abkari, Halim Saidi

    Luxation erecta

Issam Elouakili, Redouane Ouakrim, Younes Ouchrif, Mohammed Kharmaz, Farid Ismael, Mly Omar Lamrani, Ahmed El Bardouni, Mustapha Mahfoud, Mohamed Saleh Berrada, Mouradh El Yaacoubi

    Youssef Nader, Khalid Idrissi Koulali

Issam Elouakili, Younes Ouchrif, Redouane Ouakrim, Omar Lamrani, Mohammed Kharmaz, Farid Ismael, Abdou Lahlou, Ahmed El Bardouni, Mustapha Mahfoud, Mohamed Saleh Berrada, Moradh El Yaacoubi
