Antimicrobial use and stewardship opportunities in Burkina Faso: findings from the first Global Point Prevalence Survey in eight hospitals

André Yi Yè Aburi Nagalo, Ann Versporten, Odilon D. Kaboré, Orokia S. Momo, Noutin F. Michodigni, Bintou Sanogo, Jacques Zoungrana, Ines Pauwels, Ibrahim Traoré, Armel Poda, Mahamoudou Sanou, Herman Goossens, Abdoul-Salam Ouedraogo

Objective:To describe the prevalence, patterns, and quality indicators of antimicrobial use (AMU) in Burkinabe hospitals and identify priorities for stewardship.Design:Multicentre, cross-sectional point prevalence survey (PPS) following the Global Point Prevalence Survey protocol.Setting:Eight public hospitals across six cities in Burkina Faso (3 primary, 3 secondary, and 2 tertiary), February–June 2019.Participants:All inpatients present on the survey day at 8:00. Medical records were reviewed for those receiving systemic antimicrobial agents. Methods:Standardized ward- and patient-level data were collected on indications, agents, routes, and WHO AWaRe categories, alongside quality indicators (documented indication, stop/review date, guideline compliance, and targeted vs empirical therapy). Descriptive analyses compared hospital tiers. Results:Of 994 inpatients, 729 (73.3%) received ≥1 antimicrobial (range by tier: tertiary 69.7%, secondary 79.2%, primary 79.2%). Community-acquired infections accounted for 96.0% of therapeutic indications. Leading reasons were skin/soft tissue infections (12.2%), gastrointestinal infections (10.7%), and pneumonia (10.4%). Parenteral administration predominated. The most used agents were ceftriaxone (27.8%), metronidazole (15.7%), and gentamicin (9.4%). By AWaRe, Access agents comprised ∼ 50%, Watch agents ∼ 50% overall, with higher Watch use in tertiary hospitals; no Reserve antibiotics were recorded. Quality indicators were suboptimal: the indication was documented in 15.6%, the stop/review date was absent in 93.0%, and 41.1% of prescriptions were guideline-compliant. Therapy was largely empirical (98.5%). Prolonged surgical prophylaxis (>1 day) represented 86.8% of prophylaxis courses. Conclusions:Antimicrobial use (AMU) prevalence in Burkinabe hospitals is high, dominated by empirical therapy and Watch-class ceftriaxone. Deficits in prescribing quality and limited diagnostic use highlight urgent needs for context-adapted antimicrob

Antioxidant and Anti-Inflammatory Activities of Fractions from Bidens engleri O.E. Schulz (Asteraceae) and Boerhavia erecta L. (Nyctaginaceae)

Moussa COMPAORE, Sahabi BAKASSO, Roland Nâg Tiero MEDA, Odile Germaine NACOULMA

Background: According to recent studies, reactive oxygen is the leader of human metabolic disease development. The use of natural antioxidants is the best way to stop or prevent this problem. Therefore, the aim of this study was to evaluate the antioxidant and anti-inflammatory activities and to determine the polyphenolic contents of the Bidens engleri and Boerhavia erecta fractions. Methods: Plant fractions were obtained using Soxhlet procedures with hexane, dichloromethane, acetonitrile, ethyl acetate, methanol, and butanol solvent, successively. The different fractions were compared according to their antioxidant, anti-inflammatory activities, total phenolic, and total flavonoid contents. The phenolic contribution to the biological activity was evaluated. Result: The Bidens engleri and Boerhavia erecta fractions showed the highest antioxidant abilities, notably the polar fractions, which inhibited significantly the radical 2,2-diphenyl-1-picrylhydrazyl (DPPH) and 2,2-O-azinobis(3-ethylbenzoline-6-sulphonate) (ABTS). The butanol fraction from Bidens engleri and methanol fraction from Boerhavia erecta have presented the best iron (III) reduction power with 211.68 and 198.55 mgAAE/g, respectively. Butanol and acetonitrile were the best solvents for extracting phenolic compounds from Bidens engleri and Boerhavia erecta, respectively. In contrast, dichloromethane was the best solvent for extracting a flavonoid from two plants with anti-COX-2 and anti-LOX-15 active compounds. The phenolic compound contributed significantly to antioxidant activity (r > 0.80). Conclusion: The Bidens engleri and Boerhavia erecta fractions possessed a potential antioxidant for fighting oxidative stress and helping to prevent diabetes, hypertension, and cardiovascular diseases. The uses of this plant could be promoted in Burkina Faso.

Antioxidant and Phytochemical Studies of 31 Cowpeas (Vigna unguiculata (L. Walp.)) Genotypes from Burkina Faso

Pierre Alexandre Eric Djifaby Sombié, Moussa Compaoré, Ahmed Yacouba Coulibaly, Jeremy Tinga Ouédraogo, Jean-Baptiste De La Salle Tignégré, Martin Kiendrébéogo

Antioxidant compounds of dietary plants have been widely studied because of their bioactive properties. The objective of this research study was to analyse the health enhancing attributes of 31 cowpeas varieties from Burkina Faso. Significant variations were observed in the phenolic content as well as the antioxidant and anti-lipid peroxidation activities amongst the cowpea varieties. Pearson correlation coefficient analysis showed that the ferric reducing ability (r = 0.954) and anti-lipid peroxidation (r = 0.616) were positively correlated with the total phenolic content. A significant relationship between cowpea ferric reducing ability and anti-lipid peroxidation (r = 0.64) was also revealed. However, nitric oxide scavenging potential was found not to be related to its total phenolic and total flavonoid content. 2,2-diphenyl-1-picrylhydrazyl (DPPH) and hydroxyl radicals scavenging potentials were not correlated with the total flavonoid content. The pigmented seeds of cowpea varieties possess higher total phenolic, total flavonoid content, ferric reduction ability, and anti-lipid peroxidation activities than the colourless ones. The results obtained from this study suggest that Burkina Faso cowpea cultivars are rich in phenolic compounds and have significant antioxidant and anti-lipid peroxidation activities. Consumption of the cowpea, particularly of coloured cowpea seed varieties, should be beneficial for chronic human diseases prevention.

Antiretroviral treatment and quality of life in Africans living with HIV: 12-month follow-up in Burkina Faso

Antoine Jaquet, Franck Garanet, Eric Balestre, Didier K. Ekouevi, Jean Claude Azani, René Bognounou, Elias Dah, Jean Charlemagne Kondombo, François Dabis, Joseph Drabo

IntroductionThe scale-up of highly active antiretroviral therapy (HAART) has led to a significant improvement in survival of the HIV-positive patient but its effects on health-related quality of life (HRQOL) are less known and context-dependent. Our aim was to assess the temporal changes and factors associated with HRQOL among HIV-positive adults initiating HAART in Burkina Faso.MethodsHIV-positive people initiating HAART were prospectively included and followed over a one-year period in three HIV clinics of Ouagadougou. HRQOL was assessed at baseline and at each follow-up visit using physical (PHS) and mental (MHS) summary scores derived from the Medical Outcome Study 36-Item short-form health survey (MOS SF-36) questionnaire. Toxicity related to HAART modification and self-reported symptoms were recorded during follow-up visits. Determinants associated with baseline and changes in both scores over a one-year period were assessed using a mixed linear model.ResultsA total of 344 patients were included. Their median age at baseline was 37 years [interquartile range (IQR) 30–44] and their median CD4 count was 181 cells/mm3 (IQR 97–269). The mean [standard deviation (SD)] PHS score increased from 45.4 (11.1) at baseline to 60.0 (3.1) at 12 months (p<10−4) and the mean (SD) MHS score from 42.2 (8.7) to 43.9 (3.4) (p<10−2). After one year of treatment, patients that experienced on average two symptoms during follow-up presented with significantly lower PHS (63.9) and MHS (43.8) scores compared to patients that presented no symptoms with PHS and MHS of 68.2 (p<10−4) and 45.3 (p<10−3), respectively.DiscussionThe use of HAART was associated with a significant increase in both physical and mental aspects of the HRQOL over a 12-month period in this urban African population. Perceived symptoms experienced during follow-up visits were associated with a significant impairment in HRQOL. The appropriate and timely management of reported symptoms during the follow-up of HAART-treat

Any better? A follow-up content analysis of adolescent sexual and reproductive health inclusion in global financing facility country planning documents

Ulla Walmisley, Mary V. Kinney, Joël Arthur Kiendrébéogo, Yamba Kafando, Asha S. George

ABSTRACTBackgroundThe Global Financing Facility (GFF) supports national reproductive, maternal, newborn, child, adolescent health, and nutrition needs. Previous analysis examined how adolescent sexual and reproductive health was represented in GFF national planning documents for 11 GFF partner countries.ObjectivesThis paper furthers that analysis for 16 GFF partner countries as part of a Special Series.MethodsContent analysis was conducted on publicly available GFF planning documents for Afghanistan, Burkina Faso, Cambodia, CAR, Côte d’Ivoire, Guinea, Haiti, Indonesia, Madagascar, Malawi, Mali, Rwanda, Senegal, Sierra Leone, Tajikistan, Vietnam. Analysis considered adolescent health content (mindset), indicators (measure) and funding (money) relative to adolescent sexual and reproductive health needs, using a tracer indicator. Results Countries with higher rates of adolescent pregnancy had more content relating to adolescent reproductive health, with exceptions in fragile contexts. Investment cases had more adolescent content than project appraisal documents. Content gradually weakened from mindset to measures to money. Related conditions, such as fistula, abortion, and mental health, were insufficiently addressed. Documents from Burkina Faso and Malawi demonstrated it is possible to include adolescent programming even within a context of shifting or selective priorities.ConclusionTracing prioritisation and translation of commitments into plans provides a foundation for discussing global funding for adolescents. We highlight positive aspects of programming and areas for strengthening and suggest broadening the perspective of adolescent health beyond the reproductive health to encompass issues, such as mental health. This paper forms part of a growing body of accountability literature, supporting advocacy work for adolescent programming and funding. Paper context Main findings: Adolescent health content is inconsistently included in the Global Financing Facility count

Aphantasie : implications cliniques dans le champ orthophonique

    Alia Bendani

Kaoutar Zinelabidine, Idrissi Rhizlane, Meriame Meziane, Fatima Zahra Mernissi

Olga Ntumba-Tshitenge, Célestin Kaputu-Kalala-Malu, Caritas Mukampunga, Kavulu Mayamba Mukendi

APOBEC3G Variants and Protection against HIV-1 Infection in Burkina Faso

Studies on host factors, particularly the APOBEC3G gene, have previously found an association with AIDS progression in some populations and against some HIV-1 strains but not others. Our study had two main objectives: firstly, to screen a population from Burkina Faso for three variants of APOBEC3G previously described, and secondly to analyze the effect of these three variants and their haplotypes on HIV-1 infection with Circulating Recombinant Forms (CRFs) present in Burkina Faso. This case control study involved 708 seropositive and seronegative individuals. Genotyping was done by the TaqMan allelic discrimination method. Minor allele frequencies of rs6001417 (p<0.05), rs8177832 (P<0.05), and rs35228531 (P<0.001) were higher in seronegative subjects. The rs6001417 and rs8177832 SNPs were associated with HIV-1 infection in an additive model (P<0.01). Furthermore the SNP rs35228531 was also associated with HIV-1 infection in a dominant model (P<0.001). Odds ratio analysis of genotypes and alleles of the different APOBEC3G variants showed that there is a strong association between the minor genetic variants, genotype of the three SNPs, and HIV-1 status. Haplotype analysis demonstrated that rs6001417, rs8177832, and rs35228531 are in linkage disequilibrium. The haplotype GGT from the rs6001417, rs8177832 and rs35228531 respectively has a protective effect OR = 0.54 [0.43–0.68] with P<0.001. There was also associations between the haplotypes GGC OR = 1.6 [1.1;-2.3] P<0.05, and CGC OR = 5.21 [2.4–11.3] P<0.001, which increase the risk of infection by HIV-1 from almost two (2) to five (5) fold. This study demonstrates an association of rs6001417, rs8177832, and rs35228531 of APOBEC3G with HIV-1 infection in a population from Burkina Faso.

Houcine Maghrebi, Helmi Slama, Rachid Ksantini, Amine Makni, Fadhel Fteriche, Sofiene Ayadi, Wael Rebai, Amine Daghfous, Faouzi Chebbi, Adel Ammous, Mohamed Jouini, Montassar Kacem, Zoubeir Ben Safta

Aziz Elmadil, Hanan Bouamama, Mohammed Ramil, Khalid Khattal, Abderrahman Afifi, Youssef Bouabdallah

Fatima Zohra Fdili Alaoui, Kamilia Laabadi, Hekmat Chaara, Hekmat Bouguern, Abdilah Melhouf

Ahmed El Khader, Mohamed Lahkim, Rachid El Barni, Abdessamad Achour

Hind Cherrabi, Salahoudine Idrissa, Hind Abouljaoud, Abdoulaye Harouna diallo, Karima Atarraf, Aziz El Madi, Khalid Khattala, Youssef Bouabdallah

Diverticulum of the appendix is defined as the existence of diverticulum (s) arising from the cecal appendix resulting in polymorphic clinical manifestations characterized by infection of the appendix and the periappendicular region. We here report the case of a 28-year-old female patient without a significant medical history, presenting to the Emergency Department with fever, right iliac fossa pain associated with vomiting. Clinical examination showed a temperature of 38.5°C, clear susceptibility of the right iliac fossa with positive Blumberg sign and right rectal pain on touch. Differential diagnoses of urinary tract infection were excluded based on negative cytobacteriological examination of the urine. Other diagnoses, such as Crohn´s disease and colon cancers, were also excluded based on patient´s history and context. The diagnosis of acute appendicitis was made and the patient underwent coelioscopy that revealed internal laterocecal appendix with phlegmon and two diverticules at the tip and body of the appendix (A: a coelioscopic view of the appendix phlegmon, seat of the diverticulum). Appendicectomy was performed (B: macroscopic view of the appendix showing the diverticula) without incident. Histological examination showed acute appendicitis with uncomplicated diverticulitis. The postoperative course was uneventful.
