Mass azithromycin for prevention of child mortality among children with acute malnutrition: A subgroup analysis of a cluster randomized controlled trial

Children with acute malnutrition are at high risk of morality. Mass azithromycin distribution reduces all-cause mortality among children aged 1–59 months, and effects may be greater in underweight infants. Here, we evaluate the efficacy of azithromycin for reducing all-cause mortality in children aged 6–59 months with acute malnutrition (mid-upper arm circumference, MUAC, < 12.5 cm). Communities in Nouna District, Burkina Faso were 1:1 randomized to biannual mass distribution of single dose azithromycin or placebo to all children aged 1–59 months. Mortality was assessed during each census and treatment round. MUAC measurements were collected for all children. We evaluated the effect of azithromycin on mortality in subgroups of children aged 6–59 months defined by acute malnutrition (MUAC < 12.5 cm versus MUAC ≥ 12.5 cm). In children with MUAC < 12.5 cm, mortality rates were 51% lower among those living in azithromycin communities compared to placebo (incidence rate ratio 0.49, 95% confidence interval, CI, 0.25 to 0.99; incidence rate difference -18.1 deaths per 1,000 person-years, 95% CI -37.0 to -0.01), which was greater than the reduction in mortality among children with MUAC ≥ 12.5 cm (P-value for interaction on the relative scale = 0.09; P-value for interaction of the additive scale = 0.03). Children with acute malnutrition may benefit from single dose azithromycin above and beyond those without acute malnutrition.Trial registration: ClinicalTrials.gov NCT03676764; https://clinicaltrials.gov/study/NCT03676764

Mass media exposure and its impact on malaria prevention behaviour among adult women in sub-Saharan Africa: results from malaria indicator surveys

Sanni Yaya, Olalekan A. Uthman, Agbessi Amouzou, Ghose Bishwajit

BackgroundMass media exposure plays a pivotal role in health communication and adoption of a healthy lifestyle. In this study, we aimed to measure the prevalence of malaria prevention behaviour among adult women in eight malaria-endemic countries in sub-Saharan Africa (SSA), and assess the influence of mass media exposure in the adoption of those behaviours.MethodsFor this study, we collected cross-sectional data on 46,822 women aged between 15 and 49 years from the Malaria Indicator Surveys (MIS) conducted in Burkina Faso, Ghana, Mali, Malawi, Kenya, Nigeria, Sierra Leone and Uganda. As the outcome variable, malaria prevention behaviour was proxied by the use of insecticide treated nets (ITNs) and uptake of antimalarial drugs in last pregnancy.ResultsThe overall prevalence of sleeping under ITN and that of taking antimalarial drug during the last pregnancy was respectively 67.9% (95%CI = 66.6–69.2) and 72.8% (95%CI = 71.3–74.2). However, there were disparities in the prevalence of using ITN and antimalarial drug use across the study countries. In the multivariable regression analysis, not receiving malaria related information from radio, poster/billboards, community events, and health workers were found to be significantly associated with reduction in the odds of using ITN the previous night. For the use of antimalarial drugs during last pregnancy, the odds were 23% [OR = 0.773, 95%CI = 0.625–0.956] lower for those who did not receive malaria information on radio compared with those who received.ConclusionsThese findings indicate a potentially important role of malaria information received through mass media on utilisation of ITN among women in SSA. More research is needed to explore the factors that limit the accessibility to malaria information through mass media.

Anass Ayad, Badr Ettouhami, Benouachane Thami, Abdelali Bentahila

Amina Ben Salem, Sana Yahyaoui, Amal Messoud, Houda El Mhabrech, Rajaa Faleh, Chiraz Hafsa

Dondo Mara, Moussa Sylla, Seydou Ly, Ismaël Dandakoye Soumana, Badr Alami, Mustapha Maâroufi, Khalid Maazaz, Youssef Alaoui Lamrani

Alpha Boubacar Conte, Fatima Zohra Fdili Alaoui, Sofia Jayi, Hikmat Chaara, Moulay Abdelilah Melhouf

Valentine Séréna Ndong, Ghita Taki, Hicham Harmouche

Maternal alcohol consumption during pregnancy and child’s cognitive performance at 6–8 years of age in rural Burkina Faso: an observational study

BackgroundIn Burkina Faso, it is not uncommon for mothers to drink alcohol, even during pregnancy. We aimed to study the association between maternal alcohol consumption during pregnancy and the child’s cognitive performance using the Kaufman Assessment Battery for Children, 2nd edition (KABC-II) and the Children’s Category Test Level 1 (CCT-1) in rural Burkina Faso.MethodsWe conducted a follow-up study of a community cluster-randomised Exclusive breastfeeding trial, and re-enrolled the children in rural Burkina Faso. A total of 518 children (268 boys and 250 girls) aged 6–8 years were assessed using the KABC-II and the CCT-1. We examined the effect size difference using Cohen’s d and conducted a linear regression analysis to examine the association.ResultsSelf-reported alcohol consumption during pregnancy was 18.5% (96/518). Children whose mothers reported alcohol consumption during pregnancy performed significantly poorly for memory and spatial abilities tests from small effect size difference for ‘Atlantis’ (0.27) and ‘Triangle’ (0.29) to moderate effect size difference for ‘Number recall’ (0.72) compared to children whose mothers did not consume alcohol during pregnancy; the exposed children scored significantly higher errors with a small effect size (0.37) at problem solving (CCT-1) test compared to unexposed children. At unstandardized and standardized multivariable analysis, children whose mothers reported alcohol consumption during pregnancy performed significantly poorer for memory-‘Atlantis’ (p = 0.03) and ‘Number recall’ (p = 0.0001), and spatial ability tests-‘Triangle’ (p = 0.03); they scored significantly higher errors at problem solving CCT-1 test (p = 0.002); all the results were adjusted for age, sex, schooling, stunting, father’s education, mother’s employment and the promotion of exclusive breastfeeding. No statistical association was found for visual abilities-‘Conceptual Thinking’, ‘Face recognition’, ‘Story completion’, and reasoning tests-‘Rove

Maternal anaemia and duration of zidovudine in antiretroviral regimens for preventing mother-to-child transmission: a randomized trial in three African countries

Benn KD Sartorius, Matthew F Chersich, Mary Mwaura, Nicolas Meda, Marleen Temmerman, Marie Louise Newell, Timothy MM Farley, Stanley Luchters

BackgroundAlthough substantiated by little evidence, concerns about zidovudine-related anaemia in pregnancy have influenced antiretroviral (ARV) regimen choice for preventing mother-to-child transmission of HIV-1, especially in settings where anaemia is common.MethodsEligible HIV-infected pregnant women in Burkina Faso, Kenya and South Africa were followed from 28 weeks of pregnancy until 12–24 months after delivery (n = 1070). Women with a CD4 count of 200-500cells/mm3 and gestational age 28–36 weeks were randomly assigned to zidovudine-containing triple-ARV prophylaxis continued during breastfeeding up to 6-months, or to zidovudine during pregnancy plus single-dose nevirapine (sd-NVP) at labour. Additionally, two cohorts were established, women with CD4 counts: <200 cells/mm3 initiated antiretroviral therapy, and >500 cells/mm3 received zidovudine during pregnancy plus sd-NVP at labour. Mild (haemoglobin 8.0-10.9 g/dl) and severe anaemia (haemoglobin < 8.0 g/dl) occurrence were assessed across study arms, using Kaplan-Meier and multivariable Cox proportional hazards models.ResultsAt enrolment (corresponded to a median 32 weeks gestation), median haemoglobin was 10.3 g/dl (IQR = 9.2-11.1). Severe anaemia occurred subsequently in 194 (18.1%) women, mostly in those with low baseline haemoglobin, lowest socio-economic category, advanced HIV disease, prolonged breastfeeding (≥6 months) and shorter ARV exposure. Severe anaemia incidence was similar in the randomized arms (equivalence P-value = 0.32). After 1–2 months of ARV’s, severe anaemia was significantly reduced in all groups, though remained highest in the low CD4 cohort.ConclusionsSevere anaemia occurs at a similar rate in women receiving longer triple zidovudine-containing regimens or shorter prophylaxis. Pregnant women with pre-existing anaemia and advanced HIV disease require close monitoring.Trial registration numberISRCTN71468401

Maternal balanced energy-protein supplementation reshapes the maternal gut microbiome and enhances carbohydrate metabolism in infants: a randomized controlled trial

Lishi Deng, Steff Taelman, Matthew R. Olm, Laeticia Celine Toe, Eva Balini, Lionel Olivier Ouédraogo, Yuri Bastos-Moreira, Alemayehu Argaw, Kokeb Tesfamariam, Erica D. Sonnenburg, Giles T. Hanley-Cook, Moctar Ouédraogo, Rasmané Ganaba, Wim Van Criekinge, Lieven Huybregts, Michiel Stock, Patrick Kolsteren, Justin L. Sonnenburg, Carl Lachat, Trenton Dailey-Chwalibóg

Balanced energy-protein (BEP) supplementation during pregnancy and lactation can improve birth outcomes and infant growth, with the gut microbiome as a potential mediator. The MISAME-III randomized controlled trial (ClinicalTrial.gov: NCT03533712) assessed the effect of BEP supplementation, provided during pregnancy and the first six months of lactation, on small-for-gestational age prevalence and length-for-age Z-scores at six months in rural Burkina Faso. Nested within MISAME-III, this sub-study examines the impact of BEP supplementation on maternal and infant gut microbiomes and their mediating role in birth outcomes and infant growth. A total of 152 mother-infant dyads (n = 71 intervention, n = 81 control) were included for metagenomic sequencing, with stool samples collected at the second and third trimesters, and at 1–2 and 5–6 months postpartum. BEP supplementation significantly altered maternal gut microbiome diversity, composition, and function, particularly those with immune-modulatory properties. Pathways linked to lipopolysaccharide biosynthesis were depleted and the species Bacteroides fragilis was enriched in BEP-supplemented mothers. Maternal BEP supplementation also accelerated infant microbiome changes and enhanced carbohydrate metabolism. Causal mediation analyses identified specific taxa mediating the effect of BEP on birth outcomes and infant growth. These findings suggest that maternal supplementation modulates gut microbiome composition and influences early-life development in resource-limited settings. Here, in a sub-study of the MISAME-III randomized controlled trial, the authors show that maternal balanced energy-protein supplementation during pregnancy and lactation alters the maternal gut microbiome, leading to an enhanced infant carbohydrate metabolism, and identify microbial taxa with a potential role in mediating the effects.

Maternal health care utilization in Nairobi and Ouagadougou: evidence from HDSS

Clémentine Rossier, Kanyiva Muindi, Abdramane Soura, Blessing Mberu, Bruno Lankoande, Caroline Kabiru, Roch Millogo

BackgroundMaternal mortality is higher and skilled attendance at delivery is lower in the slums of Nairobi (Kenya) compared to Ouagadougou (Burkina Faso). Lower numbers of public health facilities, greater distance to facilities, and higher costs of maternal health services in Nairobi could explain these differences.ObjectiveBy comparing the use of maternal health care services among women with similar characteristics in the two cities, we will produce a more nuanced picture of the contextual factors at play.DesignWe use birth statistics collected between 2009 and 2011 in all households living in several poor neighborhoods followed by the Nairobi and the Ouagadougou Health and Demographic Surveillances Systems (n=3,346 and 4,239 births). We compare the socioeconomic characteristics associated with antenatal care (ANC) use and deliveries at health facilities, controlling for demographic variables.ResultsANC use is greater in Nairobi than in Ouagadougou for every category of women. In Ouagadougou, there are few differentials in having at least one ANC visit and in delivering at a health facility; however, differences are observed for completing all four ANC visits. In Nairobi, less-educated, poorer, non-Kikuyu women, and women living in the neighborhood farther from public health services have poorer ANC and deliver more often outside of a health facility.ConclusionsThese results suggest that women are more aware of the importance of ANC utilization in Nairobi compared to Ouagadougou. The presence of numerous for-profit health facilities within slums in Nairobi may also help women have all four ANC visits, although the services received may be of substandard quality. In Ouagadougou, the lack of socioeconomic differentials in having at least one ANC visit and in delivering at a health facility suggests that these practices stem from the application of well-enforced maternal health regulations; however, these regulations do not cover the entire set of four ANC visits.

Maternal perceptions of the quality of Care in the Free Maternal Care Policy in sub-Sahara Africa: a systematic scoping review

Monica Ansu-Mensah, Frederick I. Danquah, Vitalis Bawontuo, Peter Ansu-Mensah, Desmond Kuupiel

BackgroundThe world aims to achieve universal health coverage by removing all forms of financial barriers to improve access to healthcare as well as reduce maternal and child deaths by 2030. Although free maternal healthcare has been embraced as a major intervention towards this course in some countries in sub-Saharan Africa (SSA), the perception of the quality of healthcare may influence utilization and maternal health outcomes. We systematically mapped literature and described the evidence on maternal perceptions of the quality of care under the free care financing policies in SSA.MethodsWe employed the Arskey and O’Malley’s framework to guide this scoping review. We searched without date limitations to 19th May 2019 for relevant published articles in PubMed, Google Scholar, Web of Science, Science Direct, and CINAHL using a combination of keywords, Boolean terms, and medical subject headings. We included primary studies that involved pregnant/post-natal mothers, free maternal care policy, quality of care, and was conduct in an SSA country. Two reviewers independently screened the articles at the abstract and full-text screening guided by inclusion and exclusion criteria. All relevant data were extracted and organized into themes and a summary of the results reported narratively. The recent version of the mixed methods appraisal tool was used to assess the methodological quality of the included studies.ResultsOut of 390 studies, 13 were identified to have evidence of free maternal healthcare and client perceived quality of care. All the 13 studies were conducted in 7 different countries. We found three studies each from Ghana and Kenya, two each in Burkina Faso and Nigeria, and a study each from Niger, Sierra Leone, and Tanzania. Of the 13 included studies, eight reported that pregnant women perceived the quality of care under the free maternal healthcare policy to be poor. The following reasons accounted for the poor perception of service quality: long waiting ti

Mathematical models of human mobility of relevance to malaria transmission in Africa

John M. Marshall, Sean L. Wu, Hector M. Sanchez C., Samson S. Kiware, Micky Ndhlovu, André Lin Ouédraogo, Mahamoudou B. Touré, Hugh J. Sturrock, Azra C. Ghani, Neil M. Ferguson

As Africa-wide malaria prevalence declines, an understanding of human movement patterns is essential to inform how best to target interventions. We fitted movement models to trip data from surveys conducted at 3–5 sites throughout each of Mali, Burkina Faso, Zambia and Tanzania. Two models were compared in terms of their ability to predict the observed movement patterns – a gravity model, in which movement rates between pairs of locations increase with population size and decrease with distance, and a radiation model, in which travelers are cumulatively “absorbed” as they move outwards from their origin of travel. The gravity model provided a better fit to the data overall and for travel to large populations, while the radiation model provided a better fit for nearby populations. One strength of the data set was that trips could be categorized according to traveler group – namely, women traveling with children in all survey countries and youth workers in Mali. For gravity models fitted to data specific to these groups, youth workers were found to have a higher travel frequency to large population centers, and women traveling with children a lower frequency. These models may help predict the spatial transmission of malaria parasites and inform strategies to control their spread.

    L. Fauquembergue, G. Valentin, B. Buscail, A. Descatha

Introduction. - Sectors-Job retention/disability matrix was used on Reunion Island from 2017 to 2021 to prevent health related job losses. We aim to update the study with three years to determine the evolution and assess the different factors between restriction and job loss. Method. - A similar study will use data from the two occupational health and prevention units on Reunion Island. The data will cover the period from December 1, 2017, to December 1, 2024. It will include the sector matrix (Nomenclature Franc, aise des activites 2008, NAF), the number of job losses, and the number of employees benefiting from the obligation to employ disabled workers (BOETH) per year. For 2023 and 2024, skills with restrictions have been added as a job category according to the 2003 classification by professions and socio-professional categories (PCS 2003), in addition to sex, age, and type of company. The MADE jobs-exposure matrix on arduous exposure was applied. Results. - From 2017 to 2024, more BOETHs were found, and job raised and stabilized. The home care sector still was the first activity sectors since 2017, without any effect related to the pandemic. People with job loss in 2023-24 compared to those who benefit from restriction were older, more often blue-collar workers, and more exposed to difficult work. Conclusion. - Updating these job-incapacity/disability matrices showed that action is needed to prevent occupational health related job losses.

AbstractObjectivesTo explore the feasibility of the confidante methodology to measure past-year intimate partner violence (IPV) experiences in Burkina Faso and the Democratic Republic of the Congo (DRC) through (1) comparison of direct assessment with indirect estimation via the confidante method and (2) assessment of the performance of each confidante method assumption.DesignCross-sectional study with nationally and subnationally representative data collected from December 2020 to March 2021 in Burkina Faso (national) and from December 2021 to April 2022 in Kinshasa and Kongo Central, DRC (subnational).SettingBurkina Faso; Kinshasa, DRC; Kongo Central, DRC.ParticipantsPartnered women (married or cohabiting) aged 15–49 in Burkina Faso (N=3047), Kinshasa, DRC (N=702) and Kongo Central, DRC (N=688) and their partnered confidantes aged 15–49 (N=2064 in Burkina Faso, N=304 in Kinshasa, DRC, N=393 women in Kongo Central, DRC).Primary and secondary outcome measuresPast-year IPV (emotional, physical, sexual, any) comparing differences in prevalence between the direct respondent sample and the indirect confidante sample, adjusting for confidante method assumptions.ResultsThe confidante method produced comparable IPV estimates to respondents’ direct reports across sites (35.3% respondent vs 36.1% confidante in Kinshasa, DRC; 29.7% respondent vs 39.0% confidante in Kongo Central, DRC; 25.7% respondent vs 26.0% confidante in Burkina Faso, differences not statistically significant). Of note, there were differences in IPV estimates between respondents and confidantes by IPV subtype, with physical IPV consistently lower among respondents across sites and sexual IPV lower among confidantes in Kinshasa, DRC and Burkina Faso, though generally not statistically significant.ConclusionsThe confidante methodology did not afford advantages over standard, direct assessment for IPV. Overall, findings indicate the reliability of population-based surveys with direct IPV questions when implem

Measurement properties of the project-level Women's Empowerment in Agriculture Index

Kathryn M. Yount, Yuk Fai Cheong, Lauren Maxwell, Jessica Heckert, Elena M. Martinez, Gregory Seymour

Highlights•We tested the measurement properties of the project-level Women’s Empowerment in Agriculture Index in two projects/sites.•We used IRT methods to assess item sets used in pro-WEAI indicators for intrinsic, instrumental, and collective agency.•One item set capturing intrinsic agency in the right to bodily integrity was measurement equivalent across the two projects.•We recommend refining the other item sets to improve their measurement properties.•We offer steps to test, refine, validate, and shorten pro-WEAI and other women’s empowerment scales in development studies. Women’s empowerment is a process that includes increases in intrinsic agency (power within); instrumental agency (power to); and collective agency (power with). We used baseline data from two studies—Targeting and Realigning Agriculture for Improved Nutrition (TRAIN) in Bangladesh and Building Resilience in Burkina Faso (BRB)—to assess the measurement properties of survey questions operationalizing selected dimensions of intrinsic, instrumental, and collective agency in the project-level Women’s Empowerment in Agricultural Index (pro-WEAI). We applied unidimensional item-response models to question (item) sets to assess their measurement properties, and when possible, their cross-context measurement equivalence—a requirement of measures designed for cross-group comparisons. For intrinsic agency in the right to bodily integrity, measured with five attitudinal questions about intimate partner violence (IPV) against women, model assumptions of unidimensionality and local independence were met. Four items showed good model fit and measurement equivalence across TRAIN and BRB. For item sets designed to capture autonomy in income, intrinsic agency in livelihoods activities, and instrumental agency in: livelihoods activities, the sale or use of outputs, the use of income, and borrowing from financial services, model assumptions were not met, model fit was poor, and items generally were weakly relate

Measuring adult mortality from mobile phone surveys in Burkina Faso, Malawi and the Democratic Republic of the Congo

AbstractIn many low and middle-income countries, adult mortality estimates are derived from surveys and censuses conducted through face-to-face interviews. These interviews can be time-intensive and are often impractical during health crises or humanitarian emergencies. The expansion in cellphone ownership and network coverage has created new opportunities for collecting demographic data through mobile phone surveys, but our understanding of selection biases and reporting errors of such data remains incomplete. This study reports on adult mortality estimates obtained through mobile phone surveys conducted in Burkina Faso, Malawi and the Democratic Republic of the Congo in 2021 and 2022. To mitigate respondent fatigue and network interruptions, we used a shortened version of the set of questions generally used in surveys to ask about the survival of respondents’ siblings. Mortality estimates obtained from mobile phone interviews were lower than those from face-to-face demographic surveys. Mortality rates from the mobile phone surveys were also approximately half those expected from World Population Prospects (WPP) estimates. We attribute this underestimation primarily to reporting errors, including inaccuracies in the ages and timing of sibling deaths collected through the shortened instrument. Coverage biases due to mobile phone ownership likely played a secondary role in the reduced mortality estimates. After imputing ages and dates based on full sibling histories collected in previous face-to-face surveys, mortality rates were more consistent with WPP and Demographic and Health Survey estimates. However, estimates would be improved with more accurate age at death and time of death reports. Mobile phone surveys offer a promising alternative for monitoring adult mortality in settings where face-to-face data collection is not feasible, but they seem to be susceptible to more frequent reporting errors.
