Missed Opportunities for Early Access to Care of HIV-Infected Infants in Burkina Faso

ObjectiveThe World Health Organization (WHO) has recommended a universal antiretroviral therapy (ART) for all HIV-infected children before the age of two since 2010, but this implies an early identification of these infants. We described the Prevention of Mother-to-Child HIV Transmission (PMTCT) cascade, the staffing and the quality of infrastructures in pediatric HIV care facilities, in Ouagadougou, Burkina Faso.MethodsWe conducted a cross-sectional survey in 2011 in all health care facilities involved in PMTCT and pediatric HIV care in Ouagadougou. We assessed them according to their coverage in pediatric HIV care and WHO standards, through a desk review of medical registers and a semi-structured questionnaire administered to health-care workers (HCW).ResultsIn 2011, there was no offer of care in primary health care facilities for HIV-infected children in Ouagadougou. Six district hospitals and two university hospitals provided pediatric HIV care. Among the 67 592 pregnant women attending antenatal clinics in 2011, 85.9% were tested for HIV. The prevalence of HIV was 1.8% (95% Confidence Interval: 1.7%–1.9%). Among the 1 064 HIV-infected pregnant women attending antenatal clinics, 41.4% received a mother-to-child HIV transmission prevention intervention. Among the HIV-exposed infants, 313 (29.4%) had an early infant HIV test, and 306 (97.8%) of these infants tested received their result within a four-month period. Among the 40 children initially tested HIV-infected, 33 (82.5%) were referred to a health care facility, 3 (9.0%) were false positive, and 27 (90.0%) were initiated on ART. Although health care facilities were adequately supplied with HIV drugs, they were hindered by operational challenges such as shortage of infrastructures, laboratory reagents, and trained HCW.ConclusionsThe PMTCT cascade revealed bottle necks in PMTCT intervention and HIV early infant diagnosis. The staffing in HIV care and quality of health care infrastructures were also insufficient

Missed opportunities of inclusion in a cohort of HIV-infected children to initiate antiretroviral treatment before the age of two in West Africa, 2011 to 2013

Désiré L Dahourou, Madeleine Amorissani-Folquet, Malik Coulibaly, Divine Avit-Edi, Nicolas Meda, Marguerite Timite-Konan, Vic Arendt, Diarra Ye, Clarisse Amani-Bosse, Roger Salamon, Philippe Lepage, Valériane Leroy

IntroductionThe World Health Organization (WHO) 2010 guidelines recommended to treat all HIV-infected children less than two years of age. We described the inclusion process and its correlates of HIV-infected children initiated on early antiretroviral therapy (EART) at less than two years of age in Abidjan, Côte d'Ivoire, and Ouagadougou, Burkina Faso.MethodsAll children with HIV-1 infection confirmed with a DNA PCR test of a blood sample, aged less than two years, living at a distance less than two hours from the centres and whose parents (or mother if she was the only legal guardian or the legal caregiver if parents were not alive) agreed to participate in the MONOD ANRS 12206 project were included in a cohort to receive EART based on lopinavir/r. We used logistic regression to identify correlates of inclusion.ResultsAmong the 217 children screened and referred to the MONOD centres, 161 (74%) were included and initiated on EART. The main reasons of non-inclusion were fear of father's refusal (48%), mortality (24%), false-positive HIV infection test (16%) and other ineligibility reasons (12%). Having previously disclosed the child's and mother's HIV status to the father (adjusted odds ratio (aOR): 3.20; 95% confidence interval (95% CI): 1.55 to 6.69) and being older than 12 months (aOR: 2.05; 95% CI: 1.02 to 4.12) were correlates of EART initiation. At EART initiation, the median age was 13.5 months, 70% had reached WHO Stage 3/4 and 57% had a severe immune deficiency.ConclusionsFear of stigmatization by the father and early competing mortality were the major reasons for missed opportunities of EART initiation. There is an urgent need to involve fathers in the care of their HIV-exposed children and to promote early infant diagnosis to improve their future access to EART and survival.

Mitigation of aflatoxin contamination of maize, groundnut, and sorghum by commercial biocontrol products in farmers’ fields across Burkina Faso, Mali, Niger, and Togo

Saïdou Bonkoungou, Karim Dagno, Adamou Basso, Tedihou Ekanao, Joseph Atehnkeng, Daniel Agbetiameh, Adama Neya, Mahama Toure, Assiata Tiendrebeogo, Mamadou Konate, Bibata Outani, Matieyedou Konlambigue, Kenneth A. Callicott, Peter J. Cotty, Ibnou Dieng, Titilayo D. O. Falade, Ranajit Bandyopadhyay, Alejandro Ortega-Beltran

BackgroundAflatoxin contamination by Aspergillus section Flavi fungi poses a significant threat to food security and public health in sub-Saharan Africa (SSA). Maize, groundnut, and sorghum are staple crops frequently contaminated with aflatoxins, sometimes at dangerous levels. Despite its detrimental effects, many farmers in SSA lack access to effective tools for mitigating aflatoxin contamination. Biocontrol based on atoxigenic isolates of A. flavus is an effective tool to limit aflatoxin contamination.MethodsThe development, testing, registration, and commercial use of the aflatoxin biocontrol product Aflasafe BF01 for use in Burkina Faso is described. In addition, the deployment of the biocontrol technology across Mali, Niger, and Togo is documented, and for the first time, the use of aflatoxin biocontrol in sorghum is reported.ResultsIn all four countries, treated crops had significantly (P < 0.05) less aflatoxins than crops from untreated fields. Most treated crops met the stringent tolerance threshold for human consumption, 4 ppb total aflatoxin. Using native atoxigenic isolates of A. flavus and employing a multi-disciplinary approach, aflatoxin biocontrol products have demonstrated significant success in reducing aflatoxin levels in treated crops compared to untreated ones.ConclusionsThis multi-year, multi-funded source study underscores the effectiveness of biocontrol strategies in mitigating aflatoxin contamination at scale, offering a regional approach for sustainable management in West Africa and potentially unlocking significant health and economic benefits for the region.Supplementary InformationThe online version contains supplementary material available at 10.1186/s43170-024-00313-3.

Mitochondrial DNA analysis of field populations of Helicoverpa armigera (Lepidoptera: Noctuidae) and of its relationship to H. zea

Gajanan T Behere, Wee Tek Tay, Derek A Russell, David G Heckel, Belinda R Appleton, Keshav R Kranthi, Philip Batterham

BackgroundHelicoverpa armigera and H. zea are amongst the most significant polyphagous pest lepidopteran species in the Old and New Worlds respectively. Separation of H. armigera and H. zea is difficult and is usually only achieved through morphological differences in the genitalia. They are capable of interbreeding to produce fertile offspring. The single species status of H. armigera has been doubted, due to its wide distribution and plant host range across the Old World. This study explores the global genetic diversity of H. armigera and its evolutionary relationship to H zea.ResultsWe obtained partial (511 bp) mitochondrial DNA (mtDNA) Cytochrome Oxidase-I (COI) sequences for 249 individuals of H. armigera sampled from Australia, Burkina Faso, Uganda, China, India and Pakistan which were associated with various host plants. Single nucleotide polymorphisms (SNPs) within the partial COI gene differentiated H. armigera populations into 33 mtDNA haplotypes. Shared haplotypes between continents, low F-statistic values and low nucleotide diversity between countries (0.0017 – 0.0038) suggests high mobility in this pest. Phylogenetic analysis of four major Helicoverpa pest species indicates that H. punctigera is basal to H. assulta, which is in turn basal to H. armigera and H. zea. Samples from North and South America suggest that H. zea is also a single species across its distribution. Our data reveal short genetic distances between H. armigera and H. zea which seem to have been established via a founder event from H. armigera stock at around 1.5 million years ago.ConclusionOur mitochondrial DNA sequence data supports the single species status of H. armigera across Africa, Asia and Australia. The evidence for inter-continental gene flow observed in this study is consistent with published evidence of the capacity of this species to migrate over long distances. The finding of high genetic similarity between Old World H. armigera and New World H. zea emphasises the need t

Mixture for Controlling Insecticide-Resistant Malaria Vectors

Cédric Pennetier, Carlo Costantini, Vincent Corbel, Séverine Licciardi, Roch K. Dabiré, Bruno Lapied, Fabrice Chandre, Jean-Marc Hougard

Impregnating mosquito nets with an insect repellent and a low dose of organophosphorous insecticide combination was effective. The spread of resistance to pyrethroids in the major Afrotropical malaria vectors Anopheles gambiae s.s. necessitates the development of new strategies to control resistant mosquito populations. To test the efficacy of nets treated with repellent and insecticide against susceptible and insecticide-resistant An. gambiae mosquito populations, we impregnated mosquito bed nets with an insect repellent mixed with a low dose of organophosphorous insecticide and tested them in a rice-growing area near Bobo-Dioulasso, Burkina Faso. During the first 2 weeks posttreatment, the mixture was as effective as deltamethrin alone and was more effective at killing An. gambiae that carried knockdown resistance (kdr) or insensitive acetylcholinesterase resistance (Ace1R) genes. The mixture seemed to not kill more susceptible genotypes for the kdr or Ace1R alleles. Mixing repellents and organophosphates on bed nets could be used to control insecticide-resistant malaria vectors if residual activity of the mixture is extended and safety is verified.

Mobile Microbiological Laboratory Support for Evaluation of a Meningitis Epidemic in Northern Benin

BackgroundFixed laboratory capacity in Africa may be inadequate; mobile microbiological laboratories may address this issue but their utility has seldom been evaluated.MethodsDuring 2012, the Benin Ministry of Health requested mobile microbiological laboratory (LaboMobil®) support following the failure of polysaccharide meningococcal A+C vaccine to prevent an epidemic in five Northern districts. Within four days, the intervention was initiated. A fixed site in Northern Togo, Pasteur Institutes in Côte d’Ivoire and France, and a research laboratory in Burkina Faso provided additional laboratory support.ResultsLocal laboratories initially reported most cases to have Gram-positive diplococci suggestive of pneumococcal meningitis. The LaboMobil® evaluated 200 cerebrospinal fluid (CSF) samples and 59 stored isolates collected from 149 individuals. Of the 74 individuals with etiologic confirmation, 60 (81%) had NmW135 and 11 (15%) NmX identified; no pneumococci were identified. Testing in France on 30 NmW135 and 3 NmX confirmed the etiology in all cases. All five districts had crossed the epidemic threshold (10 cases per 100,000 per week), all had NmW135 identified and four had NmX identified. NmX were identified as X:ST-181:ccST-181∶5-1∶10-1:F1–31 and NmW135 as W:ST-11: ccST-11∶5∶2:F1-1.ConclusionsIn an area with limited local laboratory capacity, a mobile microbiology laboratory intervention occurred in four days through the cooperation of four African and one European country. Results were different from those reported by local laboratories. Despite the introduction of serogroup A meningococcal and 13-valent pneumococcal conjugate vaccines, endemic and epidemic meningitis will continue in the region, emphasizing the usefulness of the LaboMobil® in the short and medium term.

Majda Sebbani, Latifa Adarmouch, Mohamed Amine, Mohamed Cherkaoui

Mobilizing stakeholders for implant removals in Burkina Faso using landscape assessment data

Justin Tiendrebeogo, Bethany Arnold, Yacouba Ouedraogo, Rachel Haws, Lea Pepin Garane, Virginie Ouedraogo, Maria Gouem, Alimata Coulibaly, Mathieu Bougma

BackgroundSuccessful efforts to encourage uptake of subdermal contraceptive implants, with a lifespan of three to five years, necessitate planning to ensure that quality removal services are available when desired. In Burkina Faso, implant use has tripled over the past 8 years and now comprises almost half of the contraceptive method mix. Population Monitoring for Action (PMA) surveys identified barriers to obtaining quality removal when desired, particularly when the implant is not palpable, or providers lack needed skills or supplies. The Expanding Family Planning Choices (EFPC) project supported ministries of health in four countries with evaluation and strengthening of implant removal services.MethodsAn implant removal landscape assessment was conducted at 24 health facilities in three regions of Burkina Faso with high implant use that included provider observations of implant removal, interviews with providers and health facility managers, and facility readiness surveys. The project used landscape data to mobilize stakeholders through a series of participatory workshops to develop a collaborative roadmap and commit to actions supporting quality implant removals.ResultsLandscape findings revealed key gaps in provision of quality removal services, including high levels of provider confidence for implant insertion and removal (82% and 71%, respectively), low competence performing simple and difficult removals (19.2% and 11.1%, respectively), inadequate supplies and equipment (no facilities had all necessary materials for removal), lack of difficult removal management systems, and a lack of standard data collection tools for removal. Exposure to the data convinced stakeholders to focus on removals rather than expanding insertion services. While not all roadmap commitments were achieved, the process led to critical investments in quality implant removals.ConclusionLandscape data revealed that facilities lack needed supplies and equipment, and providers lack skills n

Caroline Bonnet, Anne Buttard, Tony Foglia, Anne Griffond-Boitier, Bérénice Lambert, Yoann Martin, Clément Previtali, Maylis Sposito-Tourier

Dhoha Ben Salah, Nabila Rekik, Lilia Affes, Mouna Elleuch, Fatma Mnif, Mouna Mnif, Mohamed Abid

Primary hyperparathyroidism usually affects the elderly. Juvenile hyperparathyroidism is rare and poses specific diagnostic and therapeutic problems. We report the case of two young female patients, aged 14 and 19 years, with primary hyperparathyroidism detected due to femoral head involvement. One of those patients had slipped upper femoral epiphysis. Hyperparathyroidism only affected bone in both cases. No patient had a family history of this disease. Primary hyperparathyroidism was associated with solitary and benign parathyroid adenoma in both patients. They underwent complicated adenomectomy by persistent hypocalcaemia. This last was associated with hypoparathyroidism exacerbated by hungry bone syndrome, requiring substitutive vitaminocalcic therapy. It is well known that primary hyperparathyroidism rarely occurs in young subjects. Renal involvement is the most common clinical

Ryme El Harraqui, Ilham Karimi, Abdeljalil Chemlal, Fatiha Alaoui Ismaili, Intissar Haddiya

Amina Atmane, Sanaa Hammi, Asmâa Regragui, Mohammed Raoufi, Karima Marc, Mouna Soualhi, Rachida Zahraoui, Jouda Benamor, Jamal Eddine Bourkadi

Joseph Nsambi, Olivier Mukuku, Prosper Kakudji, Jean-Baptiste Kakoma

Laetitia Poirier, Eric Ghigo, David Daudé, Eric Chabriere

Modeling and Validation of Environmental Suitability for Schistosomiasis Transmission Using Remote Sensing

BackgroundSchistosomiasis is the most widespread water-based disease in sub-Saharan Africa. Transmission is governed by the spatial distribution of specific freshwater snails that act as intermediate hosts and human water contact patterns. Remote sensing data have been utilized for spatially explicit risk profiling of schistosomiasis. We investigated the potential of remote sensing to characterize habitat conditions of parasite and intermediate host snails and discuss the relevance for public health.MethodologyWe employed high-resolution remote sensing data, environmental field measurements, and ecological data to model environmental suitability for schistosomiasis-related parasite and snail species. The model was developed for Burkina Faso using a habitat suitability index (HSI). The plausibility of remote sensing habitat variables was validated using field measurements. The established model was transferred to different ecological settings in Côte d’Ivoire and validated against readily available survey data from school-aged children.Principal FindingsEnvironmental suitability for schistosomiasis transmission was spatially delineated and quantified by seven habitat variables derived from remote sensing data. The strengths and weaknesses highlighted by the plausibility analysis showed that temporal dynamic water and vegetation measures were particularly useful to model parasite and snail habitat suitability, whereas the measurement of water surface temperature and topographic variables did not perform appropriately. The transferability of the model showed significant relations between the HSI and infection prevalence in study sites of Côte d’Ivoire.Conclusions/SignificanceA predictive map of environmental suitability for schistosomiasis transmission can support measures to gain and sustain control. This is particularly relevant as emphasis is shifting from morbidity control to interrupting transmission. Further validation of our mechanistic model needs to be complem

Modeling Long-term Vaccination Strategies With MenAfriVac in the African Meningitis Belt

Andromachi Karachaliou, Andrew J. K. Conlan, Marie-Pierre Preziosi, Caroline L. Trotter

Background. The introduction of MenAfriVac in campaigns targeting people aged 1–29 years across the African meningitis belt has successfully reduced meningitis incidence and carriage due to Neisseria meningitidis group A (MenA). It is important to consider how best to sustain population protection in the long term.Methods. We created a mathematical model of MenA transmission and disease to investigate the potential impact of a range of immunization strategies. The model is age structured; includes classes of susceptible, carrier, ill, and immune people (who may be vaccinated or unvaccinated); and incorporates seasonal transmission and a stochastic forcing term that models between year variation in rates of transmission. Model parameters were primarily derived from African sources. The model can describe the typical annual incidence of meningitis in the prevaccine era, with irregular epidemics of varying size. Parameter and structural uncertainty were explored in sensitivity analyses.Results. Following MenAfriVac introduction at high uptake, the model predicts excellent short-term disease control. With no subsequent immunization, strong resurgences in disease incidence were predicted after approximately 15 years (assuming 10 years’ average vaccine protection). Routine immunization at 9 months of age resulted in lower average annual incidence than regular mass campaigns of 1- to 4-year-olds, provided coverage was above approximately 60%. The strategy with the lowest overall average annual incidence and longest time to resurgence was achieved using a combination strategy of introduction into the Expanded Programme on Immunization at 9 months, 5 years after the initial mass campaigns, with a catch-up targeting unvaccinated 1- to 4-year-olds.Conclusions. These results can be used to inform policy recommendations for long-term vaccination strategies with MenAfriVac.

Modeling Malaria Infection and Immunity against Variant Surface Antigens in Príncipe Island, West Africa

After remarkable success of vector control campaigns worldwide, concerns about loss of immunity against Plasmodium falciparum due to lack of exposure to the parasite are relevant since an increase of severe cases in less immune individuals is expected. We present a mathematical model to investigate the impact of reducing exposure to the parasite on the immune repertoire against P. falciparum erythrocyte membrane protein 1 (PfEMP1) variants. The model was parameterized with data from Príncipe Island, West Africa, and applied to simulate two alternative transmission scenarios: one where control measures are continued to eventually drive the system to elimination; and another where the effort is interrupted after 6 years of its initiation and the system returns to the initial transmission potential. Population dynamics of parasite prevalence predict that in a few years infection levels return to the pre-control values, while the re-acquisition of the immune repertoire against PfEMP1 is slower, creating a window for increased severity. The model illustrates the consequences of loss of immune repertoire against PfEMP1 in a given setting and can be applied to other regions where similar data may be available.

AbstractSince 2010, the introduction of an effective serogroup A meningococcal conjugate vaccine has led to the near-elimination of invasive Neisseria meningitidis serogroup A disease in Africa’s meningitis belt. However, a significant burden of disease and epidemics due to other bacterial meningitis pathogens remain in the region. High-quality surveillance data with laboratory confirmation is important to monitor circulating bacterial meningitis pathogens and design appropriate interventions, but complete testing of all reported cases is often infeasible. Here, we use case-based surveillance data from 5 countries in the meningitis belt to determine how accurately estimates of the distribution of causative pathogens would represent the true distribution under different laboratory testing strategies. Detailed case-based surveillance data was collected by the MenAfriNet surveillance consortium in up to 3 seasons from participating districts in 5 countries. For each unique country-season pair, we simulated the accuracy of laboratory surveillance by repeatedly drawing subsets of tested cases and calculating the margin of error of the estimated proportion of cases caused by each pathogen (the greatest pathogen-specific absolute error in proportions between the subset and the full set of cases). Across the 12 country-season pairs analyzed, the 95% credible intervals around estimates of the proportion of cases caused by each pathogen had median widths of ±0.13, ±0.07, and ±0.05, respectively, when random samples of 25%, 50%, and 75% of cases were selected for testing. The level of geographic stratification in the sampling process did not meaningfully affect accuracy estimates. These findings can inform testing thresholds for laboratory surveillance programs in the meningitis belt.

BackgroundThe socioeconomic consequences of schistosomiasis have forced the integration of control strategies. Currently, all control efforts have tended to involve the control of snail intermediate hosts. However, the integration of snail control into the general schistosomiasis control strategy has not yet been effective in Burkina Faso. While snail occurrence data have been collected intermittently since the last systematic assessment in 1980, there has been no recent synthesis of the distribution of intermediate host snails in Burkina Faso. The aim of this study was to integrate historical and recent datasets to assess temporal shifts in species distribution, identify key climatic drivers, and produce spatially explicit risk maps to support targeted control strategies in Burkina Faso.MethodsSnail occurrence data were compiled for three time periods: 1980, 2018, and 2021. Data from 1980 and 2018 were obtained through a comprehensive literature review, while field sampling was conducted in 2021 in two basins: the Mouhoun basin and the Nakanbé basin. The Maximum Entropy (Maxent) modeling approach was used to predict changes in species distributions over time by integrating occurrence records from each period with relevant environmental variables. Model performance was evaluated using the Area Under the Curve (AUC) of the Receiver Operating Characteristic (ROC) curve and the Boyce index. The contribution of each environmental variable was assessed using a jackknife test.ResultsFive schistosomiasis intermediate hosts were collected: Bulinus truncatus, Bulinus forskalii, Bulinus senegalensis, Bulinus globosus, and Biomphalaria pfeifferi. The number of snail intermediate host species for schistosomes did not significantly change between 1980 and 2021. Except for B. umbilicatus, the results show that the two basins host almost the same species of snail intermediate host of schistosomes. The rivers, reservoirs, and irrigated plains provide favorable conditions for the oc

    Clara Lacourarie, Lisa Georges
