Œdème papillaire revelant une malformation d’Arnold Chiara type 1 : à propos d’un cas

Mouhoub Imane, Maadane Asmae, Ramdani Toufik, Sekhsoukh Rachid

Moulay Elmehdi El Hassani, Farid Kassidi, Youssef Benabdejlil, Jaouad Kouach, Driss Rahali Moussaoui, Mohammed Dehayni

Amine Razzak, Anass Kassimi, Adil Mchachi, Leila Benhmidoune, Abderrahim Chakib, Rayad Rachid, Mohamed Elbelhadji, Abdelouahed Amraoui, Hassan Elkabli

Abdelillah Rachid, Erraji Moncef, Abbassi Najib, Najib Abdeljaouad, Daudi Abdelkarim, Yacoubi Hicham

Older persons experiences of healthcare in rural Burkina Faso: Results of a cross sectional household survey

Ensuring responsive healthcare which meets patient expectations and generates trust is important to increase rates of access and retention. This need is important for aging populations where non-communicable diseases (NCDs) are a growing cause of morbidity and mortality. We performed a cross-sectional household survey including socio-demographic; morbidities; and patient-reported health system utilization, responsiveness, and quality outcomes in individuals 40 and older in northwestern Burkina Faso. We describe results and use exploratory factor analysis to derive a contextually appropriate grouping of health system responsiveness (HSR) variables. We used linear or logistic regression to explore associations between socio-demographics, morbidities, and the grouped-variable, then between these variables and health system quality outcomes. Of 2,639 eligible respondents, 26.8% had least one NCD, 56.3% were frail or pre-frail and 23.9% had a recent healthcare visit, including only 1/3 of those with an NCD. Highest ratings of care experience (excellent/very good) included ease of following instructions (86.1%) and trust in provider skills (81.1%). The HSR grouping with the greatest factor loading included involvement in decision-making, clarity in communication, trust in the provider, and confidence in providers’ skills, labelled Shared Understanding and Decision Making (SUDM). In multivariable analysis, higher quality of life (OR 1.02,95%CI 1.01–1.04), frailty (OR 1.47,95%CI 1.00–2.16), and SUDM (OR 1.06,95%CI 1.05–1.09) were associated with greater health system trust and confidence. SUDM was associated with overall positive assessment of the healthcare system (OR 1.02,95%CI 1.01–1.03) and met healthcare needs (OR 1.09,95%CI 1.08–1.11). Younger age and highest wealth quintile were also associated with higher met needs. Recent healthcare access was low for people with existing NCDs, and SUDM was the most consistent factor associated with higher health system quality out

    Omphalolith: Une présentation déroutante

Mariem Bounouar, Asmae Hatimi, Mariame Meziane, Ouafae Mikou, Fatimazahra Mernissi, Amal Bennani, Sanae Chehbouni, Taoufik Harmouch, Afaf Amarti

On the forecastability of food insecurity

Pietro Foini, Michele Tizzoni, Giulia Martini, Daniela Paolotti, Elisa Omodei

Food insecurity, defined as the lack of physical or economic access to safe, nutritious and sufficient food, remains one of the main challenges included in the 2030 Agenda for Sustainable Development. Near real-time data on the food insecurity situation collected by international organizations such as the World Food Programme can be crucial to monitor and forecast time trends of insufficient food consumption levels in countries at risk. Here, using food consumption observations in combination with secondary data on conflict, extreme weather events and economic shocks, we build a forecasting model based on gradient boosted regression trees to create predictions on the evolution of insufficient food consumption trends up to 30 days in to the future in 6 countries (Burkina Faso, Cameroon, Mali, Nigeria, Syria and Yemen). Results show that the number of available historical observations is a key element for the forecasting model performance. Among the 6 countries studied in this work, for those with the longest food insecurity time series, that is Syria and Yemen, the proposed forecasting model allows to forecast the prevalence of people with insufficient food consumption up to 30 days into the future with higher accuracy than a naive approach based on the last measured prevalence only. The framework developed in this work could provide decision makers with a tool to assess how the food insecurity situation will evolve in the near future in countries at risk. Results clearly point to the added value of continuous near real-time data collection at sub-national level.

On the use of generative models for demographic inference in malaria vectors from genomic data

AbstractMalaria in sub-Saharan Africa is transmitted by mosquitoes from the Anopheles genus. Efforts to control the spread of malaria have often focused on these vectors, but little is known about the demographic history of populations and species of Anopheles mosquitoes. Here, we adapt and apply an innovative generative deep learning algorithm to infer the joint evolutionary history of Anopheles gambiae populations sampled in Guinea and Burkina Faso. We further develop a model selection approach and discover that an evolutionary model with migration fits this pair of populations better than a model without post-split migration. For the migration model, we find that our method accurately captures population genetic differentiation. These findings demonstrate that machine learning and generative models are a valuable direction for future understanding of the evolution of malaria vectors, including the joint inference of demography and natural selection. Understanding changes in population size, migration patterns, and adaptation in hosts, vectors, and pathogens will assist malaria control interventions, with the ultimate goal of predicting nuanced outcomes from insecticide resistance to population collapse. Efforts to control the spread of malaria have often focused on mosquitoes, but relatively little is known about their evolutionary history. Here, Eneli et al. adapt and apply an innovative, generative, deep-learning algorithm to populations sampled from Guinea and Burkina Faso, West Africa. The authors further develop a novel model selection approach and discover that an evolutionary model with migration fits these populations better than a model without migration. Understanding changes in population size, migration patterns, and adaptation in hosts, vectors, and pathogens will assist malaria control interventions, with the ultimate goal of predicting nuanced outcomes from insecticide resistance to population collapse.

On-site evaluation as external quality assessment of microbiology laboratories involved in sentinel laboratory-based antimicrobial resistance surveillance network in Burkina Faso

Emmanuel Zongo, Emilie Dama, Dame Yenyetou, Merci Muhigwa, Abdoulaye Nikiema, Georges Anicet Dahourou, Abdoul-Salam Ouedraogo

BackgroundThe laboratory-based surveillance of antimicrobial resistance (AMR) is a priority component of the multisectoral national action plan to combat AMR in Burkina Faso. This study aimed to assess the QMS of microbiology laboratories involved in the Sentinel laboratory-based antimicrobial resistance surveillance network in Burkina Faso.MethodsA cross-sectional study was conducted from September 1st to November 30th, 2022. The external quality assessment (EQA) method used was on-site evaluation using a checklist that was developed and validated by a technical committee of experts. Teams of two, including an antimicrobial susceptibility test (AST) specialist and a QMS specialist, were trained on this checklist to conduct the assessment. Satisfactory performance was defined as any on-site evaluation score 80% and above with the aim of developing action plans to address gaps.ResultsAll 18 laboratories were evaluated. The overall average performance score of the participating laboratories was 40%. The highest overall performance score was 58%, and the lowest overall performance score was 26%. The average overall scores were not significantly different between private and public laboratories (p value = 0.78). The only section of the checklist with the satisfactory performance concerned the “Analytical step of AST”, with 76.5% (13/17) of the sentinel laboratories having a score ≥ 80%.ConclusionThe performance of the QMS of the sentinel laboratories in Burkina Faso for AMR surveillance was unsatisfactory, and a corrective action plan was proposed to support these laboratories in improving their QMS over the next 3 years.

Wail Bouzoubaa, Meryam Laadioui, Sofia Jayi, Fatime Zahra Fdili Alaoui, Hakima Bouguern, Hikmat Chaara, Moulay Abdelilah Melhouf

One Health Approach to Study the Occurrence and Antimicrobial Resistance of Extended-Spectrum β-Lactamase- and Carbapenemase-Producing Escherichia coli and Klebsiella spp. in Urban Agriculture in Burkina Faso

Data on antimicrobial resistance in Burkina Faso’s agricultural sector is still limited. This study assessed the occurrence of extended-spectrum β-lactamase-producing Escherichia coli (ESBL-Ec) and Klebsiella spp. (ESBL-K) in lettuce, environment, and gardeners’ stools in market gardens in Ouagadougou, Burkina Faso. A total of 356 samples were collected from three vegetable gardens (76 lettuce, 76 soil, 62 manure, 63 irrigation water, and 79 human stools). The ESBL-selective medium was used for initial selection of ESBL-producing bacteria, and the isolates were further identified using biochemical tests. An antibiotic susceptibility test was performed using the disk diffusion method. The overall prevalence of ESBL-Ec and/or ESBL-K in the samples was 232/356 (65.2%). Of the lettuce samples, the prevalence of ESBL-Ec was 19/76 (25.0%) and ESBL-K 33/76 (43.4%). In the market gardens environment, the prevalence of ESBL-Ec was 32/201 (15.9%) and ESBL-K 124/201 (61.7%). In the gardeners’ stools, the prevalence of ESBL-Ec was 42/79 (53.2%) and ESBL-K 24/79 (30.4%). Two ESBL-K isolates were found to produce NDM carbapenemase. Due to the high prevalence of ESBL-producing bacteria, which may also be carbapenemase producers, it is necessary to monitor pathogens in agricultural products with a “One Health” approach to limit and prevent infections in the population.

Ontario healthcare workers who sought treatment for their mental health during the first five waves of the COVID-19 pandemic: a snapshot of self-referrals across the province

Judith M. Laposa, Duncan Cameron, Kim Corace, Heather L. Bullock, Lauren Flavelle, Natalie Quick, Karen Rowa, Sara de la Salle, Katherin Creighton-Taylor, Alice Strachan, Stephanie Carter, Paul Kurdyak, Vanessa Saldanha, Randi E. McCabe

AbstractIntroduction:Healthcare workers (HCWs) have reported COVID-19 pandemic-related adverse mental health impacts. We examined the demographic profile of HCWs who self-referred for mental health treatment, how referrals changed over time in relation to waves of COVID-19, what the main problem was for which HCWs sought treatment, and how this changed during the pandemic. Methods:Five major healthcare institutions provided mental health supports to HCWs across Ontario during the pandemic. Data from May 2020 to March 2022 were collected from 2725 HCW self-referrals regarding referral frequency, main presenting mental health problem and demographic information including ethnicity, gender, age, healthcare setting, profession and whether the HCW had a prior mental health diagnosis or had received prior mental health treatment.Results:Treatment-seeking HCWs who self-referred predominantly self-identified as female and White. Almost half were nurses, and almost half had received previous mental health treatment; a slightly higher percentage reported a prior mental health diagnosis. Over 60% of the overall sample of HCWs worked in hospitals. The timing of increases and decreases in monthly new referrals roughly aligned with the onset and ending, respectively, of COVID-19 waves. The top five most common presenting problems for treatment-seeking were generalized anxiety/worry symptoms, depression, situational crisis/acute stress response, difficulty with stress/occupational or financial, and posttraumatic stress symptoms. Conclusion:Ontario HCWs self-referred to access mental health supports during the COVID-19 pandemic. The majority sought treatment for generalized anxiety/worry or depression symptoms. Results of this study may inform system planning for future pandemics, as well as for HCW wellness programs for continued workplace stress in the postpandemic period.

The French Society of Pathology organized two data challenges, with the support of the Health Data Hub: TissueNet in 2020, focusing on cervical lesions, and Visiomel in 2023 dedicated to melanoma in partnership with the French Society of Dermatology and the National Professional Council of Pathologists. The datasets were open a few weeks to competitors worldwide during each challenge. However, these data were then closed, awaiting a complete and permanent opening, known as open data. This ultimate step allows free access to the data to promote global research programs in these diseases. The data from the two data challenges have been available as open data on data.gouv.fr since January 2025 for TissueNet (https://www.data.gouv.fr/datasets/tissuenet-detect-lesions-in-uterine-cervix-specimens-open-data-set/) and since September 2025 for Visiomel (https://www.data.gouv.fr/datasets/visiomel-database-predicting-cutaneous-melanoma-relapse/). Making health data open is a demanding multi-step process - organizationally, technically, and regulatorily - and it is this process that we discuss here.

Operational analysis of the national sickle cell screening programme in the Republic of Uganda

Arielle G. Hernandez, Charles Kiyaga, Thad A. Howard, Isaac Ssewanyana, Grace Ndeezi, Jane R. Aceng, Russell E. Ware

BackgroundSickle cell anaemia is a common global life-threatening haematological disorder. Most affected births occur in sub-Saharan Africa where children usually go undiagnosed and die early in life. Uganda’s national sickle cell screening programme was developed in response to a 2014 sickle cell surveillance study that documented a high disease prevalence.ObjectiveThis study describes the temporal and financial aspects of Uganda’s 2014–2019 sickle cell screening programme.MethodsNational sickle cell screening data from Uganda’s Central Public Health Laboratories were used to calculate turn-around times (TATs) from sample collection to delivery, testing, and result reporting for blood samples collected from February 2014 to March 2019. The parameters affecting specific TATs were assessed. The exact programme expenditures were analysed to determine cost per test and per positive sickle cell disease case detected.ResultsA total of 278 651 samples were analysed. The median TAT from sample collection to laboratory receipt was 8 days (interquartile range [IQR]: 6–12), receipt to testing was 3 days (IQR: 1–7), and testing to result reporting was 6 days (IQR: 3–12). Altogether, the sample continuum averaged 16 days (IQR: 11–24). Lower level healthcare facilities were associated with longer sample delivery TATs. Calendar months (January and December) and larger sample volumes impacted testing and result reporting TATs. The cost per test was $4.46 (United States dollars [USD]) and $483.74 USD per positive case detected.ConclusionUganda’s sickle cell screening programme is efficient and cost-effective. Universal newborn screening is the best strategy for detecting sickle cell anaemia in Uganda.

Operational Differences between Product Development Partnership, Pharmaceutical Industry, and Investigator Initiated Clinical Trials

Medicine development is a lengthy endeavour. Increasing regulatory stringency and trial complexity might lead to reduced efficiency, dwindled output, and elevated costs. However, alternative models are possible. We compared the operational differences between pharmaceutical industry sponsored trials, product development partnership trials, and investigator-initiated trials to identify key drivers of inefficiency in clinical research. We conducted an exploratory mixed-methods study with stakeholders, including clinical trial sponsors, contract research organisations, and investigators. The qualitative component included 40 semi-structured interviews, document reviews of 12 studies and observations through work shadowing in research institutions in Burkina Faso, Mali, and Switzerland. The findings were triangulated with an online survey polling clinical research professionals. The operational differences were grouped under five categories: (i) trial start-up differences including governance and management structure; (ii) study complexity; (iii) site structural and organisational differences; (iv) study conduct, quality approaches, and standard operating procedures; and (v) site capacity strengthening and collaboration. Early involvement of sites in the planning and tailored quality approaches were considered critical for clinical operations performance. Differences between the types of trials reviewed pertained to planning, operational complexities, quality approaches, and support to the sites. Integration of quality-by-design components has the potential to alleviate unnecessary process burden.
