Biological Hazards and Indicators Found in Products of Animal Origin in Cambodia from 2000 to 2022: A Systematic Review

Biological hazards in products of animal origin pose a significant threat to human health. In Cambodia, there are few comprehensive data and information on the causes of foodborne diseases or risks. To date, there has been no known published study similar to this review. This systematic review is aimed to investigate the prevalence of biological hazards and their indicators in products of animal origin from 2000 to 2022. The main objective of this study was also to contribute to strengthening Cambodia’s food control system. This review followed the established “Preferred Reporting Items for Systematic Reviews and Meta-Analyses” (PRISMA) guidelines. In total, 46 studies were retained for complete review. Most studies (n = 40) had been conducted by or with external researchers, reflecting the under-resourcing of the National Food Control System in terms of surveillance; areas outside the capital were relatively understudied, reflecting evidence found in Ethiopia and Burkina Faso. Five categories of hazards were reported with the highest number of studies on fish parasites. Marketed fish, often originating from different countries, had a higher mean value of parasite prevalence (58.85%) than wild-caught fish (16.46%). Viral pathogens in bat meat presented a potential spillover risk. Many potentially important hazards had not yet been studied or reported (e.g., Norovirus, Shigella, toxin-producing Escherichia coli, and Vibrio cholerae). The findings of our review highlighted significant urgencies for national competent authorities to enhance food hygiene practices along the production chain, tackle import control, and enforce the implementation of a traceability system, alongside more research collaboration with neighboring countries and key trading partners. It is crucial to conduct more extensive research on food safety risk analysis, focusing on the identification and understanding of various biological hazards and their associated risk factors in food.

Biological larviciding against malaria vector mosquitoes with Bacillus thuringiensis israelensis (Bti) – Long term observations and assessment of repeatability during an additional intervention year of a large-scale field trial in rural Burkina Faso

Peter Dambach, Volker Winkler, Till Bärnighausen, Issouf Traoré, Saidou Ouedraogo, Ali Sié, Rainer Sauerborn, Norbert Becker, Valérie R. Louis

ABSTRACTThe first line of malaria vector control to date mainly relies on the use of long-lasting insecticidal nets (LLINs) and indoor residual spraying (IRS). For integrated vector management, targeting the vector larvae with biological larvicides such as Bacillus thuringiensis israelensis (Bti) can be an effective additional mainstay. This study presents data from the second intervention year of a large-scale trial on biological larviciding with Bti that was carried out in 127 rural villages and a semi-urban town in Burkina Faso. Here we present the reductions in malaria mosquitoes that were achieved by continuing the initial interventions for an additional year, important to assess sustainability and repeatability of the results from the first intervention year. Larviciding was performed applying two different larviciding choices ((a) treatment of all environmental breeding sites, and (b) selective treatment of those that were most productive for Anopheles larvae indicated by remote sensing based risk maps). Adult Anopheles spp. mosquito abundance was reduced by 77.4% (full treatment) and 63.5% (guided treatment) compared to the baseline year. The results showed that malaria vector abundance can be dramatically reduced using biological larviciding and that this effect can be achieved and maintained over several consecutive transmission seasons.

Biological Status and Dietary Intakes of Iron, Zinc and Vitamin A among Women and Preschool Children in Rural Burkina Faso

BackgroundFood-based approaches such as biofortification are meant to sustainably address micronutrient deficiencies in poor settings. Knowing more about micronutrient intakes and deficiencies is a prerequisite to designing and evaluating interventions.ObjectiveThe objectives of the study were to assess biological status and dietary intakes of iron, zinc and vitamin A among women and children aged 36–59 months in rural Burkina Faso and to study relationships between intake and status to better inform future food-based interventions.DesignA cross-sectional survey was carried out in two rural provinces of Burkina Faso on a random cluster sample of 480 mother-child pairs. Dietary data was obtained by 24-hour recalls repeated on a random sub-selection of 37.5% of subjects to allow calculation of nutrient’s probability of adequacy (PA). Biomarkers were measured on a sub-sample of 180 mother-child pairs. Blood samples were analyzed for hemoglobin, serum ferritin, soluble transferrin receptors (sTfR), C-reactive protein, alpha-1-glycoprotein, serum zinc concentration (SZnC) and retinol. For each micronutrient the relationship between biomarker and dietary intake was investigated by multiple linear regression models accounting for inflammatory biomarkers.ResultsMean PA for iron, zinc and vitamin A was 0.49, 0.87 and 0.21 among women and 0.61, 0.95 and 0.33 among children, respectively. Prevalence of anemia, corrected low serum ferritin and high sTfR was 37.6%, 4.0% and 77.5% among women and 72.1%, 1.5% and 87.6% among children, respectively. Prevalence of low SZnC and corrected low serum retinol was 39.4% and 12.0% among women and 63.7% and 24.8% among children, respectively. There was a tendency for a positive relationship between vitamin A intakes and serum retinol among women (β = 0.0003, P = 0.06). Otherwise, no link was found between micronutrients biomarkers and intakes.ConclusionOur study depicted different images of micronutrient deficiencies when based on dietary i

    Stefano Bosi, Claire Rogel Gaillard

Biophysical Profiling of Sickle Cell Disease Using Holographic Cytometry and Deep Learning

Sickle cell disease (SCD) is an inherited hematological disorder associated with high mortality rates, particularly in sub-Saharan Africa. SCD arises due to the polymerization of sickle hemoglobin, which reduces flexibility of red blood cells (RBCs), causing blood vessel occlusion and leading to severe morbidity and early mortality rates if untreated. While sickle solubility tests are available to sub-Saharan African population as a means for detecting sickle hemoglobin (HbS), the test falls short in assessing the severity of the disease and visualizing the degree of cellular deformation. Here, we propose use of holographic cytometry (HC), a high throughput, label-free imaging modality, for comprehensive morphological profiling of RBCs as a means to detect SCD. For this study, more than 2.5 million single-cell holographic images from normal and SCD patient samples were collected using the HC system. We have developed an approach for specially defining training data to improve machine learning classification. Here, we demonstrate the deep learning classifier developed using this approach can produce highly accurate classification, even on unknown patient samples.

Birth-Preparedness for Maternal Health: Findings from Koupéla District, Burkina Faso

Allisyn C. Moran, Gabriel Sangli, Rebecca Dineen, Barbara Rawlins, Mathias Yaméogo, Banza Baya

Maternal mortality is a global burden, with more than 500,000 women dying each year due to pregnancy and childbirth-related complications. Birth-preparedness and complication readiness is a comprehensive strategy to improve the use of skilled providers at birth, the key intervention to decrease maternal mortality. Birth-preparedness and complication readiness include many elements, including: (a) knowledge of danger signs; (b) plan for where to give birth; (c) plan for a birth attendant; (d) plan for transportation; and (e) plan for saving money. The 2003 Burkina Faso Demographic and Health Survey indicated that only 38.5% of women gave birth with the assistance of a skilled provider. The Maternal and Neonatal Health Program of JHPIEGO implemented a district-based model service-delivery system in Koupéla, Burkina Faso, during 2001–2004, to increase the use of skilled providers during pregnancy and childbirth. In 2004, a cross-sectional survey with a random sample of respondents was conducted to measure the impact of birth-preparedness and complication readiness on the use of skilled providers at birth. Of the 180 women who had given birth within 12 months of the survey, 46.1% had a plan for transportation, and 83.3% had a plan to save money. Women with these plans were more likely to give birth with the assistance of a skilled provider (p=0.07 and p=0.03 respectively). Controlling for education, parity, average distance to health facility, and the number of antenatal care visits, planning to save money was associated with giving birth with the assistance of a skilled provider (p=0.05). Qualitative interviews with women who had given birth within 12 months of the survey (n=30) support these findings. Most women saved money for delivery, but had less concrete plans for transportation. These findings highlight how birth-preparedness and complication readiness may be useful in increasing the use of skilled providers at birth, especially for women with a plan for saving

Birthweight, gestational age, and early school trajectory

Rabi Joël Gansaonré, Lynne Moore, Jean-François Kobiané, Ali Sié, Slim Haddad

BackgroundBirthweight and gestational age are important factors of not only newborn health by also child development and can contribute to delayed cognitive abilities. However, no study has analyzed the association of birthweight and gestational age with school trajectory measured simultaneously by school entry, grade repetition, and school dropout. This study aims, first, to analyze the association of birthweight or gestational age with school entry, and second, to explore the relationship between birthweight or gestational age and grade repetition and school dropout among children in Ouagadougou, Burkina Faso.MethodsThis study used longitudinal data from the Ouagadougou Health and Demographic Surveillance System. Our samples consisted of children born between 2008 and 2014 who were at least three years old at the beginning of the 2017–18 school year. Samples included 13,676, 3152, and 3498 children for the analysis of the school entry, grade repetition, and dropout, respectively. A discrete-time survival model was used to examine the relationship between birthweight or gestational age and school entry, grade repetition, and dropout. The association between birthweight or gestational age and age at school entry were assessed using a Poisson regression.ResultsThe incidence rate of school entry was 18.1 per 100 people-years. The incidence of first repetition and dropout were 12.6 and 5.9, respectively. The probability of school entry decreased by 31% (HR:0.69, 95%CI: 0.56–0.85) and 8% (HR:0.92, 95%CI: 0.85–0.99) for children weighing less than 2000 g and those weighing between 2000 and 2499 g, respectively, compared to those born with a normal weight (weight ≥ 2500 g). The age at school entry of children with a birthweight less than 2000 g and between 2000 and 2499 g was 7% (IRR: 1.07, 95%CI: 1.06–1.08) and 3% (IRR: 1.03, 95%CI: 1.00-1.06) higher than children born at a normal birthweight, respectively. Gestational age was not associated with school entry or age at s

Fatima-Zahra Joudar, Dounia El Moujtahide, Elhoucine Sebbar, Mohammed Choukri

Sophie Ndaw, Danièle Jargot, Aurélie Martin Remy, Catherine Champmartin, Lisa Chedik, Fabrice Marquet, Alain A. Robert

Black-focused social prescribing:the importance of an Afrocentric approach

Sofia Ramirez, Natasha Beaudin, Jennifer Rayner, Neil Price, Daniel Townsend

AbstractThe Black-Focused Social Prescribing (BFSP) project is a unique initiative by the Alliance for Healthier Communities that intertwines Afrocentric principles with social prescribing. Going beyond conventional social prescribing models, BFSP addresses specific health needs within Black communities. It is rooted in the Alliance Black Health Strategy, advocates for Black health, and is guided by Afrocentric principles. The evaluation framework prioritizes client voices, ensuring cultural safety and, by taking time for trust-building, underscores the importance of an inclusive approach. BFSP holds the potential to foster community trust and engagement, and enhance health outcomes in the Black community.

    Blastomycose pulmonaire

Fatma Chermiti Ben Abdallah, Imène Bachouch, Nidhal Belloumi, Marwa Kacem, Mouna Mlika, Faouzi El Mezni, Soraya Fenniche

Sahar Mouram, Ibtissam Fellat, Mohamed Cherti

Euclides N. M. Sacomboio, Santo D. Zua, Adelino T. Tchivango, António D. Pululu, Adilson C. D. Caumba, Adelina B. M. Paciência, Danilson V. Sati, Sabina G. Agostinho, Yolanda S. Agostinho, Fernando G. Mazanga, Neusa B. Ntambo, Cruz S. Sebastião, Joana P. Paixão, Joana Morais

IntroductionIntroduction: Malaria continues to be the leading cause of hospitalization and death in Angola, a country in sub- Saharan Africa. In 2023, in the first quarter, 2,744,682 cases were registered, and of these 2,673 patients died due to malaria disease. Previous studies have shown that the ABO blood group can affect the progression of malaria to severe conditions after P. falciparum infection, while the sickle cell gene offers relative protection.ObjectiveWe investigated changes in the blood count according to blood groups (ABO/Rh) and sickle cell trait in patients with malaria in Luanda, capital of Angola.Methodology This was a longitudinal, prospective and observational study with 198 patients hospitalized for malaria. Results Of the 198 patients studied, 13(6.6%) were ABRh(+), 4(2.0%) were ARh(-), 49(24.7%) were ARh(+), 42(21, 2%) were BRh (+), 5(2.5%) were ORh(-) and 85(42.9%) were ORh(+). For sickle cell trait, 145(73.2%) were AA, 37(18.7%) were AS and 16(8.1%) were SS. No statistical relationship was observed between age group, sex, parasitemia, clinical picture, hematocrit, MCV, HCM, MCHC, leukocytes, NEUT, LINF and PTL values with blood groups (p<0.05), but there was a relationship between values of hemoglobin and ABO/Rh blood groups (p>0.05). There was no relationship between age, parasitemia, clinical condition, MCV, HCM and MCHC values, leukocytes, NEUT and LINF with sickle cell trait (p<0.05), but there was a relationship between sex, hemoglobin and PTL and sickle cell values. sickle cell trait (p>0.05).ConclusionIt is imperative to differentiate patients with malaria based on blood groups and sickle cell trait, taking into account mainly the blood count parameters that demonstrate that there are patients who, depending on blood group or sickle cell trait, may react weakly to malaria infection regardless of the degree of parasitemia and medical prognosis.

Blood feeding behaviour comparison and contribution of Anopheles coluzzii and Anopheles gambiae, two sibling species living in sympatry, to malaria transmission in Alibori and Donga region, northern Benin, West Africa

Martin C. Akogbéto, Albert Sourou Salako, Fortuné Dagnon, Rock Aïkpon, Michelle Kouletio, Arthur Sovi, Michel Sezonlin

BackgroundThe main goal of this study was to assess the blood feeding behaviour and the contribution Anopheles coluzzii and Anopheles gambiae, 2 sibling species of An. gambiae sensu stricto. present and living in sympatry in 2 regions of northern Benin targeted for indoor residual spraying (IRS).MethodsThe study was carried out in 6 districts of 2 regions of Benin (Alibori and Donga). Human landing catches (HLC) performed inside and outside of the households and pyrethrum spray captures (PSC) carried out in bedrooms were used to sample vector populations (An. gambiae and An. coluzzii). Collected mosquitoes were analysed to estimate the human biting rate indoors and outdoors, the circumsporozoite antigen positivity, and the anthropophagic index using ELISA methodology. Polymerase chain reaction was used to estimate the frequency of the knockdown resistance (kdr) L1014F and the ace-1 mutations, 2 markers associated respectively with pyrethroids and carbamate/organophosphate insecticide resistance.ResultsA higher blood feeding rate was observed in An. gambiae compared to An. coluzzii as well as, a non-pronounced outdoor biting behavior in both species. The latter showed similar anthropophagic and sporozoite rates. However the analysis indicates a seasonal difference in the contribution of each species to malaria transmission associated with shifts in resting behaviour. Anopheles coluzzii females accounted for most of the detected infections: 86% in Alibori and 79% in Donga, during the dry season versus 14.4% and 21.2%, respectively for An. gambiae during the same period. This relationship was reversed in Donga during the rainy season (66% for An. gambiae against 34% for An. coluzzii). Results also indicated lower frequencies of kdr L1014F and ace-1 in An. coluzzii versus An. gambiae.ConclusionDespite similarity in some parameters related to malaria transmission in both surveyed species, An. coluzzii is potentially a more important malaria vector because of high density

Blood transfusion requirements among children with severe malarial anemia: a cross-sectional study in a second level reference hospital in Burkina Faso

Salam Sawadogo, Koumpingnin Nébié, Tieba Millogo, Eléonore Kafando

Introductionblood transfusion (BT) is an important part of pediatrics healthcare in sub-Saharan Africa because of anemia due to malaria, malnutrition and hereditary anomalies of red blood cells. However, BT services experienced chronic blood shortage, unsafe blood products and poor procedures of clinical use of blood. This results in inadequate management of severe anemia.Methodsto assess the quality of BT requirements in severe malarial anemia at the regional hospital center of Koudougou in Burkina Faso, we carried out a cross-sectional study including 402 children with severe malaria (WHO 2000 criteria).Resultsover the study period, severe malaria represented 45.6% (402/882) of pediatric admissions. Anemia was observed in 97.5% (392/402) of cases and BT was required for 78.4% (315/402). The median age was 16 months (IQR 9-27) and the average hemoglobin was 51.4±22.2 g/L. The prescriptions were in accordance with WHO and national guidelines respectively in 63.8% and 92.7%. Blood units were issued in 99.4% (350/352) of blood orderings. Out of 350 blood units delivered, blood was administered in 98% (343/350). The median actual time to transfusion was 65 minutes (IQR: 45-100) and median transfusion duration was 73.8 minutes (IQR: 47.5-110). The signs of intolerance to anemia disappeared in 134/138 cases (97.1%) and the average haemoglobin increased of 37.9±17.6 g/L. Death occurred in 23 cases (5.7%).Conclusionthe management of severe malaria requires blood transfusion in almost half of cases. Blood was available to meet most requests. However, efforts are still required for proper use of the blood.
