Cutaneous diphtheria in a child returning from visiting friends & relatives in Burkina Faso: a case report

    G. Pagani, M. Ticozzelli, F. Bassani, S. Sciarrabba, R. Agistri, L. Pogliani, B. Osnaghi, C. Pavia, L. Vismara, S. Rusconi

We present a case report of an 8-year-old African origin, Italian born girl, fully immunized for Diphtheria, with multiple ulcers caused by Corynebacterium diphtheria. The clinical event was preceded by a recent trip to Burkina Faso in a rural area. The atypical presentation with ulcers spread to the entire body and particularly to one leg, led to a bacterial swab, from whose culture the pathogen emerged. The girl was initially empirically treated with amoxicillin/clavulanate and then azithromycin. Cutaneous forms are the most common of all forms and have a relevant spreading role. This case is of particular concern in the context of a rising number of cases in Europe, and it’s peculiar since it doesn’t regard immigrants and refugees, whereas it regards a fully immunized child who traveled from an endemical country.

Cuticular hydrocarbons are associated with mating success and insecticide resistance in malaria vectors

Kelsey L. Adams, Simon P. Sawadogo, Charles Nignan, Abdoulaye Niang, Douglas G. Paton, W. Robert Shaw, Adam South, Jennifer Wang, Maurice A. Itoe, Kristine Werling, Roch K. Dabiré, Abdoulaye Diabaté, Flaminia Catteruccia

Anopheles coluzzii females, important malaria vectors in Africa, mate only once in their lifetime. Mating occurs in aerial swarms with a high male-to-female ratio, where traits underlying male mating success are largely unknown. Here, we investigated whether cuticular hydrocarbons (CHCs) influence mating success in natural mating swarms in Burkina Faso. As insecticides are widely used in this area for malaria control, we also determined whether CHCs affect insecticide resistance levels. We find that mated males have higher CHC abundance than unmated controls, suggesting CHCs could be determinants of mating success. Additionally, mated males have higher insecticide resistance under pyrethroid challenge, and we show a link between resistance intensity and CHC abundance. Taken together, our results suggest that CHC abundance may be subject to sexual selection in addition to selection by insecticide pressure. This has implications for insecticide resistance management, as these traits may be sustained in the population due to their benefits in mating even in the absence of insecticides. In this study, Adams et al. investigate the effect of cuticular hydrocarbons on mating success in natural mosquito mating swarms. These hydrocarbons confer both higher mating success and increased resistance to pyrethroid, suggesting sexual selection for insecticide resistance in this population secondary to mating success.

Cutis laxa congénital: à propos d’un cas

Aziza El Ouali, Manal Azizi, Siham Dikhaye, Noufissa Benajiba

Boukind Samira, Dlimi Meriem, Elatiqi Oumkeltoum, Elamrani Driss, Benchamkha Yassine, Ettalbi Saloua

CYP2D6 Pharmacogenetics in Nigerian Sickle Cell Disease: Phase 1 of Implementing Pharmacogenomics Testing for Effective Care and Treatment in Africa (iPROTECTA) program

Babatunde Adeagbo, Olusola Olarewaju, Ochuko Orherhe, Zedias Chikwambi, Adrian Mazhindu, Rahman Bolarinwa, Oluseye Bolaji, Collen Masimirembwa

BackgroundSickle Cell Disease (SCD) is highly prevalent in Nigeria, with severe pain crises being a primary cause of morbidity. Codeine and tramadol are frequently used opioids, but their effectiveness and safety are significantly influenced by CYP2D6 genetic variations. Clinical Pharmacogenetics Implementation Consortium (CPIC) guidelines exist for opioid therapy based on CYP2D6 phenotypes. There’s a critical need for pre-emptive pharmacogenomic (PGx) testing in African SCD patients to guide opioid selection. This study aimed to determine CYP2D6 allele, phenotype frequencies and evaluate the feasibility of implementing pre-emptive pharmacogenomic (PGx) testing to guide opioid therapy for SCD patients in Nigeria.MethodsThis prospective, multicenter implementation study recruited 503 consenting SCD patients (HbSS or HbSC) aged ≥15 years from five Nigerian sites. Blood samples were collected for DNA extraction. CYP2D6 single-nucleotide polymorphisms and copy number variations were determined using Taqman assays based open array, GenoPharm. Phenotypes were assigned based on Clinical Pharmacogenetics Implementation Consortium (CPIC) guidelines using the Genomics Information Management System (GIMS). and patient-specific medication safety cards were generated.ResultsWe successfully genotyped 503 SCD patients with a mean age of 25.1 years, while 61.4% were female, and hydroxyurea use was less than 9.4%. Actionable CYP2D6 variants were found in 36.6% of participants. The predicted phenotype distribution was 8.8% Ultrarapid Metabolizers (UM), 54.1% Normal Metabolizers (NM), 26.0% Intermediate Metabolizers (IM), and 1.8% Poor Metabolizers (PM), with 9.3% undetermined. Patient medication safety cards were provided to guide prescriptions.ConclusionsThis study successfully established a genotyped cohort of 503 Nigerian SCD patients, demonstrating the feasibility of pre-emptive pharmacogenetic testing through a Pan-African collaborative model in a resource-limited setting. CYP6 P450 Enzymes and ACE-1 Duplication Produce Extreme and Multiple Insecticide Resistance in the Malaria Mosquito Anopheles gambiae

Malaria control relies heavily on pyrethroid insecticides, to which susceptibility is declining in Anopheles mosquitoes. To combat pyrethroid resistance, application of alternative insecticides is advocated for indoor residual spraying (IRS), and carbamates are increasingly important. Emergence of a very strong carbamate resistance phenotype in Anopheles gambiae from Tiassalé, Côte d'Ivoire, West Africa, is therefore a potentially major operational challenge, particularly because these malaria vectors now exhibit resistance to multiple insecticide classes. We investigated the genetic basis of resistance to the most commonly-applied carbamate, bendiocarb, in An. gambiae from Tiassalé. Geographically-replicated whole genome microarray experiments identified elevated P450 enzyme expression as associated with bendiocarb resistance, most notably genes from the CYP6 subfamily. P450s were further implicated in resistance phenotypes by induction of significantly elevated mortality to bendiocarb by the synergist piperonyl butoxide (PBO), which also enhanced the action of pyrethroids and an organophosphate. CYP6P3 and especially CYP6M2 produced bendiocarb resistance via transgenic expression in Drosophila in addition to pyrethroid resistance for both genes, and DDT resistance for CYP6M2 expression. CYP6M2 can thus cause resistance to three distinct classes of insecticide although the biochemical mechanism for carbamates is unclear because, in contrast to CYP6P3, recombinant CYP6M2 did not metabolise bendiocarb in vitro. Strongly bendiocarb resistant mosquitoes also displayed elevated expression of the acetylcholinesterase ACE-1 gene, arising at least in part from gene duplication, which confers a survival advantage to carriers of additional copies of resistant ACE-1 G119S alleles. Our results are alarming for vector-based malaria control. Extreme carbamate resistance in Tiassalé An. gambiae results from coupling of over-expressed target site allelic variants with heightened C

Hedfi Mohamed, Belghachem Dorra, Bouhafa Hela, Abdelhedi Cherif, Sridi Azza, Sassi Karim, Bellil Khadija, Chouchene Adnen

Bruce Shinga Wembulua, Kalilou Diallo, Moussa Diallo, Marie Antoinette Daba Dione, Abdoulaye Diop, Noel Magloire Manga

Cytokine-Driven Immune Phenotypes at Delivery as Indicators of Malaria Infection Among Primigravidae in Burkina Faso: An Exploratory Analysis

In malaria-endemic regions, women remain vulnerable to Plasmodium falciparum infection at the time of delivery. However, the immunological mechanisms underlying infection-associated inflammation in primigravid women remain poorly characterized. This exploratory study investigated cytokine-based immune profiles reflecting malaria infection status at delivery. We assessed 33 primigravid women from Nanoro, Burkina Faso (mean age 19 years; range 18–20.5) at childbirth. Antibody responses to P. falciparum antigens (PfCSP, PfAMA-1, and EBA-175) and plasma levels of cytokines (IL-4, IL-10, IL-6, TNF-α, and IFN-γ) were quantified using enzyme immunoassays. Multivariate analyses, including principal component analysis (PCA) and hierarchical clustering, identified three distinct immune profiles: (1) a low-inflammatory cluster with reduced IL-6 and TNF-α, (2) a TNF-α–dominant cluster, and (3) a highly pro-inflammatory cluster with elevated IL-6 and TNF-α. Cluster stability was supported by bootstrap analysis (AU ≥ 92%). All women in the most inflammatory cluster were P. falciparum–positive at delivery (Fisher’s exact test, p = 0.04; exploratory association). These cytokine-driven profiles reflect biologically distinct inflammatory states associated with concurrent infection at delivery rather than predictive immune predispositions. The findings underscore the potential of cytokine profiling as a hypothesis-generating tool to guide future longitudinal studies on immune regulation and the postpartum period.

Cytotoxic, Antiproliferative and Pro-Apoptotic Effects of 5-Hydroxyl-6,7,3′,4′,5′-Pentamethoxyflavone Isolated from Lantana ukambensis

Wamtinga Richard Sawadogo, Claudia Cerella, Ali Al-Mourabit, Céline Moriou, Marie-Hélène Teiten, Innocent Pierre Guissou, Mario Dicato, Marc Diederich

Lantana ukambensis (Vatke) Verdc. is an African food and medicinal plant. Its red fruits are eaten and highly appreciated by the rural population. This plant was extensively used in African folk medicinal traditions to treat chronic wounds but also as anti-leishmanial or cytotoxic remedies, especially in Burkina Faso, Tanzania, Kenya, or Ethiopia. This study investigates the in vitro bioactivity of polymethoxyflavones extracted from a L. ukambensis as anti-proliferative and pro-apoptotic agents. We isolated two known polymethoxyflavones, 5,6,7,3′,4′,5′-hexamethoxyflavone (1) and 5-hydroxy-6,7,3′,4′,5′-pentamethoxyflavone (2) from the whole plant of L. ukambensis. Their chemical structures were determined by spectroscopic analysis and comparison with published data. These molecules were tested for the anti-proliferative, cytotoxic and pro-apoptotic effects on human cancer cells. Among them, 5-hydroxy-6,7,3′,4′,5′-pentamethoxyflavone (2) was selectively cytotoxic against monocytic lymphoma (U937), acute T cell leukemia (Jurkat), and chronic myelogenous leukemia (K562) cell lines, but not against peripheral blood mononuclear cells (PBMCs) from healthy donors, at all tested concentrations. Moreover, this compound exhibited significant anti-proliferative and pro-apoptotic effects against U937 acute myelogenous leukemia cells. This study highlights the anti-proliferative and pro-apoptotic effects of 5-hydroxy-6,7,3′,4′,5′-pentamethoxyflavone (2) and provides a scientific basis of traditional use of L. ukambensis.

Cytotoxicity of Particulate Matter PM10 Samples from Ouagadougou, Burkina Faso

Joelle Nicole Guissou, Isabelle Baudrimont, Abdoul Karim Ouattara, Jacques Simpore, Jean Sakande

Particulate matter (PM) is one of the main air pollutants with 257,000 deaths per year in Africa. Studying their toxic mechanisms of action could provide a better understanding of their effects on the population health. The objective of this study was to describe the PM10 toxic mechanism of action collected in 3 districts of Ouagadougou. Once per month and per site between November 2015 and February 2016, PM10 was sampled for 24 hours using the MiniVol TAS (AirMetrics, Eugene, USA). The collected filters were then stored in Petri dishes at room temperature for in vitro toxicological studies using human pulmonary artery endothelial cells (HPAEC) at the Bordeaux INSERM-U1045 Cardio-thoracic Research Center. The three study districts were classified based on PM10 level (high, intermediate, and low, respectively, for districts 2, 3, and 4). PM10 induced a concentration-dependent decrease in cell viability. A significant decrease in cell viability was observed at 1 µg/cm2, 10 µg/cm2, and 25 µg/cm2 for, respectively, districts 2, 3, and 4. A significant increase in the production of reactive oxygen species (ROS) was observed at 10 µg/cm2 for district 2 versus 5 µg/cm2 and 1 µg/cm2 for districts 3 and 4, respectively. Finally, a significant production of IL-6 was recorded from 5 µg/cm2 for district 4 versus 10 µg/cm2 for districts 2 and 3. Consequently, Ouagadougou is subjected to PM10 pollution, which can induce a significant production of ROS and IL-6 to cause adverse effects on the health of the population.

Dairy farming systems driven by the market and low-cost intensification in West Africa: the case of Burkina Faso

Eric Vall, Ollo Sib, Arielle Vidal, Jethro Barkwende Delma

The increase in demand for dairy products in Burkina Faso is encouraging livestock producers to develop milk production. Three types of dairy systems (pastoralists, agropastoralists and market-oriented dairy farms) have been characterised based on a sample of 60 producers operating in the West and centre of the country. Pastoralists’ dairy operations consist mainly of zebus, rely on pasture for feed, store little fodder, and recover little manure. Milk yields are low (1.4 l/tropical livestock unit (TLU)/day) and milk sales are limited, but mostly benefit women. Agropastoralists’ dairy operations consist mainly of zebus, store more fodder for feed, use more concentrate and recover manure better. Milk yields are higher (3.1 l/TLU/day) and milk sales are threefold those of pastoralists, but less of the money generated by milk sales goes to women. Market-oriented dairy farmers’ operations are mainly made up of crossbreds, reared indoors and fed on fodder and feeds, store much more fodder and recover manure even better. They generate the highest milk yields (7.3 l/TLU/day), and milk sales are 2.5-fold those of agropastoralists. However, money earned from milk sales mainly benefits men. The study shows that the improvement in dairy systems’ technical and economic performance, which mostly rests on genetics and cow feed, but also on better recycling of agricultural by-products, is driven by a low-cost intensification and market opportunity (raising processors demand).

    Dans le sillage d'Alphonse Laveran

    Patrick BERCHE
