Does attitude towards wife beating determine infant feeding practices during diarrheal illness in sub-Saharan Africa?

Betregiorgis Zegeye, Nicholas Kofi Adjei, Bright Opoku Ahinkorah, Edward Kwabena Ameyaw, Abdul-Aziz Seidu, Comfort Z. Olorunsaiye, Sanni Yaya

BackgroundInappropriate feeding practices of children during illness remains a public health problem globally, particularly in sub-Saharan Africa (SSA). One strategy to improve child health outcomes is through women empowerment—measured by wife beating attitude. However, the role of attitude towards wife beating in child feeding practices has not been comprehensively studied. Therefore, we investigated the association between women's attitude towards wife beating and child feeding practices during childhood diarrhea in 28 countries in SSA.MethodsWe analyzed data from the Demographic and Health Survey on 40,720 children under 5 years. Bivariate and multivariate binary logistic regression analyses were applied to assess the association between women's attitude towards wife beating and child feeding practices. The results were presented using adjusted odds ratio (aOR) with 95% confidence intervals (CIs).ResultsThe pooled results showed that appropriate feeding practices during diarrheal illness among under-five children was 9.3% in SSA, varying from 0.4% in Burkina Faso to 21.1% in Kenya. Regarding regional coverage, the highest coverage was observed in Central Africa (9.3%) followed by East Africa (5.5%), Southern Africa (4.8%), and West Africa (4.2%). Women who disagreed with wife-beating practices had higher odds of proper child feeding practices during childhood diarrhea compared to those who justified wife-beating practices (aOR = 2.02, 95% CI; 1.17–3.48).ConclusionThe findings suggest that women’s disagreement with wife beating is strongly associated with proper child feeding practices during diarrheal illness in SSA. Proactive measures and interventions designed to change attitudes towards wife-beating practices are crucial to improving proper feeding practices in SSA.

Does education sculpt healthcare choices? Exploring factors influencing healthcare utilization among female youths in eight low and lower-middle-income countries

IntroductionEmphasizing the global commitment to universal health coverage, this research addresses geographical variations and challenges faced by young females across low and lower-middle-income countries. Therefore, the objective of this study is to determine the factors that influence the decision-making of young females when it comes to seeking healthcare services in low or lower-middle-income countries.MethodologyWe examined healthcare usage among female youth across eight countries. We used data from the DHS and employed regression and Chi2 tests for our analysis. Our focus was on females aged 15 to 24 and their visits to healthcare facilities. To ensure the validity of our findings, we used weighted sampling to represent the population.ResultsWe had a total sample of 51,298 female youth groups between the ages of 15 and 24 who participated in our study. When it comes to the demographics of the participants, most of those in the 15-19 age group were from Burkina Faso (54.26%), while most of those in the 20-24 age group were from Ghana (50.19%). The impact of education varied across countries; primary education led to increased healthcare utilization in Kenya, Tanzania, and Cambodia, but unexpected trends were observed in Burkina Faso.ConclusionsEducation significantly influences healthcare utilization, positively impacting both primary and secondary education. Rural residents face challenges accessing timely healthcare. Geographical challenges, like diseases and limited accessibility, contribute to varied healthcare usage in Kenya and the Philippines. Addressing infrastructure issues is critical, emphasizing education and promoting transparency to enhance healthcare equity.

Does enrollment status in community-based insurance lead to poorer quality of care? Evidence from Burkina Faso

Paul Jacob Robyn, Till Bärnighausen, Aurélia Souares, Germain Savadogo, Brice Bicaba, Ali Sié, Rainer Sauerborn

IntroductionIn 2004, a community-based health insurance (CBI) scheme was introduced in Nouna health district, Burkina Faso, with the objective of improving financial access to high quality health services. We investigate the role of CBI enrollment in the quality of care provided at primary-care facilities in Nouna district, and measure differences in objective and perceived quality of care and patient satisfaction between enrolled and non-enrolled populations who visit the facilities.MethodsWe interviewed a systematic random sample of 398 patients after their visit to one of the thirteen primary-care facilities contracted with the scheme; 34% (n = 135) of the patients were currently enrolled in the CBI scheme. We assessed objective quality of care as consultation, diagnostic and counselling tasks performed by providers during outpatient visits, perceived quality of care as patient evaluations of the structures and processes of service delivery, and overall patient satisfaction. Two-sample t-tests were performed for group comparison and ordinal logistic regression (OLR) analysis was used to estimate the association between CBI enrollment and overall patient satisfaction.ResultsObjective quality of care evaluations show that CBI enrollees received substantially less comprehensive care for outpatient services than non-enrollees. In contrast, CBI enrollment was positively associated with overall patient satisfaction (aOR = 1.51, p = 0.014), controlling for potential confounders such as patient socio-economic status, illness symptoms, history of illness and characteristics of care received.ConclusionsCBI patients perceived better quality of care, while objectively receiving worse quality of care, compared to patients who were not enrolled in CBI. Systematic differences in quality of care expectations between CBI enrollees and non-enrollees may explain this finding. One factor influencing quality of care may be the type of provider payment used by the CBI scheme, which ha

Does extreme asymmetric dominance promote hybridization between Anopheles coluzzii and Anopheles gambiae s.s. in seasonal malaria mosquito communities of West Africa?

Abdoulaye Niang, Patric S. Epopa, Simon P. Sawadogo, Hamidou Maïga, Lassana Konaté, Ousmane Faye, Roch K. Dabiré, Frédéric Tripet, Abdoulaye Diabaté

BackgroundAnopheles gambiae s.s. and An. coluzzii are two of the most important malaria vector species in sub-Saharan Africa. These recently-diverged sibling species do not exhibit intrinsic post-zygotic barriers to reproduction and are thought to be separated by strong assortative mating combined with selection against hybrids. At present, little is known about the ecological conditions that determine hybridization and introgression between these cryptic taxa.MethodsSwarm segregation and assortative mating were studied in Western Burkina Faso in the villages of Vallée du Kou (VK7) and Soumousso which differed in terms of which sibling species was much rarer than the other, and in Bana where both occurred in similar proportions. Swarms and pairs in copula were collected and genotyped, the proportion of intra and interspecific mating determined, and interspecific sperm transfer checked genetically. Females were collected through larval and adult indoor collections and genotyped or sexed-and-genotyped via a novel multiplex PCR.ResultsA total of 3,687 males and 220 females were collected and genotyped from 109 swarms. Only 3 swarms were composed of males from both species, and these were from the village of VK7 where An. gambiae s.s. was comparatively rare. Mixed-species pairs captured in copula were only detected in that area and made for 3.62 % and 100 % of mating pairs involving An. coluzzii and An. gambiae s.s. individuals, respectively. The high An. gambiae s.s. cross-mating rate was mirrored by high rates of hybridizations estimated from female larvae and adults indoor collections. This contrasted with Soumousso, where despite being much less common than An. gambiae s.s., An. coluzzii males did not form mixed swarms, females were not found in interspecific swarms or copula and hybridization rates were low in both sibling species.ConclusionsThese data suggest that ecological conditions leading to rare An. gambiae s.s. in populations dominated by An. coluzzii may p

Does investment in COVID-19 information systems strengthen national digital health architecture? Lessons learned from Burkina Faso, Indonesia, Mali and Suriname

Colleen Boyle, Caitlin Madevu-Matson, Agus Rachmanto, Derek Kunaka, Lauren Gilliss, Leah McManus, Madina Kouyate, Maria Aruan, Nony Parmawaty, Rahim Kebe, Vijay Sewradj, Stephanie Watson-Grant

AbstractThere is limited research on how emergency investments can support national health information system strengthening both for a given health emergency response and for routine health program areas. This study examined the effect of COVID-19 vaccine digital investments on advancing national digital health system architectures in Burkina Faso, Indonesia, Mali and Suriname. Primary qualitative data were collected through purposively sampled key informant interviews and focus group discussions with 67 key stakeholders from government institutions, donors and implementing partners from February to May 2023. Primary data were triangulated with secondary data collected through desk review and were analyzed deductively and inductively using thematic coding. The study found that although USAID’s COVID-19 vaccine digital investments in Burkina Faso, Indonesia, Mali and Suriname did not contribute to the development of formal national digital health architecture documentation, progress was made in strengthening components of national digital health architectures, specifically around standards and interoperability and around governance. Progress was also made in motivating government stakeholders to invest in formal digital health transformation efforts. Lessons learned from this study suggest that connecting investments in system development and enhancement with investments in governance and infrastructure can improve country data management processes and data use for decision-making in routine health and emergency responses.

Domestic Accidents of Children in the Orodara District of Burkina Faso: Mothers’ Knowledge of First-Aid Practices

First-aid practices after a domestic accident are not always known, especially in Africa. This study aimed to measure mothers’ knowledge of emergency procedures and attitudes in the event of domestic accidents in children. We conducted a cross-sectional study in the Orodara health district, Kénédougou province, Burkina Faso, among mothers of children aged 0–14 years. The dependent variable was the mothers’ knowledge of domestic accident first-aid practices, and the independent variables were the sociodemographic characteristics of the households and the mothers. Determinants were identified using linear regression with a threshold of 5%. A total of 798 mothers were surveyed. The mean knowledge score was 6.9 (standard deviation = 1.5) out of 19. Upon our multivariate analysis, the factors associated with the mothers’ knowledge about first-aid practices were the mothers’ age, the number of children under 14 years old living in the same household, the household size, the score for knowledge of non-recommended attitudes, the mothers’ level of education, and the place of residence. This study showed that awareness campaigns, especially in rural areas, seem important in improving mothers’ knowledge of first-aid practices in domestic accidents and, therefore, reducing the morbidity and mortality associated with domestic accidents.

Domestic Larval Control Practices and Malaria Prevalence among Under-Five Children in Burkina Faso

IntroductionLarval source management has contributed to malaria decline over the past years. However, little is known about the impact of larval control practices undertaken at the household level on malaria transmission.MethodsThe study was conducted in Kaya health district after the 2010 mass distribution of insecticide treated-nets and the initiation of malaria awareness campaigns in Burkina Faso. The aim was to (i) estimate the level of domestic larval control practices (cleaning of the house and its surroundings, eradication of larval sources, and elimination of hollow objects that might collect water); (ii) identify key determinants; and (iii) explore the structural relationships between these practices, participation in awareness-raising activities and mothers’ knowledge/attitudes/practices, and malaria prevalence among under-five children.ResultsOverall, 2004 households were surveyed and 1,705 under-five children were examined. Half of the mothers undertook at least one action to control larval proliferation. Mothers who had gone to school had better knowledge about malaria and were more likely to undertake domestic larval control practices. Living in highly exposed rural areas significantly decreased the odds of undertaking larval control actions. Mothers’ participation in malaria information sessions increased the adoption of vector control actions and bednet use. Malaria prevalence was statistically lower among children in households where mothers had undertaken at least one vector control action or used bed-nets. There was a 0.16 standard deviation decrease in malaria prevalence for every standard deviation increase in vector control practices. The effect of bednet use on malaria prevalence was of the same magnitude.ConclusionCleaning the house and its surroundings, eradicating breeding sites, and eliminating hollow objects that might collect water play a substantial role in preventing malaria among under-five. There is a need for national malaria contro

Domestic migration and mobile phones: A qualitative case study focused on recent migrants to Ouagadougou, Burkina Faso

Migration has long been used as a strategy for livelihood diversification in rural, subsistence communities. Yet in order for migration to effectively serve as a livelihood diversification strategy, it should meet certain conditions: migration should ease financial burdens, should confer access to economically valuable resources and information, and should broaden social networks. Using qualitative data gathered in 25 interviews with rural migrants to Ouagadougou, Burkina Faso, we examine how mobile phone technology has impacted migration as a livelihood diversification strategy. Our results show that while mobile phones facilitate migration, the advantages conferred may benefit migrants at the expense of the home communities. Mobile phones alleviate financial constraints, enable access to broader networks, and facilitate informational and resource support among migrants. Our results show limited evidence of migrants using mobile phone technology to provide resources or information to the home community. Our results highlight the need to reconsider the ways in which migration can be used as a livelihood diversification strategy in light of changing communication technologies to promote the economic success of both migrants and their home communities.

Nicolas Fanantenana Herinirina, Lova Hasina Ny Ony Narindra Rajaonarison, Andry Roussel Herijoelison, Olivat Aimée Alson Rakoto, Ahmad Ahmad

Najat Handor, Sanae Elalami, Mounya Bouabdellah, Abdelatif Srifi, Hicham Esselmani, Laila Benchekroun, Layachi Chabraoui

Sihame Lemouakni, Amale Hassani, Hakim Elyajouri, Mohammed Kmari, Hakim Ourrai, Rachid Abilkacem, Aomar Agadr

Redouane Ouakrim, Younes Ouchrif, Issam El Ouakili, Mohammed Kharmaz, Moulay Omar Elamrani, Mohammed Elouadghiri, Mustapha Mahfoud, Ahmed Elbardouni, Abdou Lahlou, Mohammed Saleh Berrada

Rakotoniaina Masinarivo Daniella, Rasolonjatovo Anjaramalala, Ramiandrisoa, Raveloson Freddie, Rakotoarimanana Solofonirina, Rabearivony Nirina
