Mouhamadou Bamba Ndiaye, Maboury Diao, Adama Kane, Malick Bodian, Alassane Mbaye, Mouhamadoul Mounir Dia, Moustapha Sarr, Abdoul Kane, Serigne Abdou Ba

Mohammed Bachrif, Houssam Laachach, Ilham Benahmed, Mehdi Berrajaa, Alaa Fliti, Mustapha Aziouaz, Nabila Ismaili, Noha Elouafi

Hanane Boussir, Amine Ghalem, Nabila Ismaili, Noha El Ouafi

Omar Sow, William Valentin, Diouf Cheikh, Barboza Denis, Faye Samba Thiapato, Diallo Ibrahima, Gueye Serigne Modou Kane

Mohamed Hedfi, Nada Essid, Ferdaous Trabelsi, Hakim Znaidi

Houda Elbahraoui, Abderrahman Elmazghi, Hanane Bouziane, Adil Elghanmi, Amina Lakhdar, Driss Ferhati

Kamilia Laabadi, Fatima Zohra Fdili Alaoui, Sofia Jayi, Hakima Bouguern-Hekmat Chaara, Moulay Abdilah Melhouf

Georges Antoine Bazolo Ba Ngouala, Loubna Bourgi, Joao Arm Indo Da Veiga, Arona Sakho

Endothelial Protein C Receptor Gene Variants Not Associated with Severe Malaria in Ghanaian Children

BackgroundTwo recent reports have identified the Endothelial Protein C Receptor (EPCR) as a key molecule implicated in severe malaria pathology. First, it was shown that EPCR in the human microvasculature mediates sequestration of Plasmodium falciparum-infected erythrocytes. Second, microvascular thrombosis, one of the major processes causing cerebral malaria, was linked to a reduction in EPCR expression in cerebral endothelial layers. It was speculated that genetic variation affecting EPCR functionality could influence susceptibility to severe malaria phenotypes, rendering PROCR, the gene encoding EPCR, a promising candidate for an association study.MethodsHere, we performed an association study including high-resolution variant discovery of rare and frequent genetic variants in the PROCR gene. The study group, which previously has proven to be a valuable tool for studying the genetics of malaria, comprised 1,905 severe malaria cases aged 1–156 months and 1,866 apparently healthy children aged 2–161 months from the Ashanti Region in Ghana, West Africa, where malaria is highly endemic. Association of genetic variation with severe malaria phenotypes was examined on the basis of single variants, reconstructed haplotypes, and rare variant analyses.ResultsA total of 41 genetic variants were detected in regulatory and coding regions of PROCR, 17 of which were previously unknown genetic variants. In association tests, none of the single variants, haplotypes or rare variants showed evidence for an association with severe malaria, cerebral malaria, or severe malaria anemia.ConclusionHere we present the first analysis of genetic variation in the PROCR gene in the context of severe malaria in African subjects and show that genetic variation in the PROCR gene in our study population does not influence susceptibility to major severe malaria phenotypes.

Engagement and social acceptance in genome editing for human benefit: Reflections on research and practice in a global context

Sebastián Barbosa, Lea Pare Toe, Delphine Thizy, Manjulika Vaz, Lucy Carter

While there are both practical and ethical reasons for public engagement in science and innovation, real-world detailed examples of engagement practice and the lessons to come from these are still hard to find. This paper showcases three contextually diverse case studies of engagement practice. Case 1 recounts the experiences of a government-funded initiative to involve scientists and policy makers as science communicators for the purpose of engaging the Argentine public on gene editing. Case 2 describes the research methodologies used to elicit diverse stakeholder views in the face of political uncertainty and institutional distrust in India. Finally, case 3 unpacks the tensions and gaps with existing international guidelines for ensuring local voices are respected in community decision-making in Burkina Faso. Each case shares its own compelling rationale for selecting the engagement method chosen and details the challenges encountered along the way. Each case shares its vision for creating legitimate opportunities for broader societal involvement in the planning, conduct and delivery of responsible science. These cases demonstrate the nuances, sensitivities and challenges of engaging with publics and broader stakeholders in discussions about genome editing for human benefit.

Engaging diverse communities participating in clinical trials: case examples from across Africa

Aceme Nyika, Roma Chilengi, Deus Ishengoma, Sally Mtenga, Mahamadou A Thera, Mahamadou S Sissoko, John Lusingu, Alfred B Tiono, Ogobara Doumbo, Sodiomon B Sirima, Martha Lemnge, Wen L Kilama

BackgroundIn the advent of increasing international collaborative research involving participants drawn from populations with diverse cultural backgrounds, community engagement becomes very critical for the smooth conduction of the research. The African Malaria Network Trust (AMANET) is a pan-African non-governmental organization that sponsors and technically supports malaria vaccine trials in various African countries.Case descriptionAMANET sponsored phase Ib or IIb clinical trials of several malaria vaccine candidates in various Africa countries. In Burkina Faso, Mali and Tanzania trials of the merozoite surface protein 3 -- in its Long Synthetic Peptide configuration (MSP3 LSP) -- were conducted. In Mali, the apical membrane antigen 1 (AMA1) was tested, while a hybrid of glutamate rich protein (GLURP) and MSP3 (GMZ2) was tested in Gabon. AMANET recognizes the importance of engaging with the communities from which trial participants are drawn, hence community engagement was given priority in all project activities conducted in the various countries.Discussion and evaluationExisting local social systems were used to engage the communities from which clinical trial participants were drawn. This article focuses on community engagement activities employed at various AMANET-supported clinical trial sites in different countries, highlighting subtle differences in the approaches used. The paper also gives some general pros and cons of community engagement.ConclusionsCommunity engagement enables two-way sharing of accurate information and ideas between researchers and researched communities, which helps to create an environment conducive to smooth research activities with enhanced sense of research ownership by the communities.

Enhanced effect of seasonal malaria chemoprevention when coupled with nutrients supplementation for preventing malaria in children under 5 years old in Burkina Faso: a randomized open label trial

Paul Sondo, Bérenger Kaboré, Toussaint Rouamba, Eulalie Compaoré, Yssimini Nadège Guillène Tibiri, Hyacinthe Abd-El Latif Faïçal Kaboré, Karim Derra, Marc Christian Tahita, Hamidou Ilboudo, Gauthier Tougri, Ismaïla Bouda, Tikanou Dakyo, Hyacinthe Kafando, Florence Ouédraogo, Eli Rouamba, So-vii Franck Hien, Adama Kazienga, Cheick Saïd Compaoré, Estelle Bambara, Macaire Nana, Prabin Dahal, Franck Garanet, William Kaboré, Thierry Léfèvre, Philippe Guerin, Halidou Tinto

BackgroundIn rural African settings, most of the children under the coverage of Seasonal Malaria Chemoprevention (SMC) are also undernourished at the time of SMC delivery, justifying the need for packaging malarial and nutritional interventions. This study aimed at assessing the impact of SMC by coupling the intervention with nutrients supplementation for preventing malaria in children less than 5 years old in Burkina Faso.MethodsA randomized trial was carried out between July 2020 and June 2021 in the health district of Nanoro, Burkina Faso. Children (n = 1059) under SMC coverage were randomly assigned to one of the three study arms SMC + Vitamin A (SMC-A, n = 353) or SMC + Vitamin A + Zinc (SMC-AZc, n = 353) or SMC + Vitamin A + PlumpyDoz(tm) (SMC-APd, n = 353)-a medium quantity—lipid-based nutrient supplement (MQ-LNS). Children were followed up for one year that included an active follow-up period of 6 months with scheduled monthly home visits followed by 6 months passive follow-up. At each visit, capillary blood sample was collected for malaria diagnosis by rapid diagnosis test (RDT).ResultsAdding nutritional supplements to SMC had an effect on the incidence of malaria. A reduction of 23% (adjusted IRR = 0.77 (95%CI 0.61–0.97) in the odds of having uncomplicated malaria in SMC-APd arm but not with SMC-AZc arm adjusted IRR = 0.82 (95%CI 0.65–1.04) compare to control arm was observed. A reduction of 52%, adjusted IRR = 0.48 (95%CI 0.23–0.98) in the odds of having severe malaria was observed in SMC-APd arm compared to control arm. Besides the effect on malaria, this combined strategy had an effect on all-cause morbidity. More specifically, a reduction of morbidity odds of 24%, adjusted IRR = 0.76 (95%CI 0.60–0.94) in SMC-APd arm compared to control arm was observed. Unlike clinical episodes, no effect of nutrient supplementation on cross sectional asymptomatic infections was observed.ConclusionAdding nutritional supplements to SMC significantly increases the impact

Enhanced quality of nutrition services during antenatal care through interventions to improve maternal nutrition in Bangladesh, Burkina Faso, Ethiopia, and India

Phuong H Nguyen, Lan M Tran, Shivani Kachwaha, Tina Sanghvi, Zeba Mahmud, Maurice G Zafimanjaka, Tamirat Walissa, Sebanti Ghosh, Sunny S Kim

BackgroundQuality antenatal care (ANC) services are critical for maternal health and nutrition. Information on the quality of nutrition interventions during ANC is scarce in low- and middle-income countries. We examined the effects of intensified maternal nutrition interventions during ANC on service readiness, provision of care, and experience of care and assessed the inter-relationships between the dimensions of quality.MethodsWe used data from impact evaluations of maternal nutrition interventions in Bangladesh, Burkina Faso, Ethiopia, and India. We calculated the quality of nutrition services during ANC using information from health facility assessments, health care provider interviews, ANC observations, and client exit interviews. We used structural equation models to examine relationships between the dimensions of quality.ResultsHealth facilities in all four countries had a high service readiness component in terms of basic amenities, equipment and supplies, medicines and commodities, and guidelines (mean (x̄) = 8–10 in Bangladesh and Burkina Faso, x̄ = 7–9 in Ethiopia, and x̄ = 6–8 in India). Scores for provision of care were low across the countries but higher in intervention compared to control areas in Bangladesh (5.2 vs. 2.9) and Burkina Faso (5.6 vs. 4.8), but not significantly different in Ethiopia (range = 4.7–5.0) and India (range = 2.6–3.5). For experience of care, client satisfaction scores were high and similar between intervention and control areas in all countries (range = 8.3–9.7), but client experience scores were lower with statistically significant differences observed only in Bangladesh (x̄ = 8.2 in intervention vs. x̄ = 7.1 in control areas). The interventions had significant direct effects on service readiness in Bangladesh (β = 0.07), Burkina Faso (β = 1.20), and Ethiopia (β = 1.0), on the provision of care in Bangladesh (β = 2.27), Burkina Faso (β = 1.27), and India (β = 0.96), and experience of care in Bangladesh (β = 0.21).ConclusionsI

Enhancement of the geological mapping in weathered plutonic and metamorphic rocks areas using village water supply wells data (lithology and hydrochemistry)

Cheik Abba Cissé Ouangaré, Séta Naba, Patrick Lachassagne

In weathered plutonic and metamorphic rocks areas, because rock outcrops are rare due to the extensive regolith cover, geological mapping is largely based on the interpretation of airborne data and imagery (aerial photographs, satellite images, airborne geophysics when available, etc.). In the sub-Saharan Africa, numerous village water supply campaigns were performed during the last 40 years. Most hydrogeological and geological data from these campaigns are stored and safeguard in country scale databases. In this study, we develop a methodology to improve the existing geological maps in a such geological context with lithological data from the existing databases. These data were not used during past geological mapping; yet they provide a complementary access to the lithology, notably in areas where outcrops are very scarce. We propose a coherent methodology to use and validate such geological data, complementary to field observations on the scarce outcrops, to cope with their uncertainties. We also show that groundwater hydrochemistry, namely the silica content, can be of use to complete the geological mapping. Using data from 735 boreholes drilled in the Koudougou and Léo square study area of Burkina Faso, we validate this new methodological approach and locally propose a more accurate 1:200,000 geological map. Highlights•We propose a new cartographic approach in basement zones with thick regolith cover, based on data from lithological descriptions of village hydraulics water boreholes.•We demonstrate the value of hydrochemistry, in particular the silica content of borehole water, for discriminating major lithological units in basement zones.•We propose a methodology to check and validate the boreholes' lithology and, endly and if necessary, to correct the existing geological maps.

Enhancing the Introduction and Scale Up of Self-Administered Injectable Contraception (DMPA-SC) in Health Systems (the EASIER Project): Protocol for Embedded Implementation Research

BackgroundThe introduction of self-administered injectable contraception presents an opportunity to address the unmet need for family planning. As ministries of health scale up self-administered injectable contraception, there is a scarcity of knowledge on the implementation practices and contextual conditions that help and hinder these efforts. The World Health Organization has launched the “enhancing self-administered family planning through embedded research project” (EASIER) to address this challenge.ObjectiveEASIER’s objectives are to: (1) assess the coverage of self-injectable contraception, and the readiness of health systems to integrate it into the contraceptive method mix; (2) document strategies used to introduce and scale up self-injectable contraception and understand practices that have led to success and challenges; (3) identify the contextual factors that affect the adoption and implementation of self-injectable contraception throughout health systems; (4) understand whether implementation addresses users’ preferences and needs; (5) strengthen collaboration between decision makers, researchers, and implementers; support and build capacity to use evidence.MethodsEASIER developed a global protocol that implementation research (IR) teams in Burkina Faso, Ghana, and Kenya adapted into country-level embedded IR projects. In all countries (1) at the national level, IR teams evaluate the policy environment for scaling up by conducting a desk review and in-depth interviews; (2) at the local level, IR teams implement quantitative questionnaires on structural and organizational readiness to integrate self-injection into the method mix; (3) in “case study” localities, IR teams conduct in-depth interviews and focus group discussions with implementers, method users, and community members; and (4) IR teams use participatory action research to elicit stakeholder participation and translate findings into programmatic decisions.ResultsEASIER has been launched in all

Henri Lagarde, Pauline Martin, Philippe Barre, René Baumont, Mickaël Brochard, Philippine Coeugnet, Pascal Croiseau, Luc Delaby, Anne de la Torre, Nicolas C Friggens, Raphael Guatteo, Frédéric Joly, Marie-Angelina Magne, Sandra Novak, Christophe Perrot, Laurent Schibler, Patrick Veysset, Aurélie Vinet, Didier Boichard

    Ahmed Rouihi, Bouchaib Hemmaoui

Non-Hodgkin's lymphoma is a rare histological type of nasopharyngeal cancer. Most of these cancers are undifferentiated carcinomas or undifferentiated nasopharyngeal carcinomas (UNC). We report a case of nasopharyngeal non-Hodgkin's lymphoma. Immunohistochemical examination showed B-cell non-Hodgkin's lymphoma. The study involved a 47-year old patient with no particular past medical history presenting with right laterocervical swelling associated with right nasal obstruction and rhinorrhea that had persisted for 6 months. Clinical examination showed right voluminous painless adherent laterocervicale lymphadenopathy without signs of inflammation. Endoscopic examination of the nasopharynx showed enormous process extending to the posterior nares. Anatomo-pathologic study of the biopsy with immunohistochemical study showed B-cell non-Hodgkin's lymphoma. Patient's outcome was favorable under chemotherapy with no relapse during 6-month follow-up period. Primary non-Hodgkin's lymphoma of the nasopharynx is a rare localization of lymphomas. It occurs in less than 10% of patients with lymphoma of the head and of the neck. It often poses a problem in positive clinical and histological diagnosis. Symptoms are generally little specific. Peripheral lymphadenopathies occur in 50% of cases. Diagnosis is based on biopsy with immunohistochemical examination. Therapeutic progress have been achieved through the more frequent use of chemotherapy associated with radiotherapy.

Enorme kyste amygdaloïde cervical: à propos d'un cas

Ahmed Rouihi, Bouchaib Hemmaoui, Noureddine Errami, Fouad Benariba
