Laurence Kern, Lilian Fautrelle, Nathalie Godart

Jean Berchmans Bizimana, Mansourou Mohamed Lawani, Barnabé Akplogan, Charles Gaturagi

    Jean-Arthur Micoulaud-Franchi, Armelle Rancillac

    Laure Peter-Derex, Armelle Rancillac

Marine Saour, Pascal Colson, Philippe Gaudard

Acute and subchronic toxicity of aqueous extracts of Combretum micranthum (G. Don) and Gardenia sokotensis (Hutch) having ethnobotanical uses in Burkina Faso

OUEDRAOGO Elisabeth, ZABRE Généviève, TINDANO Basile, YOUGBARE Wendyam Joëlle Raymonde, OWONA Pascal Emmanuel, BAYALA Balé

Medicinal plants are the major sources of drugs used to treat diseases. Scientific studies were performed on some plants, but few data are available on the medicinal plants used to manage bone diseases in Burkina Faso. This study was conducted to identify medicinal plants used in the treatment of osteoporosis and investigate the acute and subchronic toxicity of Combretum micranthum and Gardenia sokotensis aqueous extracts. A survey was carried out through a structured interview with traditional practitioners. Phytochemical screening was performed using a validated thin-layer chromatographic method. The acute oral toxicity study of extracts was validated at 2000 mg/kg in mice. In the subchronic toxicity, rats were orally administered 100, 200, and 400 mg/kg of each extract for 90 days. Results show sixty-one plant species divided into 33 families. C. micranthum and G. sokotensis were most cited. Phytochemical screening of aqueous extracts of plants revealed flavonoids, tannins, and terpenoids. Acute toxicity study indicated up to 2000 mg/kg of each extract was tolerated without death or any signs of toxicity. In the subchronic toxicity test, physiological, serum biochemistry, and hematology examination, no features suggestive of each extract's toxicity were observed at doses of 100 and 200 mg/kg. The hepatic balance (aspartate and alanine aminotransferases) was significantly (p < 0.0001) reduced at doses of 100 and 200 mg/kg. A significant (p < 0.001) decrease in triglyceride and cholesterol levels was observed. To conclude, extracts were non-toxic and could be used for their ethnopharmacological properties, but experimental therapeutic evidence is still needed. Graphical Abstract Highlights•Combretum micranthum and Gardenia sokotensis are hepatoprotective and nephroprotective.•Combretum micranthum and Gardenia sokotensis present a hepatic risk at high doses.•Combretum micranthum and Gardenia sokotensis reduce triglyceride levels.

Acute Effects of Cocoa Flavanols on Blood Pressure and Peripheral Vascular Reactivity in Type 2 Diabetes Mellitus and Essential Hypertension: A Protocol for an Acute, Randomized, Double-Blinded, Placebo-Controlled Cross-Over Trial.

Anouk Tanghe, Bert Celie, Samyah Shadid, Ernst Rietzschel, Jos Op 'T Roodt, Koen D Reesink, Elsa Heyman, Patrick Calders

Introduction: Patients with type 2 diabetes mellitus are at high risk to develop vascular complications resulting in high morbidity and mortality. Cocoa flavanols are promising nutraceuticals with possible beneficial vascular effects in humans. However, limited research is currently available on the vascular effects in a diabetic population with inconsistent results. Possible reasons for this inconsistency might be heterogeneity in the given intervention (dose per time and day, single dose vs. split-dose, placebo formula) and the studied population (blood pressure at baseline, duration of diabetes, use of vasoactive antihypertensive and antidiabetic drugs, sex). Therefore, we aimed to develop a randomized, double-blinded, placebo-controlled cross-over trial to investigate whether cocoa flavanols have an acute impact on blood pressure and vascular reactivity in patients with type 2 diabetes with and without arterial hypertension. Methods and Analysis: We will include participants in four groups: (i) patients with type 2 diabetes without arterial hypertension, (ii) patients with type 2 diabetes with arterial hypertension and 1 antihypertensive drug, (iii) non-diabetic participants with essential hypertension and 1 antihypertensive drug, and (iv) healthy controls. All participants will complete the same protocol on both testing days, consuming high-flavanol cocoa extract (790 mg flavanols) or placebo. Macrovascular endothelial function (flow-mediated dilation) and blood pressure will be measured before and after capsule ingestion. Forearm muscle vasoreactivity (near-infrared spectroscopy) and brachial artery blood flow (echo-doppler) will be assessed in response to a dynamic handgrip exercise test after capsule ingestion. Data will be analyzed with a random intercept model in mixed models. Clinical Trial Registration: www.Clinicaltrials.gov, identifier: NCT03722199.

Acute intestinal intussusception among children under five years of age admitted in an Ouagadougou hospital, Burkina Faso, 2008-2013: epidemiological, clinical and therapeutic aspects

Tapsoba Wendlamita Toussaint, Albert Wandaogo, Idrissa Clétus Yaméogo W, Isso Ouédraogo, Somkièta Modeste Francis Ouédraogo, Olivier Zampou, Bernadette Béré, Negar Aliabadi, Eyal Leshem, Moumouni Nikièma, Ma Ouattara, Jason M Mwenda, Isidore Bonkoungou, Emile Bandré, Umesh D Parashar, Jacqueline E Tate

Introductionacute intestinal intussusception is a life-threatening surgical condition. In some settings, rotavirus vaccines have been associated with a low-level increased risk of intussusception. We describe the epidemiology, clinical manifestations and management of intussusception in a tertiary referral hospital in Burkina Faso prior to the introduction of rotavirus vaccine in October 2013.Methodswe retrospectively reviewed medical records of all children under 5 years of age treated at the Charles de Gaulle Pediatric Hospital for intussusception meeting the Brighton level 1 diagnostic criteria, from October 31st, 2008 to October 30th, 2013. We report the incidence of intussusception as well as descriptive characteristics of these cases.Resultsa total of 107 Brighton level 1 intussusception cases were identified, representing a hospital incidence of 21.4 cases / year. There were 69 males and 38 females (sex ratio of 1.8), with a median age of 8 months (range 2 months to 4 years). Sixty-two percent of intussusception cases occurred among infants (n = 67 cases). The average time from symptom onset to seeking medical consultation was 3.8 days +/- 2.7 (range 0 to 14 days). Treatment was mainly surgical (105 patients, 98.1%) with 35 patients (32.7%) undergoing intestinal resection. Thirty-seven patients (35.5%) experienced post-operative complications. The mortality rate was 9.3%. Intestinal resection was a risk factor for death from intussusception.Conclusionin this review of intussusception hospitalizations prior to rotavirus vaccine introduction in Burkina Faso, delays in seeking care were common and were associated with mortality.

Acute undifferentiated febrile illness in a traveler returning from Burkina Faso, West Africa to Puducherry, India- a case report

Srikanth Srirama, Krishan Kumar Sihag, Anand Kumar Chandrasekaran, Neethi Prasannan Latha, Waseema Arif, Balaji Sampath Kumar, Panneer Devaraju

BackgroundImported malaria remains a significant diagnostic challenge, particularly in regions approaching elimination. This case report is novel in its exploration of the complexities involved in diagnosing malaria imported from a high-endemic area to a low-endemic region, emphasizing the critical role of detailed travel history and molecular diagnostics in identifying the disease. The case underscores the potential public health implications of missing such diagnoses in regions where routine malaria testing is not performed due to low endemicity.Case presentationA 25-year-old male presented to a tertiary care hospital in Puducherry, India, with symptoms of intermittent fever, headache, loss of appetite, and vomiting, which persisted over a week. Initial laboratory findings showed thrombocytopenia and leukopenia, with negative tests for dengue, chikungunya, and scrub typhus. Further investigation using molecular diagnostic techniques identified Plasmodium falciparum. The patient had recently returned from Burkina Faso, West Africa, where he had experienced similar episodes of fever, establishing the diagnosis of imported malaria.ConclusionsThis case highlights the necessity of considering imported malaria in the differential diagnosis of febrile illnesses in India, a country nearing malaria elimination. It illustrates the importance of incorporating travel history into clinical evaluations and supports the use of molecular diagnostics to effectively diagnose malaria in settings where traditional diagnostic methods may overlook imported cases. The findings advocate for enhanced surveillance and diagnostic preparedness to manage imported cases of malaria, thereby supporting ongoing elimination efforts and preventing the re-establishment of local transmission.Clinical trial numberNot applicable.

Adapting a safe water storage container to improve household stored water quality in rural Burkina Faso: a cluster randomized trial

Darcy M. Anderson, Michael B. Fisher, Osborn Kwena, Hermann Kambou, Romain Broseus, Ashley R. Williams, Kaida Liang, Rohit Ramaswamy, Jamie Bartram

Safe water storage protects household drinking water from microbial contamination, maintaining water quality and preventing diarrhea and other water-borne diseases. However, achieving high adoption and sustained use of safe storage is challenging. Systematic adaptation can address these challenges by improving contextual fit while retaining core functionality to protect water quality. We applied Plan-Do-Study-Act (PDSA) cycles to systematically adapt a safe water storage container (SWSC) intervention for implementation in rural Burkina Faso. This study describes the adaptation process and the impacts of the SWSC on Escherichia coli contamination in household stored water in a cluster-randomized trial with 49 intervention villages (274 households) and 50 no-intervention control villages (290 households). SWSC adoption among intervention households was high (88.9%). The intervention achieved approximately a 0.4 log reduction in E. coli contamination. Intervention impact was likely moderated by differential changes in improved source use across intervention and control households. Safe storage improves water quality when used consistently. PDSA frameworks can guide the adaptation of safe storage interventions to optimize adoption and sustained use in new contexts while preserving core functions that protect water quality.

Adapting Agriculture to Climate Change: Are Climate‐Smart Practices Important in Burkina Faso?

Benoit Bado, Noël Thiombiano, Nestor Tiehi Tito

ABSTRACTIn Burkina Faso, smallholder farmers rely heavily on rain‐fed agriculture, which is affected by climate change. The adoption of climate‐smart practices is essential to strengthen the resilience of agricultural systems to climate change and improve household food security and, consequently, global food security. Despite the great potential of these practices to combat the effects of climate change on agriculture, their adoption by farmers remains low or limited. The reasons for this low adoption are varied, suggesting that the factors are largely contextual. This research analyzes the determinants of the adoption of climate‐smart practices among farmers in Burkina Faso in the context of innovation diffusion. To do this, a multivariate probit regression model was used on survey data from 48,159 plots owned by farmers in the country. The results show that age, gender, access to credit, access to extension services, property rights, livestock ownership, and education are the main determinants of the adoption of climate‐smart practices in Burkina Faso. Large‐scale awareness‐raising, training, and promotion, while promoting access to credit and land ownership documents, are necessary for better adoption of climate‐smart practices.

Adapting field-mosquito collection techniques in a perspective of near-infrared spectroscopy implementation

Bernard Mouonniba Somé, Dari F. Da, Ruth McCabe, Nicaise Denis C. Djègbè, Lawata Inès Géraldine Paré, Kadidia Wermé, Karine Mouline, Thierry Lefèvre, Anicet Georges Ouédraogo, Thomas S. Churcher, Roch Kounbobr Dabiré

BackgroundNear-infrared spectroscopy (NIRS) has the potential to be a useful tool for assessing key entomological parameters of malaria-transmitting mosquitoes, including age, infectious status and species identity. However, before NIRS can be reliably used in the field at scale, methods for killing mosquitoes and conserving samples prior to NIRS scanning need to be further optimized. Historically, mosquitoes used in studies have been killed with chloroform, although this approach is not without health hazards and should not be used in human dwellings. For the application of NIRS scanning it is also unclear which mosquito preservation method to use. The aim of the study reported here was to investigate the use of pyrethrum spray, a commercially available insecticide spray in Burkina Faso, for killing mosquitoesMethodsLaboratory-reared Anopheles gambiae and Anopheles coluzzii were killed using either a pyrethrum insecticide spray routinely used in studies involving indoor mosquito collections (Kaltox Paalga®; Saphyto, Bobo-Dioulasso, Burkina Faso) or chloroform (“gold standard”). Preservative methods were also investigated to determine their impact on NIRS accuracy in predicting the species of laboratory-reared Anopheles and wild-caught mosquito species. After analysis of fresh samples, mosquitoes were stored in 80% ethanol or in silica gel for 2 weeks and re-analyzed by NIRS. In addition, experimentally infected An. coluzzii and wild-caught An. gambiae sensu lato (s.l.) were scanned as fresh samples to determine whether they contained sporozoites, then stored in the preservatives mentioned above for 2 weeks before being re-analyzed.ResultsThe difference in the accuracy of NIRS to differentiate between laboratory-reared An. gambiae mosquitoes and An. coluzzii mosquitoes killed with either insecticide (90%) or chloroform (92%) was not substantial. NIRS had an accuracy of 90% in determining mosquito species for mosquitoes killed with chloroform and preserved in ethanol

Kadidiatou R. Kourouma, Tieba Millogo, Aissatou Diallo, Wambi M.E. Yaméogo, Marie L. Agbré-Yacé, Mamadou D. Baldé, Issaka Tiembré, Alexandre Delamou, Séni Kouanda

BackgroundThe World Health Organization Safe Childbirth Checklist (WHO SCC) was developed to increase the uptake of essential birth practices; however, only a few studies have adopted this process in French West African countries.AimThis study aimed to describe the WHO SCC adaptation process across Burkina Faso, Côte d’Ivoire, and Guinea, and the lessons learned.SettingAdaptation processes were conducted in the above-mentioned countries.MethodsFrom May 2022 to November 2022, a cross-country adaptation of the WHO SCC was carried out using a co-creation approach following a modified Delphi process. This process included the contextual adaptation of the tool by local technical advisory groups in each country based on national guidelines, the harmonisation and production of a single modified SCC by the Cross-Country Technical Advisory Group, a pre-test of the modified SCC, and adoption.ResultsMinor modifications were made on 27 items. No items were deleted. Two items related to hand hygiene and the use of protective equipment were added at the ‘just before pushing’ pause point. The modified SCC implemented in each country consisted of 31 items, with variations observed in the timing of the monitoring signs when plotting the partograph. The tool was introduced following the A3 and kakemono formats.ConclusionThe study emphasised the importance of engaging all stakeholders and end users in the adaptation process for a sustainable use of the tool.ContributionThis collaborative effort between countries to develop a unified SCC highlights the importance of adaptation based on national guidelines and local contexts.

Adaptive deletion in resistance gene duplications in the malaria vector Anopheles gambiae

Benoît S. Assogba, Haoues Alout, Alphonsine Koffi, Cédric Penetier, Luc S. Djogbénou, Patrick Makoundou, Mylène Weill, Pierrick Labbé

AbstractWhile gene copy‐number variations play major roles in long‐term evolution, their early dynamics remains largely unknown. However, examples of their role in short‐term adaptation are accumulating: identical repetitions of a locus (homogeneous duplications) can provide a quantitative advantage, while the association of differing alleles (heterogeneous duplications) allows carrying two functions simultaneously. Such duplications often result from rearrangements of sometimes relatively large chromosome fragments, and even when adaptive, they can be associated with deleterious side effects that should, however, be reduced by subsequent evolution. Here, we took advantage of the unique model provided by the malaria mosquito Anopheles gambiae s.l. to investigate the early evolution of several duplications, heterogeneous and homogeneous, segregating in natural populations from West Africa. These duplications encompass ~200 kb and 11 genes, including the adaptive insecticide resistance ace‐1 locus. Through the survey of several populations from three countries over 3–4 years, we showed that an internal deletion of all coamplified genes except ace‐1 is currently spreading in West Africa and introgressing from An. gambiae s.s. to An. coluzzii. Both observations provide evidences of its selection, most likely due to reducing the gene‐dosage disturbances caused by the excessive copies of the nonadaptive genes. Our study thus provides a unique example of the early adaptive trajectory of duplications and underlines the role of the environmental conditions (insecticide treatment practices and species ecology). It also emphasizes the striking diversity of adaptive responses in these mosquitoes and reveals a worrisome process of resistance/cost trade‐off evolution that could impact the control of malaria vectors in Africa.

Adaptive Potential of Hybridization among Malaria Vectors: Introgression at the Immune Locus TEP1 between Anopheles coluzzii and A. gambiae in ‘Far-West’ Africa

“Far-West” Africa is known to be a secondary contact zone between the two major malaria vectors Anopheles coluzzii and A. gambiae. We investigated gene-flow and potentially adaptive introgression between these species along a west-to-east transect in Guinea Bissau, the putative core of this hybrid zone. To evaluate the extent and direction of gene flow, we genotyped site 702 in Intron-1 of the para Voltage-Gated Sodium Channel gene, a species-diagnostic nucleotide position throughout most of A. coluzzii and A. gambiae sympatric range. We also analyzed polymorphism in the thioester-binding domain (TED) of the innate immunity-linked thioester-containing protein 1 (TEP1) to investigate whether elevated hybridization might facilitate the exchange of variants linked to adaptive immunity and Plasmodium refractoriness. Our results confirm asymmetric introgression of genetic material from A. coluzzii to A. gambiae and disruption of linkage between the centromeric "genomic islands" of inter-specific divergence. We report that A. gambiae from the Guinean hybrid zone possesses an introgressed TEP1 resistant allelic class, found exclusively in A. coluzzii elsewhere and apparently swept to fixation in West Africa (i.e. Mali and Burkina Faso). However, no detectable fixation of this allele was found in Guinea Bissau, which may suggest that ecological pressures driving segregation between the two species in larval habitats in this region may be different from those experienced in northern and more arid parts of the species’ range. Finally, our results also suggest a genetic subdivision between coastal and inland A. gambiae Guinean populations and provide clues on the importance of ecological factors in intra-specific differentiation processes.

Adding artesunate to sulphadoxine-pyrimethamine greatly improves the treatment efficacy in children with uncomplicated falciparum malaria on the coast of Benin, West Africa

BackgroundBenin has recently shifted its national antimalarial drug policy from monotherapies to combinations containing artemisinin derivatives. When this decision was taken, the available information on alternatives to chloroquine and sulphadoxine-pyrimethamine, the first- and second-line treatment, was sparse.MethodsIn 2003 – 2005, before the drug policy change, a randomized, open-label, clinical trial was carried out on the efficacy of chloroquine, and sulphadoxine-pyrimethamine alone or combined with artesunate, with the aim of providing policy makers with the information needed to formulate a new antimalarial drug policy. Children between six and 59 months of age, with uncomplicated malaria and living in the lagoon costal area in southern Benin, were randomly allocated to one of the three study arms and followed up for 28 days.ResultsTreatment failure (PCR corrected) was significantly lower in the artesunate + sulphadoxine-pyrimethamine group (4/77, 5.3%) than in chloroquine group(51/71, 71.8%) or the sulphadoxine-pyrimethamine alone group (30/70, 44.1%) (p < 0.001). Despite high sulphadoxine-pyrimethamine failure, its combination with artesunate greatly improved treatment efficacy.ConclusionIn Benin, artesunate + sulphadoxine-pyrimethamine is efficacious and could be used when the recommended artemisinin-based combinations (artemether-lumefantrine and amodiaquine-artesunate) are not available. However, because sulphadoxine-pyrimethamine is also used in pregnant women as intermittent preventive treatment, its combination with artesunate should not be widely employed in malaria patients as this may compromise the efficacy of intermittent preventive treatment.
