Human papillomavirus genotypes and factors associated with major cervical smear abnormalities in a sickle cell endemic area of Kisangani, Democratic Republic of the Congo

BackgroundCervical cancer due to high-risk human papillomavirus (HR-HPV) is the leading gynecological cancer in Sub-Saharan Africa and in the Democratic Republic of the Congo (DRC), where approximately one-quarter of people carries the sickle cell allele. This study aims to identify the HR-HPV genotypes and factors associated with major cervical abnormalities among women living in the sickle cell endemic area of Kisangani, DRC.MethodsThis hospital-based cross-sectional study consisted to a face-to-face and semi-structured questionnaire for collecting data of the socio-demographic, clinical and sexual behaviors characteristics of participants. The sickle cell status was confirmed using liquid chromatography coupled with mass spectrometry. Cervical smears were collected using the ThinPrep® Pap-test solution (Hologic Inc, Marlborough, USA). The HR-HPV molecular analysis was performed using the Cobas® 6800 system (Roche Molecular Systems, Somerville, USA) for real time polymerase chain reaction (RT-PCR) and the Oxford Nanopore platform (Oxford Nanopore Technologies, Oxford, England) for the viral genome sequencing. Cytological analyses were performed using the ThinPrep 5000 processor (Hologic), and the results were reported according to the 2014 Bethesda System classification. The logistic regression model was used to estimate the adjusted Odd ratio (aOR) during the risk factor assessments.ResultsAmong 712 women enrolled, the prevalence of HR-HPV was 28.4% (95% CI: 25.1–31.7). HPV35, HPV52 and HPV31 were the most common genotypes; while the sequencing identified two new variants of HPV103 and HPV223. The following factors were associated with HR-HPV infection: (i) age between 25 and 34 years (aOR: 2.10, 95% CI:1.22–3.69); (ii) being unmarried (aOR: 1.54, 95% CI: 1.05–2.27) and (iii) HIV-positive status (aOR: 14.85, 95% CI: 2.36–288.25). No correlation was observed between hemoglobin AS (HbAS) status and HR-HPV infection or cytological abnormalities. However, women aged

Humoral and cell-mediated immunity to MSP3 peptides in adults immunized with MSP3 in malaria endemic area, Burkina Faso

    I NEBIE, A DIARRA, A OUEDRAOGO, A B TIONO, A T KONATE, A GANSANE, I SOULAMA, S COUSENS, O LEROY, S B SIRIMA

We performed a single-blind, randomized phase 1 trial of the long synthetic peptide (LSP) of merozoite surface protein-3 (MSP3) in adults living in Burkina Faso. Thirty eligible volunteers were randomized to receive either the MSP3-LSP candidate vaccine or tetanus toxoid vaccine as a control. A dose of each vaccine was administered on days 0, 28 and 112 and the vaccine was formulated with aluminium hydroxide. Humoral immune responses were assessed by ELISA at days 0, 28, 56, 112, 140, 252 and 365 and cell-mediated immune responses by lymphoproliferation assay and by ELISA on days 0, 56 and 140. IgG responses to four peptides of MSP3 were similar in both vaccine groups. Higher IgG concentrations were recorded after the beginning of malaria high transmission season in both vaccine groups. The lymphocyte proliferation and the production of IFN-γ in response to stimulation with the four overlapping peptides increased following vaccination in the MSP3-LSP vaccine group, but did not change appreciably in the control group. In contrast to natural infection, MSP3-LSP did not boost humoral responses to the four overlapping peptides of MSP3 to any detectable degree in our semi-immune adult. MSP3-LSP may be more immunogenic in young children with little or no acquired immunity.

Humoral Response to the Anopheles gambiae Salivary Protein gSG6: A Serological Indicator of Exposure to Afrotropical Malaria Vectors

Salivary proteins injected by blood feeding arthropods into their hosts evoke a saliva-specific humoral response which can be useful to evaluate exposure to bites of disease vectors. However, saliva of hematophagous arthropods is a complex cocktail of bioactive factors and its use in immunoassays can be misleading because of potential cross-reactivity to other antigens. Toward the development of a serological marker of exposure to Afrotropical malaria vectors we expressed the Anopheles gambiae gSG6, a small anopheline-specific salivary protein, and we measured the anti-gSG6 IgG response in individuals from a malaria hyperendemic area of Burkina Faso, West Africa. The gSG6 protein was immunogenic and anti-gSG6 IgG levels and/or prevalence increased in exposed individuals during the malaria transmission/rainy season. Moreover, this response dropped during the intervening low transmission/dry season, suggesting it is sensitive enough to detect variation in vector density. Members of the Fulani ethnic group showed higher anti-gSG6 IgG response as compared to Mossi, a result consistent with the stronger immune reactivity reported in this group. Remarkably, anti-gSG6 IgG levels among responders were high in children and gradually declined with age. This unusual pattern, opposite to the one observed with Plasmodium antigens, is compatible with a progressive desensitization to mosquito saliva and may be linked to the continued exposure to bites of anopheline mosquitoes. Overall, the humoral anti-gSG6 IgG response appears a reliable serological indicator of exposure to bites of the main African malaria vectors (An. gambiae, Anopheles arabiensis and, possibly, Anopheles funestus) and it may be exploited for malaria epidemiological studies, development of risk maps and evaluation of anti-vector measures. In addition, the gSG6 protein may represent a powerful model system to get a deeper understanding of molecular and cellular mechanisms underlying the immune tolerance and prog

Abdennassar El Kharras, Mehdi Atmane, Jamal El Fenni, Souad Chaouir, Touria Amil

Mamadou Bata Dianka, Tarek El Hamdani, Ibrahima Kaba, Abdessamad Naja, Khadidia Ibahioin, Abdessamad El Azhari

Manal BOUIKHIF, Sana FARHANE, Mohamed LYAGOUBI, Sarra AOUFI

Lamyae Amro, Saloua El Fadili, Hind Serhane, Hafsa Sajiai, Salma Ait Batahar

Hicham Fenane, El mehdi Maidi, Mohamed Bouchikh, Damsane Lamboni, Abdellah Achir, Fahd Ouchen, Mbola Oyali, Mohamed Caidi, Said Al Aziz, Abdellatif Benosman

Nathalie Dinganga Kapessa, Manix Ilunga Banza, Jeff Ntalaja, Kibangula Kasanga Trésor, Christelle Ngoie Ngoie, Daniel Bokar Nyamezawa, Tshilombo Katombe François, Willy Arung Kalau

Sidi Salem-Memou, Sidiya Chavey, Hamdy Elmoustapha, Abdallahi Mamoune, Ahmedou Moctar, Sidimohamed Salihy, Najat Boukhrissi

Romuald Kouitcheu, Dominique N'Dri Oka, Guy Varlet

Hydropower development and malaria transmission: A geospatial econometric study

    Callum J. Thomas

BackgroundIn Western Africa, the causal relationship between hydropower project implementation and malaria transmission, remains understudied.AimThis study assesses whether a causal correlation exists between hydropower development and malaria transmission outcomes across locally affected communities, using malaria incidence and prevalence as key indicators. Malaria incidence is measured as the number of clinical Plasmodium falciparum cases per person, while prevalence is the parasite rate of P. falciparum in children aged 2–10 years. The analysis focuses on P. falciparum given its severity across West Africa, along with the availability of consistent geospatial data.SettingThe study was conducted in Ghana, Côte d’Ivoire, and Gabon.MethodsUtilising multivariate Difference-in-Differences (DiD) regression models and geospatial analysis across pre- and post-dam periods, this study evaluates malaria outcomes within 15 km of hydropower sites.ResultsThe DiD estimator (Treatment_Post variable) suggests no statistically significant increase in malaria transmission following hydropower project implementation. Estimated effects are insignificant in Côte d’Ivoire (incidence: p = 0.210, prevalence: p = 0.200), Gabon (incidence: p = 0.990, prevalence: p = 0.990), and Ghana (incidence: p = 0.089, prevalence: p = 0.102), indicating no strong causal link at the 5% level. By contrast, environmental and socio-economic variables such as urbanisation, elevation, and climate factors consistently showed strong associations with malaria transmission (p < 0.01).ConclusionHydropower presence alone is not a primary driver of malaria dynamics.ContributionThis study provides the first large-scale geospatial analysis of malaria trends across hydropower projects in Western Africa, challenging traditional assumptions of a direct causal link and highlighting the need for interventions shaped by environmental and socio-economic factors.

Hydroxyurea - Pragmatic Reduction In Mortality and Economic burden (H-PRIME): A 2x2x2 factorial randomised open-label trial investigating practical approaches to the treatment of sickle cell disease at four sites in Eastern Uganda

Peter Olupot-Olupot, Denis Amorut, Paul Ongodia, Charles B. Okalebo, Julian Abeso, Florence Aloroker, Sarah Asio, Amos Odiit, Charles Kiyaga, Rita Muhindo, Sophie Uyoga, George Mochamah, Asad Muyinda, Grace Abongo, Gideon Nyutu, Christabel Mogaka, Kathryn Maitland, Diana Gibb, Ann Sarah Walker, Roisin Connon, Elizabeth George, Russell E Ware, Thomas N. Williams

BackgroundSickle cell disease is the most common and severe genetic disease in humans worldwide. The majority of those affected are born in sub-Saharan Africa, where resources to diagnose and manage them are often limited. Fresh approaches to the pragmatic management of children with sickle cell disease in Africa that improve both morbidity and mortality are urgently needed.MethodsHydroxyurea - Pragmatic Reduction In Mortality and Economic burden (H-PRIME) is a 2 × 2 × 2 factorial randomized open-label trial that is investigating three separate interventions. Eighteen hundred children aged 1-10 years attending sickle cell clinics at one of four sites in Eastern Uganda are being randomized to: (1) hydroxyurea administered at a low dose (10 mg/kg/day) versus high dose (25 mg/kg/day), prescribed pragmatically through a weight-band-based approach and with clinically, rather than laboratory-guided monitoring; (2) enhanced malaria prophylaxis with weekly doses of dihydroartemisinin-piperaquine versus monthly doses of sulfadoxine-pyrimethamine (standard of care); and (3) enhanced antibacterial prophylaxis with daily cotrimoxazole throughout life versus twice daily penicillin until the age of 5 years (standard of care). All children will be followed up for 48 months after the date on which the first child was randomized. The primary endpoint for the hydroxyurea randomization will be mortality, for the antimalarial random will be malaria-associated hospital admission, and for the antimicrobial randomization will be all-cause hospital admission. Secondary outcomes will include the incidence of sickle-specific complications, receipt of blood transfusions, hemoglobin and fetal hemoglobin concentrations, and economic costs and benefits of the three interventions.ConclusionsThe first participant was recruited for the trial on January 16, 2024. In total, 1487 of the 1800 target children were recruited by February 17, 2025. H-PRIME will efficiently answer three important pragmatic

Hydroxyurea mobile directly observed therapy versus standard monitoring in patients with sickle cell anemia: a phase 2 randomized trial

Philip Sasi, Abel Makubi, Raphael Z. Sangeda, Mariam Y. Ngaeje, Bruno P. Mmbando, Joseph Soka, Caterina Rosano, Alex S. Magesa, Sharon E. Cox, Julie Makani, Enrico M. Novelli

BackgroundSickle cell anemia (SCA) prevalence remains high in sub-Saharan Africa. Long-term treatment with hydroxyurea (HU) increases survival, however, poor adherence to treatment could limit effectiveness. Whilst HU treatment adherence is currently high, this might decrease over time.MethodsWe conducted a single-center, randomized, open-label, parallel group phase 2 controlled clinical trial to determine whether mobile Directly Observed Therapy (m-DOT) increases HU treatment adherence (NCT02844673). Eligible participants were adults with homozygous SCA. People on a chronic blood transfusion program, with hemoglobin (Hb) A levels greater than 20% of the total Hb, total Hb less than 4 g/dL, pregnant or HIV positive were excluded. After a 3-month pre-treatment period participants were randomized to either m-DOT or standard monitoring arm. All participants received smart mobile phones and were treated with HU (15 mg/kg) daily for three months. In the m-DOT arm, drug intake was video recorded on cell phone by the participant and the video sent to the study team. The primary objective was to evaluate the effect of m-DOT on adherence to HU treatment by medication possession ratio (MPR).ResultsOf the 86 participants randomized, 76 completed the trial (26.13 ± 6.97 years, 63.5 % female). Adherence was high (MPR > 95 %) in both groups, 29 (80.6 %) in m-DOT versus 37 (94.9 %) in the standard monitoring arm (P = 0.079). No HU treatment was withheld from participants due to safety concerns.Conclusionsm-DOT did not increase adherence to HU treatment. We recommend that further testing in larger trials with a longer follow up period be undertaken. Plain language summarySickle cell anemia (SCA) is an inherited blood disorder in which there is an abnormal protein inside red blood cells. This results in red blood cells becoming sickle shaped and more easily destroyed in the body. Long-term treatment with hydroxyurea can reduce the frequency of illness and hospitalization. However, o

Hydroxyurea Therapy for Children With Sickle Cell Anemia in Sub‐Saharan Africa: Rationale and Design of the REACH Trial

Patrick T. McGann, Léon Tshilolo, Brigida Santos, George A. Tomlinson, Susan Stuber, Teresa Latham, Banu Aygun, Stephen K. Obaro, Peter Olupot‐Olupot, Thomas N. Williams, Isaac Odame, Russell E. Ware

BackgroundSickle cell anemia (SCA) is an inherited hematological disorder that causes a large but neglected global health burden, particularly in Africa. Hydroxyurea represents the only available disease‐modifying therapy for SCA, and has proven safety and efficacy in high‐resource countries. In sub‐Saharan Africa, there is minimal use of hydroxyurea, due to lack of data, absence of evidence‐based guidelines, and inexperience among healthcare providers.ProcedureA partnership was established between investigators in North America and sub‐Saharan Africa, to develop a prospective multicenter research protocol designed to provide data on the safety, feasibility, and benefits of hydroxyurea for children with SCA.ResultsThe Realizing Effectiveness Across Continents with Hydroxyurea (REACH, ClinicalTrials.gov NCT01966731) trial is a prospective, phase I/II open‐label dose escalation study of hydroxyurea that will treat a total of 600 children age 1–10 years with SCA: 150 at each of four different clinical sites within sub‐Saharan Africa (Angola, Democratic Republic of Congo, Kenya, and Uganda). The primary study endpoint will be severe hematological toxicities that occur during the fixed‐dose treatment phase. REACH has an adaptive statistical design that allows for careful assessment of toxicities to accurately identify a safe hydroxyurea dose.ConclusionsREACH will provide data that address critical gaps in knowledge for the treatment of SCA in sub‐Saharan Africa. By developing local expertise with the use of hydroxyurea and helping to establish treatment guidelines, the REACH trial results will have the potential to transform care for children with SCA in Africa. Hydroxyurea-Induced miRNA Expression in Sickle Cell Disease Patients in Africa

Khuthala Mnika, Gaston K. Mazandu, Mario Jonas, Gift D. Pule, Emile R. Chimusa, Neil A. Hanchard, Ambroise Wonkam

Hydroxyurea (HU) is clinically beneficial in sickle cell disease (SCD) through fetal hemoglobin (HbF) induction; however, the mechanism of HU is not yet fully elucidated. Selected miRNAs have been associated with HU-induced HbF production. We have investigated differential HU-induced global miRNA expression in peripheral blood of adult SCD patients in patients from Congo, living in South Africa. We found 22 of 798 miRNAs evaluated that were differentially expressed under HU treatment, with the majority (13/22) being functionally associated with HbF-regulatory genes, including BCL11A (miR-148b-3p, miR-32-5p, miR-340-5p, and miR-29c-3p), MYB (miR-105-5p), and KLF-3 (miR-106b-5), and SP1 (miR-29b-3p, miR-625-5p, miR-324-5p, miR-125a-5p, miR-99b-5p, miR-374b-5p, and miR-145-5p). The preliminary study provides potential additional miRNA candidates for therapeutic exploration.
