Insight into the genetic diversity of Mycobacterium bovis isolated from cattle in Ghana

ABSTRACTBovine tuberculosis (bTB), caused primarily by Mycobacterium bovis, poses significant risks to cattle and public health in Ghana. Previous studies reported varying prevalence (3%–21%) and M. bovis strain distributions, necessitating a deeper understanding of the molecular epidemiology of bTB to guide control strategies. This study investigated the genetic diversity and phylogenetic relationships of M. bovis isolates from cattle in Ghanaian abattoirs and compared results with strains from other African countries. Spoligotyping analysis of 28 M. bovis isolates confirmed 6 distinct spoligotypes, with SB0944 being the most prevalent (64.3%). Mycobacterial Interspersed Repetitive Units–Variable Number Tandem Repeats (MIRU-VNTR) genotyping analysis further resolved the isolates into 22 genotypes, with loci MIRU26, QUB26, and ETR-A showing the highest allelic diversity. Mixed infections were found in 10% of culture-confirmed cases, indicating a high risk of co-infection with multiple strains in the area. Comparative analysis revealed that Ghanaian isolates clustered within the African 1 (Af1) clonal complex, consistent with strains from Nigeria, Burkina Faso, and Cameroon, but distinct from the BCG-like and other strains that were dominant in Algeria and, to some extent, in Mali. Minimal strain homogenization of West-Central Africa with other regions suggests localized evolution, shaped by ecological barriers, regulated transhumance, and possible competitive exclusion among strains. These findings provide evidence for the need for region-specific bTB control measures that reflect Africa’s unique livestock dynamics and micro-epidemic patterns. Enhanced surveillance with advanced genotyping methods could improve outbreak tracking and inform targeted interventions to mitigate bTB’s impact on livestock in this region.IMPORTANCEBovine tuberculosis (bTB), caused primarily by Mycobacterium bovis, is a neglected disease in many low- and middle-income countries, including G

Insights into Novel Viral Threats in Sweetpotato from Burkina Faso: Characterisation of Unexplored Pathogens

Sweetpotato is a key staple crop in tropical and subtropical regions. Its vegetative propagation makes it a persistent reservoir, facilitating the emergence and spread of complex infections. Understanding its virome is crucial for disease management and food security. We investigated the sweetpotato virome in Burkina Faso using rolling circle amplification and Oxford Nanopore sequencing. Eight symptomatic leaf samples, previously undiagnosed using conventional methods, were analysed. Bioinformatic pipelines were employed followed by phylogenetic comparisons. Two viruses known to infect sweetpotato, namely sweet potato leaf curl virus (SPLCV) and sweet potato leaf curl deltasatellite 3 (SPLCD3), were consistently detected in all samples. Additionally, pepper yellow vein Mali virus (PepYVMV), cotton leaf curl Gezira alphasatellite (CLCuGeA) and cotton leaf curl Gezira betasatellite (CLCuGeB) were identified for the first time in this crop. Phylogenetic analysis confirmed their genetic proximity to isolates from tomato, okra and pepper. Their co-occurrence with SPLCV and SPLCD3 indicates a complex viral landscape that could influence disease severity. This study highlights the underestimated role of sweetpotato as a viral reservoir, influencing virus evolution and transmission. Further studies should assess their pathogenicity, co-infection dynamics and vector-mediated transmission to improve crop productivity.

Mouhamed Cherif Mboup, Khadidiatou Dia, Pape Diadie Fall

Gloria Akimana, Remy Onana, Siham Lkhooyali, Hassan Errihani

Yawovi Mawufemo Tsevi, Badomta Dolaama, Komlan Georges Tona, Anani Amen Tevi, Edem Cruz Affanou, Amah Daniel Amede, Jannat Haroune-Traore, Yoan Eyram Makafui Amekoudi, Akomola Kossi Sabi, Weu Mélanie Tia, Delphine Amélie Lagou

Insulin Growth Factor-I in Protein-Energy Malnutrition during Rehabilitation in Two Nutritional Rehabilitation Centres in Burkina Faso

    S. Kouanda, B. Doulougou, V. De Coninck, L. Habimana, B. Sondo, R. Tonglet, J. M. Ketelslegers, A. Robert

Objective. To investigate the relationship between IGF-I and the nutritional status of West-African children hospitalised for nutritional rehabilitation. Patients and methods. A cohort study was performed in two centres for nutritional rehabilitation and education (CREN) in Burkina Faso. Children were followed and the anthropometric data as well as the capillary blood samples were taken on the 7th and on the 14th days after their admission. IGF-I levels were determined from dried blood spots on filter paper on IGF-I RIA, after separation of the IGF-I from its binding proteins, using Sep-Pak chromatography. Results. A total of 59 children was included in the cohort. The IGF-I mean geometric values (SD) were 6.3 (1.4) μg/L on admission, 8.6 (1.8) μg/L at day 7 and 13.6 (2.0) μg/L at day 14. The differences between these values were statistically significant (P < .001). There is a significant correlation between the changes of IGF-I with the change of weight for height Z-score (P = .01). Conclusion. These results suggest that IGF-I can be considered as a potential marker to follow the nutritional status of children admitted in hospital for protein and energy malnutrition.

Faten Hadjkacem, Mahdi Kalthoum, Dorra Ghorbel, Mouna Ammar, Mouna Elleuch, Nadia Charfi, Mouna Mnif, Mohamed Abid

Nadia Anoun, Hamza Hasnaoui, Firdaous Ameziane Hassani, Mehdi Bourakkadi Idrissi, Abdelmalek Ousadden, Hanan El Ouahabi, Khalid Ait Taleb

Integrated monitoring and evaluation and environmental risk factors for urogenital schistosomiasis and active trachoma in Burkina Faso before preventative chemotherapy using sentinel sites

Artemis Koukounari, Seydou Touré, Christl A Donnelly, Amadou Ouedraogo, Bernadette Yoda, Cesaire Ky, Martin Kaboré, Elisa Bosqué-Oliva, María-Gloria Basáñez, Alan Fenwick, Joanne P Webster

BackgroundOver 1 billion of the world's poorest inhabitants are afflicted by neglected tropical diseases (NTDs). Integrated control programmes aimed at tackling these debilitating NTDs have been recently initiated, mainly using preventative chemotherapy. Monitoring and evaluation (M&E) of these integrated programs presents particular challenges over and above those required for single disease vertical programmes. We used baseline data from the National NTD Control Programme in Burkina Faso in order to assess the feasibility of an integrated survey design, as well as to elucidate the contribution of environmental variables to the risk of either Schistosoma haematobium, trachoma, or both among school-aged children.MethodsS. haematobium infection was diagnosed by detecting eggs in urine. A trachoma case was defined by the presence of Trachomatous inflammation-Follicular (TF) and/or Trachomatous inflammation-Intense (TI) in either eye. Baseline data collected from 3,324 children aged 7-11 years in 21 sentinel sites across 11 regions of Burkina Faso were analyzed using simple and multivariable hierarchical binomial logistic regression models fitted by Markov Chain Monte Carlo estimation methods. Probabilities of the risk of belonging to each infection/disease category were estimated as a function of age, gender (individual level), and environmental variables (at sentinel site level, interpolated from national meteorological stations).ResultsOverall prevalence at the sentinel sites was 11.79% (95% CI: 10.70-12.89) for S. haematobium; 13.30% (12.14-14.45) for trachoma and 0.84% (0.53-1.15) for co-infections. The only significant predictor of S. haematobium infection was altitude. There were significant negative associations between the prevalence of active trachoma signs and minimum temperature, and air pressure. Conditional upon these predictors, these data are consistent with the two pathogens being independent.ConclusionsUrogenital schistosomiasis and trachoma constitut

Integrated primary health care in low- and middle-income countries: a double challenge

    Thomas Druetz

BackgroundThe lack of primary healthcare integration has been identified as one of the main limits to programs’ efficacy in low- and middle-income countries. This is especially relevant to the Millennium Development Goals, whose health objectives were not attained in many countries at their term in 2015. While global health scholars and decision-makers are unanimous in calling for integration, the objective here is to go further and contribute to its promotion by presenting two of the most important challenges to be met for its achievement: 1) developing a “crosswise approach” to implementation that is operational and effective; and 2) creating synergy between national programs and interventions driven by non-State actors.Main bodyThe argument for urgently addressing this double challenge is illustrated by drawing on observations made and lessons learned during a four-year research project (2011–2014) evaluating the effects of interventions against malaria in Burkina Faso. The way interventions were framed was mostly vertical, leaving little room for local adaptation. In addition, many non-governmental organizations intervened and contributed to a fragmented and heteronomous health governance system. Important ethical issues stem from how interventions against malaria were shaped and implemented in Burkina Faso. To further explore this issue, a scoping literature review was conducted in August 2016 on the theme of integrated primary healthcare. It revealed that no clear definition of the concept has been advanced or endorsed thus far. We call for caution in conceptualizing it as a simple juxtaposition of different tasks or missions at the primary care level. It is time to go beyond the debate around selective versus comprehensive approaches or fragmentation versus cohesion. Integration should be thought of as a process to reconcile these tensions.ConclusionsIn the context that characterizes many low- and middle-income countries today, better aid coordination and pub

Integrating field surveys and remote sensing to optimize phosphorus resource management for rainfed rice production in the Central plateau of Burkina Faso

Rice production in sub-Saharan Africa (SSA) is restricted by low water availability, soil fertility, and fertilizer input, and phosphate rock (PR) application is expected to increase production. Soil water conditions and soil types affect the efficacy of phosphorus fertilization in improving productivity. However, these factors are rarely discussed together. In this study, we aimed to investigate the soil types and soil water conditions in the fields, as well as their effects on rice productivity after phosphorus fertilization, and optimize the findings using remote sensing techniques. A soil profiling survey, followed by a field experiment in seven farmer fields, was performed in the Central plateau of Burkina Faso. The following treatments were applied: nitrogen and potassium fertilization without phosphorus (NK), PR application with NK (NK+PR), and triple super phosphate (TSP) application with NK (NK+TSP). Submergence duration and cumulative water depth were recorded manually. The inundation score, estimated using a digital elevation model, explained the distribution of soil types and soil water conditions and correlated negatively with sand content and positively with silt and clay content, indicating an illuvial accumulation of fine soil particles with nutrient transportation. The field experiment showed that although grain yield was significantly restricted by phosphorus deficiency, the increase in yield after phosphorus fertilization was higher in Lixisols and Luvisols than in Cambisols because of the low Bray-2-phosphorus content of Lixisols and Luvisols. The inundation score correlated positively with grain yields after NK+PR and NK+TSP treatments. In conclusion, soils with low inundation scores (mainly Lixisols and Luvisols) showed a drastic increase in grain yield after TSP application, whereas those with high inundation scores showed comparable yields after PR and TSP application despite the low phosphorus fertilization effect. Our findings would help op

AbstractBackgroundAzithromycin mass drug administration (MDA) to ages 1–59 months can reduce childhood mortality; however, more evidence is needed to support targeting a narrower age range of 1–11 months. This trial assesses the efficacy of azithromycin MDA to 1–11-month-old children in reducing mortality in a real-world setting with integration of vitamin A delivery within the established Child Health Days platform in Burkina Faso.MethodsMortalite Infantile Reduite par l’Administration de Masse de l’Azithromycine is a double-masked, cluster-randomised, placebo-controlled trial in Child Health Days communities in Burkina Faso. Primary healthcare centre catchment areas (Centres de Santé et de Promotion Sociale (CSPS)) were randomised 2:1 to deliver biannual azithromycin or placebo for 1–11-month-old children. Birth history data at the study endpoint were used to calculate mortality rates and compare between groups.FindingsFrom September 2021 to January 2024, 201 709 children in 303 CSPS received azithromycin and 100 959 children in 158 CSPS received placebo, with an overall treatment coverage of 85%. Mortality rates were 2.6 (95% CI 2.1 to 3.1) deaths per 1000 person-years in the azithromycin arm and 2.5 (95% CI 1.8 to 3.2) per 1000 person-years in the placebo arm. There was no significant difference in the mortality rates by arm (incidence rate ratio: 1.04; 95% CI 0.75 to 1.46; p value 0.80). There were 16 non-serious adverse events and no serious adverse events recorded during the trial.InterpretationThis trial demonstrates that azithromycin MDA for child survival can be scaled up and integrated into existing child health programmes but was unable to demonstrate an effect of azithromycin distribution on infant mortality. These findings indicate future policy decisions should consider treatment delivery to the larger age group of children up to 5 years old.FundingThe trial was funded by the Bill and Melinda Gates Foundation (INV-005395).Trial registration numberNCT0

Integration of postpartum care into child health and immunization services in Burkina Faso: findings from a cross-sectional study

Danielle Yugbaré Belemsaga, Anne Goujon, Aristide Bado, Seni Kouanda, Els Duysburgh, Marleen Temmerman, Olivier Degomme

BackgroundThe Missed Opportunities for Maternal and Infant Health (MOMI) project, which aimed at upgrading maternal and infant postpartum care (PPC), implemented a package of interventions including the integration of maternal PPC in infant immunization services in 12 health facilities in Kaya Health district in Burkina Faso from 2013 to 2015. This paper assesses the coverage and the quality of combined mother-infant PPC in reproductive, maternal, newborn and child health services (RMNCH).MethodsWe conducted a mixed methods study with cross-sectional surveys before and after the intervention in the Kaya health and demographic surveillance system. On the quantitative side, two household surveys were performed in 2012 (N = 757) and in 2014 (N = 754) among mothers within one year postpartum. The analysis examines the result of the intervention by the date of delivery at three key time points in the PPC schedule: the first 48 h, days 6–10 and during weeks 6–8 and beyond. On the qualitative side, in depth interviews, focus group discussions and observations were conducted in four health facilities in 2012 and 2015. They involved mothers in the postpartum period, facility and community health workers, and other stakeholders. We performed a descriptive analysis and a two-sample test of proportions of the quantitative data. The qualitative data were recorded, transcribed and analysed along the themes relevant for the intervention.ResultsThe findings show that the WHO guidelines, in terms of content and improvement of maternal PPC, were followed for physical examinations and consultations. They also show a significant increase in the coverage of maternal PPC services from 50% (372/752) before the intervention to 81% (544/672) one year after the start of the intervention. However, more women were assessed at days 6–10 than at later visits. Integration of maternal PPC was low, with little improvements in history taking and physical examination of mothers in immunization servic

The mechanisms behind the ability of Plasmodium falciparum to evade host immune system are poorly understood and are a major roadblock in achieving malaria elimination. Here, we use integrative genomic profiling and a longitudinal pediatric cohort in Burkina Faso to demonstrate the role of post-transcriptional regulation in host immune response in malaria. We report a strong signature of miRNA expression differentiation associated with P. falciparum infection (127 out of 320 miRNAs, B-H FDR 5%) and parasitemia (72 miRNAs, B-H FDR 5%). Integrative miRNA-mRNA analysis implicates several infection-responsive miRNAs (e.g., miR-16-5p, miR-15a-5p and miR-181c-5p) promoting lymphocyte cell death. miRNA cis-eQTL analysis using whole-genome sequencing data identified 1,376 genetic variants associated with the expression of 34 miRNAs (B-H FDR 5%). We report a protective effect of rs114136945 minor allele on parasitemia mediated through miR-598-3p expression. These results highlight the impact of post-transcriptional regulation, immune cell death processes and host genetic regulatory control in malaria. Here, the authors identify signatures of miRNA expression differentiation associated with Plasmodium falciparum infection and parasitemia in a longitudinal pediatric cohort in Burkina Faso. In particular, expression of several miRNAs known to promote lymphocyte cell death is affected during infection.
