Ouo-Ouo LOUA, Issa KONATÉ, Yacouba CISSOKO, Mamadou Saliou SOW

Ines Trabelsi, Mouadh Ben Ali, Manel Ben Romdhane, Hanen Smaoui, Imen Bel Hadj, Khadija Boussetta

Mostafa Rafai, Naoufal Chouaib, Saad Zidouh, Hicham Bakkali, Lahcen Belyamani

Khalid Mouhadi, Tarik Boulahri, Abdelhadi Rouimi

    Mehdi Hasnaoui, Rihab Guizani

The incidence of sphenoid sinus meningoencephalocele is very low. It is estimated one person out of 700 000 live births. This clinical entity is rarer in adults. We here report the case of a 51-year old woman presenting with feeling of intranasal heaviness that had progressed since the young age without rhinorrhea or nasal obstruction. She had no dysosmia or headache. She did not have a history of recurrent meningitis, skull base surgery or head trauma. Both nasal fossae were free. Computed tomography (CT) scan of the facial skeleton showed subtotal filling of the right sphenoid sinus, with spontaneously hyperdense content at places, without significant contrast enhancement associated with lysis of the lateral wall of the right sphenoid sinus. Brain and facial skeleton magnetic resonance imaging (MRI) showed dehiscence of the lateral wall of the right sphenoid sinus with herniation of the cerebrospinal fluid presenting hyposinal on T1, hypersignal on T2, cerebral parenchyma in T1 and T2 isosignal. We opted for therapeutic abstention because the patient was asymptomatic.

Méningo-vascularite bactérienne révélant un adénome hypophysaire

Siham Bouchal, Hanane Razzouki, Salma Ibn Elkhyat, Mustafa Maaroufi, Ouarda El Ouali, Ouafae Messouak, Mohammed Faouzi Belahsen

Patrick Rakotozanany, Miraulle Tien Yu Song, Njara Francia Ranoasy, Ketsia Rakotovao, Willy Ratovondrainy

Meningococcal A conjugate vaccine coverage in the meningitis belt of Africa from 2010 to 2021: a modelling study

Rose G. Bender, Jasmine Shen, Aleksandr Aravkin, André Arsène Bita Fouda, Ado M. Bwaka, Natalie C. Galles, Emily Haeuser, Simon I. Hay, Anderson Latt, Jason M. Mwenda, Emma L.B. Rogowski, Alyssa N. Sbarra, Reed J.D. Sorensen, Avina Vongpradith, Claire Wright, Peng Zheng, Jonathan F. Mosser, Hmwe H. Kyu

SummaryBackgroundAs of the end of 2021, twenty-four countries in the African meningitis belt have rolled out mass campaigns of MenAfriVac®, a meningococcal A conjugate vaccine (MACV) first introduced in 2010. Twelve have completed introduction of MACV into routine immunisation (RI) schedules. Although select post-campaign coverage data are published, no study currently comprehensively estimates MACV coverage from both routine and campaign sources in the meningitis belt across age, country, and time.MethodsIn this modelling study, we assembled campaign data from the twenty-four countries that had introduced any immunisation activity during or before the year 2021 (Benin, Burkina Faso, Burundi, Cameroon, Central African Republic, Chad, Côte d’Ivoire, Democratic Republic of the Congo, Ethiopia, Eritrea, the Gambia, Ghana, Guinea, Guinea Bissau, Kenya, Mali, Mauritania, Niger, Nigeria, Senegal, South Sudan, Sudan, Togo and Uganda) via WHO reports and RI data via systematic review. Next, we modelled RI coverage using Spatiotemporal Gaussian Process Regression. Then, we synthesized these estimates with campaign data into a cohort model, tracking coverage for each age cohort from age 1 to 29 years over time for each country.FindingsCoverage in high-risk locations amongst children aged 1–4 in 2021 was estimated to be highest in Togo with 96.0% (95% uncertainty interval [UI] 92.0–99.0), followed by Niger with 87.2% (95% UI 85.3–89.0) and Burkina Faso, with 86.4% (95% UI 85.1–87.6). These countries had high coverage values driven by an initial successful mass immunisation campaign, followed by a catch-up campaign, followed by introduction of RI. Due to the influence of older mass vaccination campaigns, coverage proportions skewed higher in the 1–29 age group than the 1–4 group, with a median coverage of 82.9% in 2021 in the broader age group compared to 45.6% in the narrower age group.InterpretationThese estimates highlight where gaps in immunisation remain and emphasise the

Meningococcal carriage 7 years after introduction of a serogroup A meningococcal conjugate vaccine in Burkina Faso: results from four cross-sectional carriage surveys

Sarah Mbaeyi, Emmanuel Sampo, Kambiré Dinanibè, Issaka Yaméogo, Malika Congo-Ouédraogo, Mamadou Tamboura, Guetawendé Sawadogo, Kalifa Ouattara, Mahamadou Sanou, Tanga Kiemtoré, Gerard Dioma, Barnabé Sanon, Hermann Somlaré, Augustin Kyetega, Absatou Ky Ba, Flavien Aké, Félix Tarbangdo, Frederic Acho Aboua, Yvette Donnou, Idrissa Kamaté, Jaymin C Patel, Susanna Schmink, Michael W Spiller, Nadav Topaz, Ryan Novak, Xin Wang, Brice Bicaba, Lassana Sangaré, Rasmata Ouédraogo-Traoré, Paul A Kristiansen

SummaryBackgroundIn the first 2 years after a nationwide mass vaccination campaign of 1–29-year-olds with a meningococcal serogroup A conjugate vaccine (MenAfriVac) in Burkina Faso, carriage and disease due to serogroup A Neisseria meningitidis were nearly eliminated. We aimed to assess the long-term effect of MenAfriVac vaccination on meningococcal carriage and herd immunity.MethodsWe did four cross-sectional studies of meningococcal carriage in people aged 9 months to 36 years in two districts of Burkina Faso between May 2, 2016, and Nov 6, 2017. Demographic information and oropharyngeal swabs were collected. Meningococcal isolates were characterised using whole-genome sequencing.FindingsOf 14 295 eligible people, 13 758 consented and had specimens collected and laboratory results available, 1035 of whom were meningococcal carriers. Accounting for the complex survey design, prevalence of meningococcal carriage was 7·60% (95% CI 5·67–9·52), including 6·98% (4·86–9·11) non-groupable, 0·48% (0·01–0·95) serogroup W, 0·10% (0·01–0·18) serogroup C, 0·03% (0·00–0·80) serogroup E, and 0% serogroup A. Prevalence ranged from 5·44% (95% CI 4·18–6·69) to 9·14% (6·01–12·27) by district, from 4·67% (2·71–6·64) to 11·17% (6·75–15·59) by round, and from 3·39% (0·00–8·30) to 10·43% (8·08–12·79) by age group. By clonal complex, 822 (88%) of 934 non-groupable isolates were CC192, all 83 (100%) serogroup W isolates were CC11, and nine (69%) of 13 serogroup C isolates were CC10217.InterpretationOur results show the continued effect of MenAfriVac on serogroup A meningococcal carriage, for at least 7 years, among vaccinated and unvaccinated cohorts. Carriage prevalence of epidemic-prone serogroup C CC10217 and serogroup W CC11 was low. Continued monitoring of N meningitidis carriage will be crucial to further assess the effect of MenAfriVac and inform the vaccination strategy for future multivalent meningococcal vaccines.FundingBill & Melinda Gates Foundation and Gavi, the Vaccine Allia

Meningococcal carriage in the African meningitis belt

A meningococcal serogroup A polysaccharide/tetanus toxoid conjugate vaccine (PsA-TT) (MenAfriVac#x2122;) is being deployed in countries of the African meningitis belt. Experience with other polysaccharide/protein conjugate vaccines has shown that an important part of their success has been their ability to prevent the acquisition of pharyngeal carriage and hence to stop transmission and induce herd immunity. If PsA-TT is to achieve the goal of preventing epidemics, it must be able to prevent the acquisition of pharyngeal carriage as well as invasive meningococcal disease and whether PsA-TT can prevent pharyngeal carriage needs to be determined. To address this issue, a consortium (the African Meningococcal Carriage (MenAfriCar) consortium) was established in 2009 to investigate the pattern of meningococcal carriage in countries of the African meningitis belt prior to and after the introduction of PsA-TT. This article describes how the consortium was established, its objectives and the standardised field and laboratory methods that were used to achieve these objectives. The experience of the MenAfriCar consortium will help in planning future studies on the epidemiology of meningococcal carriage in countries of the African meningitis belt and elsewhere.

Meningococcal carriage within households in the African meningitis belt: A longitudinal pilot study

Nicole E. Basta, Abdoulaye Berthe, Mahamadou Keita, Uma Onwuchekwa, Boubou Tamboura, Awa Traore, Musa Hassan-King, Olivier Manigart, Maria Nascimento, James M. Stuart, Caroline Trotter, Jayne Blake, Anthony D. Carr, Stephen J. Gray, Lynne S. Newbold, Yangqing Deng, Julian Wolfson, M. Elizabeth Halloran, Brian Greenwood, Ray Borrow, Samba O. Sow

Highlights•We demonstrated the feasibility of conducting a longitudinal, household-based study of meningococcal carriage in the African meningitis belt.•During the cross-sectional screening visit, the carriage prevalence was 5% (20 carriers among 400 participants from 116 households).•Over 17 months, 1422 swabs were obtained from 202 people in 20 households. 73 carrier isolates identified; 33 people (16.3%) carried at least once.•Among all swabs collected, 84% of isolates were non-groupable, though 6 W and 9 Y isolates were identified. No A, C, or X carriers were found.•The meningococcal carriage duration, any serogroup, was 2.9 months (95% CI: 1.6, 5.4). The acquisition rate was 2.3% per month (95% CI: 1.3, 3.8). SummaryObjectivesCarriers of Neisseria meningitidis are a key source of transmission. In the African meningitis belt, where risk of meningococcal disease is highest, a greater understanding of meningococcal carriage dynamics is needed.MethodsWe randomly selected an age-stratified sample of 400 residents from 116 households in Bamako, Mali, and collected pharyngeal swabs in May 2010. A month later, we enrolled all 202 residents of 20 of these households (6 with known carriers) and collected swabs monthly for 6 months prior to MenAfriVac vaccine introduction and returned 10 months later to collect swabs monthly for 3 months. We used standard bacteriological methods to identify N. meningitidis carriers and fit hidden Markov models to assess acquisition and clearance overall and by sex and age.ResultsDuring the cross-sectional study 5.0% of individuals (20/400) were carriers. During the longitudinal study, 73 carriage events were identified from 1422 swabs analyzed, and 16.3% of individuals (33/202) were identified as carriers at least once. The majority of isolates were non-groupable; no serogroup A carriers were identified.ConclusionsOur results suggest that the duration of carriage with any N. meningitidis averages 2.9 months and that males and children acq

Meningococcal Meningitis Outbreaks in the African Meningitis Belt After Meningococcal Serogroup A Conjugate Vaccine Introduction, 2011–2017

Katya Fernandez, Clément Lingani, Olaolu Moses Aderinola, Kadadé Goumbi, Brice Bicaba, Zewdu Assefa Edea, Clément Glèlè, Badu Sarkodie, Agbeko Tamekloe, Armelle Ngomba, Mamoudou Djingarey, Ado Bwaka, William Perea, Olivier Ronveaux

AbstractBackgroundIn 2010–2017, meningococcal serogroup A conjugate vaccine (MACV) was introduced in 21 African meningitis belt countries. Neisseria meningitidis A epidemics have been eliminated here; however, non-A serogroup epidemics continue.MethodsWe reviewed epidemiological and laboratory World Health Organization data after MACV introduction in 20 countries. Information from the International Coordinating Group documented reactive vaccination.ResultsIn 2011–2017, 17 outbreaks were reported (31 786 suspected cases from 8 countries, 1–6 outbreaks/year). Outbreaks were of 18–14 542 cases in 113 districts (median 3 districts/outbreak). The most affected countries were Nigeria (17 375 cases) and Niger (9343 cases). Cumulative average attack rates per outbreak were 37–203 cases/100 000 population (median 112). Serogroup C accounted for 11 outbreaks and W for 6. The median proportion of laboratory confirmed cases was 20%. Reactive vaccination was conducted during 14 outbreaks (5.7 million people vaccinated, median response time 36 days).ConclusionOutbreaks due to non-A serogroup meningococci continue to be a significant burden in this region. Until an affordable multivalent conjugate vaccine becomes available, the need for timely reactive vaccination and an emergency vaccine stockpile remains high. Countries must continue to strengthen detection, confirmation, and timeliness of outbreak control measures.

Meningococcal Meningitis Surveillance in the African Meningitis Belt, 2004–2013

Clément Lingani, Cassi Bergeron-Caron, James M. Stuart, Katya Fernandez, Mamoudou H. Djingarey, Olivier Ronveaux, Johannes C. Schnitzler, William A. Perea

Background. An enhanced meningitis surveillance network was established across the meningitis belt of sub-Saharan Africa in 2003 to rapidly collect, disseminate, and use district weekly data on meningitis incidence. Following 10 years’ experience with enhanced surveillance that included the introduction of a group A meningococcal conjugate vaccine, PsA-TT (MenAfriVac), in 2010, we analyzed the data on meningitis incidence and case fatality from countries reporting to the network.Methods. After de-duplication and reconciliation, data were extracted from the surveillance bulletins and the central database held by the World Health Organization Inter-country Support Team in Burkina Faso for countries reporting consistently from 2004 through 2013 (Benin, Burkina Faso, Chad, Democratic Republic of Congo, Ghana, Côte d'Ivoire, Mali, Niger, Nigeria, Togo).Results. The 10 study countries reported 341 562 suspected and confirmed cases over the 10-year study period, with a marked peak in 2009 due to a large epidemic of group A Neisseria meningitidis (NmA) meningitis. Case fatality was lowest (5.9%) during this year. A mean of 71 and 67 districts annually crossed the alert and epidemic thresholds, respectively. The incidence rate of NmA meningitis fell >10-fold, from 0.27 per 100 000 in 2004–2010 to 0.02 per 100 000 in 2011–2013 (P < .0001).Conclusions. In addition to supporting timely outbreak response, the enhanced meningitis surveillance system provides a global overview of the epidemiology of meningitis in the region, despite limitations in data quality and completeness. This study confirms a dramatic fall in NmA incidence after the introduction of PsA-TT.

Meningococcal Seroepidemiology 1 Year After the PsA-TT Mass Immunization Campaign in Burkina Faso

Haoua Tall, Seydou Yaro, Hervé B. N. Kpoda, Soumeya Ouangraoua, Caroline L. Trotter, Berthe-Marie Njanpop Lafourcade, Helen Findlow, Xilian Bai, Catherine Martin, Ikenna Nwakamma, Jean Bosco Ouedraogo, Bradford D. Gessner, Ray Borrow, Judith E. Mueller

Background. A group A meningococcal (MenA) conjugate vaccine, PsA-TT (MenAfriVac), was introduced in Burkina Faso via mass campaigns between September and December 2010, targeting the 1- to 29-year-old population. This study describes specific antibody titers in the general population 11 months later and compares them to preintroduction data obtained during 2008 using the same protocol.Methods. During October–November 2011, we recruited a representative sample of the population of urban Bobo-Dioulasso aged 6 months to 29 years, who underwent standardized interviews and blood draws. We assessed anti-MenA immunoglobulin G (IgG) concentrations (n = 200) and, using rabbit complement, serum bactericidal antibody (SBA) titers against 2 group A strains: reference strain F8238 (SBAref) (n = 562) and strain 3125 (SBA3125) (n = 200).Results. Among the 562 participants, 481 (86%) were aged ≥23 months and had been eligible for the PsA-TT campaign. Among them, vaccine coverage was 86.3% (95% confidence interval [CI], 82.7%–89.9%). Prevalence of putatively protective antibodies among vaccine-eligible age groups was 97.3% (95% CI, 95.9%–98.7%) for SBAref titers ≥128, 83.6% (95% CI, 77.6%–89.7%) for SBA3125 ≥128, and 84.2% (95% CI, 78.7%–89.7%) for anti-MenA IgG ≥2 µg/mL. Compared to the population aged 23 months to 29 years during 2008, geometric mean titers of SBAref were 7.59-fold higher during 2011, 51.88-fold for SBA3125, and 10.56-fold for IgG.Conclusions. This study shows high seroprevalence against group A meningococci in Burkina Faso following MenAfriVac introduction. Follow-up surveys will provide evidence on the persistence of population-level immunity and the optimal vaccination strategy for long-term control of MenA meningitis in the African meningitis belt.

Mental health and urban living in sub-Saharan Africa: major depressive episodes among the urban poor in Ouagadougou, Burkina Faso

Géraldine Duthé, Clémentine Rossier, Doris Bonnet, Abdramane Bassiahi Soura, Jamaica Corker

BackgroundIn sub-Saharan African cities, the epidemiological transition has shifted a greater proportion of the burden of non-communicable diseases, including mental and behavioral disorder, to the adult population. The burden of major depressive disorder and its social risk factors in the urban sub-Saharan African population are not well understood and estimates vary widely. We conducted a study in Ouagadougou, Burkina Faso, in order to estimate the prevalence of major depressive episodes among adults in this urban setting.MethodsThe Ouagadougou Health and Demographic System Site (HDSS) has followed the inhabitants of five outlying neighborhoods of the city since 2008. In 2010, a representative sample of 2,187 adults (aged 15 and over) from the Ouaga HDSS was interviewed in depth regarding their physical and mental health. Using criteria from the Mini International Neuropsychiatric Interview (MINI), we identified the prevalence of a major depressive episode at the time of the interview among respondents and analyzed its association with demographic, socioeconomic, and health characteristics through a multivariate analysis.ResultsMajor depressive episode prevalence was 4.3 % (95 % CI: 3.1–5.5 %) among the survey respondents. We found a strong association between major depressive episode and reported chronic health problems, functional limitations, ethnicity and religion, household food shortages, having been recently a victim of physical violence and regularly drinking alcohol. Results show a U-shaped association of the relationship between major depressive episode and standard of living, with individuals in both the poorest and richest groups most likely to suffer from major depressive disorder than those in the middle. Though, the poorest group remains the most vulnerable one, even when controlling by health characteristics.ConclusionsMajor depressive disorder is a reality for many urban residents in Burkina Faso and likely urbanites throughout sub-Saharan Africa.

Imane Ouafki, Lamyae Nouiakh, Raihana Boujarnija, Afaf Amarti, Lamiae Amaadour, Karima Oualla, Zineb Benbrahim, Samia Arifi, Nawfel Mellas

    Asmaa Sqalli Houssaini, Badreddine Hassam

Image en médecineDermocorticoids (DC) are the first line treatment for mild to moderate psoriasis. Despite their relative safety, long-term use and most of all self-medication practice could expose them to a risk of skin atrophy. We report the case of a 50-year old female patient, treated for psoriasis with chronic application of topical dermocorticoids over several years. Her skin appeared to be very thin, displaying the underlying venous network, telangiectasias as well as stretch marks which are manifestations of dermo-epidermal atrophy. Skin atrophy is the most common adverse effect of dermocorticoids and it is due to their strong antiproliferative effect on keratinocytes, melanocytes and fibroblasts which synthesize the collagen. It may be associated with delayed wound healing and various infectious complications; hence the importance of providing proper patient education.

Djibo Douma Maiga, Houdou Seyni, Ali Ousmane Moussa, Amadou Sidikou

Naoual Nchinech, Afaf Elgharbi, Fatima Zahra Aglili, Yamna Kriouile, Yahia Cherrah, Asmaa Aaloui Mdaghri, Samira Serragui

Meta-analysis of heritability estimates and genome-wide association for tick-borne haemoparasites in African cattle

Valentina Riggio, Maxime Madder, Michel Labuschagne, Rebecca Callaby, Rongrong Zhao, Appolinaire Djikeng, Josephus Fourie, James G. D. Prendergast, Liam J. Morrison

The control of tick-borne haemoparasites in cattle largely relies on the use of acaricide drugs against the tick vectors, with some vaccination also being used against selected pathogens. These interventions can be difficult in Africa, where accessibility and cost of vaccines can be issues, and the increasing resistance of tick vectors to the widely used acaricides is a complication to disease control. A potential complementary control strategy could be the exploitation of any natural host genetic resistance to the pathogens. However, there are currently very few estimates of the extent of host resistance to tick-borne haemoparasites, and a significant contributing factor to this knowledge gap is likely to be the difficulty of collecting appropriate samples and data in the smallholder systems that predominate livestock production in low- and middle-income countries, particularly at scale. In this study, we have estimated the heritability for the presence/absence of several important haemoparasite species (including Anaplasma marginale, Babesia bigemina, Babesia bovis, and Ehrlichia ruminantium), as well as for relevant traits such as body weight and body condition score (BCS), in 1,694 cattle from four African countries (Burkina Faso, Ghana, Nigeria, and Tanzania). Heritability estimates within countries were mostly not significant, ranging from 0.05 to 0.84 across traits and countries, with standard errors between 0.07 and 0.91. However, the weighted mean of heritability estimates was moderate and significant for body weight and BCS (0.40 and 0.49, respectively), with significant heritabilities also observed for the presence of A. marginale (0.16) and E. ruminantium (0.19). In a meta-analysis of genome-wide association studies (GWAS) for these traits, two peaks were identified as reaching the suggestive significance threshold (p < 1.91 × 10−7 and p < 1.89 × 10−7, respectively): one on chromosome 24 for BCS and one on chromosome 8 for the E. ruminantium infection st
