Methods for Identifying Neisseria meningitidis Carriers: A Multi-Center Study in the African Meningitis Belt

ObjectiveDetection of meningococcal carriers is key to understanding the epidemiology of Neisseria meningitidis, yet no gold standard has been established. Here, we directly compare two methods for collecting pharyngeal swabs to identify meningococcal carriers.MethodsWe conducted cross-sectional surveys of schoolchildren at multiple sites in Africa to compare swabbing the posterior pharynx behind the uvula (U) to swabbing the posterior pharynx behind the uvula plus one tonsil (T). Swabs were cultured immediately and analyzed using molecular methods.ResultsOne thousand and six paired swab samples collected from schoolchildren in four countries were analyzed. Prevalence of meningococcal carriage was 6.9% (95% CI: 5.4-8.6%) based on the results from both swabs, but the observed prevalence was lower based on one swab type alone. Prevalence based on the T swab or the U swab alone was similar (5.2% (95% CI: 3.8-6.7%) versus 4.9% (95% CI: 3.6-6.4%) respectively (p=0.6)). The concordance between the two methods was 96.3% and the kappa was 0.61 (95% CI: 0.50-0.73), indicating good agreement.ConclusionsThese two commonly used methods for collecting pharyngeal swabs provide consistent estimates of the prevalence of carriage, but both methods misclassified carriers to some degree, leading to underestimates of the prevalence.

Méthotrexate et psoriasis: à propos de 46 cas

Kawtar Inani, Mariame Meziane, Fatimazahra Mernissi

Methylene blue for malaria in Africa: results from a dose-finding study in combination with chloroquine

Peter E Meissner, Germain Mandi, Boubacar Coulibaly, Steffen Witte, Théophile Tapsoba, Ulrich Mansmann, Jens Rengelshausen, Wolfgang Schiek, Albrecht Jahn, Ingeborg Walter-Sack, Gerd Mikus, Jürgen Burhenne, Klaus-Dieter Riedel, R Heiner Schirmer, Bocar Kouyaté, Olaf Müller

The development of safe, effective and affordable drug combinations against malaria in Africa is a public health priority. Methylene blue (MB) has a similar mode of action as chloroquine (CQ) and has moreover been shown to selectively inhibit the Plasmodium falciparum glutathione reductase. In 2004, an uncontrolled dose-finding study on the combination MB-CQ was performed in 435 young children with uncomplicated falciparum malaria in Burkina Faso (CQ monotherapy had a > 50% clinical failure rate in this area in 2003). Three serious adverse events (SAE) occurred of which one was probably attributable to the study medication. In the per protocol safety analysis, there were no dose specific effects. The overall clinical and parasitological failure rates by day 14 were 10% [95% CI (7.5%, 14.0%)] and 24% [95% CI (19.4%, 28.3%)], respectively. MB appears to have efficacy against malaria, but the combination of CQ-MB is clearly not effective in the treatment of malaria in Africa.

mHealth-Enabled Stroke Screening for Pediatric Sickle Cell Disease in Low-Resource Settings: Systematic Literature Review of Critical Barriers, Emerging Technologies, and AI-Driven Solutions

AbstractBackgroundSickle cell disease (SCD) is a genetic blood disorder affecting millions globally, with life-threatening complications, and most patients live in sub-Saharan Africa. Particularly, children with SCD have a high risk of stroke. Although early screening for stroke could help prevent many cases, access to effective stroke screening remains limited in low-resource settings (LRS). Existing traditional approaches are highly operator-dependent, costly, resource-intensive, or difficult to deploy at scale in pediatric care. These limitations highlight the urgent need for accessible, scalable, and child-appropriate stroke screening and assessment tools suitable for low-resource health care contexts.ObjectiveThe aims of this systematic literature review are to (1) uncover system-level barriers affecting stroke screening accessibility for patients with pediatric sickle cell disease (PSCD) in LRS, including underserved contexts within high-income countries; (2) identify existing and emerging stroke screening and assessment technologies and their implementation characteristics, such as feasibility, scalability, portability, and training requirements; and (3) propose a user-centered mobile health (mHealth) framework for stroke screening that improves accessibility and feasibility in resource-constrained health care settings.MethodsPRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines were followed to organize the search process. A systematic search was conducted using an advanced query and defined eligibility criteria in the academic databases of PubMed, IEEE Xplore, Wiley Online Library, and Google Scholar. Studies published in English between January 1, 2021, and October 31, 2025, were selected. Collected data were arranged in a preformatted Microsoft Excel spreadsheet for analysis. Risk-of-bias assessment was performed using various risk-of-bias assessment tools because of the heterogeneity of the included studies. Narrative syn

Ines Barka, Emna Dendana, Nesrine Chikhrouhou, Amel Maroufi, Maha Kacem, Molka Chadli, Koussay Ach

Micro-epidemiology of Plasmodium falciparum malaria: Is there any difference in transmission risk between neighbouring villages?

Yazoumé Yé, Catherine Kyobutungi, Valérie R Louis, Rainer Sauerborn

BackgroundMalaria control strategies are designed as a solution for either the whole region or the whole country and are assumed to suit every setting. There is a need to shift from this assumption because transmission may be different from one local setting to another. The aim of this study was to assess the risk of clinical malaria given the village of residence among under-five children in rural north-western Burkina Faso.Methods867 children (6–59 months) were randomly selected from four sites. Interviewers visited the children weekly at home over a one-year period and tested them for fever. Children with fever were tested for malaria parasites. An episode of clinical malaria was defined as fever (axillary temperature ≥ 37.5°C) + parasites density ≥ 5,000 parasites/μl. Logistic regression was used to assess the risk of clinical malaria among children at a given site of residence.ResultsChildren accumulated 758 person years (PYs). Overall, 597 episodes of clinical malaria were observed, giving an incidence rate of 0.79 per PY. The risk of clinical malaria varied amongst the four sites. Taking one village as reference the odds ratio for the other three sites ranged from 0.66; 95%CI: 0.44–0.98 to 1.49; 95%CI: 1.10–2.01.ConclusionMalaria control strategies should be designed to fit the local context. The heterogeneity of transmission should be assessed at the district level to allow cost-effective resource allocation that gives priority to locations with high risk. Functional routine health information systems could provide the necessary data for context specific risk assessment.

Micro-genetic environmental sensitivity across macro-environments of chickens reared in Burkina Faso and France

Mette Dam Madsen, Naomi Duijvesteijn, Julius van der Werf, Sam Clark

BackgroundCommercial poultry production systems follow a pyramidal structure with a nucleus of purebred animals under controlled conditions at the top and crossbred animals under commercial production conditions at the bottom. Genetic correlations between the same phenotypes on nucleus and production animals can therefore be influenced by differences both in purebred-crossbred genotypes and in genotype-by-environment interactions across the two environments, known as macro-genetic environmental sensitivity (GES). Within each environment, genotype-by-environment interactions can also occur due to so-called micro-GES. Micro-GES causes heritable variation in phenotypes and decreases uniformity. In this study, genetic variances of body weight (BW) and of micro-GES of BW and the impacts of purebred-crossbred differences and macro-environmental differences on micro-GES of BW were estimated. The dataset contained three subpopulations of slow-growing broiler chickens: purebred chickens (PB) reared in France, and crossbred chickens reared in France (FR) under the same conditions as PB or reared in Burkina Faso (BF) under local conditions. The crossbred chickens were offspring of the same dam line and had PB as their sire line.ResultsEstimates of heritability of BW and micro-GES of BW were 0.54 (SE of 0.02) and 0.06 (0.01), 0.67 (0.03) and 0.03 (0.01), and 0.68 (0.04) and 0.02 (0.01) for the BF, FR, and PB subpopulations, respectively. Estimates of the genetic correlations for BW between the three subpopulations were moderately positive (0.37 to 0.53) and those for micro-GES were weakly to moderately positive (0.01 to 0.44).ConclusionsThe results show that the heritability of the micro-GES of BW varies with macro-environment, which indicates that responses to selection are expected to differ between macro-environments. The weak to moderate positive genetic correlations between subpopulations indicate that both macro-environmental differences and purebred-crossbred differences

Microbial gut evaluation in an angolan paediatric population with sickle cell disease

Mariana Delgadinho, Catarina Ginete, Brígida Santos, Joana Mendes, Armandina Miranda, Jocelyne Vasconcelos, Miguel Brito

AbstractSickle cell disease (SCD) is one of the most common genetic conditions worldwide. It can contribute up to 90% of under‐5 mortality in sub‐Saharan Africa. Clinical manifestations are very heterogeneous, and the intestinal microbiome appears to be crucial in the modulation of inflammation, cell adhesion and induction of aged neutrophils, the main interveners of recurrent vaso‐occlusive crisis. Enterocyte injury, increased permeability, altered microbial composition and bacterial overgrowth have all been documented as microbial and pathophysiologic changes in the gut microbiome of SCD patients in recent studies. Our aim was to sequence the bacterial 16S rRNA gene in order to characterize the gut microbiome of Angolan children with SCA and healthy siblings as a control. A total of 72 stool samples were obtained from children between 3 and 14 years old. Our data showed that the two groups exhibit some notable differences in microbiota relative abundance at different classification levels. Children with SCA have a higher number of the phylum Actinobacteria. As for the genus level, Clostridium cluster XI bacteria was more prevalent in the SCA children, whereas the siblings had a higher abundance of Blautia, Aestuariispira, Campylobacter, Helicobacter, Polaribacter and Anaerorhabdus. In this study, we have presented the first microbiota analysis in an Angolan paediatric population with SCD and provided a detailed view of the microbial differences between patients and healthy controls. There is still much to learn before fully relying on the therapeutic approaches for gut modulation, which is why more research in this field is crucial to making this a reality.

BackgroundLocally processed agro-food products are central to West African diets, infant and complementary feeding, local entrepreneurship and access to culturally appropriate foods. Post-production handling, packaging, transport, storage and retail exposure may compromise microbial and physicochemical quality. This study assessed ten locally processed agro-food products marketed in Burkina Faso and evaluated product-specific post-production supply-chain vulnerabilities at the retail level.ResultsThirty samples were collected, comprising three independent batches of each of the following products: infant pearl millet–soy–peanut flour (P1), fortified infant flour (P2), instant multi-cereal porridge (P3), dehydrated attiéké (P4), dried mango (P5), natural honey (P6), cottonseed cooking oil (P7), hibiscus juice (P8), Moringa oleifera leaf powder (P9) and roasted cashew nuts (P10). All continuous variables were z-score standardized prior to multivariate analysis. Principal component analysis (PCA) retained two components explaining 76.9% of total variance (PC1: 53.3%; PC2: 23.6%). The highest post-production vulnerability composite index (PPVCI) scores were observed for Moringa oleifera leaf powder (P9, rank 1) and hibiscus juice (P8, rank 2); intermediate profiles were identified for infant flours (P1, rank 3) and multi-cereal porridge (P3, rank 4); and the lowest profiles were recorded for honey (P6), cottonseed oil (P7) and roasted cashew nuts (P10).ConclusionsThe study provides baseline evidence for targeted quality-assurance interventions. Priority actions include: strengthened hygiene control during drying, milling and packaging; improved moisture-proof packaging for powders and dried products; cold-chain and water-quality control for hibiscus juice; oxidative stability monitoring for oil and lipid-rich products; and enhanced surveillance of infant foods and nutritional supplements. Future studies should incorporate mycotoxin screening and molecular confirmation m

Microbiological quality assessment of five common foods sold at different points of sale in Burkina-Faso

The aim of the present study was to assess the microbial quality of five ready-to-eat food such as bread, pasta, rice with sauce, beans and milk sold in five localities of Burkina Faso namely, Ouagadougou, Bobo-Dioulasso, Dakola, Cinkansé and Niangoloko. One hundred and one samples were collected and microbial quality were assessed by evaluating the food hygiene indicators such as total aerobic mesophilic flora, total coliforms, thermotolerant coliforms, yeast and mould. Food safety indicators such as Escherichia coli, Salmonella, coagulase-positive staphylococci, Clostridium perfringens and Bacillus cereus were also tested for contamination. Samples were tested according to ISO guidelines for all parameters. The results showed that 74 (73.27%) of samples were satisfactory while 15 (14.85%) were acceptable and 12 (11.88%) were not satisfactory according to international standards. Among the food safety indicators sought, Escherichia coli was detected in two samples and Bacillus cereus in four samples. Most of the analyzed food exhibited good hygiene behavior within the acceptable limits and the highest of not satisfactory rate was observed in milk powder and rice with sauce. Ouagadougou samples recorded the highest number of not satisfactory samples.Despite the general quality was satisfactory, the presence of specific microorganisms such as coliforms is indicative of the poor hygiene surrounded these foods. It is therefore necessary to train and follow up the vendors in the handling of equipment, hand-washing practices and selling environment hygiene for better improvement of the quality of the street foods.

Microbiological safety of flours used in follow up for infant formulas produced in Ouagadougou, Burkina Faso

Larissa Y. Waré, Augustini P. Nikièma, Jean C. Meile, Saïdou Kaboré, Angélique Fontana, Noël Durand, Didier Montet, Nicolas Barro

The prevalence of diarrheal diseases in children aged from 6 to 24 months in Burkina Faso is 38%. These diarrheas may be due to the consumption of contaminated weaning food. Therefore, the microbiological quality of follow up infant flours used as supplement foods is a key-point to reduce children diseases. In this study, the microbiological safety of locally-produced infant flours was investigated. One hundred and ninety-nine (199) samples were collected mainly in retails outlets and in Recovery and Nutrition Education Centers. According to the Burkina Faso regulations, microbiological analyses were carried out for Total Aerobic Mesophilic Flora (TAMF), thermotolerant coliforms, Salmonella spp. and yeasts/molds. The bacterial and fungal isolates were identified using phenotypic and genotypic methods and the study of the production of mycotoxins was carried out from the fungal isolates. In collected samples, the TAMF count ranged from 0 to 1.8 × 106 CFU/g with a total average of 6.3 × 104 CFU/g. About 2% of the samples had a microbial load exceeding the standards (105 CFU/g). No Salmonella spp. was isolated in the final infant flours. However, the presence of Enterobacteriaceae (Klebsiella spp. Enterobacter spp. and Cronobacter spp.) was detected and molecular characterization revealed also the presence of fungal species of the genus Aspergillus spp., Penicillium spp. and Fusarium spp. Some of these species were found to produce aflatoxins, ochratoxin A and fumonisins, which are potential carcinogenic toxins. These results demonstrated the need for a preventive approach based on the application of Hazard Analysis Critical Control Point in the food industry to ensure food safety of infant flours in Burkina Faso.

Microsatellite analysis supports clonal propagation and reduced divergence of Trypanosoma vivax from asymptomatic to fatally infected livestock in South America compared to West Africa

Herakles A Garcia, Adriana C Rodrigues, Carla MF Rodrigues, Zakaria Bengaly, Antonio HH Minervino, Franklin Riet-Correa, Rosangela Z Machado, Fernando Paiva, Jael S Batista, Luis Neves, Patrick B Hamilton, Marta MG Teixeira

BackgroundMechanical transmission of the major livestock pathogen Trypanosoma vivax by other biting flies than tsetse allows its spread from Africa to the New World. Genetic studies are restricted to a small number of isolates and based on molecular markers that evolve too slowly to resolve the relationships between American and West African populations and, thus, unable us to uncover the recent history of T. vivax in the New World.MethodsT. vivax genetic diversity, population structure and the source of outbreaks was investigated through the microsatellite multiloci (7 loci) genotype (MLGs) analysis in South America (47isolates from Brazil, Venezuela and French Guiana) and West Africa (12 isolates from The Gambia, Burkina Faso, Ghana, Benin and Nigeria). Relationships among MLGs were explored using phylogenetic, principal component and STRUCTURE analyses.ResultsAlthough closely phylogenetically related, for the first time, genetic differences were detected between T. vivax isolates from South America (11 genotypes/47 isolates) and West Africa (12 genotypes/12 isolates) with no MLGs in common. Diversity was far greater across West Africa than in South America, where genotypes from Brazil (MLG1-6), Venezuela (MLG7-10) and French Guiana (MLG11) shared similar but not identical allele composition. No MLG was exclusive to asymptomatic (endemic areas) or sick (outbreaks in non-endemic areas) animals, but only MLGs1, 2 and 3 were responsible for severe haematological and neurological disorders.ConclusionsOur results revealed closely related genotypes of T. vivax in Brazil and Venezuela, regardless of endemicity and clinical conditions of the infected livestock. The MLGs analysis from T. vivax across SA and WA support clonal propagation, and is consistent with the hypothesis that the SA populations examined here derived from common ancestors recently introduced from West Africa. The molecular markers defined here are valuable to assess the genetic diversity, to track the s

    Microscopie et interactions hôtes/micro-organismes

    Jeannine Lherminier, Marie Huyez-Levrat

Mid-Upper Arm Circumference Based Nutrition Programming: Evidence for a New Approach in Regions with High Burden of Acute Malnutrition

BackgroundIn therapeutic feeding programs (TFP), mid-upper arm circumference (MUAC) shows advantages over weight-for-height Z score (WHZ) and is recommended by the World Health Organization (WHO) as an independent criterion for screening children 6–59 months old. Here we report outcomes and treatment response from a TFP using MUAC ≤118 mm or oedema as sole admission criteria for severe acute malnutrition (SAM).MethodsPatient data from September 2007 to March 2009 for children admitted by MUAC ≤118 mm or oedema to a Médecins Sans Frontières (MSF) TFP in Burkina Faso were retrospectively analyzed. Analysis included anthropometric measurements at admission and discharge, program outcomes and treatment response.ResultsOf 24,792 patient outcomes analyzed, nearly half (48.8%; n = 12,090) were admitted with MUAC 116–118 mm. Most patients (88.7%; n = 21,983) were 6–24 months old. At admission, 52.7% (n = 5,041) of those with MUAC 116–118 mm had a WHZ <−3 SD. At discharge, 89.1% (n = 22,094) recovered (15% weight gain or oedema resolution), 7.9% (n = 1,961) defaulted, 1.5% (n = 384) failed to respond to treatment, and 1.0% (n = 260) died. Average weight gain was 5.4 g/kg/day, and average MUAC gain was 0.42 mm/day. Patients with MUAC ≤114 mm at admission had higher average daily weight and MUAC gains at discharge compared to those admitted with MUAC 116–118 mm, but those in the latter category required longer lengths of stay to achieve recovery (P<0.001).ConclusionThis analysis suggests that MUAC ≤118 mm as TFP admission criterion is a useful alternative to WHZ. Regarding treatment response, rates of weight and MUAC gain were acceptable. Applying 15% weight gain as discharge criterion resulted in longer lengths of stay for less malnourished children. Since MUAC gain parallels weight gain, it may be feasible to use MUAC as both an admission and discharge criterion.

Wail Bouzouba, Fatime Zahra Fdili Alaoui, Sofia Jayi, Hakima Bouguern, Moulay Abdelilah Melhouf

Adil Kallat, Oumar Diaby, Ali Valizadeh, Jean Agel, Claude Erb
