Diabetes knowledge and associated factors in adolescents and young adults with type 1 diabetes in Ouagadougou (Burkina Faso)

Daniel Zemba, Yempabou Sagna, Solo Traore, Lassane Zoungrana, D. Patricia Somé, S. Aimée Kissou, Oumar Guira, Téné Marceline Yaméogo

BackgroundType 1 diabetes (T1D) in youth is becoming a public health problem in Sub-Saharan Africa, including Burkina Faso. However, little is known about the level of knowledge of these patients on T1D. This study aimed to evaluate the knowledge of diabetes in adolescents and young adults about the disease, and identify the factors associated in Burkina Faso.MethodsA cross-sectional survey was conducted from April to June 2020 among youth with T1D, aged 10 to 30 years, and regularly followed in the internal medicine department of Yalgado Ouedraogo University Hospital of Ouagadougou, Burkina Faso. Data were collected using the French AJD (Aide aux Jeunes Diabétiques) validated diabetes knowledge and skills (DKS) questionnaire designed to test participants’ accuracy in knowledge about six different themes of T1D, as generalities of diabetes, hypoglycemia and hyperglycemia, management of insulin treatment, and self-monitoring blood glucose (SMBG). DKS level was determined by calculating the scores, and univariate and multivariate logistic regression were used to explore factors influencing DKS scores. This level was classified as insufficient or low ≤ 25/50, regular 26–39/50, and adequate or good ≥ 40/50.ResultsSixty-three participants with a mean age of 19.05 years and a sex ratio (W/M) of 1.17 were included in our study. The mean HbA1c level was 9.79%, and 43 (68.23%) patients had an insufficient DKS level. The mean global DKS score of correct answers was 23.63/50. The percentage of correct answers was respectively 50% for the item “generalities of diabetes”, 32.4% for the item “hypoglycemia and hyperglycemia”, 67.72% for the item “diet”, 37.34% for the item “management of insulin treatment” and 44.97% for the item “SMBG”. In univariate analysis, a better patient DKS level was associated with university education and long duration of diabetes care follow-up (> 10 years, p < 0.05). Only increasing age remain associated with a better knowledge score (p < 0.05) in mult

Diagnosing congenital malaria in a high-transmission setting: clinical relevance and usefulness of P. falciparum HRP2-based testing

Hamtandi Magloire Natama, Delwendé Florence Ouedraogo, Hermann Sorgho, Eduard Rovira-Vallbona, Elisa Serra-Casas, M. Athanase Somé, Maminata Coulibaly-Traoré, Petra F. Mens, Luc Kestens, Halidou Tinto, Anna Rosanas-Urgell

Congenital malaria diagnosis is challenging due to frequently observed low parasite density infections, while their clinical relevance during early infancy is not well characterized. In Nanoro health district (Burkina Faso), we determined the prevalence of congenital malaria by real-time quantitative PCR and we assessed the performance of rapid diagnosis test (RDT) and light microscopy (LM) to detect Plasmodium falciparum infections in cord-blood samples. In addition, we examined the usefulness of P. falciparum Histidine Rich Protein2 (PfHRP2) as surrogate biomarker of infection and explored association between congenital malaria and clinical outcomes. A prevalence of congenital malaria by qPCR of 4% (16/400) was found, which increased to 10% among newborns from mothers infected at delivery. RDT and LM showed poor performances indicating limited utility for congenital malaria screening in cord blood. Because PfHRP2 detection in cord blood could be affected by transplacental passage of parasite antigens, PfHRP2 might not be used as a surrogate biomarker of congenital malaria infections. There was no evidence of a significant clinical impact of congenital malaria on infant’s health from birth to 59 days of life. Case control studies including long-term follow up may provide additional understanding on the relevance of neonatal malaria infections.

Sibraogo Kiemtoré, Hyacinthe Zamané, Yobi Alexis Sawadogo, Rodrigue Sansan Sib, Evelyne Komboigo, Ali Ouédraogo, Blandine Bonané

BackgroundGiant ovarian cyst is very rare in gravid-puerperium period. It is a cause of a maternal-fetal morbidity. We report a case of a giant benign ovarian cyst in gravid-puerperium period which was diagnosed and managed in a hospital of a low-resource country.Case presentationData were collected by historical review, clinical examination, laboratory investigations, imaging examination, and by histopathological study of the excised surgical specimen. It is the case of a 25-year-old woman who was third gravida and third para with unknown pathological history. After she had given birth through vagina, a giant ovarian cyst, unknown during pregnancy, was diagnosed. A left oophorectomy carrying the cyst was performed after laparotomy in Yalgado Ouedraogo University Hospital Center of Ouagadougou (Burkina Faso). The cyst was 42 cm long and weighed 19.7 kg. The histology of the operative specimen revealed serous cystadenoma of the ovary. The postoperative course was uneventful.ConclusionThis case reports that vaginal delivery is possible with a giant ovarian cyst associated with pregnancy. Surgical management of the cyst can be performed in the postpartum with satisfaction.

Diagnosis and Management of Bacterial Meningitis by Pediatric Residents in Sudan

BackgroundBacterial meningitis (BM) is a life-threatening emergency requiring timely and accurate diagnosis. In Sudan, a country within the meningitis belt, pediatric residents play a key role in BM management, yet no prior studies have evaluated their practices. This study aims to assess diagnostic and management approaches among pediatric residents and identify related challenges.MethodologyA descriptive cross-sectional study was conducted among pediatric residents registered with the Sudan Medical Specialization Board (SMSB). Data were collected using a validated, self-administered questionnaire distributed via email and WhatsApp. The survey included items on diagnostic methods, lumbar puncture (LP) practices, antibiotic use, training, and follow-up care. Descriptive statistics and Chi-square or Fisher's exact tests were used to analyze associations, with a significance level set at P < 0.05.ResultsOf 283 respondents, 249 (88%) were female, and 158 (56%) were in their final residency year. BM diagnosis was confirmed in 106 (37%) cases based on cerebrospinal fluid findings consistent with BM-elevated white cell count, low glucose, and high protein - rather than culture-proven disease, while 40% relied on clinical and basic lab criteria. LP was performed by 212 (75%) of residents, but 168 (59%) reported patient or family refusal as a primary barrier. Only 73 (26%) documented consent/refusal. While 243 (86%) were trained to interpret LP results, just 157 (55%) received training on obtaining consent. Empirical treatment commonly included vancomycin-based regimens (142, 50%) or third-generation cephalosporins (138, 49%). Most residents adhered to a 10- to 14-day intravenous antibiotic regimen (264, 93%), but only 63 (22%) consistently arranged hearing assessments post-treatment.ConclusionsFindings reveal inconsistent diagnostic practices and significant training and resource gaps among pediatric residents managing BM in Sudan. Addressing these deficiencies may improve

Diagnosis of a public policy: an introduction to user fee exemptions for healthcare in the Sahel

Jean-Pierre Olivier de Sardan, Valéry Ridde

During the last ten years, Burkina Faso, Mali and Niger have opted for selective user fee exemption policies, while remaining within the general framework of cost recovery. But they have each developed their own particular institutional mechanisms, different from those of their neighbour. This was the topic of a comparative research program combining both quantitative and qualitative surveys over a four-year period. This special issue presents papers setting exemption policies in the wider context of public policy and the day-to-day functioning of health systems (part 1); presenting overarching case studies (part 2); and reflecting on our methodological approach (part 3).User fee exemption policies were introduced in Burkina Faso, Mali and Niger during the first decade of this century. They cover several sector-based measures ('free healthcare' in everyday language), and sometimes come on top of high levels of subsidies which enabled significant reductions in the cost of certain drugs and treatments. From the late 1980s, these three countries were - and still are - subject to a comprehensive system of cost recovery at the point of delivery (a policy introduced following the Bamako Initiative), or, to be more precise, a system of partial payment of drugs and services by the user. Only a small proportion of the costs are actually recovered as the amounts charged to the users do not take salaries, investments or recurrent costs, which are all paid by the state, into account, and represent only a small percentage of the overall health budget (an order of magnitude of five percent is often cited at state level [1,2]. Nevertheless, the sums recovered by health centres enabled them to buy drugs and cover certain local expenses. However, for public health reasons, cost recovery has always been subject to a variety of sector-based exceptions, determined by the nature of the disease or intervention involved. For example, mass immunization (National Immunization Days) and routin

Diagnosis of Bacterial Bloodstream Infections: A 16S Metagenomics Approach

BackgroundBacterial bloodstream infection (bBSI) is one of the leading causes of death in critically ill patients and accurate diagnosis is therefore crucial. We here report a 16S metagenomics approach for diagnosing and understanding bBSI.Methodology/Principal FindingsThe proof-of-concept was delivered in 75 children (median age 15 months) with severe febrile illness in Burkina Faso. Standard blood culture and malaria testing were conducted at the time of hospital admission. 16S metagenomics testing was done retrospectively and in duplicate on the blood of all patients. Total DNA was extracted from the blood and the V3–V4 regions of the bacterial 16S rRNA genes were amplified by PCR and deep sequenced on an Illumina MiSeq sequencer. Paired reads were curated, taxonomically labeled, and filtered. Blood culture diagnosed bBSI in 12 patients, but this number increased to 22 patients when combining blood culture and 16S metagenomics results. In addition to superior sensitivity compared to standard blood culture, 16S metagenomics revealed important novel insights into the nature of bBSI. Patients with acute malaria or recovering from malaria had a 7-fold higher risk of presenting polymicrobial bloodstream infections compared to patients with no recent malaria diagnosis (p-value = 0.046). Malaria is known to affect epithelial gut function and may thus facilitate bacterial translocation from the intestinal lumen to the blood. Importantly, patients with such polymicrobial blood infections showed a 9-fold higher risk factor for not surviving their febrile illness (p-value = 0.030).Conclusions/SignificanceOur data demonstrate that 16S metagenomics is a powerful approach for the diagnosis and understanding of bBSI. This proof-of-concept study also showed that appropriate control samples are crucial to detect background signals due to environmental contamination. Diagnosis patterns of sickle cell disease in Ghana: a secondary analysis

Alexandra M. Sims, Kwaku Osei Bonsu, Rebekah Urbonya, Fatimah Farooq, Fitz Tavernier, Marianna Yamamoto, Sheri VanOmen, Brittne Halford, Polina Gorodinsky, Rachel Issaka, Tulana Kpadenou, Rhonda Douglas, Samuel Wilson, Clementine Fu, Danielle Canter, Duña Martin, Austin Novarra, Lewis Graham, Fredericka Sey, Charles Antwi-Boasiako, Catherine Segbefia, Onike Rodrigues, Andrew Campbell

BackgroundDespite having the highest prevalence of sickle cell disease (SCD) in the world, no country in Sub-Saharan Africa has a universal screening program for the disease. We sought to capture the diagnosis patterns of SCD (age at SCD diagnosis, method of SCD diagnosis, and age of first pain crisis) in Accra, Ghana.MethodsWe administered an in-person, voluntary survey to parents of offspring with SCD between 2009 and 2013 in Accra as a part of a larger study and conducted a secondary data analysis to determine diagnosis patterns. This was conducted at a single site: a large academic medical center in the region. Univariate analyses were performed on diagnosis patterns; bivariate analyses were conducted to determine whether patterns differed by participant’s age (children: those < 18 years old whose parents completed a survey about them, compared to adults: those > = 18 years old whose parents completed a survey about them), or their disease severity based on SCD genotype. Pearson’s chi-squared were calculated.ResultsData was collected on 354 unique participants from parents. Few were diagnosed via SCD testing in the newborn period. Only 44% were diagnosed with SCD by age four; 46% had experienced a pain crisis by the same age. Most (66%) were diagnosed during pain crisis, either in acute (49%) or primary care (17%) settings. Children were diagnosed with SCD at an earlier age (74% by four years old); among the adults, parents reflected that 30% were diagnosed by four years old (p < 0.001). Half with severe forms of SCD were diagnosed by age four, compared to 31% with mild forms of the disease (p = 0.009).ConclusionsThe lack of a robust newborn screening program for SCD in Accra, Ghana, leaves children at risk for disease complications and death. People in our sample were diagnosed with SCD in the acute care setting, and in their toddler or school-age years or thereafter, meaning they are likely being excluded from important preventive care. Diagnostic à terme de jumeaux conjoints

Imane Benchiba, Mohammed Karam Saoud, Nisrine Mamouni, Sanaa Errarhay, Chahrazad Bouchikhi, Banani Abdelaziz

Diagnostic accuracy of cervical cancer screening and screening–triage strategies among women living with HIV-1 in Burkina Faso and South Africa: A cohort study

BackgroundCervical cancer screening strategies using visual inspection or cytology may have suboptimal diagnostic accuracy for detection of precancer in women living with HIV (WLHIV). The optimal screen and screen–triage strategy, age to initiate, and frequency of screening for WLHIV remain unclear. This study evaluated the sensitivity, specificity, and positive predictive value of different cervical cancer strategies in WLHIV in Africa. Methods and findingsWLHIV aged 25–50 years attending HIV treatment centres in Burkina Faso (BF) and South Africa (SA) from 5 December 2011 to 30 October 2012 were enrolled in a prospective evaluation study of visual inspection using acetic acid (VIA) or visual inspection using Lugol’s iodine (VILI), high-risk human papillomavirus DNA test (Hybrid Capture 2 [HC2] or careHPV), and cytology for histology-verified high-grade cervical intraepithelial neoplasia (CIN2+/CIN3+) at baseline and endline, a median 16 months later. Among 1,238 women (BF: 615; SA: 623), median age was 36 and 34 years (p < 0.001), 28.6% and 49.6% ever had prior cervical cancer screening (p < 0.001), and 69.9% and 64.2% were taking ART at enrolment (p = 0.045) in BF and SA, respectively. CIN2+ prevalence was 5.8% and 22.4% in BF and SA (p < 0.001), respectively. VIA had low sensitivity for CIN2+ (44.7%, 95% confidence interval [CI] 36.9%–52.7%) and CIN3+ (56.1%, 95% CI 43.3%–68.3%) in both countries, with specificity for ≤CIN1 of 78.7% (95% CI 76.0%–81.3%). HC2 had sensitivity of 88.8% (95% CI 82.9%–93.2%) for CIN2+ and 86.4% (95% CI 75.7%–93.6%) for CIN3+. Specificity for ≤CIN1 was 55.4% (95% CI 52.2%–58.6%), and screen positivity was 51.3%. Specificity was higher with a restricted genotype (HPV16/18/31/33/35/45/52/58) approach (73.5%, 95% CI 70.6%–76.2%), with lower screen positivity (33.7%), although there was lower sensitivity for CIN3+ (77.3%, 95% CI 65.3%–86.7%). In BF, HC2 was more sensitive for CIN2+/CIN3+ compared to VIA/VILI (relative sensitivity for CIN2+

Hekmat Chaara, Hind Adadi, Imane Attar, Sofia Jayi, Fatima-Zahra Fdili Alaoui, Moulay Abdelilah Melhouf

While impaired eruption of permanent molars is a rare phenomenon, its prevalence appears to be increasing. Among these disorders, a distinction can be made between impactions, in which a tooth is blocked along its eruption path by a physical obstacle, and primary retention, in which eruption ceases in the absence of such a barrier. Within primary retention, primary failure of eruption (PFE) is a genetically determined condition that can severely affect one or more teeth and impact adjacent teeth. The clinical management of PFE is complex and multidisciplinary, and orthodontic traction is not recommended, as it often results in progressive ankylosis. Early identification of PFE is therefore essential to guide the therapeutic strategy.

Jihane Toughza, Aomar Agadr, Mouad Nejjari, Insaf Al Ammari

Kays Chaker, Abdelrazek Bouzouita, Marwa Gharbi, Ahlem Blel, Marouene Chakroun, Haroun Ayed, Mohamed Cherif, Mohamed Riadh Ben Slama, Soumaya Rammeh, Amine Derouiche, Mohamed Chebil

Zouheir Hafidi, Hanan Handor, Mina Laghmari, Najat Handor, Lalla Ouafae Cherkaoui, Samira Tachfouti, Myriame Seffar, Rajae Daoudi

Fatma Chaker, Melika Chihaoui, Meriem Yazidi, Hedia Slimane

Karidia Werme, Cyrille Bisseye, Issiaka Ouedraogo, Albert Théophane Yonli, Djénèba Ouermi, Florencia Djigma, Rémy Moret, Charlemagne Gnoula, Jean-Baptiste Nikiema, Jacques Simpore

Ilboudo Désire, Bisseye Cyrille, Djigma Florencia, Diande Souba, Yonli Albert, Bazie Jean Telesphore Valerie, Compaore Rebecca, Gnoula Charlemagne, Djibril Tamboura, Moret Rémy, Pietra Virginio, Karou Damintoti Simplice, Ouedraogo Martial, Simpore Jacques
